        외상환자의 중증도분류를 위한 Triage Score와 Triage-Revised Trauma Score의 비교

        Background: The triage-revised trauma score(t-RTS) is used for triage in trauma patients in many countries. Recently, the triage score was developed as a new triage method for trauma patients in Korea. The objective of this study was to assess the accuracy of triage for the t-RTS and the triage score. Methods: The medical records of 1575 consecutive trauma patients visiting the Emergency Center, Masan Samsung Hospital, from March to August 1999 were retrospectively analyzed. The triage score and the t-RTS were taken from the charts completed by the emrgency room doctors and nurses. The accuracies of the two triage methods were compared by using the undertriage and the overtriage rates, receiver operating characteristic(ROC) curve analysis, and a correlation analysis with many physiologic, anatomical, biochemical, and mixed scores(Glasgow coma scale, injury severity score, TRISS, APACHE II and III scores, operation, survival, etc.). Results: The undertriage rates for the triage score and the t-RTS were 22.8% and 38.2%, respectively(p≤0.001), the overtriage rates were 6.3% and 3.1%, respectively,(p≤0.001), and the accurate triage rates were 91.3% and 91.7%, respectively. The areas under ROC curves for the triage score and the t-RTS were 0.871±0.033 and 0.800±0.039, respectively(95% confidence interval, p<0.001). In general, the correlations of the two triage methods with many physiologic, anatomical, biochemical, and mixed scores were moderately high, but there were no statistical differences between the correlation coefficients of the two triage methods for those scores. Conclusion: The triage score was equal or superior to the t-RTS for triage in trauma patients. The former was simpler and could be used easily by emergency medical personnel. Therefore, the authors conclude that the triage score can be used instead of the t-RTS for triage in trauma patients.

        2.9% 과산화수소를 함유한 부착형 미백제의 임상적 효능과 안정성에 관한 연구

        2.9%의 과산화수소를 함유한 시험용 부착형 미백제(Medison dental whitening strip, Samsung medical Co.,Anyang,Korea)를 실험군 피험자 23명, 시험용 미백제에서 과산화수소를 제외한 동일한 제재를 대조약으로 대조군 피험자 24명에게 매일 1시간 30분씩, 2주간 적용하여 임상적 효능과 안전성을 평가하였다. 미백 효과는 Shade Vision, Chroma Mete 및 Vitapan classical shade guide를 이용하여 색변화를 측정하여 다음과 같은 결론을 얻었다. 1. 실험군에서의 전체 치아의 색변화량(ΔE^(*))은 세 가지 측정법에서 모두 대조군에 비해 통계적으로 유의한 차이를 보였고 (p < 0.05). 쉽게 인지할 수 있는 정도였다. 2. 색변화는 색좌표 상에서 명도의 증가와 녹색조 및 청색조로의 변화에 의해 얻어졌다. 3. 치아별로는 어두운 견치의 색변화량이 가장 컸고, 측절치, 중절치 순으로 감소하였다. 4. 47명의 피험자 중 5명이 경도의 과민증을 경험하였으나 곧 완화되었다, 치은염증지수와 전기치수검사 결과 시험 전후에 차이가 없어 안전성을 확인하였다. This study evaluated the effectiveness and safety of an experimental bleaching strip (Medison dental whitening strip, Samsung medical Co., Anyang, Korea) containing 2.9% hydrogen peroxide. Twenty-three volunteers used the bleaching strips for one and a half hour daily for 2 weeks. As control group, the same strips in which hydrogen peroxide was not included were used by 24 volunteers with the same protocol. The shade chang (ΔE^(*), color differnce) of twelve anterior teeth was measured using Shade Vision (X-Rite Inc., S. W. Grandville, MI, USA), Chroma Meter (Minota Co., Ltd. Osaka, Japan) and Vitapan classical shade guide (Vita Zahnfabrik, Germany). The shade change of overall teeth in the experimental group was significantly greater than that in the control group (p < 0.05) and was easily perceivable. The change resulted from the increase of lightness (CIE L^(*) value) and the decrease of redness (CIE a^(*) value) and yellowness (CIE b^(*) value). The shade change of individual tooth was greatest in canine, can smallest in central incisor. The safety of the bleaching strip was also confirmed. 〔J Kor Acad Cons Dent 31(4):269-281, 2006〕

        석류씨 추출물의 항산화 및 항균활성

        This study was investigated on antioxidative and antimicrobial activities of PSW(pomegranate seed water extract), PSE(pomegranale seed ethanol extract) and PSO(pomegranate seed oil). 'rile extraction yields of PSW, PSE and PSO were 28,9, 13.0 and 4.9%, respectively. Total phenol contents of PSW, PSE and PSO were 47, 78 mg/g(dry basis) and 40 mg/g, respectively. Electron donating abilities of PSW, PSE and PSO at 1,000 ppm were 18.8, 28.5 and 9.7%, respectively. Antioxidative activities in linoleic acid substrates at 500 ppm were in order of 1'SE > a-tocophcrol > PSW > PSO. Antioxidative activities in linoleic acid emulsion substrates at 200 ppm were in order of α-tocopberol > PSE > PSW > PSO, In antimicrobial activity, PSO showed growth inhibition effect against Micrococcus luteus, Salmonella enteritidis and PSW showed growth inhibition effect against Bacillus cereus, Escherichia coll. Whereas antimicrobial activity' of PSE was not observed. The nitrite-scavenging abilities of PSW, PSE and PSO at 2,000 ppm were 27.5, 23.7 and 39.6%, respectively. And the SOD-like activities of PSW, PSE and PSO at 1,000 ppm were 15.9, 34.9 and 0.10%, respectively.

        충수염의 임상적 고찰과 초음파의 유용성

        Appendicitis is the most common acute abdomen in Emergency Medical Center & outpatient department. But sometimes, it is difficult for us to diagnose because of clinically variable symptoms. We reviewed the ultrasonographic findings and analyse 521 cases of appendectomy patients about its distribution of sex and age, seasonal prevalence, clinical symptom and signs, laboratory findings, duration from onset of symptom to arrival at hospital, duration from arrival at hospital to operation, vital signs, operative results, and associated diseases, which were treated at the department of General Surgery & Emergency Medical Center in Chung Ang Gil Hospital from July, 1994 to June, 1995. The results were as follows : 1. The most common age group was second decade. And male to female ratio was 1.43:1. Seasonal variation was not significant. 2. Of 521 cases, 141 cases(27.1%) visited the hospital within 24 hours from the onset of symptom, and 259 cases(49.7%) visited between 24 to 48 hours. 3. In duration from arrival to operation, the cases within 6 hours were 263 (50.5%), the cases between 6 and 12 hours were 131(25..1%). 4. In systolic blood pressure, cases below 90mmHg were 19(3.6%), cases above 160mmHg were 23(4.4%). Below 60mmHg diastolic pressure were 55 cases(10.6%), cases above 110mmHg were 9(1.7%). In pulse rate, above 90 rate/min were 171(32.8%). 5. On admission leukocytosis was found in 413 cases(79.3%) and 342 cases of them were pathologic appendicitis. 6. Ultrasoundgraphy was performed in the 87 cases, but 23 cases were suggested appendicitis. Among 23 cases, 22(95.7%) revealed pathologic appendicitis at ooperation and labortatory examination. Of the 64 cases which were normal on ultrasonography, only 2 cases(3.1%) were proved to be appendicitis. 7. The results of 496 operations without ultrasonography were appendicitis in 390 cases(78.6%), mesenteric lymphadenopathy 36(7.3%), pelvic inflammatory disease 28(5.6%), fecalith 15(3.0%), terminal ileitis 8(1.6%), diverticulitis 7(1.4%), urinary tract stones 5(1.0%), cecal cancer 2 cases, right ovarian tumor 2 cases, tubo-ovarian abscess 2 cases, grater omentum internal hernia 1 case. Our results showed that the sensitivity of ultrasonohraphy was 95.8% and the specificity 98.4% in the diagnosis of appendicitis, It suggests that the ultrasonography is very helpful in the diagnosis & differential diagnosis of acute appeendiditis.

        Preparation and Characterization of SiO₂/TiO₂ Core/Shell Composite Particles using TiO₂ Nanoparticles Via Heterocoagulation in a Water System

        SiO₂/TiO₂core/shell composites were prepared using SiO₂and nano-crystalline TiO₂sols as the core and shell particles, respectively, via heterocoagulation in a water system. The dispersion stability of the sols has been investigated as a function of pH and concentration of surfactant, on the basis of the DLVO and heterocoagulation theories. The core/shell composite particles were characterized as a function of the molar ratio of SiO₂and TiO₂by various techniques, such as TGA, FT-IR spectroscopy, XRD, FE-SEM, N₂adsorption, apparent surface coverage, and their photocatalytic activity for the decomposition of methylene blue (MB) in water. The optimum value of pH for the preparation of the SiO₂/TiO₂core/shell particles was 3.5. A homogeneous TiO₂coating was obtained when the molar ratio of SiO₂and TiO₂was 88:12. The apparent surface coverage and BET surface area of the SiO₂/TiO₂(88/12) particles prepared from SiO₂(Dpso: 692 ㎚) were 90% and 48.9 ㎡/g, respectively, while those of the composite particles prepared from SiO₂(Dpso: 331 ㎚) were 90% and 65.4 ㎡/g, respectively. The SiO₂/TiO₂ core/shell particles, prepared by the heterocoagulation method, exhibited efficient photocatalytic activities toward the decomposition of MB, which increased with the UV irradiation time and the TiO₂ loading.

        Interferon-induced protein 56 (<i>IFI56</i>) is induced by VHSV infection but not by bacterial infection in olive flounder (<i>Paralichthys olivaceus</i>)

        <P><B>Abstract</B></P> <P>Interferon-inducible protein 56 (<I>IFI56,</I> also known as ISG56/<I>IFIT1</I>, interferon-induced protein with tetratricopeptide repeats 1) is strongly induced in response to interferon and a potent inhibitor of viral replication and translational initiation. Here, we describe the identification of <I>IFI56</I> (<I>OfIFI56</I>) in olive flounder, its characteristic features, and expression levels in various tissues before and after viral hemorrhagic septicemia virus (VHSV) infection. The full-length <I>OfIFI56</I> sequence was identified from rapid amplification of cDNA ends PCR. The complete coding sequence of <I>OfIFI56</I> is 1971 bp in length and encodes 431 amino acids. The putative <I>OfIFI56</I> protein has multiple tetratricopeptide (TPR) motifs, which regulate diverse biological processes, such as organelle targeting, protein import, and vesicle fusion. Based on sequence analysis, the <I>Larimichthys crocea</I> IFI56 protein (61%) had the highest sequence homology to <I>OfIFI56.</I> In healthy olive flounder, <I>OfIFI56</I> mRNA expression was detected in many tissues such as intestine, gill, head kidney, heart, spleen, and trunk kidney tissues. After VHSV challenge, <I>OfIFI56</I> mRNA was significantly up-regulated in these tissues. Additionally, <I>OfIFI56</I> expression was induced by poly I:C but not by <I>Streptococcus parauberis</I> and <I>S. iniae</I> infection or lipopolysaccharide injection in kidney and spleen tissues of olive flounder. These results demonstrate that piscine <I>OfIFI56</I> expression is not induced by bacterial infection but is selectively induced by viral infection, especially VHSV, and that <I>OfIFI56</I> may play an important role in the host response against VHSV infection.</P> <P><B>Highlights</B></P> <P> <UL> <LI> The fish IFI56/IFIT1 family including OfIFI56 grouped with IFI56 and not fish IFI58. </LI> <LI> OfIFI56 mRNA was up-regulated in all tissues except muscle after VHS virus challenge. </LI> <LI> OfIFI56 expression was not induced by <I>Streptococcus parauberis</I> and <I>S. iniae</I> infection and LPS injection in olive flounder. </LI> <LI> OfIFI56 mRNA expression markedly increased in all tissues after poly I: C infection. </LI> </UL> </P>

        Prognostic Determinants in Patients with Traumatic Pancreatic Injuries

        The aim of this study was to identify factors that predict morbidity and mortality in patients with traumatic pancreatic injuries. A retrospective review was performed on 75 consecutive patients with traumatic pancreatic injuries admitted to the Emergency Medical Center at Masan Samsung Hospital and subsequently underwent laparotomy during the period January 2000 to December 2005. Overall mortality and morbidity rates were 13.3% and 49.3%, respectively. A multivariate regression analysis revealed that greater than 12 blood transfusions and an initial base deficit of less than -11 mM/L were the most important predictors of mortality (p<0.05). On the other hand, the most important predictors of morbidity were surgical complexity and an initial base deficit of less than -5.8 mM/L (p<0.01). These data suggests that early efforts to prevent shock and rapidly control of bleeding are most likely to improve the outcome in patients with traumatic pancreatic injuries. The severity of pancreatic injury per se influenced only morbidity.

        Intrawound Vancomycin Powder Application for Preventing Surgical Site Infection Following Cranioplasty

        Objective : Surgical site infection is the most detrimental complication following cranioplasty. In other surgical fields, intrawound vancomycin powder application has been introduced to prevent surgical site infection and is widely used based on results in multiple studies. This study evaluated the effect of intrawound vancomycin powder in cranioplasty compared with the conventional method without topical antibiotics. Methods : This retrospective study included 580 patients with skull defects who underwent cranioplasty between August 1, 1998 and December 31, 2021. The conventional method was used in 475 (81.9%; conventional group) and vancomycin powder (1 g) was applied on the dura mater and bone flap in 105 patients (18.1%; vancomycin powder group). Surgical site infection was defined as infection of the incision, organ, or space that occurred after cranioplasty. Surgical site infection within 1-year surveillance period was compared between the conventional and vancomycin powder groups with logistic regression analysis. Penalized likelihood estimation method was used in logistic regression to deal with zero events. All local and systemic adverse events associated with topical vancomycin application were also evaluated. Results : Surgical site infection occurred in 31 patients (5.3%) and all were observed in the conventional group. The median time between cranioplasty and detection of surgical site infection was 13 days (range, 4–333). Staphylococci were the most common organisms and identified in 25 (80.6%) of 31 cases with surgical site infections. The surgical site infection rate in the vancomycin powder group (0/105, 0.0%) was significantly lower than that in the conventional group (31/475, 6.5%; crude odds ratio [OR], 0.067; 95% confidence interval [CI], 0.006–0.762; adjusted OR, 0.068; 95% CI, 0.006–0.731; p=0.026). No adverse events associated with intrawound vancomycin powder were observed during the follow-up. Conclusion : Intrawound vancomycin powder effectively prevented surgical site infections following cranioplasty without local or systemic adverse events. Our results suggest that intrawound vancomycin powder is an effective and safe strategy for patients undergoing cranioplasty.

        Olive flounder CD276 (B7-H3) a coinhibitory molecule for T cells: Responses during viral hemorrhagic septicemia virus (VHSV) stimulation

        <P>Coinhibitory pathways in the B7-CD28 family provide critical inhibitory signals that regulate immune homeostasis, defense and protect tissue integrity. CD276 (B7-H3) is an important immune checkpoint member of this family, which is induced on antigen-presenting cells (APCs), and plays an important role in the inhibition of T cell function. We have characterized the CD276 gene of olive flounder, Paralichthys olivaceus. OfCD276 has an ORF of 912 bp that codes for 303 amino acids with a predicted molecular mass of 33 kDa. It is a type I trans membrane protein with a single extracellular V- and C-like Ig domains, a transmembrane region, and a highly diverse cytoplasmic tail. This gene was distinctly expressed in gill, spleen, and skin, and sparsely expressed in other tissues. Pathogen stimulation by VHSV revealed that transcription of OfCD276 was induced on early hours in liver and expressed late in head kidney, spleen, intestine and gill tissues. Flow cytometry analysis of leukocytes revealed the percentage of granulocytes and lymphocytes that expressed OfCD276 molecules on their cell surface was 85.1% and 3.1%, respectively. Our study shows a significant role played by this coinhibitory molecule that participate in the regulation of the cell mediated immune response.</P>
