Inter-radicular distance between the maxillary second premolar and the first molar

The aim of this study was to evaluate the effect of the curvature of mesiobuccal root of the maxillary first molar in the space between the maxillary second premolar and the first molar, which is the preferred site for mini-implant placement. In this study, cone-beam computed tomography (CBCT) data of 85 patients (36 male, 49 female; 14-38 years old) were used to measure the mesiobuccal root length of the maxillary first molar and the distance from the cementoenamel junction (CEJ) to the most curved point of the mesiobuccal root of the maxillary first molar. The interradicular distance between the maxillary second premolar and the first molar, and the buccolingual width of alveolar bone and cortical bone thickness at the plane 4-, 6-, and 8-mm apical to the CEJ were also measured. The length of the mesiobuccal root of the maxillary first molar was 11.2 ± 1.1 mm, and the distance from CEJ to the most curved point was 6.4 ± 0.7 mm, which was about 57.4% of the root length. The interradicular distances were almost same at the 4- and 6-mm planes, but increased significantly between the 6- and 8-mm planes. Although the buccolingual widths of the alveolar bone was increased apically, cortical bone thicknesses were not different among the 4-, 6-, and 8-mm planes. The interradicular distance does not increase between the 4-mm and 6-mm planes from CEJ because of the presence of the curvature of the mesiobuccal root of maxillary first molar.

As an alternative to the conventional fixed appliance that uses orthodontic brackets, a simple round tube without a bonding base can be bonded to the tooth surface by covering the tube with flowable resin. In this technique, bent wires cannot be inserted into the simple tubes; therefore, repositioning of the simple tubes is often required for adjustments. To reduce repositioning of simple tubes, a dome-shaped resin covering of the simple tube can be designed with a customized in-and-out compensation, using three-dimensional computer-aided design software based on digital simulation of orthodontic tooth movement. In the present case, the use of simple tubes bonded with customized resin coverings in a Class I nonextraction case is described in a 17-year-old male, in whom moderate crowding of the anterior teeth was treated over an 8-month period. This case shows that simple tubes can be used as an alternative to brackets in some Class I nonextraction cases, with the potential benefit of reducing decalcification.

This case report describes the orthodontic treatment of a patient with severe anterior open bite and skeletal class II malocclusion with temporomandibular joint (TMJ) osteoarthritis (OA) of the left condyle. The 21-year-old male patient had open-bite malocclusion, mild crowding, and protrusion of the anterior teeth. Mild erosive changes were detected in the anterior part of the left mandibular condyle on cone-beam computed tomography; however, because no clinical symptoms were present, orthodontic treatment was performed. It is imperative to consider the potential implications of orthodontic treatment on the stability of the TMJ throughout the duration of treatment, as any instability can exacerbate TMJ OA. Hence, it is crucial to opt for the least invasive treatment modality available. In this regard, orthodontic treatment using a skeletal anchorage system as an alternative to conventional orthognathic surgery for patients with open bite holds great promise, as it not only ensures mandibular stability but also significantly ameliorates the open-bite condition.

Understanding the classification of malocclusion is a crucial issue in Orthodontics. It can also help us to diagnose, treat, and understand malocclusion to establish a standard for definite class of patients. Principal component analysis (PCA) and k-means algorithms have been emerging as data analytic methods for cephalometric measurements, due to their intuitive concepts and application potentials. This study analyzed the macro- and meso-scale classification structure and feature basis vectors of 1020 (415 male, 605 female; mean age, 25 years) orthodontic patients using statistical preprocessing, PCA, random matrix theory (RMT) and k-means algorithms. RMT results show that 7 principal components (PCs) are significant standard in the extraction of features. Using k-means algorithms, 3 and 6 clusters were identified and the axes of PC1~3 were determined to be significant for patient classification. Macro-scale classification denotes skeletal Class I, II, III and PC1 means anteroposterior discrepancy of the maxilla and mandible and mandibular position. PC2 and PC3 means vertical pattern and maxillary position respectively; they played significant roles in the meso-scale classification. In conclusion, the typical patient profile (TPP) of each class showed that the data-based classification corresponds with the clinical classification of orthodontic patients. This data-based study can provide insight into the development of new diagnostic classifications.

<P>Ka-mi-kae-kyuk-tang (KMKKT) is an Oriental herbal medicinal cocktail. Our collaborative team has shown that it has potent anti-angiogenic, anti-cancer and anti-metastatic activities in vivo without observable side effects. We have documented evidence for KMKKT to alleviate drug-induced hematotoxicity in vivo. In the present study, we investigated the mechanistic and signaling events through which KMKKT enhances hematopoiesis, using hematopoietic stem cells (HSCs) isolated from the bone marrow of 8-12 week-old C57BL/6 mice. Our results show that KMKKT significantly increased the expression of the hematopoietic cytokines interleukin (IL)-3, stem cell factor (SCF), granulocyte-macrophage-colony stimulating factor (GM-CSF), thrombopoietin (TPO) and erythropoietin (EPO) at the level of mRNA and secretion in HSCs. KMKKT also increased the expression of c-Kit, a cytokine receptor expressed in HSCs. In addition, KMKKT enhanced phosphorylation of Janus kinase 2 (JAK2) and signal transducer and activator of transcription 5 (STAT5), and increased the binding activity of STAT5 to gamma interferon activated sites (GAS) that mediate JAK2 downstream signaling. Furthermore, we found that KMKKT significantly enhanced the growth rate of colony-forming unit granulocyte erythrocyte monocyte macrophages (CFU-GEMM) and burst forming unit erythroid (BFU-E) of mouse HSCs (mHSCs) stimulated by IL-3/EPO. Overall, our results demonstrated that KMKKT alleviated drug-induced side effects through enhanced hematopoiesis, at least in part through cytokine-mediated JAK2/STAT5 signaling.</P>
임신 중기의 모체 혈청 Alpha-Fetoprotein치에 대한 연구

비정상적으로 높거나 낮은 임신 중기의 모체 혈청 alpha-fetoprotein치는 태아의 염색체 이상이나 신경판 결손 등과 관련이 있는 것으로 알려져 왔다. 한국 정상 임부에서 임신 중기 모체 혈청 alpha-fetoprotein의 정상치를 구하는데 도움이 되고 본원 고유의 정상 범위 설정을 위해 임신 중기의 각 임신 주수별 모체혈청 alpha-fetoprotein치의 중앙값 및 0.4, 2.5, 3.5MOM을 사용한 상한치와 하한치를 측정하였다. 본 연구에서는 3.5MOM이보다 높은 모체 혈청 alpha-fetoprotein치가 신경관 결손, 복벽 결손, 낭포성 하이그로마 같은 기형아와 관련이 있는 것으로 나타났다. Abnormally high and low levels of material serum alpha-fetoprotein(MSAFP) in the second trimester of pregnancy have been associated with various poor pregnancy outcomes such as neural tube defects and chromosomal abnormalities. A tonal of 659 patients participated in a maternal serum alpha-fetoprotein screening program in Inje University, Sanggye Paik Hospital between July 1, 1991 and December 31, 1992. The authors determined the median value of MSAFP on each gestational week of midpregnancy and established the cutoff value using 0.4, 2.5, 3.5 multiples of the median(MOM) for the determinations of normal range in Korean mother. In this study, MSAFP values higher than 3.5 multiples of the median(MOM) were associated with poor pregnancy outcomes such as neural tube defects, abdominal wall defects, and cystic hygroma.
