A 72-year-old female with a colonic laterally spreading tumor (LST) was referred to our department. A total colonoscopy revealed a large nongranular LST, 30 mm in diameter, in the ascending colon. Detailed examination with chromoendoscopy confirmed that the lesion was an intramucosal tumor, and endoscopic submucosal dissection (ESD) was performed. After a circumferential incision around the lifted lesion with a submucosal fluid cushion, diffuse adipose tissue was observed in the submucosal layer beneath the lesion. The endoscopic view was blurred when dissecting the submucosal layer due to fat adhering to the lens. Since this made it difficult to continue the procedures, we infused water into the lumen and kept the endoscope tip immersed in the collected water. The resulting improved view made it possible to complete all procedures without withdrawing the endoscope to wipe the lens. The lesion was successfully resected en bloc without complications. The pathological examination indicated the curative resection of a tubulovillous adenoma. We propose that a submerged ESD could also be an effective procedure for colonic neoplasms with submucosal fat by avoiding blurring of the endoscopic view. (Gut and Liver 2008;2:209-212)
MARCH-I: a New Regulator of Dendritic Cell Function
Satoshi Ishido,Yohei Matsuki,Eiji Goto,Mizuho Kajikawa,Mari Ohmura-Hoshino 한국분자세포생물학회 2010 Molecules and cells Vol.29 No.3
We and other groups have demonstrated that the expres-sion level of MHC class II (MHC II) is regulated through ubiquitination of the MHC II chain. We also reported that MARCH-I, an E3 ubiquitin ligase, is critical for this process. At present, however, the importance of MARCH-I-mediated MHC II regulation in vivo is still unknown. In this review, we will summarize recent advances in our understanding of MARCH-I-mediated MHC II ubiquitination, and discuss how we can overcome the difficulties inherent in these studies.
Short-Term Healing Process of Artificial Ulcers after Gastric Endoscopic Submucosal Dissection

Background/Aims: The relationship between the appearance of an ulcer and postoperative bleeding after gastric endoscopic submucosal dissection (ESD) is not well understood. To explore this potential relationship, we retrospectively analyzed the short-term healing process of ESD. Methods: A total of 520 consecutive lesions in 434 patients seen between January 2004 and December 2009 were retrospectively investigated. At the second-look endoscopy, which occurred between 1 and 8 days after ESD, artificial ulcers were categorized into 6 patterns according to Forrest`s classification: spurting bleeding, oozing bleeding, non-bleeding visible vessel, adherent clot, black base/spot, and clean base. From these data, a short-term healing model of the artificial ulcer was generated. Results: Ulcers were observed to change gradually from a bloody to a clean surface. The bleeding or non-bleeding visible vessel categories, which occurred in approximately one quarter of the ulcers within 3 days of ESD, were rarely observed 4 days after ESD. Conclusions: Ulcers that occur after gastric ESD heal in line with a specific time course, and it appears that most healing occurs without massive bleeding. (Gut Liver 2011;5:293-297)

We and other groups have demonstrated that the expression level of MHC class II (MHC II) is regulated through ubiquitination of the MHC II ${\beta}$ chain.

Purpose: Few reports have been made on the therapeutic effects as well as pathological features of an antithrombin preparation in patients diagnosed with septic disseminated intravascular coagulation (DIC) by the diagnostic criteria for acute DIC. Materials and Methods: A total of 88 sepsis patients who had received inpatient hospital care during the period from January 2000 through December 2008 were divided into two groups, an antithrombin group and a non-antithrombin group, to study the outcomes. Furthermore, the relationship between sepsis-related factors and DIC in 44 patients was studied. Results: The antithrombin group contained 34 patients, and the non-antithrombin group contained 54 patients. The outcomes were significantly better in the antithrombin group. The levels of protein C were low in DIC patients. Conclusion: Our results suggest that early administration of antithrombin might improve outcomes of septic DIC patients in the diagnostic criteria for Japanese Association for Acute Medicine acute DIC.

In this papar, we have examined reproduction of waveform and stability that needs to be cleared in order to introduce the delta-sigma modulator into the multi-channel measurement systems. Our study has proved that the delta-sigma modulator can be applied to the multi-channel measurement system.

Purpose: It has been reported that the Pulse Contour Cardiac Output (PiCCO) is very useful mainly in the field of intensive care and treatment to grasp the pathophysiologicalconditions of pulmonary edema because of its capability of obtainingdata such as Pulmonary Vascular Permeability Index (PVPI) and Extra VascularLung Water (EVLW). Furthermore, a high degree of usability of various markers has been reported for better understanding of the pathological conditions in cases with septicemia. Materials and Methods: The correlation between the cardiorespiratory status based upon the PiCCO monitor (EVLW and PVPI) and inflammatorymarkers including C reactive protein, procalcitonin (PC), and EndotoxinActivity Assay (EAA) were evaluated in 11 severe cases that required treatmentwith a respirator in an intensive care unit. Results: The EAA values were significantly higher in patients with abnormal EVLW at 0.46±0.20 compared to the normal EVLW group at 0.21±0.19 (p=0.0064). In a similar fashion, patients with abnormal PVPI values tended to have higher PC levels at 18.9±21.8 comparedto normal PVPI cases at 2.4±2.2 (p=0.0676). On the other hand, PVPI was significantly higher in the abnormal EAA group at 3.55±0.48 in comparison with the normal EAA group at 1.99±0.68 (p=0.0029). The abnormal EAA group tended to have higher PVPI values than the normal EAA group. Conclusion: The EAA is a measurement method designed to estimate the activity of endotoxins in the whole blood. Our results suggest that the EAA value, which had the greatest correlationwith lung disorders diagnosed by the PiCCO monitoring, reflects inflammatoryreactions predominantly in the lungs.
HIGH EFFICIENCY ARCHITECTURE FOR DVB-S2 BASED LDPC DECODER
Xiongxin ZHAO,Zhixiang CHEN,Satoshi Goto 대한전자공학회 2009 ITC-CSCC :International Technical Conference on Ci Vol.2009 No.7
The proposed architecture is based on the structured quasi-cyclic (QC)-LDPC codes. The main contribution of this work is rearrangement for the original PCMs in DVB-S2 standard to QC type and utilize the property of QC codes structure achieving variable code lengths (64800 for normal and 16200 for short) and multiple code rates (1/4; 1/3; 2/5; 1/2; 3/5; 2/3; 3/4; 4/5; 5/6; 8/9; 9/10) for DVB-S2 based LDPC decoder architecture flexibly.
