We developed the educational purpose mobile application, named "Wild Flowers of Bukhansan National Park (version 1.0)", aiming for easy identification of wild flowers for students and visitors in the park. When visitors find a flower or part of plant in the park, visitors can search for its name utilizing the pictures and characters provided in their own smartphone mobile devices or tablet PCs. The application provides pictures of wild flowers in the park and character-based searching system based on 12 diagnostic features (e.g., growth form, leaf arrangement, flower symmetry, petal color, petal number, sepal number, etc). We adopted the complete floristic survey of Chung and Lee (1962) and added species that we confirmed their distribution in the park during the development of this application. In summary, number of vascular plants in this park was estimated to be 428 taxa including 100 families, 280 genera, 327 species, 1 subspecies, 50 varieties, and 5 formas. We provided a total of 588 pictures representing 358 taxa and each taxon includes multiple pictures in many cases. Included identification quizzes can be an efficient educational tool as well as fun activity for students and visitors who are learning plant species in Korea. Our next step will include GPS function in the application for indicating visitor's location and for providing previously reported sites of the species that we interested in the map of the park. The future application which includes GPS function will be a valuable tool for the monitoring of rare plants, plant researches related to the climate changes, etc. We currently provide Korean iPhone version only, and English version and both of android versions will be serviced soon.

Background: Hemangioma is the most common benign hepatic tumor. Many hepatic hemangiomas tend to be found incidentally, but should be differentiated from malignant tumors. We report an interesting case of hepatic hemangioma in patient who has high risks of hepatocellular carcinoma. Case: A 52-year-old woman, who was a chronic alcoholic abuser and hepatitis C virus carrier with no treatment experience, was admitted due to diarrhea. The patient had suffered from intermittent abdominal pain in the right subcostal area for recent 3 months. Also the patient had easy bruising and frequent epistaxis. She had used oral contraceptives for almost 20 years. Systolic blood pressure was 160 mmHg and diastolic blood pressure was 80 mmHg. Body temperature was normal. Physical examination revealed icteric sclerae and tenderness in right upper quadrant of the abdomen. Laboratory study showed platelet count of 31,000/ul, AST/ALT of 302/127 IU/L, and total bilirubin of 2.7 mg/dl. 3-phased contrast-enhanced abdominal computed tomography (CT) revealed a 4 cm × 4 cm sized hepatic mass involving both segment 5 and 6. Abdominal CT finding suggested hepatic hemangioma, however, typical finding of hemagioma was unclear, therefore, could not rule out the malignancy. Since she had risk factors for hepatocellular carcinoma, abdominal ultrasonography was performed for further evaluation. But abdominal ultrasonography also showed atypical finding. The confirmative diagnosis could be done by dynamic magnetic resonance imaging (MRI) using primovist®, which is an innovative liver cell-specific contrast medium. Dynamic MRI presented peripheral globular enhancement and a centripetal fill-in pattern, confirmatory findings of hepatic hemangioma, on the delayed image. The patient got relief from abdominal pain and diarrhea during admission, now is being followed-up at a general out-patient clinics. Conclusions: Hemangiomas with atypical abdominal CT and ultrasonographic findings in patients with high risks of hepatocellular carcinoma can be easily characterized with primovist-enhanced dynamic MRI.
The Longitudinal Trend of Cardiac Surgery in Korea from 2003 to 2013

Background: The purpose of this study was to investigate longitudinal changes of the utilization of operational and surgical medical care inside and outside a metropolitan area over 10 years, analyzing the residential areas of patients and the locations of medical facilities for major cardiovascular surgery. Methods: Data analysis was conducted by classifying the addresses of patients and the locations of medical care facilities of metropolitan cities and provinces, using data from the National Health Insurance Corporation from January 2003 to December 2013. Results: There is serious concentration of major heart surgery to medical facilities in Seoul; this problem has not improved over time. There were differences in percentages of surgical procedures performed in the metropolitan areas according to major diseases. In the case of Busan and Daegu provinces, at least 50% of the patients underwent surgery in medical facilities in the city, but there are other regions where the percentage is less than 50%. In the case of provinces, the percentage of surgical procedures performed in medical facilities in Seoul or nearby metropolitan cities is very high. Conclusion: Policies to strengthen the regional capabilities of heart surgery and to secure human resources are required to mitigate the concentration of patients in the capital area. Many regional multi-centers must be designated to minimize unnecessary competition among regional university hospitals and activate a win-win partnership model for medical services.

<P>Ischemic preconditioning (IPC) is a condition of sublethal transient global ischemia and exhibits neuroprotective effects against subsequent lethal ischemic insult. We, in this study, examined the neuroprotective effects of IPC and its effects on immunoreactive changes of antioxidant enzymes including superoxide dismutase (SOD) 1 and SOD2, catalase (CAT) and glutathione peroxidase (GPX) in the gerbil hippocampal CA1 region after transient forebrain ischemia. Pyramidal neurons of the stratum pyramidale (SP) in the hippocampal CA1 region of animals died 5 days after lethal transient ischemia without IPC (8.6% (ratio of remanent neurons) of the sham-operated group); however, IPC prevented the pyramidal neurons from subsequent lethal ischemic injury (92.3% (ratio of remanent neurons) of the sham-operated group). SOD1, SOD2, CAT and GPX immunoreactivities in the sham-operated animals were easily detected in pyramidal neurons in the stratum pyramidale (SP) of the hippocampal CA1 region, while all of these immunoreactivities were rarely detected in the stratum pyramidale at 5 days after lethal transient ischemia without IPC. Meanwhile, their immunoreactivities in the sham-operated animals with IPC were similar to (SOD1, SOD2 and CAT) or higher (GPX) than those in the sham-operated animals without IPC. Furthermore, their immunoreactivities in the stratum pyramidale of the ischemia-operated animals with IPC were steadily maintained after lethal ischemia/reperfusion. Results of western blot analysis for SOD1, SOD2, CAT and GPX were similar to immunohistochemical data. In conclusion, IPC maintained or increased the expression of antioxidant enzymes in the stratum pyramidale of the hippocampal CA1 region after subsequent lethal transient forebrain ischemia and IPC exhibited neuroprotective effects in the hippocampal CA1 region against transient forebrain ischemia.</P>

Kaempferol, one of the phytoestrogens, is found in berries and Brassica and Allium species and is known to have antioxidative and anti-inflammatory properties. In the present study, we examined the molecular mechanisms underlying the anti-inflammation effect of kaempferol in an aged animal model. To examine the effect of kaempferol in aged Sprague-Dawley rats, kaempferol was fed at 2 or 4 mg/kg/day for 10 days. The data show that kaempferol exhibited the ability to maintain redox balance. Kaempferol suppressed nuclear factor-${\kappa}B$ (NF-${\kappa}B$) activation and expression of its target genes cyclooxygenase-2, inducible nitric oxide synthase, monocyte chemoattractant protein-1, and regulated upon activation, and normal T-cell expressed and secreted in aged rat kidney and in tert-butylhydroperoxide-induced YPEN-1 cells. Furthermore, kaempferol suppressed the increase of the pro-inflammatory NF-${\kappa}B$ cascade through modulation of nuclear factor-inducing kinase (NIK)/$I{\kappa}B$ kinase (IKK) and mitogen-activated protein kinases (MAPKs) in aged rat kidney. Based on these results, we concluded that anti-oxidative kaempferol suppressed the activation of inflammatory NF-${\kappa}B$ transcription factor through NIK/IKK and MAPKs in aged rat kidney.

Deutzia paniculata Nakai, a rare and endemic plant, has limited distribution throughout the North and South Gyeongsang provinces of South Korea. The D. paniculata community grows mostly on the stony slopes of forests, valley edges, and rock layers at 250-960 m in altitude, where deciduous trees are dominant and high humidity is maintained. Correlation analysis of vegetation and environmental factors found that the Walter's dogwood-mulberry community was correlated with soil acidity (pH). Whilst the queritron community had correlations with distance from the valley, rock rate and slope. The natural habitat of the Palgongsan Mountain in Daegu is known to have high genetic diversity, had eight D. paniculata individuals recorded from 2014 to 2018, and 12 individuals recorded in 2020 (new individuals due to a newly created space within the herbaceous layer caused by grass mowing works), it is therefore unlikely that the community would perish unless there was an artificial disturbance. To conserve the natural habitats of D. paniculata, oppression by Sasa borealis, damage, increase in crown density of the upper layer, overexploitation, and absence of seedlings should be carefully investigated. In addition, response measures should also be established and the impact on seed fullness and the reproductive characteristics of D. paniculata recorded. To restore declined genetic diversity, individuals from high genetic diversity regions, such as Palgongsan Mountain, should be artificially transplanted.
Results of Decompressive Craniectomy in Severe Head Injury

1985년 4월부터 1990년 6월까지 순천향대학 천안병원 신경외과에 입원하여 감압 두개절제술을 받은 환자 63례를 대상으로하여 임상적 소견과 방사선학적 소견 그리고 결과를 소급 조사 분석하였다. 대상환자는 모두 수술당시 혼수상태(글라스고우 혼수계수 : 3∼8)였다. 남녀비는 42 : 21이었고 평균 연령은 43세(7세∼80세)였다. 양측 전두 감압술을 시행한 경우가 15례(23.8%), 전두측두두정 감압술을 시행한 경우가 48례(76.2%)였다. 내원당시 동공 상태는 41례(65.1%)가 비정상이었으나 수술전에는 60례(95.2%)가 비정상이었다. 수술후 30일 이내 사망(수술사망율)이 49례(77.8%)였고 2례는 30일 이후 사망하여 전체적인 사망률은 81.0%였다. 생존한 12례중 양호한 예후(회복과 중등도 장애)는 8례(12.7%)뿐이었다. 수술방법은 예후에 영향을 주지 않았다. 원 병소중 가장 흔한 병소는 급성 경막하혈종으로 35례(55.6%)에서 발견되었다. 두개골 골절은 48례(76.2%)에서 관찰되었다. 수술후 CT는 18례(28.6%)에서 시행하였다. 이들 중 14례(77.8%)에서 중등도·중증의 부종이 관찰되었고 13례(72.2%)에서는 지연성 병소가 발견되었다. 특히 9례(50.0%)에서는 수술후 정중편위가 수술전 보다 더 심해졌다. 이러한 결과로 보아 감압 두개절제술은 사망율과 이환율이 높고 악영향을 줄 가능성이 있기 때문에 수술방법은 아니라고 생각한다. We present a retrospective study on 63 patients who underwent decompressive craniectomy from April 1985 to June 1990. All patients were comatose(Glasgow Coma Score ; 3-8) at the time of operation. There were 42 males and 21 females with an age range of 7 to 80 years(mean age, 42 years). Bifrontal craniectomy was performed in 15 patients(23.8%) and frontotemporoparietal craniectomy was done in 48 patients(76.2%). Pupils were abnormal in 41 patients(65.1%) on arrival. However, those became abnormal in 60 patients(95.2%) preoperatively. Fourty nine of the 63 patients died within the first 30 days of surgery (operative mortality, 77.8%). Late mortality was 3.2%, making the total mortality for this series 81.0%. Only eight patients(12.7%) could have favorable outcome(good recovery and moderate disability). The operative method did not affect on the prognosis. The most common primary mass lesion was acute subdural hematoma(35 patients, 55.6%). Skull fracture was found in 48 patients(76.2%). Postoperative CT scaning were obtained in 18 patients(28.6%). Moderate to marked degree of edema was found in 14 patients(77.8%). Delayed lesions were found in 13 patients(72.2%). Intracerebral hematoma was the most common delayed lesion. In nine patients(50.0%). the degree of midline shift was more severe than the preoperative CT scans. These results stroly suggested that the WDCE was not an effective surgical method due to its high mortality, morbidity and possible harmful effect such as enhanced edema formation and hastened delayed lesions.
