        Risk factors of medication-related osteonecrosis of the jaw: a retrospective study in a Turkish subpopulation

        Objectives: Medication-related osteonecrosis of the jaw (MRONJ) is a well-known side effect of certain drugs that are used to influence bone metabolism to treat osteometabolic disease or cancers. The purpose of our study was to investigate how high-concentration and low-concentration bisphosphonate (BP) intake affects the disease severity. Materials and Methods: Data collected from the medical records of 52 patients treated with BPs, antiresorptive, antiangiogenic drugs and diagnosed with MRONJ were included in this study. Age, sex, type of systemic disease, type of drug, duration of drug treatment, jaw area with MRONJ, drug administration protocol, and MRONJ clinical and radiological findings were obtained. Patients were divided into two groups: anti-neoplastic (Group I, n=23) and anti-osteoporotic (Group II, n=29). Statistical evaluations were performed using the IBM SPSS ver. 21.0 program. Results: In both groups, more females had MRONJ. MRONJ was found in the mandibles of 30 patients (Group I, n=14; Group II, n=16). When we classified patients according to the American Association of Oral and Maxillofacial Surgeons staging system, significant differences were seen between groups (χ 2 =12.23, P<0.01). More patients with advanced stage (stage 2-3) MRONJ were found in Group I (60.9%). Conclusion: According to our results, high-concentration BP intake, age and duration of drug intake increased disease severity.

        Serum Levels of 8-Iso-Prostaglandin F2α and Raftlin in Patients with Generalized Anxiety Disorder

        Objective: Although there are neurobiological studies of patients with generalized anxiety disorder (GAD), the topic is still open to research. Lipid peroxidation can generate new molecular signal sequences by altering protein amounts and activity. 8-Iso-Prostaglandin F2α (8-iso-PGF2α) is known to be an important lipid peroxidation marker. Raftlin, which is defined as a major lipid raft protein, is important for the regulation of signal transduction and inflammatory processes. Methods: Our aim in this study was to compare the 8-iso-PGF2α and Raftlin levels of forty patients diagnosed with GAD and 40 healthy controls (age-sex and body mass index-matched). Results: In the present study, increased serum 8-iso-PGF2α and Raftlin levels were found in patients with GAD compared to healthy controls. Conclusion: To our knowledge, this is the first study to examine 8-iso-PGF2α and Raftlin levels in patients with GAD. These results expand our knowledge of oxidative stress and inflammatory processes in patients with GAD. Our study should be considered preliminary and further studies should be performed with larger sample groups comparing values before and after treatment.

      • Comparison between Radiological and Invasive Diagnostic Modalities in Diagnosis of Breast Cancer

        Background: Breast cancer is the most common cause of deaths of cancer in women. Nowadays, following completion of imaging methods, mainly fine needle aspiration biopsy (FNAB) and core biopsy methods have been used for establishing cytopathological diagnosis although discussions regarding superiority continue. Materials and Methods: Those with a complaint of "mass in breast" along with those diagnosed to have a mass as a result of routine physical examination among all patients applying to our clinic between 01.01.2009 and 31.12.2011 were retrospectively assessed. Totals of 146 and 64 patients with complete radiological observation who had undergone FNAB and core biopsies, respectively, were evaluated. Postoperative pathological results of patients of both groups receiving surgery were also taken into consideration. All results were compared in terms of false positivity/negativity, sensitivity/specifity, surgery types and distribution of postoperative results with regard to diagnoses along with those of malignant/benign masses with regard to quadrants determined. Results: Diagnostic malignancy power of mammographic BIRADS classification was 87.3%. However, the value was 75% in the core biopsy group. Sensitivity and specifity following comparison of FNAB and postoperative pathology results of those receiving surgery were 85.4% and 92.9% while they were 93.5% and 100% in the core biopsy group. Diagnostic malignancy power, calculated by determining AUC in ROC analysis, of FNAB was 89.1% while that of core biopsy was 96.7%. Conclusions: It was shown that core biopsy is superior to FNAB in terms of sensitivity, specificity and accurate histopathological classification. However; quick, cheap and basic diagnosis by means of FNAB should not be ignored. Sensitivity of FNAB is rather high in experienced hands and furthermore it would be expected to be lower than with core biopsy.

        Series tuned mass dampers in vibration control of continuous railway bridges

        This paper presents the applicability of series tuned mass dampers (STMDs) to reduce the multiple resonant responses of continuous railway bridges under high-speed train. The bridge is modeled by two-span Bernoulli-Euler beam with uniform cross-section, and a STMD device consisting of two TMD units installed on the bridge to reduce its multiple resonant vibrations. The system is assumed to be under the action of a high-speed train passage which is modeled as a series of moving forces. Sequential Programming Technique (SQP) is carried out to find the optimal parameters of the STMD that minimizes the maximum peak responses of the bridge. Comparisons with the results available in the literature are presented to demonstrate the effectiveness and robustness of STMD system in reducing the multiple resonant responses of the continuous railway bridges under high-speed trains.

        Effects of Cognitive Behavioral Therapy, Existential Psychotherapy and Supportive Counselling on Facial Emotion Recognition Among Patients with Mild or Moderate Depression

        Objective This study compared the effects of cognitive behavioral therapy (CBT), existential psychotherapy (ExP) and supportive counseling (SUP) on facial emotion recognition among mildly and moderately depressed patients. Methods 21 patients for CBT, and 20 each for ExP and SUP groups with 60 healthy controls were investigated. Eight consecutive weekly sessions and following two monthly boosters were performed. Prior to the sessions, all subjects received Sociodemographic Data Form, the Structured Clinical Interview for DSM-IV Axis I Disorders (SCID-1), and Facial Emotion Recognition Test (FERT). Patients received Hamilton Depression Rating Scale (HDRS) and FERT at the onset and after weekly and booster sessions. Results Patients’ ability to recognize surprised and neutral emotions were lower than controls. ExP group improved recognition of almost all emotions, CBT group improved only happy emotions and SUP group did not improve any emotions. HDRS scores declined in all patient groups, ExP and CBT groups had lower scores than SUP. Conclusion MDD patients recognized surprised and neutral emotions lower than controls. ExP improved ability to recognize almost all emotions, CBT improved only happy emotions, SUP did not improve at all. ExP, CBT and SUP all led to a reduction in MDD. ExP and CBT had comparable effects and both were more helpful than SUP.

        The Evaluation of Exogenous Melatonin Administration in Supraspinatus Overuse Tendinopathy in an Experimental Rat Model

        Background: Increased oxidative stress and inflammation play a critical role in the etiopathogenesis of chronic tendinopathy. Melatonin is an endogenous molecule that exhibits antioxidant and anti-inflammatory activity. The aim of this study was to evaluate the biochemi-cal and histopathological effects of exogenous melatonin administrations in supraspinatus overuse tendinopathy.Methods: Fifty rats were divided into the following four groups: cage activity, melatonin treatment, corticosteriod therapy, and control. Melatonin (10 mg/kg, intraperitoneal; twice a day) and triamcinolone (0.3 mg/kg, subacromial; weekly) were administered to the treat-ment groups after the overuse period. Biochemical and histopathological evaluations were performed on serum samples and biopsies obtained from rats. Plasma inducible nitric oxide synthase (iNOS), vascular endothelial growth factor (VEGF), total antioxidant status (TAS), total oxidant status (TOS), and oxidative stress index (OSI) levels were evaluated biochemically. Results: The TAS, TOS, OSI, iNOS, and VEGF values were significantly lower than the pre-treatment levels in rats receiving exogenous melatonin treatment (3 or 6 weeks) (p<0.05). TOS, iNOS, VEGF, and OSI values after 3 weeks of triamcinolone administration, and TOS, VEGF, and OSI levels after 6 weeks of triamcinolone application, were significantly lower than the pre-treatment levels (p<0.05). Conclusions: Exogenous melatonin application in overuse tendinopathy reduces oxidative stress and inflammation. Melatonin might be an alternative potential molecule to corticosteroids in the treatment of chronic tendinopathy.

        A Methodology for Optimal Layout Design of Pressure Cells for Concrete Faced Rockfill Dams

        In this study, a methodology for optimal layout design of pressure cells for concrete faced rockfill dams is developed. Arepresentative dimensionless stress distribution model was formed for obtaining the magnitudes and location of different stress zonesas a function of dam height. This information enabled development of a procedure for proper location and the number of pressurecells throughout the dam body. A vertical placement algorithm based on error minimization was first developed, which is followedby an approach to find the number and location of pressure cells on a particular elevation of the dam body. The effects of face slabcracking and earthquake are interpreted. Furthermore, the performance of the proposed model was also tested when instruments areinstalled to different elevations in the dam body than those recommended by the model developed. The proposed optimizationscheme provides a basis for economical layout design of pressure cells with sufficient information allowing realistic assessment ofthe structural behavior. The application of the proposed model is illustrated for some existing dams. It is observed that this algorithmgives satisfactory results.

        Energy-based design base shear for RC frames considering global failure mechanism and reduced hysteretic behavior

        A nonlinear static procedure considering work-energy principle and global failure mechanism to estimate base shears of reinforced concrete (RC) frame-type structures is presented. The relative energy equation comprising of elastic vibrational energy, plastic strain energy and seismic input energy is obtained. The input energy is modified with a factor depending on damping ratio and ductility, and the energy that contributes to damage is obtained. The plastic energy is decreased with a factor to consider the reduced hysteretic behavior of RC members. Given the pre-selected failure mechanism, the modified energy balance equality is written using various approximations for modification factors of input energy and plastic energy in scientific literature. External work done by the design lateral forces distributed to story levels in accordance with Turkish Seismic Design Code is calculated considering the target plastic drift. Equating the plastic energy obtained from energy balance to external work done by the equivalent inertia forces considering, a total of 16 energy-based base shears for each frame are derived considering different combinations of modification factors. Ductility related parameters of modification factors are determined from pushover analysis. Relative input energy of multi degree of freedom (MDOF) system is approximated by using the modal-energy-decomposition approach. Energy-based design base shears are compared with those obtained from nonlinear time history (NLTH) analysis using recorded accelerograms. It is found that some of the energy-based base shears are in reasonable agreement with the mean base shear obtained from NLTH analysis.
