        Estimating Cognitive Load in a Mobile Personal Health Record Application: A Cognitive Task Analysis Approach

        Objectives: Mobile health applications that are designed without considering usability criteria can lead to cognitive overload,resulting in the rejection of these apps. To avoid this problem, the user interface of mobile health applications should be evaluatedfor cognitive load. This evaluation can contribute to the improvement of the user interface and help prevent cognitiveoverload for the user. Methods: In this study, we evaluated a mobile personal health records application using the cognitivetask analysis method, specifically the goals, operators, methods, and selection rules (GOMS) approach, along with the relatedupdated GOMS model and gesture-level model techniques. The GOMS method allowed us to determine the steps of the tasksand categorize them as physical or cognitive tasks. We then estimated the completion times of these tasks using the updatedGOMS model and gesture-level model. Results: All 10 identified tasks were split into 398 steps consisting of mental andphysical operators. The time to complete all the tasks was 5.70 minutes and 5.45 minutes according to the updated GOMSmodel and gesture-level model, respectively. Mental operators covered 73% of the total fulfillment time of the tasks accordingto the updated GOMS model and 76% according to the gesture-level model. The inter-rater reliability analysis yielded anaverage of 0.80, indicating good reliability for the evaluation method. Conclusions: The majority of the task execution timescomprised mental operators, suggesting that the cognitive load on users is high. To enhance the application's implementation,the number of mental operators should be reduced.

        Familial Actinic Lichen Planus: Three Cases from the Same Family

        Actinic lichen planus (ALP) that affects sun-exposed areas of the skin is an uncommon variantof lichen planus. While ALP is commonly triggered by ultraviolet radiation exposure,genetic predisposition may also be important in the pathogenesis of the disease. Herein, wereport three patients with ALP from the same family, which supports the genetic etiopathogeneticfactors of ALP.

        Evaluation and follow-up of pain, fatigue, and quality of life in COVID-19 patients

        Objectives: We evaluated pain, fatigue, anxiety, depression, and quality of life in patients hospitalized for coronavirus disease 2019 (COVID-19) and observed them over a period of 3 months. We also investigated the relationship of these symptoms to age, sex, disease severity, and levels of anxiety and depression. Methods: The study included 100 confirmed COVID-19 patients (i.e., positive on a polymerase chain reaction test) between the ages of 18 and 75 years. Pain (visual analog scale [VAS]), fatigue (fatigue severity scale), anxiety, and depression (hospital anxiety and depression scales) were evaluated on the first day of hospitalization and at 1-month and 3-month follow-ups. The short form-12 questionnaire was used to measure quality of life at the 1-month and 3-month follow-ups. Results: No differences were found in pain, fatigue, anxiety levels, depression levels, and quality of life according to disease severity. High VAS scores at hospital admission were related to continued pain at the 3-month follow-up (odds ratio [OR], 1.067; p<0.001). High VAS (OR, 1.072; p=0.003) and anxiety levels (OR, 1.360; p=0.007) were related to severe fatigue at the 3-month evaluation. Conclusion: Pain, fatigue, anxiety, and depression appear to be long-term sequelae of COVID-19 and can affect quality of life. High VAS and anxiety levels were found to be associated with long-term fatigue.

        An Analysis of Myocardial Efficiency in Patients with Severe Asymptomatic Mitral Regurgitation

        BACKGROUND: It is difficult to determine left ventricular systolic performance in patients with severe mitral regurgitation (MR) since left ventricular ejection fraction (EF) could be preserved until the end stages of the disease. Myocardial efficiency (MEf) describes the amount of external work (EW) done by the left ventricle per unit of oxygen consumed (mVO2). In the present study, we aimed to investigate MEf in patients with asymptomatic severe MR using a novel echocardiographic method. METHODS: A total of 27 patients with severe asymptomatic MR and 26 healthy volunteers were included in this cross-sectional study. EW was measured using stroke volume and blood pressure, while mVO2 was estimated using double product and left ventricular mass. RESULTS: There were no differences between the groups with regards to EF (66% ± 5% vs. 69% ± 7%), while MEf was significantly reduced in patients with severe MR (25% ± 11% vs. 44% ± 12%, p < 0.001). This difference was maintained even after adjustment for age, gender and body surface area (adjusted ̅ : 0.44, 95% CI: 0.39–0.49 for controls and adjusted ̅ : 0.24, 95% CI: 0.19–0.29 for patients with severe MR). Further analysis showed that this reduction was due to an increase in total mVO2 in the severe MR group. MEf of thepatients who were both on β-blockers and angiotensin converting enzyme inhibitors/angiotensin receptor blockers were higher than those who were not on any drugs, but this difference was not statistically significant (32% ± 15% vs. 23% ± 9%, p = 0.41). CONCLUSIONS: MEf was significantly lower in patients with asymptomatic severe MR and preserved EF.

      • Conformation changes of polysilanes during the polaron formation

        Quantum mechanical studies of polaron formation in oligo[methyl(phenyl)silane] (OMPSi) was performed. The conformationsof the neutral, positively and negatively charged OMPSi were optimized by means of B3LYP method. For the evaluation of thesubstituent eects, the conformation of the unsubstituted oligosilane chain was also calculated. The negative polaron conformationshows a signicant stretching of the chain, whereas the positive polaron conformation is shrunk. This fact is associated with changesof electron density distribution. There is also a strong inuence of thep-conjugated substituent on the conformation of the negativepolaron. On the other hand, the inuence of the substituents on the conformation of the positive polaron is small. The absorptionspectra of the defect-free and polaron oligomers are discussed.. 2002 Elsevier Science B.V. All rights reserved.

        Whole body ultrasound in the operating room and intensive care unit

        Whole body ultrasound can be used to improve the speed and accuracy of evaluation of an increasing number of organ systems in the critically ill. Cardiac and abdominal ultrasound can be used to identify the mechanisms and etiology of hemodynamic instability. In hypoxemia or hypercarbia, lung ultrasound can rapidly identify the etiology of the condition with an accuracy that is equivalent to that of computed tomography. For encephalopathy, ocular ultrasound and transcranial Doppler can identify elevated intracranial pressure and midline shift. Renal and bladder ultrasound can identify the mechanisms and etiology of renal failure. Ultrasound can also improve the accuracy and safety of percutaneous procedures and should be currently used routinely for central vein catheterization and percutaneous tracheostomy.

        Confocal imaging of biomarkers at a single-cell resolution: quantifying 'living' in 3D-printable engineered living material based on Pluronic F-127 and yeast Saccharomyces cerevisiae

        Engineered living materials (ELMs) combine living cells with non-living scaffolds to obtain life-like characteristics, such as biosensing, growth, and self-repair. Some ELMs can be 3D-printed and are called bioinks, and their scaffolds are mostly hydrogel-based. One such scaffold is polymer Pluronic F127, a liquid at 4 °C but a biocompatible hydrogel at room temperature. In such thermally-reversible hydrogel, the microorganism-hydrogel interactions remain uncharacterized, making truly durable 3D-bioprinted ELMs elusive.We demonstrate the methodology to assess cell-scaffold interactions by characterizing intact alive yeast cells in cross-linked F127-based hydrogels, using genetically encoded ratiometric biosensors to measure intracellular ATP and cytosolic pH at a single-cell level through confocal imaging.When embedded in hydrogel, cells were ATP-rich, in exponential or stationary phase, and assembled into microcolonies, which sometimes merged into larger superstructures. The hydrogels supported (micro)aerobic conditions and induced a nutrient gradient that limited microcolony size. External compounds could diffuse at least 2.7 mm into the hydrogels, although for optimal yeast growth bioprinted structures should be thinner than 0.6 mm. Moreover, the hydrogels could carry whole-cell copper biosensors, shielding them from contaminations and providing them with nutrients.F127-based hydrogels are promising scaffolds for 3D-bioprinted ELMs, supporting a heterogeneous cell population primarily shaped by nutrient availability.
