        A “Radial Ready” Tricoaxial Setup for Anterior Circulation Mechanical Thrombectomy: Technical Aspects and Preliminary Results

        Purpose: Mechanical thrombectomy (MT) is the standard of care for acute ischemic stroke (AIS) due to large vessel occlusion (LVO). The choice of a transradial approach (TRA) for anterior circulation LVOs is still debatable; the use of a specific tricoaxial system could help mitigate numerous issues related to transradial MT.Materials and Methods: From November 2022 to November 2023, 22 patients underwent TRA-MT for anterior circulation LVOs, both as first-line and rescue from transfemoral approach (TFA) failure, with the same triaxial setup consisting of a 7F introducer sheath, 7F guide catheter, and aspiration catheters ranging from 5.5F to 5F in relation to the occlusion site. Choice of thrombectomy technique was at operator discretion. Patients’ demographic data, clinical presentation, treatment details, complications, rate of crossover to TFA, successful revascularization (modified thrombolysis in cerebral infarction [mTICI] score ≥2b), and good clinical outcome at 3 months (modified Rankin scale [mRS] 0-2) were reported.Results: Of 20 patients selected, 10 (50%) had occlusion of M1 segment of middle cerebral artery (MCA), 6 (30%) of internal carotid artery (ICA) terminus, and 4 (20%) with M2 MCA occlusions; 12/20 (60%) were right-sided occlusions and 8/20 (40%) were left-sided. The mean National Institutes of Health Stroke Scale score was 9.25 at admission. Successful revascularization to mTICI 2b-3 was achieved in 18/20 patients (90%). Intracranial complications were reported in 2 (10%) patients. Rate of radial artery occlusion at 24 hours was 10,6%; no access-site haemorrhagic complications were reported. Symptomatic intracranial hemorrhage occurred in 2 (10%) patients. mRS score 0-2 at 3 months was 50%.Conclusion: The high technical effectiveness and good safety profile of this specific tricoaxial setup for TRA-MT in AIS, even for large proximal LVOs, could constitute a viable alternative to TFA-MT in selected cases.

      • Potential for probing three-body decays of Long-Lived Particles with MATHUSLA

        <P><B>Abstract</B></P> <P>Several extensions of the Standard Model predict the existence of Long-Lived Neutral Particles (LLNPs) with masses in the multi-GeV range and decay lengths of O ( 100 m ) or longer. These particles could be copiously produced at the LHC, but the decay products cannot be detected with the ATLAS or CMS detectors. MATHUSLA is a proposed large-volume surface detector installed near ATLAS or CMS aimed to probe scenarios with LLNPs which offers good prospects for disentangling the physics underlying two-body decays into visible particles. In this work we focus on LLNP decays into three particles with one of them being invisible, which are relevant for scenarios with low scale supersymmetry breaking, feebly interacting dark matter or sterile neutrinos, among others. We analyze the MATHUSLA prospects to discriminate between two- and three-body LLNP decays, as well as the prospects for reconstructing the underlying model parameters.</P>

        Persistent Nonbilious Vomiting in a Child: Possible Duodenal Webbing

        An association between malrotation and congenital duodenal webbing is rare. We present our experience with four patients at two centers, and a review of published reports. There are currently 94 reported cases of duodenal pathology associated with malrotation. However, only 15 of the 94 cases (15.9%) include patients with malrotation and a duodenal web. We suggest that nonbilious vomiting in a child must prompt the surgeon to consider duodenal pathology even in the presence of malrotation.

        Preoperative Narcotic Use, Impaired Ambulation Status, and Increased Intraoperative Blood Loss Are Independent Risk Factors for Complications Following Posterior Cervical Laminectomy and Fusion Surgery

        Objective: This retrospective cohort study seeks to identify risk factors associated with complications following posterior cervical laminectomy and fusion (PCLF) surgery. Methods: Adults undergoing PCLF from 2012 through 2018 at a single center were identified. Demographic and radiographic data, surgical characteristics, and complication rates were compared. Multivariate logistic regression models identified independent predictors of complications following surgery. Results: A total of 196 patients met the inclusion criteria and were included in the study. The medical, surgical, and overall complication rates were 10.2%, 23.0%, and 29.1% respectively. Risk factors associated with medical complications in multivariate analysis included impaired ambulation status (odds ratio [OR], 2.27; p=0.02) and estimated blood loss over 500 mL (OR, 3.67; p=0.02). Multivariate analysis revealed preoperative narcotic use (OR, 2.43; p=0.02) and operative time (OR, 1.005; p=0.03) as risk factors for surgical complication, whereas antidepressant use was a protective factor (OR, 0.21; p=0.01). Overall complication was associated with preoperative narcotic use (OR, 1.97; p=0.04) and higher intraoperative blood loss (OR, 1.0007; p=0.03). Conclusion: Preoperative narcotic use and estimated blood loss predicted the incidence of complications following PCLF for CSM. Ambulation status was a significant predictor of the development of a medical complication specifically. These results may help surgeons in counseling patients who may be at increased risk of complication following surgery.

        Novosphingobium sp. PP1Y as a novel source of outer membrane vesicles

        Outer membrane vesicles (OMVs) are nanostructures of 20– 200 nm diameter deriving from the surface of several Gramnegative bacteria. OMVs are emerging as shuttles involved in several mechanisms of communication and environmental adaptation. In this work, OMVs were isolated and characterized from Novosphingobium sp. PP1Y, a Gram-negative non-pathogenic microorganism lacking LPS on the outer membrane surface and whose genome was sequenced and annotated. Scanning electron microscopy performed on samples obtained from a culture in minimal medium highlighted the presence of PP1Y cells embedded in an extracellular matrix rich in vesicular structures. OMVs were collected from the exhausted growth medium during the mid-exponential phase, and purified by ultracentrifugation on a sucrose gradient. Atomic force microscopy, dynamic light scattering and nanoparticle tracking analysis showed that purified PP1Y OMVs had a spherical morphology with a diameter of ca. 150 nm and were homogenous in size and shape. Moreover, proteomic and fatty acid analysis of purified OMVs revealed a specific biochemical “fingerprint”, suggesting interesting details concerning their biogenesis and physiological role. Moreover, these extracellular nanostructures do not appear to be cytotoxic on HaCaT cell line, thus paving the way to their future use as novel drug delivery systems.

        <P>The focus of this paper lies on the possible experimental tests of leptogenesis scenarios. We consider both leptogenesis generated from oscillations, as well as leptogenesis from out-of-equilibrium decays. As the Akhmedov-Rubakov-Smirnov (ARS) mechanism allows for heavy neutrinos in the GeV range, this opens up a plethora of possible experimental tests, e.g. at neutrino oscillation experiments, neutrinoless double beta decay, and direct searches for neutral heavy leptons at future facilities. In contrast, testing leptogenesis from out-of-equilibrium decays is a quite difficult task. We comment on the necessary conditions for having successful leptogenesis at the TeV-scale. We further discuss possible realizations and their model specific testability in extended seesaw models, models with extended gauge sectors, and supersymmetric leptogenesis. Not being able to test high-scale leptogenesis directly, we present a way to falsify such scenarios by focusing on their washout processes. This is discussed specifically for the left-right symmetric model and the observation of a heavy <TEX>$ W_{R}$</TEX>, as well as model independently when measuring <TEX>$ \Updelta L = 2$</TEX> washout processes at the LHC or neutrinoless double beta decay.</P>

        The lipopolysaccharide core oligosaccharide of <i>Burkholderia</i> plays a critical role in maintaining a proper gut symbiosis with the bean bug <i>Riptortus pedestris</i>

        <P>Lipopolysaccharide, the outer cell-wall component of Gram-negative bacteria, has been shown to be important for symbiotic associations. We recently reported that the lipopolysaccharide O-antigen of <I>Burkholderia</I> enhances the initial colonization of the midgut of the bean bug, <I>Riptortus pedestris</I>. However, the midgut-colonizing <I>Burkholderia</I> symbionts lack the O-antigen but display the core oligosaccharide on the cell surface. In this study, we investigated the role of the core oligosaccharide, which directly interacts with the host midgut, in the <I>Riptortus–Burkholderia</I> symbiosis. To this end, we generated the core oligosaccharide mutant strains, Δ<I>wabS</I>, Δ<I>wabO</I>, Δ<I>waaF,</I> and Δ<I>waaC,</I> and determined the chemical structures of their oligosaccharides, which exhibited different compositions. The symbiotic properties of these mutant strains were compared with those of the wild-type and O-antigen–deficient Δ<I>wbiG</I> strains. Upon introduction into <I>Riptortus</I> via the oral route, the core oligosaccharide mutant strains exhibited different rates of colonization of the insect midgut. The symbiont titers in fifth-instar insects revealed significantly reduced population sizes of the inner core oligosaccharide mutant strains Δ<I>waaF</I> and Δ<I>waaC</I>. These two strains also negatively affected host growth rate and fitness. Furthermore, <I>R. pedestris</I> individuals colonized with the Δ<I>waaF</I> and Δ<I>waaC</I> strains were vulnerable to septic bacterial challenge, similar to insects without a <I>Burkholderia</I> symbiont. Taken together, these results suggest that the core oligosaccharide from <I>Burkholderia</I> symbionts plays a critical role in maintaining a proper symbiont population and in supporting the beneficial effects of the symbiont on its host in the <I>Riptortus–Burkholderia</I> symbiosis.</P>
