        Diosgenin from <i>Dioscorea nipponica</i> Ameliorates Diabetic Neuropathy by Inducing Nerve Growth Factor

        <P>Diabetic neuropathy is characterized by axonal degeneration, demyelination, and atrophy in association with failed axonal regeneration, remyelination, and synaptogenesis. Recent reports suggest that reduced levels of nerve growth factor (NGF) may play a significant role in the pathogenesis of diabetic polyneuropathy. In this study, we investigated the regulation of NGF by steroid diosgenin (DG) in a diabetic neuropathy rodent model. We found that DG, the primary spirostane-type steroid in several <I>Dioscorea</I> species, increased NGF levels in the sciatic nerve of diabetic rats. Additionally, DG increased neurite outgrowth in PC12 cells and enhanced nerve conduction velocities in the diabetic neuropathy mouse model. DG-treated diabetic mice showed reduced disarrangement of the myelin sheath and increased area of myelinated axons by electron microscope studies and exhibited improvement in the damaged axons. Our data further suggest that DG increased the nerve conduction velocity through induction of NGF. Thus, our findings indicate that DG, a major sapogenin obtained from <I>Dioscorea nipponica</I>, reverses functional and ultrastructural changes and induces neural regeneration in a diabetic neuropathy model.</P>

        초등학교 영어 교과서 자료의 진정성 고찰

        The purpose of this study is to investigate the authenticity of the expressions in elementary school English textbooks in South Korea. 14 native English speaking instructors from different national backgrounds analyzed 5 elementary school English textbooks for 6<SUP>th</SUP> grade with the 2 criteria of situational and linguistic authenticity. Situational authenticity consists of situational probability and consensus building with students. Linguistic authenticity consists of language expressions, style of formality, and conversation flow. The major results of this study are as follows. First, there are total 43 expressions found of which authenticity is insufficient: 5 in situational authenticity; 38 in linguistic authenticity. Second, most expressions whose linguistic authenticity is insufficient are due to language expressions and conversation flow. Therefore, textbook writers should use more natural and authentic expressions for students to have more confidence when they are using English.

        Chitosan-Based Film of Tyrothricin for Enhanced Antimicrobial Activity against Common Skin Pathogens Including Staphylococcus aureus

        Chitosan-based film-forming gel is regarded as a promising vehicle for topical delivery of antimicrobial agents to skin wounds, since it protects from microbial infection and the cationic polymer itself possesses antibacterial activity. In this study, possible synergistic interaction against common skin pathogens between the cationic polymer and tyrothricin (TRC), a cyclic polypeptide antibiotic, was investigated, by determining the concentration to inhibit 90% of bacterial isolates (MIC). The addition of the polysaccharide to TRC dramatically reduced the MIC values of TRC by 1/33 and 1/4 against both methicillin-resistant and methicillinsusceptible Staphylococcus aureus, respectively. The synergism of TRC and chitosan combination against both strains was demonstrated by the checkerboard method, with a fractional inhibitory concentration index below 0.5. Moreover, co-treatment of TRC and chitosan exhibited antibacterial activity against Pseudomonas aeruginosa, due to the antibacterial activity of chitosan, whereas TRC itself did not inhibit the gram-negative bacterial growth. These findings suggested that the use of chitosan-based film for topical delivery of TRC could be an alternative to improve TRC antimicrobial activity against strains that are abundant in skin wounds.

      • Theraplay-Based Group Therapy for Juvenile Delinquents : A Randomized and Controlled Trial in Korea

        목 적 : 본 연구의 목적은 소년범에 대한 중재프로그램으로써 치료놀이 기반 그룹치료의 효과를 평가하는 것이다. 방 법 : 2016년 1월에서 4월까지 서울 소년원의 재소중인 소년범들 중 ADHD와 세 가지의 공병질환이 있는 30명을 대상으로 각각 치료놀 이군과 대조군으로 무작위 배정하여 치료군에는 치료놀이를, 대조군에는 레크레이션 프로그램을 21세션 제공하였다. 모든 참가자들은 프로그램 시행 전과 후에 한국 청소년 자기행동 평가척도(K-YSR), 소아용 삶의 질 검사(PedsQL), Barratt 충동성 척도(Barratt Impulsiveness Scale), 자기통제력 척도(self-control rating), 자기 자존감 척도(self-esteem scale), 소아우울척도(Children’s Depression Inventory)에 답하였다. 결 과 : 프로그램 전 후 변화에서 학업수행점수(p=0.043)는 치료놀이군에서만 통계적으로 유의한 수준의 호전을 보였으며, 자아존중감 점수의 경우 치료군에서는 상승한 반면(p<0.001), 대조군에서는 하락하였다(p=0.036). 치료 전후 그룹간의 변화에서는 자아존중감, 문제행동, 외현화 문제 항목에서 유의미한 차이를 보였다(순서대로 p<0.001, p=0.023, p=0.023). 결 론 : 치료놀이를 시행한 소년범에서 학업수행 점수와 자아존중감 점수가 유의미하게 상승하였다. 하지만 본 연구의 결과를 일반화하기 위해서는 추후 여자 소년범을 포함하고 보다 다양한 범위의 정신병리를 포함한 연구가 진행되어야 할 것이다. Objectives : The aim of this study is to evaluate the effectiveness of Theraplay-based group therapy with juvenile delinquents. Methods : From January–April 2016, 30 participants of Seoul Reformatory who had Attention-Deficit Hyperactivity Disorder (ADHD) with three other comorbidities were randomly assigned to either a Theraplay treatment group or a control group. Every participant completed pre- and post-test self-reporting questionnaires to include the Korean Youth Self-Report (KYSR), the Pediatric Quality of Life Inventory (PedsQL), the Barratt Impulsiveness Scale, a self-control rating, a self-esteem scale, and the Children’s Depression Inventory. We offered 21 Theraplay sessions for the treatment group and offered recreational play for the control group during the test period. Results : Only the Theraplay treatment group showed statistically significant enhancements in school functioning (p=0.043). The Theraplay treatment group improved in their self-esteem scores (p<0.001) while the control group showed a decrease in these scores (p=0.036). A comparison between the groups regarding changes from baseline to endpoint scores for certain variables showed significant differences for self-esteem, delinquent behavior, and externalizing behavior (p<0.001, p=0.023, p=0.023, respectively). Conclusion : The adolescents who received Theraplay displayed significant changes in their school functioning and self-esteem scores. In order to generalize the results of this study, it is important that future studies include female adolescents and a more diverse range of psychopathology.

        Comparison of Piroxicam Pharmacokinetics and Anti-Inflammatory Effect in Rats after Intra-Articular and Intramuscular Administration

        This study evaluated the pharmacokinetic profile and therapeutic efficacy of piroxicam (PX), a long acting non-steroidal antiinflammatory drug for the treatment of arthritis, following intra-articular (IA) injection in comparison to the pharmacokinetic profile and therapeutic efficacy of PX after intramuscular (IM) injection. In the pharmacokinetic study in rats, systemic exposure and pharmacokinetic parameters of PX after a single IA dose were compared with systemic exposure and pharmacokinetic parameters of PX after administration of the same dose IM (0.6 mg/kg). The anti-inflammatory and analgesic effects of IA PX were evaluated simultaneously in a monoiodoacetate-induced osteoarthritis rat model. The plasma PX concentration rapidly rose following IA injection, and it was comparable to the plasma PX concentration following IM injection, suggesting the rapid efflux of the drug molecule from the joint cavity. However, in the efficacy study, the IA PX administration significantly reduced the knee swelling by reducing the level of prostaglandin E2 in the joint, compared to that following administration of IA vehicle and after administration of the IM PX dose. In addition, we found that the anti-inflammatory and anti-nociceptive efficacies of IA PX were synergistically increased upon co-treatment with hyaluronic acid (HA), a potent agent for the treatment of osteoarthritis, at the weight ratio of 1:1 or 1:2, and these effects were more pronounced than those following administration of HA or PX alone. In conclusion, this study demonstrated the efficacy of the IA use of PX alone and/or in combination with HA in osteoarthritis.

        Reduced Food-Effect on Intestinal Absorption of Dronedarone by Self-microemulsifying Drug Delivery System (SMEDDS).

        <P>The oral absorption of dronedarone (DRN), a benzofuran derivative with anti-arrhythmic activity, is significantly affected by food intake. The absolute bioavailability of the marketed product (Multaq, Sanofi, U.S.) was about 4% without food, but increased to 15% when administered with a high fat meal. Therefore, to reduce the food-effect on the intestinal absorption of DRN, a novel self-microemulsifying drug delivery system (SMEDDS) was formulated and the comparative in vivo absorption studies with the marketed product were carried out using male beagle dogs either in the fasted or fed state. The SMEDDS consisted of the drug, Labrafil M 1944CS, and Kolliphor EL in a weight ratio of 1?:?1?:?2, rapidly formed a fine oil-in-water emulsion with a droplet size less than 50?nm. An in vivo absorption study revealed that the area-under-curve (AUC0-24?h) and maximal plasma concentration (Cmax) were 10.4-fold (p<0.05) and 8.6-fold (p<0.05) higher, respectively, after the marketed product was orally administered to beagles in the fed state when compared to those in the fasted state. This food-effect were remarkably alleviated by SMEDDS formulation, with AUC0-24?h and Cmax 2.9-fold (p<0.05) and 2.6-fold (p<0.05) higher in the fed state when compared to the fasted state, by facilitating intestinal absorption of DRN in the fasted state. The results of this study suggest that SMEDDS may decrease the differences in oral absorption of DRN between the prandial states, improving therapeutic efficacy as well as patient compliance.</P>

        Pharmacokinetics of chlorogenic acid and corydaline in DA-9701, a new botanical gastroprokinetic agent, in rats

        <P>1. <?ri?>Few studies describing the pharmacokinetic properties of chlorogenic acid (CA) and corydaline (CRD) which are marker compounds of a new prokinetic botanical agent, DA-9701, have been reported. The aim of the present study is to evaluate the pharmacokinetic properties CA and CRD following intravenous and oral administration of pure CA (1-8 mg/kg) or CRD (1.1-4.5 mg/kg) and their equivalent dose of DA-9701 to rats.</P><P>2. <?ri?>Dose-proportional AUC and dose-independent clearance (10.3-12.1 ml/min/kg) of CA were observed following its administration. Oral administration of CA as DA-9701 did not influence the oral pharmacokinetic parameters of CA. Incomplete absorption of CA, its decomposition in the gastrointestinal tract, and/or pre-systemic metabolism resulted in extremely low oral bioavailability (<I>F</I>) of CA (0.478-0.899%).</P><P>3. <?ri?>CRD showed greater dose-normalized AUC in the higher dose group than that in lower dose group(s) after its administration due to saturation of its metabolism via decreased non-renal clearance (by 51.3%) and first-pass extraction. As a result, the <I>F</I> of CRD following 4.5 mg/kg oral CRD (21.1%) was considerably greater than those of the lower dose groups (9.10 and 13.8%). However, oral administration of CRD as DA-9701 showed linear pharmacokinetics as a result of increased AUC and <I>F</I> in lower-dose groups (by 182% and 78.5%, respectively) compared to those of pure CRD. The greater oral AUC of CRD for DA-9701 than for pure CRD could be due to decreased hepatic and/or GI first-pass extraction of CRD by other components in DA-9701.</P>
