Background/Aims: Probe-based confocal laser endomicroscopy (pCLE) requires the administration of intravenous (IV) fluorescein. This study aimed to determine the optimal dose of IV fluorescein for both upper and lower gastrointestinal (GI) tract pCLE. Methods: Patients 20 to 79 years old with gastric high-grade dysplasia (HGD) or colorectal neoplasms (CRNs) were enrolled in thestudy. The dose de-escalation method was employed with five levels. The primary endpoint of the study was the determination ofthe optimal dose of IV fluorescein for pCLE of the GI tract. The reduced dose was determined based on off-line reviews by threeendoscopists. An insufficient dose of fluorescein was defined as the dose of fluorescein with which the pCLE images were notdeemed to be visible. If all three endoscopists determined that the tissue structure was visible, the doses were de-escalated. Results: A total of 12 patients with gastric HGD and 12 patients with CRNs were enrolled in the study. Doses were de-escalated to0.5 mg/kg of fluorescein for both non-neoplastic duodenal and colorectal mucosa. All gastric HGD or CRNs were visible with pCLEwith IV fluorescein at 0.5 mg/kg. Conclusions: In the present study, pCLE with IV fluorescein 0.5 mg/kg was adequate to visualize the magnified structure of both theupper and lower GI tract.
Analysis of Coupled-Inductor Configuration for an Interleaved High Step-Up Converter
Wilmar Martinez,Jun Imaoka,Yuki Itoh,Masayoshi Yamamoto,Kazuhiro Umetani 전력전자학회 2015 ICPE(ISPE)논문집 Vol.2015 No.6
High step-up converters are widely used in sustainable energy systems and recently used in automotive applications due to their high voltage gain capability. Nevertheless, with the purpose of obtaining a higher voltage gain, in comparison with conventional boost converters, current high step-up converters often employ additional multiplier cells, which may lead to significant cost-up and low power density. Therefore, a novel two-phase interleaved high step-up converter is proposed in order to minimize additional circuit volume used to achieve large voltage gain. The proposed converter addresses the purpose by a particular coupled inductor where three windings are installed in one or two cores. As a result, the proposed converter can achieve higher voltage gain than the conventional topologies by adding a winding and two diodes to the interleaved two phase boost chopper, besides the coupled-inductor configuration. This paper evaluates two arrangements of the coupled-inductor configuration of the proposed high step-up converter: 1. Three windings integrated in only one core and 2. Two independent inductors with a shared winding. The result revealed that the proposed converter shows higher voltage gain than the normal boost converter and the magnetic integration in the coupled-inductor configuration further increases the voltage gain by 20%.
Bleeding After Gastric Endoscopic Submucosal Dissection Focused on Management of Xa Inhibitors
Ono Shoko,Ieko Masahiro,Tanaka Ikko,Shimoda Yoshihiko,Ono Masayoshi,Yamamoto Keiko,Sakamoto Naoya 대한위암학회 2022 Journal of gastric cancer Vol.22 No.1
Purpose: The use of direct oral Xa inhibitors (DXaIs) to prevent venothrombotic events is increasing. However, gastrointestinal bleeding, including that related to endoscopic resection, is a concern. In this study, we evaluated bleeding and coagulation times during the perioperative period of gastric endoscopic submucosal dissection (ESD). Materials and Methods: Patients who consecutively underwent gastric ESD from August 2016 to December 2018 were analyzed. Bleeding rates were compared among the 3 groups (antiplatelet, DXaIs, and control). DXaI administration was discontinued on the day of the procedure. Prothrombin time (PT), activated partial thromboplastin time, and the ratio of inhibited thrombin generation (RITG), which was based on dilute PT, were determined before and after ESD. Results: During the study period, 265 gastric ESDs were performed in 239 patients, where 23 and 50 patients received DXaIs and antiplatelets, respectively. Delayed bleeding occurred in 17 patients (7.4%) and 21 lesions (7.1%). The bleeding rate in the DXaI group was significantly higher than that in the other groups (30.4%, P<0.01), and the adjusted odds ratio of bleeding was 5.7 (95% confidence interval, 1.4–23.7; P=0.016). In patients using DXaIs, there was a significant (P=0.046) difference in the median RITG between bleeding cases (18.6%) and non-bleeding cases (3.8%). Conclusions: A one-day cessation of DXaIs was related to a high incidence of bleeding after gastric ESD, and monitoring of residual coagulation activity at trough levels might enable the predicted risk of delayed bleeding in patients using DXaIs.
Deformed type 0A matrix model and super-Liouville theory for fermionic black holes
Ahn, Changrim,Kim, Chanju,Park, Jaemo,Suyama, Takao,Yamamoto, Masayoshi Institute of Physics Pub 2006 The journal of high energy physics Vol.2006 No.1
We consider a &hatc; = 1 model in the fermionic black hole background. For this purpose we consider a model which contains both the N = 1 and the N = 2 super-Liouville interactions. We propose that this model is dual to a recently proposed type 0A matrix quantum mechanics model with vortex deformations. We support our conjecture by showing that non-perturbative corrections to the free energy computed by both the matrix model and the super-Liouville theories agree exactly by treating the N = 2 interaction as a small perturbation. We also show that a two-point function on sphere calculated from the deformed type 0A matrix model is consistent with that of the N = 2 super-Liouville theory when the N = 1 interaction becomes small. This duality between the matrix model and super-Liouville theories leads to a conjecture for arbitrary n-point correlation functions of the N = 1 super-Liouville theory on the sphere.
