        Evaluation of the Efficiency of Chitosan Hydrogel Containing Berberis integerrima Root Extract on a Full-Thickness Skin Wound in a Rat Model

        Some species of the Berberidaceae family and natural biopolymer chitosan have good effects on wound healing. In this research, a new formulation of chitosan hydrogel containing alcoholic root extract of Berberis integerrima (Chi-BIE) was designed and evaluated for the treatment of full-thickness skin wounds in an animal model. Alcoholic extract of Berberis integerrima root (BIE) was prepared, and its total amount of berberine was determined using UV/visible spectroscopy. Chitosan gel containing 10 and 20% BIE was prepared, and their extract release profile was assayed. The antimicrobial activity of Chi-BIE was investigated by the minimum inhibitory concentration assay. Finally, the efficiency of Chi-BIE formulations on the full-thickness wound healing was physically and histologically evaluated in an animal model. BIE was successfully loaded into the chitosan hydrogel and showed a burst release of extract during the first 4 h (60%) followed by a sustained release. Formulation of BIE in chitosan hydrogel could increase the antimicrobial activity of chitosan and BIE alone against Staphylococcus aureus, Pseudomonas aeruginosa, Bacillus subtilius, Escherichia coli, and Candida albicans. Chi-20% BIE indicated more antimicrobial activity compared to Chi-10%BIE. Formulation of BIE in chitosan hydrogel could increase the wound healing effects of chitosan and BIE alone. Chi-20% BIE had a faster restorative effect than other treatment groups due to more formation of epithelial tissue and faster maturation of bud tissue. Based on the results of this study, the chitosan hydrogel containing BIE is proposed as a suitable dressing for wound-healing.

        Biochemical and Rheological Characterization of a Protease from Fruits of Withania coagulans with a Milk-clotting Activity

        A protease from Withania coagulans fruits wasevaluated for milk-clotting properties. The optimumtemperature and pH for enzyme activity were 70oC and 4,respectively. The protease also exhibited an excellentthermal stability at 60oC for 30 min. Fractional precipitationusing ammonium sulfate showed that the 40-50% fraction(F5) possessed the highest milk-clotting activity. The F5fraction showed a Mw band of 66 kDa by SDS-PAGE. Gelformation was monitored using low amplitude oscillatoryrheology at different temperatures. An Arrhenius plot wasused to describe the temperature dependence of thegelation rate parameter. Time sweep testing showed that anincrease in temperature was accompanied by a decrease inthe gelation onset time, a higher gel formation rate, andhigher values for final gel strength. W.coagulans fruitshave potential for use as a rennet substitute.

        Effect of 38% carbamide peroxide on the microleakage of silorane-based versus methacrylate-based composite restorations

        Objectives: This study aimed to assess the effect of 38% carbamide peroxide on the microleakage of class V cavities restored with either a silorane-based composite or two methacrylate-based composites. Materials and Methods: A total of 96 class V cavities were prepared on the buccal surface of extracted human teeth with both enamel and dentin margins and were randomly assigned into three groups of Filtek P90 (3M-ESPE) + P90 system adhesive (3M-ESPE)(group A), Filtek Z250 (3M-ESPE) + Adper Prompt L-Pop (3M-ESPE)(group B) and Filtek Z350XT (3M-ESPE) + Adper Prompt L-Pop (group C). Half of the teeth were randomly underwent bleaching (38% carbamide peroxide, Day White, Discus Dental, applying for 15 min, twice a day for 14 day) while the remaining half (control) were not bleached. Dye penetration was measured following immersion in basic fuchsine. Data were statistically analyzed using Kruskal-Wallis and Mann-Whitney U tests at a level of 0.05. Results: No significant differences were found between composites in the control groups in enamel (p = 0.171) or dentin (p = 0.094) margins. After bleaching, microleakage of Z250 (in enamel [p = 0.867] or dentin [p = 0.590] margins) and Z350 (in enamel [p = 0.445] or dentin [p = 0.591]margins) did not change significantly, but the microleakage of P90 significantly increased in both enamel (p = 0.042) and dentin (p = 0.002) margins. Conclusions: No significant differences were noted between the bleached and control subgroups of two methacrylate-based composites in enamel or dentin margins. Microleakage of silorane-based composite significantly increased after bleaching.

      • Adjusting for reverse causation to estimate the effect of obesity on mortality after incident heart failure in the Atherosclerosis Risk in Communities (ARIC) study

        OBJECTIVES: The lower mortality rate of obese patients with heart failure (HF) has been partly attributed to reverse causation bias due to weight loss caused by disease. Using data about weight both before and after HF, this study aimed to adjust for reverse causation and examine the association of obesity both before and after HF with mortality. METHODS: Using the Atherosclerosis Risk in Communities (ARIC) study, 308 patients with data available from before and after the incidence of HF were included. Pre-morbid and post-morbid obesity were defined based on body mass index measurements at least three months before and after incident HF. The associations of pre-morbid and post-morbid obesity and weight change with survival after HF were evaluated using a Cox proportional hazard model. RESULTS: Pre-morbid obesity was associated with higher mortality (hazard ratio [HR], 1.61; 95% confidence interval [CI], 1.04 to 2.49) but post-morbid obesity was associated with increased survival (HR, 0.57; 95% CI, 0.37 to 0.88). Adjusting for weight change due to disease as a confounder of the obesity-mortality relationship resulted in the absence of any significant associations between post-morbid obesity and mortality. CONCLUSIONS: This study demonstrated that controlling for reverse causality by adjusting for the confounder of weight change may remove or reverse the protective effect of obesity on mortality among patients with incident HF.

        Field Study of Longshore Sediment Transport Rate Estimation over the Newly-Developed Coastline of Northern Oman Sea

        Estimations of sediment transport rate is the prerequisite of coastal development and management studies, as accretion and erosion are among main controlling considerations for port planning. The Iranian coastline of the Oman Sea, which is so-called Makran Coastline, has a complex morpho-dynamic behavior and should be considered from both scientific importance and its high potential for development in the next decade. It is while morpho-dynamic perspective of the area is not well studied so far. On the other hand, the wave climate of the area is affected by different regimes: monsoon waves, normal seas, swells coming from the Indian Ocean and tropical cyclones. There are limited researches on the contribution of each regime on the longshore sediment transport with considering the sensitivity to deep water wave direction variability. This study aims to put different pieces of knowledge together, including field measurements, numerical modeling and Geographic Information System-based (GIS-based) analysis of multi-year cross-shore profile data, to obtain a more realistic estimation of Longshore Sediment Transport (LST) rate along the un-developed Makran Coastline. The focus of this paper is mostly on the accurate wave and sediment transport modeling, verified against available field data and morphological evidences. The major challenge in this regard has been the lack of long-term data availability, as the coastline has been remained undeveloped. This has been overcome by adopting satellite imagery as a complementary source to the available periodic hydrography data, as well as data science methods for improving wave data accuracy. A numerical model is applied for simulating the transportation process of sediments along Makran Coastline. The existing extracted shoreline changes around Makran Coastline through GIS analysis of satellite images were used to assess the general morphological changes in the study area. All the simulation and coastline change analysis results are adopted for obtaining an integrated conclusion in Zarabad Port as the case study of this research. The obtained results were in fair agreement with observations and this is why the model setup and study methodology can be applied for LST rate estimations over Makran Coastline.

      • Expression of Neuronal Markers, NFP and GFAP, in Malignant Astrocytoma

        Background: Immunohistochemical markers are considered as important factors in diagnosis of malignant astrocytomas. The aim of the current study was to investigate the frequency of the immunohistochemical markers neurofilament protein (NFP) and glial fibrillary acidic protein (GFAP) in malignant astrocytoma tumors in Firoozgar and Rasool-Akram hospitals from 2005 to 2010. Materials and Methods: In this cross-sectional study, immunohistochemical analysis of NFP and GFAP was performed on 79 tissue samples of patients with the diagnosis of anaplastic and glioblastoma multiform (GBM) astrocytomas. Results: The obtained results demonstrated that all patients were positive for GFAP and only 3.8% were positive for NFP. There was no significant association between these markers and clinical, demographic, and prognostic features of patients (p>0.05). Conclusions: NFP was expressed only in GBMs and not in anaplastic astrocytomas. It would be crucial to confirm the present findings in a larger number of tumors, especially in high grade gliomas.

        Comparison of Attitudes Regarding Quality of Life between Insulin-Treated Subjects with Diabetes Mellitus and Healthy Populations

        Background: Diabetes mellitus is a chronic disease and one of the main causes of mortality in developing countries. The main objective of treating all chronic diseases, of course, is to improve well-being and attain a satisfactory quality of life (QOL). The major goal of this study is comparison of attitude toward QOL in insulin-dependent subjects with diabetes mellitus and healthy subjects. Methods: In this study, insulin-dependent subjects with diabetes mellitus and healthy subjects were gathered via convenience sampling. The subjects were asked to complete the Hanestad & Albrektsen Attitude to Quality of Life Questionnaire. The questionnaire evaluates five quality of life dimensions-physical, social, mental-emotional, behavioral-activity, and economic-using a scoring system similar to the Likert scale. The Wilcoxon test was used to compare scores between the two groups. Results: The mean total score on attitude toward QOL in the healthy control group was 53.8, and it in the insulin-dependent subjects with diabetes mellitus group was 35.9. The mean total score of attitude toward QOL in the physical dimension, mental-emotional and feelings of well-being dimension, and behavioral-activity dimension were significantly higher in the healthy population than they were in diabetes mellitus groups. Such a difference was not seen in the social and economic dimensions. Conclusion: Since the attitudes of insulin-dependent subjects with diabetes mellitus toward QOL are used as an index of individual and societal health levels, it appears that this group may benefit from education and professional counseling to improve their QOLs.

        An Iterative Method for Equilibrium and Constrained Convex Minimization Problems

        We are concerned with finding a common solution to an equilibrium problem associated with a bifunction, and a constrained convex minimization problem. We propose an iterative fixed point algorithm and prove that the algorithm generates a sequence strongly convergent to a common solution. The common solution is identified as the unique solution of a certain variational inequality.

        Study of probiotic and simvastatin additive effect on Helicobacter pylori eradication rate

        Background: Helicobacter pylori infection is a major cause of chronic gastritis, peptic, mucosa associated lymphoid tissueMisdiagnosed as lymphoma and gastric cancer. The eradication rate of H. pylori with standard treatments are decreasing worldwide. The aim of this study was to determine the efficacy of adding simvastatin or probiotic as adjuvant to quadruple therapy on the eradication of H. pylori. Methods: This randomized clinical trial study was conducted on 160 patients with H. pylori. The patients were randomly divided in to 4 groups. The group 1: standard quadruple eradication regimen consisting of two antibiotics (clarithromycin 500 mg and amoxicillin 1 g, all twice per day), a proton pump inhibitor (pantoprazole 40 mg twice daily), Bismuth (120 mg twice daily) with placebo (daily), group 2: standard regimen plus probiotic (daily east 250 mg), group 3: standard regimen supplemented with simvastatin (10 mg daily) and group 4: standard regimen plus simvastatin (20 mg daily) for 14 days was given. The Eradication was determined by stool antigen test at least 1 month after treatment. Results: The risk ratio of eradication of the infection in the simvastatin group 10 mg and simvastatin 20 mg was 0.375 and 0.625 times in compare with the placebo group, indicating a negative effect of simvastatin 10 and 20 mg on the treatment of H. pylori infection, this relationship is not statistically significant (P > 0.05). Conclusion: Due to the high efficacy of simvastatin in H. pylori eradication, further studies are needed to evaluate the use of statins as adjunctive to improve the eradication rate.

        In this paper, we consider the integral of a stochastic process with respect of a sequence of square integrable semimartingales. By this integrals, we construct a reproducing kernel Hilbert space and study the correspondence between this space with the concepts of arbitrage and viability in mathematical finance.
