        Subclinical Atherosclerosis in Patients with Cushing Syndrome: Evaluation with Carotid Intima-Media Thickness and Ankle-Brachial Index

        Background: Cushing syndrome (CS) has been described as a killing disease due its cardiovascular complications. In fact, chronic cortisol excess leads to a constellation of complications, including hypertension, hyperglycemia, adiposity, and thromboembolism. The main vascular alteration associated with CS is atherosclerosis. Methods: Aim of this study was to analyze carotid intima-media thickness (cIMT) and ankle-brachial index (ABI), two surrogate markers of subclinical atherosclerosis in a consecutive series of CS patients, compared to patients with essential hypertension (EH) and health subjects (HS). Results: Patients with CS showed a significant increase (P<0.05) of cIMT (0.89±0.17 mm) compared to EH (0.81±0.16 mm) and HS (0.75±0.4 mm), with a high prevalence of plaque (23%; P<0.03). Moreover, CS patients showed a mean ABI values (1.07±0.02) significantly lower respect to HS (1.12±0.11; P<0.05), and a higher percentage (20%) of pathological values of ABI (≤0.9; P<0.03). Conclusion: In conclusion, we confirmed and extended the data of cIMT in CS, and showed that the ABI represent another surrogate marker of subclinical atherosclerosis in this disease.

      • ENTREPRENEURIAL DISCOVERY PROCESS AND THE DEVELOPMENT OF THE WINE INDUSTRY IN GREECE: UNDERSTANDING INDUSTRY NEEDS TOWARDS THE JRC’S INNOVATIVE APPROACH

        Greece wine production has shown swinging trends: in 2013 it grew by 17,9% reaching 3.7 Million Hectoliters (USDA), but it 2014 it decreased by 15% (Greek Wine Federation), whilst grape prices went up. Greece has a huge potential, but it is still a small producer in Europe. The Greek wine industry got through several phases: before the 80s the product was quite “poor”, but with the entrance in the market of some educated winemakers who have introduced a new business philosophy and approach to wine, production has changed (GaiaWines.gr). The population of firms consist of a few larger producers, and most of the companies are relatively small. Although the quality has been improved, most of the production is addressed to the domestic market, since only few producers have established export networks (Vlachvei et al., 2012). Thus, the economic crisis that affected Greece has created difficulties also for domestic sales. Background research has outlines some of the limits of the Greek wine industry: according to Vlachvei et al. (2012),the new competitive challenges require owner and managers to achieve a better understanding of operational and marketing process that can contribute to the development of their brand in order to develop an integrated marketing communication approach. The Greek wine sector is extremely fragmented with a high predominance of small size firms; as size increases, the degree of professional organization grows, as well as the relative presence of foreign markets (Papalexiou, 2009). Thus, it has been recognized that Greek wine export is disorganized (Baiocchi, 2011; Papalexiou, 2009). The limits imposed by UE to plantings have affected Greece growth strategy: Greece is still a small producer and although it shows similarities with the south of Italy, it is impossible to think to follow the “Italian Style” (Baiocchi, 2011). Thus the urgency of undertaking a propter model for the development of the industry is seen as a priority for the Greek wine system. Wineries have seen in related businesses an effective tool for improving the competitiveness of the wine industry (Papalexiou, 2009); a lot has been done for achieving an integration between wine tourism and the wine business. Background research describes the experiments undertaken around wine routes, conceived as a useful tool for improving the wine sector (Hall & Mitchell, 2000) and for the development of rural areas (Karafolas, 2007). In 2014 the Joint Research Center (JRC) of the European Commission has undertaken a Smart Specialization pilot project that involves rural areas in Northern Greece, in order to foster local economic development by adopting an entrepreneurial discovery approach (EDP). JRC can be considered as a source of technology generation and a service that provides expert advice to the institutions of the European Union and more specifically to the European Commission and the European Parliament (Moncada-Paterno`-Castello, 2003). It can be guessed how JRC outcomes could impact citizens’ lives. From September 2014 for 15 months JRC has been involved in the European Parliament Preparatory Action; the aims of this project are: to facilitate the refinement and implementation of the RIS3 strategy in a region heavily hit by the crisis; and to serve as a model for other convergence regions in Greece and Europe (from JRC website). The project has seen the collaboration of JRC – IPTS (Institute for Prospective Technological Studies) with DG Regio and the Managing Authority of Eastern Macedonia and Thrace. The wine sector has been selected, together with few others for the preparatory action. The approach that has been adopted was the Entrepreneurial Discovery Process (EDP), in order to allocate resources under the guide of entrepreneurial actors, from the public and private sector. EDP is “a process in which the entrepreneurial actors are discovering and producing information about new business and innovation activities and the government is collecting, assessing and transforming this knowledge into policy action” (http://s3platform.jrc.ec.europa.eu/s3-governance).EDP plays a leading role in the Smart Specialization Strategy (S3): EDP suggests that the allocation of resources is the result of a process undertaken by entrepreneurial actors. It emerges how innovative this process is, if compared to past industrial policies. EDP contributed to minimise failures of market innovation. The Smart Specialisation perspective let entrepreneurs discovering the right domains of future specializations (Foray, 2012). A newer approach to regional competitiveness (Ketels, 2013) has been followed by European Commission, that, in order to promote an efficient and effective public funding policy in regional economic development (Mempel-Śnieżyk, 2013), has actively supported Smart Specialisation Initiatives. On the other side, S3 has a prior role in the EU Regional and Cohesion Policy 2014-2020 (Martínez-López & Palazuelos-Martínez, 2014). This paper examines the role that EDP and S3 have had in the definition of a strategy for the development of the local wine industry. The paper aims to answering to the following research questions: What are the elements seen as priorities from entrepreneurs for developing the local wine industry in Greece? Is there an overlapping between the findings emerging from JRC workshops and those that have been pointed out by relevant opinion makers and experts in the Greek wine industry? What about the effectiveness of EDP in wine? The paper is structured as follows: In the first part a description of the Greek wine industry will be carried out; in a second section authors will point out JRC’s EDP approach; thirdly an analysis of data collected from JRC sessions and from interviews with international experts in Greece will be performed. 9 experts that took part to the workshop will be interviewed. Data will be collected through telephone interviews and web based survey; once collected data will be analyzed through a text analysis. A literature review on EDP and theoretical issues that have inspired the organization of JRC sessions will be presented in order to support Research Questions. The papers ends up with a discussion and conclusions in order to provide inputs for professionals, and policy makers.

        Pereskia aculeata Miller Flour: Metabolic Effects and Composition

        Pereskia aculeata Miller is known in Brazil as ora-pro-nobis (OPN) and has been used commonly in the folklore medicine. The purpose of our study was to evaluate the composition and the effects of OPN flour on the metabolic profile and intestinal motility of Wistar rats. Animals were divided randomly into five groups (n = 10): G1 (control group) and G2 (treated with OPN flour). For the intestinal motility: G3 (control group), G4 (treated with senne), and G5 (treated with OPN flour). After 40 days, G1 and G2 were euthanized and metabolic profiles were analyzed (glycemia, cholesterol, triglycerides, high density lipoprotein [HDL-c], C reactive protein, AST, ALT, Lee Index, weight, and visceral fat). The flour of OPN was effective in reducing percentage of weight gain, visceral fat, levels of total cholesterol, triglycerides, low density lipoprotein, very low density lipoprotein, and increased HDL-c. Significance was also found in the distance covered by the activated charcoal from the pylorus to the beginning of the cecum, which was higher in animals treated with OPN. Our results indicate that OPN flour may bring health benefits, as the improvement of the intestinal motility, and it is associated with reduction of visceral fat and lipid profile, as well as the increase of HDL-c levels. With these results, we may suggest that the incorporation of this flour in different industrial products may be a convenient and effective way for the intake of healthier products.

        Dehydroepiandrosterone and cortisol concentrations in the cerebrospinal fluid of dogs

        Concentrations of cortisol, dehydroepiandrosterone and dehydroepiandrosterone-sulfate were measured by performing radioimmunoassay of the cerebrospinal fluid of 68 dogs diagnosed with idiopathic epilepsy or inflammatory, degenerative, or non-neurological disease. No steroid concentration differences were found among diagnoses. Dehydroepiandrosterone and dehydroepiandrosterone-sulfate concentrations were higher in males than in females and dehydroepiandrosterone-sulfate decreased with increasing age. No sex or age effects were observed on cortisol or hormone ratios. Although limited to a relatively small sample, our results show sex- and age-dependent variations in these neurosteroid concentrations in cerebrospinal fluid. The role of such variations in the pathophysiology of the dog brain warrants further investigation.

        Single nucleotide polymorphism of GSTP1 and pathological complete response in locally advanced rectal cancer patients treated with neoadjuvant concomitant radiochemotherapy

        Purpose: Standard treatment for locally advanced rectal cancer consists of neoadjuvant radiochemotherapy with concomitant fluoropyrimidine or oxaliplatin and surgery with curative intent. Pathological complete response has shown to be predictive for better outcome and survival; nevertheless there are no biological or genetic factors predictive for response to treatment. We explored the correlation between the single nucleotide polymorphisms (SNPs) GSTP1 (A313G) and XRCC1 (G28152A), and the pathological complete response and survival after neoadjuvant radiochemotherapy in locally advanced rectal cancer patients. Materials and Methods: Genotypes GSTP1 (A313G) and XRCC1 (G28152A) were determined by pyrosequencing technology in 80 patients affected by locally advanced rectal cancer. Results: The overall rate of pathological complete response in our study population was 18.75%. Patients homozygous AA for GSTP1 (A313G) presented a rate of pathological complete response of 26.6% as compared to 8.5% of the AG+GG population (p = 0.04). The heterozygous comparison (AA vs. AG) showed a significant difference in the rate of pathological complete response (26.6% vs. 6.8%; p = 0.034). GSTP1 AA+AG patients presented a 5- and 8-year cancer-specific survival longer than GSTP1 GG patients (87.7% and 83.3% vs. 44.4% and 44.4%, respectively) (p = 0.014). Overall survival showed only a trend toward significance in favor of the haplotypes GSTP1 AA+AG. No significant correlations were found for XRCC1 (G28152A). Conclusion: Our results suggest that GSTP1 (A313G) may predict a higher rate of pathological complete response after neoadjuvant radiochemotherapy and a better outcome, and should be considered in a more extensive analysis with the aim of personalization of radiation treatment.

        A Healthy Balance of Plasma Cholesterol by a Novel Annurca Apple-Based Nutraceutical Formulation: Results of a Randomized Trial

        Cardiovascular diseases are nowadays preferential targets of preventive medicine through a straightforward therapy on lipid profile. However, statins, the first-line lipid-lowering drug therapy, specifically act on low-density lipoprotein cholesterol (LDL-C), having a modest effect on plasma high-density lipoprotein cholesterol (HDL-C) concentrations. Today, a number of novel HDL-targeted therapies are emerging, along with unexpected side effects. Thus, novel and possibly safe substances, able to correct impaired lipid profile in humans, are still in great demand. Herein, based on encouraging clinical data, we formulated a nutraceutical product (AppleMetS®, AMS), based on a polyphenolic extract from Annurca apple, and demonstrated that two capsules a day of AMS, after one month, have a LDL-C lowering outcome equivalent to 40 mg of simvastatin or 10 mg of atorvastatin. Nevertheless, different from statin-based therapy, AMS exerted a notable effect on HDL (+49.2%). Based on the trial results, we can assert that AMS formulation could effectively integrate the current therapeutic arsenal to correct impaired lipid profile in humans. Specifically, AMS may be considered a complementary and/or alternative safe substance suitable for the treatment of mildly hypercholesterolemic subjects who do not present occurrence of atheromatous plaques yet.

        G2A Attenuates Propionibacterium acnes Induction of Inflammatory Cytokines in Human Monocytes

        Background: Acne vulgaris is a disease of the pilosebaceous unit characterized by increased sebum production, hyperkeratinization, and immune responses to Propionibacterium acnes (PA). Here, we explore a possible mechanism by which a lipid receptor, G2A, regulates immune responses to a commensal bacterium. Objective: To elucidate the inflammatory properties of G2A in monocytes in response to PA stimulation. Furthermore, our study sought to investigate pathways by which lipids modulate immune responses in response to PA. Methods: Our studies focused on monocytes collected from human peripheral blood mononuclear cells, the monocytic cell line THP-1, and a lab strain of PA. Our studies involved the use of enzyme-linked immunosorbent, Western blot, reverse transcription polymerase chain reaction, small interfering RNA (siRNA), and microarray analysis of human acne lesions in the measurements of inflammatory markers. Results: G2A gene expression is higher in acne lesions compared to normal skin and is inducible by the acne therapeutic, 13-cis -retinoic acid. In vitro, PA induces both the Toll-like receptor 2-dependent expression of G2A as well as the production of the G2A ligand, 9-hydroxyoctadecadienoic acid, from human monocytes. G2A gene knockdown through siRNA enhances PA stimulation of interleukin (IL)-6, IL-8, and IL-1β possibly through increased activation of the ERK1/2 MAP kinase and nuclear factor kappa B p65 pathways. Conclusion: G2A may play a role in quelling inflammatory cytokine response to PA, revealing G2A as a potential attenuator of inflammatory response in a disease associated with a commensal bacterium.
