        A Multicenter Analysis of Clinical Features and Long-Term Outcomes of POEMS Syndrome in Korea

        Background: POEMS syndrome is a rare form of plasma cell dyscrasia characterized by polyneuropathy, organomegaly, endocrinopathy, monoclonal proteins, and skin changes. Owing to its low incidence, there are few reports regarding this syndrome. This multicenter study included 84 patients diagnosed with POEMS syndrome in South Korea. Methods: We retrospectively evaluated 84 patients diagnosed with POEMS syndrome at 8 hospitals in South Korea between January 2000 and October 2022. The clinical characteristics and treatment outcomes were analyzed. Results: The median patient age was 53 years (range, 26–77 years), and 63.1% of the patients were male. All patients had peripheral neuropathy, and 81 (96.4%) had monoclonal plasma cell proliferation. Plasma vascular endothelial growth factor levels were available for 32 patients with a median of 821 pg/mL (range, 26–12,900 pg/mL). Other common features included skin changes (54.2%), volume overload (71.4%), and organomegaly (72.6%). Of the 84 patients, 75 received initial treatment (local radiotherapy, 6 [8.0%]; chemotherapy, 17 [22.7%]; both chemotherapy and local radiotherapy, 9 [12.0%]), upfront autologous stem cell transplantation (ASCT), 43 (57.3%; with induction chemotherapy, n = 12, 16.0%; without induction chemotherapy, n = 31, 41.3%). The median follow-up duration was 40.7 months. The 5-year overall survival (OS) was 78%, and the 5-year progression-free survival (PFS) was 55%. Patients who underwent upfront ASCT and were diagnosed after 2014 had a longer OS and PFS. Conclusion: The demographics of Korean patients with POEMS syndrome were similar to those reported previously. Because of the introduction of new treatment agents and the reduced rate of transplant-related mortality related to ASCT, the treatment outcomes of Korean patients with POEMS syndrome have improved in recent years.

        5L-Scale Magnesio-Milling Reduction of Nanostructured SiO<sub>2</sub> for High Capacity Silicon Anodes in Lithium-Ion Batteries

        <P>Nanostructured silicon (Si) is useful in many applications and has typically been synthesized by bottom-up colloid-based solution processes or top-down gas phase reactions at high temperatures. These methods, however, suffer from toxic precursors, low yields, and impractical processing conditions (i.e., high pressure). The magnesiothermic reduction of silicon oxide (SiO2) has also been introduced as an alternative method. Here, we demonstrate the reduction of SiO2 by a simple milling process using a lab scale planetary-ball mill and industry-scale attrition-mill. Moreover, an ignition point where the reduction begins was consistently observed for the milling processes, which could be used to accurately monitor and control the reaction. The complete conversion of rice husk SiO2 to high purity Si was demonstrated, taking advantage of the rice husk's uniform nanoporosity and global availability, using a 5L-scale attrition-mill. The resulting porous Si showed excellent performance as a Li-ion battery anode, retaining 82.8% of the initial capacity of 1466 mAh g(-1) after 200 cycles.</P>

        Identification of Aspergillus nidulans from cooked eggs produced by permitted factory

        Fungus generally doesn`t produce toxic or harmful substances so it has low chances to cause food poisoning. However it leads to change appearance, odor and characteristics of the contaminated foods and result in sanitary risk problems. Therefore the contamination of fungi should be prevented since they are not proper for human consumption. Green fungi with white outline raised from the air cell of cooked eggs which were collected by Gyeongi Livestock Veterinary Service in August, 2006. The results came out after the cultivation using Sabouraud`s Dextrose Agar(SDA). The conidium appeared white and monospore, the shape of colony was round and oval. Conidiophore was brown and granulated and wrinkles and formed. It was confirmed as Aspergillus nidulans based on the dying using Lactophenol cotton blue, the observation of septum and vesicle from the grown spores, and rDNA sequencing

      • Bioactive Metabolites from the Fruits of Psoralea corylifolia.

        <P>Twenty-four metabolites, including seven new compounds (1-7), were isolated from the dried fruits of Psoralea corylifolia. On the basis of combined spectroscopic and chemical analysis, the new compounds were determined to be six flavonoids (1-6) and a meroterpenoid (7). The absolute configurations of the natural products obtained, including the previously undetermined 16 and 17, were assigned by several methods, such as NOE spectroscopy, optical rotation, and CD spectroscopy. Several of these compounds exhibited moderate inhibitory activity toward Staphylococcus mutans-derived SrtA (2, 6, and 16) and significant stimulation of SIRT1 activity (2, 3, and 15).</P>

      • Dental hygiene and COPD exacerbations

        Background: The aim of this study was to examine the relationship of dental brushing and COPD acute exacerbation in patients with mild to moderate COPD. Methods: This was a retrospective observational study. Data were obtained from the Korean National Health and Nutrition Examination Survey (KNHANES) (2007 ~ 2012) database. Mild to moderate COPD patients were defined as 50 % ≤ FEV1 <80%. Acute exacerbation of COPD was defined as emergency room visit or hospital admission with prescription of antibiotics and/or systemic corticosteroid within 3 days. We defined the exacerbators as the patients with history of exacerbations during 6 years, and the non-exacerbators as the patients without history of exacerbations during 6 years. We examined the tooth brushing before sleeping, whether tooth brushing or not per a day, the use of tooth floss, the use of interdental tooth brush. We survey whether tooth brushing pattern affect the exacerbation. Results: A total of 241 patients (10.1%) had experienced exacerbations among 2363 mild to moderate COPD patients. Exacerbators showed older ages compared to non-exacerbators (68 yrs vs. 62 yrs, p<0.001). Exacerbators showed lower frequency of tooth brushing before sleeping compared to non-exacerbators (16.2% vs. 24%, p=0.008). Exacerbators showed lower frequency of tooth brushing before survey day (93.4% vs. 97.8%, p=0.021). Exacerbators used tooth floss less compared to non-exacerbators (4.3%, vs. 13.7%, p<0.001). Odds ratio is 0.30 (95% CI 0.15-0.62, p=0.001) of dental floss users for COPD exacerbations compared to non-users. Conclusions: Dental floss user experienced less exacerbations than non-users. Dental floss might decrease COPD exacerbations. Further studies will be needed.

        PKC-β modulates Ca2+ mobilization through Stim1 phosphorylation

        Background: Calcium ions play a pivotal role in cell proliferation, differentiation, and migration. Under basal conditions, the calcium level is tightly regulated; however, cellular activation by growth factors increase the ion level through calcium pumps in the plasma membrane and endoplasmic reticulum for calcium signaling. Orai1 is a major calcium channel in the cell membrane of non-excitable cells, and its activity depends on the stromal interaction molecule 1 (Stim1). Several groups reported that the store-operated calcium entry (SOCE) can be modulated through phosphorylation of Stim1 by protein kinases such as extracellular signal-regulated kinase (ERK), protein kinase A (PKA), and p21-activated kinase (PAK). PKC is a protein kinase that is activated by calcium and diacylglycerol (DAG), but it remains unclear what role activated PKC plays in controlling the intracellular calcium pool. Objectives: Here, we investigated whether PKC-β controls intracellular calcium dynamics through Stim1. Methods: Several biochemical methods such as immune-precipitation, site directed mutagenesis, in vitro kinase assay were employed to investigate PKC interaction with and phosphorylation of Stim1. Intracellular calcium mobilization, via Stim1 mediated SOCE channel, were studied using in the presence of PKC activator or inhibitor under a confocal microscope. Results: Our data demonstrate that PKC interacts with and phosphorylates Stim1 in vitro. phosphorylation of Stim1 at its C-terminal end appears to be important in the regulation of SOCE activity in HEK293 and HeLa cells. Additionally, transient intracellular calcium mobilization assays demonstrate that the SOCE activity was inhibited by PKC activators or activated by PKC inhibitors. Conclusion: In sum, our data suggest a repressive role of PKC in regulating calcium entry through SOCE.

        마취과 전공의 수료자의 수련교육 만족도에 관한 설문조사 분석

        Bakground: Obtaining and utilizing the feed-backs from residents who have finished four year of anesthesia residency could well contribute to improvement in training program. Therefore authors have designed a self questionnaire to analyze the degrees or measures of satisfaction from such training program and data were evaluated to provide,in future,the guideline which would improve the quality of the training program. Methods: The self-questionnaires were sent to residents(n=148),who have been through the entire four year of residency training courses under anesthesia department. The assessment was conducted to measure the degree of satisfaction based on several variables such as motivation, selection of anesthesiology as a first choice, type of training hospital, and sex. To evaluate the current problems of anesthesia residency program, we made 30 open-ended and close-ended questions. Data analysis was made using Fishers exaet test. Results: There were no statistically significant difference between the degree of satisfaction and their motivation for choosing anesthesia, anesthesia as a major, selection of anesthesiology as a first choice, and types of hospitals. As for the difference in satisfaction of training, male residents showed significantly higher satisfaction rate( n=92, 36.2%) than female residents(n=47, 17%). Conclusions: These results suggest that degrees of satisfaction was more likely related to the program of each training hospital and sex compared to other variables studied.

        소아에서 호기말 탄산가스 분압 측정시 소아용 End-Tidal CO2 Uncuffed Tracheal Tube 의 사용효과에 대한 관찰

        To determine the accuracy of end-tidal PCO_2(PetCO_2) measurement analysed with Ohmeda 5210 capnometer in the pediatric patients whose lung were ventilated with Ohio Infant Circle Absorber and Ohio Ventilator 7200, we compared PetCO_2 measurements sampled from the proximal(PetCO_2-p) and distal(PetCO_2-d) ends of the tracheal tube to arterial PCO_2(PaCO_2) in 22 healthy pediatric patients between 6.8 to 18.5 kg. The results were as follows: 1) Proximal PetCO_2 and distal PetCO_2 correlated with PaCO_2(r=0.76; SEE=2.79; r=0.70; SEE=3.01, respectively)(p<0.05). 2) Arterial to end-tidal PCO_2 difference(delta(a-et)PCO_2) was significantyly greater with distal(3.53±4.68 mmHg) than proximal(0.35±2.72 mmHg) sampling(p<0.05). 3) In subgroup of patients whose proximal to distal concentration of nitrous oxide difference was more than 2%, the delta(a-et) PCO_2 using distal sampling(7.90±5.15 mmHg) was also significantly greater than it was using proximal sampling(-1.73±2.39 mmHg)(p<0.05). We conclude that distal estimates of PetCO_2-P can not provide acceptable estimate of PaCO_2 in healthy pediatric patients who are intubated with pediatric uncuffed tracheal tube.
