요추 수술 예정 환자에서 실시한 선택적 신경근 차단술의 5년 추시 결과

목적: 하지 방사통을 호소하는 환자의 수술 준비 과정에서 선택적 척추 신경근 차단술을 실시하고, 그 효과가 얼마나 오래 유지되는지를 알아 보고자 5년 이상 추시된 환자를 대상으로 분석하여 그 결과를 보고 하고자 한다. 대상 및 방법: 2000년 1월에서 2000년 12월까지 하지 방사통으로 타 병원 혹은 본원에서 수술적 치료를 권유받은 환자 중 수술 준비 과정에서 진단 및 치료 목적으로 선택적 척추 신경근 차단술을 시행한 환자를 대상으로 하였으며, 이중 5년 이상 추시가 가능하였던 69예를 대상으로 하였다. 결과분석은 진료기록부와 전화 인터뷰를 통해 하였으며. 신경차단술 시행군과 수술을 시행한 군의 치료결과 분석은 Kim's criteria (김의 평가기준), VAS(Visual analog scale) score를 이용하였다. 결과: 연구 대상 환자 69예 중 37예에서는 차단술 후에 증상 호전이 없어 수술을 시행하였고, 32예에서는 신경차단술 만으로 증상호전 되었다. 젊은 사람(60세 미만)에서 보존적 요법보다는 수술을 하는 경향이 많았으며, 이환 기간이 길수록(6개월 이상) 신경근 차단술 보다는 수술하는 경우가 높았다. 차단술 만으로도 호전된 32예의 환자 추시 결과상 다른 치료 없이 증상 호전이 지속되었던 환자는 17예(24.6%)였고 증상이 재발하여 추가 치료를 필요로 한 환자는 15예이었다. 이중 4예는 수술적 치료를 시행하였다. 결론: 수술이 예정된 하지 방사통 환자를 대상으로 수술 준비 기간 동안에 신경근 차단술을 시행하여 그 중 일부에서는 장기 추시에서도 증상 호전이 지속 되었다. 따라서 신경차단술은 수술 전에 한번 시도해 볼 만한 방법 중의 하나로 사료된다. Purpose: To evaluate the effectiveness of a selective nerve root block (SNRB) in patients with sciatica before surgery through a more than 5 year follow up. Materials and Methods: Between January 2000 and December 2000, patients with sciatica, who were recommended to undergo surgery to diagnose and treat sciatica by other clinics or at our hospital, were selected to undergo SNRB before surgery. Among them, 69 patients were followed up for 5 years. The analysis was based on the patients' medical records and telephone interviews, and the treatment results in the SNRB group and operation group were analyzed using Kim's criteria and the Visual Analog Scale score. Results: Among the 69 patients, there was no improvement in symptoms in 37 patients after SNRB. Therefore, they underwent surgery. The symptoms of the remaining 32 patients were improved by SNRB. Overall, younger patients and those with a longer symptom duration required surgery. Conclusion: SNRB was performed on patients with sciatica who were scheduled to undergo surgery. Some patients showed improvement in their symptoms in the long term. Therefore, SNRB should be considered as a treatment option prior to surgery.
A Strength Analysis of a Hull Girder in a Rough Sea

A ship in waves is suffered from the various wave loads that comes from its motion throughout its life. Because these loads are dynamic, the analysis of a ship structure must be considered as the dynamic problem precisely. In the rationally-based design, the dynamic structural analysis is carried out using dynamic wave loads provided from the results of the ship motion calculation as a rigid body. This method is based on the linear theory assumed low wave height and small amplitude of motion. But at the rough sea condition, high wave height, compared with ship's depth, induce the large ship motion, so the ship section configuration under waterline is rapidly changed at each time. This results in a non-linear problem. Considering above situation in this paper, a strength analysis method is introduced for the hull girder among waves considering non-linear hydrodynamic forces. This paper evaluates the overall or primary level of the ship structural dynamic loading and dynamic response provided from the non-linear wave forces, and bottom flare impact forces by momentum slamming theory. For numerical calculation a ship is idealized as a hollow thin-walled box beam using thin walled beam theory and the finite element method is used. This method applied to a 40,000 ton double hull tanker and attention is paid to the influence of the response of the ship's speed, wave length and wave height compared with the linear strip theory.
Case Reports : Two Cases of Giant Epidermal Cyst Occurring in the Neck

Epidermal cysts are the most common cysts of the skin. A conventional epidermal cyst rarely reaches a size of more than 5 cm in diameter. We report on two cases of giant epidermal cyst occurring in the neck. One patient had a cyst measuring 12×9×9 cm and the other patient had a non-pulsatile, dome-shaped lesion in the neck, which measured 6×5×3 cm. The lesions were totally excised. Histopathologically, both were confirmed as giant epidermal cysts. (Ann Dermatol 23(S1) S135~S138, 2011)

<P>A first-order shear deformation plate theory model of laminated composite plates subjected to a combined electromagnetic and thermal field is developed. The coupled equations of motion are based on the electromagnetic equations (Faraday, Ampere, Ohm, and Lorentz equations) and thermal equations which are involved in constitutive equations. In order to determine the implications of a number of geometrical and physical features of the model, one special case free vibration of a composite plate immersed in a transversal magnetic field is investigated. Special coupling characteristics between the magnetic, elastic and thermal fields are described in this paper. In addition, the free vibration responses of finite composite rectangular plates immersed in a transversal magnetic field are investigated. The vibration response characteristics of a composite plate are exploited in connection with the magnetic field intensity, thermal load, and electric conductivity of fibrous composite materials. Some pertinent conclusions, which highlight the various effects induced by the electromagnetic-thermo-elastic couplings, are outlined.</P>

<P> Mepivacaine is an aminoamide-linked local anesthetic with an intermediate duration that intrinsically produces vasoconstriction both in vivo and in vitro. The aims of this in-vitro study were to examine the direct effect of mepivacaine in isolated rat aortic rings and to determine the associated cellular mechanism with a particular focus on endothelium-derived vasodilators, which modulate vascular tone. In the aortic rings with or without endothelium, cumulative mepivacaine concentration-response curves were generated in the presence or absence of the following antagonists: N<SUP>ω</SUP>-nitro-l-arginine methyl ester [l-NAME], indomethacin, fluconazole, methylene blue, 1H-[1,2,4]oxadiazolo[4,3-a]quinoxalin-1-one [ODQ], verapamil, and calcium-free Krebs solution. Mepivacaine produced vasoconstriction at low concentrations (1 × 10<SUP>−3</SUP> and 3 × 10<SUP>−3</SUP> mol/L) followed by vasodilation at a high concentration (1 × 10<SUP>−2</SUP> mol/L). The mepivacaine-induced contraction was higher in endothelium-denuded aortae than in endothelium-intact aortae. Pretreatment with l-NAME, ODQ, and methylene blue enhanced mepivacaine-induced contraction in the endothelium-intact rings, whereas fluconazole had no effect. Indomethacin slightly attenuated mepivacaine-induced contraction, whereas verapamil and calcium-free Krebs solution more strongly attenuated this contraction. The vasoconstriction induced by mepivacaine is attenuated mainly by the endothelial nitric oxide - cyclic guanosine monophosphate pathway. In addition, mepivacaine-induced contraction involves cyclooxygenase pathway activation and extracellular calcium influx via voltage-operated calcium channels. </P>
Risk Factors for Postoperative Recurrence in Korean Patients with Crohn’s Disease

Background/Aims: A considerable number of patients with Crohn’s disease still need intestinal resection surgery. Postoperative recurrence is an important issue in Crohn’s disease management, including the selection of high-risk patients. Eastern Asian patients showed several differences from Caucasian patients. Therefore, we investigated the postoperative surgical recurrence outcome and identified risk factors in Korean patients. Methods: Clinical data of 372 patients with Crohn’s disease who underwent first intestinal resection between January 2004 and August 2014 at 14 hospitals in Korea were retrospectively reviewed. Results: Over the follow-up period, 50 patients (17.1%) showed surgical recurrence. The cumulative surgical recurrence rate was 6.5% at 1 year and 15.4% at 7 years. Age under 16 (p=0.011; hazard ratio [HR], 5.136; 95% confidence interval [CI], 1.576 to 16.731), colonic involvement (p=0.023; HR , 2.011; 95% CI, 1.102 to 3.670), and the presence of perianal disease at surgery (p=0.008; HR, 2.239; 95% CI, 1.236 to 4.059) were independent risk factors associated with surgical recurrence. Postoperative thiopurine treatment (p=0.002; HR, 0.393; 95% CI, 0.218 to 0.710) was a protective factor for surgical recurrence. Conclusions: Among the disease characteristics at surgery, younger age, colonic location, and perianal lesions were independent risk factors for surgical recurrence. Postoperative thiopurine treatment significantly reduced the incidence of surgical recurrence.
Floating-gate type organic memory device with organic insulator film of plasma polymerized styrene

<P>Plasma polymerized styrene (ppS) thin films were prepared and used as gate insulator and tunneling layer in a floating-gate type organic memory device. To investigate feasibility of the ppS thin film for application in non-volatile organic memory, an organic thin film transistor (OTFT) and a floating-gate type organic memory device were fabricated. Current voltage (I-V) characteristics of the OTTT and floating-gate type organic memory device were comparatively investigated, and hysteresis in the I-V characteristics of both devices was studied. A pseudo-charge-storage phenomenon was found for the ppS insulator thin film in the OTFT and floating-gate type organic memory device. The floating-gate type organic memory device revealed a reasonable hysteresis voltage of 27 V. It was confirmed that the ppS could be applied to fabricate an alternative floating-gate type organic memory device with promising memory function. (C) 2014 The Japan Society of Applied Physics</P>
