        Purpose : To evaluate the impact of added progestogen on biochemical markers of the bone turnover undergoing hormone replacement therapy in the postmenopausal women. Materials and Methods : There are 71 postmentopausal women who are divided into three groups (Group I, Group II, Group III). Group I(continuous combined)is given conjugated equine estrogen (CEE, 0.625mg, day 1-30) plus medroxyprogesterone acetate (MPA, 2.5mg, day 1-30), Group II(unopposed estrogen) is given conjugated equine estrogen (CEE, 0.625mg, day 1-30) plus medroxyprogesterone acetate (MPA, 5mg, day 1-12). And there were measured serum osteocalcin, serum total alkaline phosphatase and urinary deoxypyridinoline excretion before starting hormone replacement therapy and 12 months later. Results : The serum osteocalcin, serum total alkaline phosphatase and urinary deoxypyridinoline excretion significantly decreased after hormone replacement therapy (Group, I, II, III). And there was no significance in decreases in biochemical markers of the bone turnover among three groups. Conclusions : It was concluded that the impact of added progestogen on the biochemical markers of the bone turnover undergoing hormone replaced therapy in postmenopausal women was not significant than nothing added group. Thus we concluded that progestogen was not impact on decreasing on the biochemical markers of the bone turnover in postmenopausal women undergoing hormone replacement therapy.

      • Survival of Alzheimer's Disease Patients in Korea

        <P>Abstract</P><P><B><I>Background/Aims:</I></B> The natural history of Alzheimer's disease (AD) has rarely been studied in the Korean population. Our study on survival analyses in Korean AD patients potentially provides a basis for cross-cultural comparisons. <B><I>Methods:</I></B> We studied 724 consecutive patients from a memory disorder clinic in a tertiary hospital in Seoul, who were diagnosed as having AD between April 1995 and December 2005. Deaths were identified by the Statistics Korea database. The Kaplan-Meier method was used for survival analysis, and a Cox proportional hazard model was used to assess factors related to patient survival. <B><I>Results:</I></B> The overall median survival from the onset of first symptoms and from the time of diagnosis was 12.6 years (95% confidence interval 11.7-13.4) and 9.3 years (95% confidence interval 8.7-9.9), respectively. The age of onset, male gender, history of diabetes mellitus, lower Mini-Mental State Examination score, and higher Clinical Dementia Rating score were negatively associated with survival. There was a reversal of risk of AD between early-onset and later-onset AD, 9.1 years after onset. <B><I>Conclusions:</I></B> The results of our study show a different pattern of survival compared to those studies carried out with western AD populations. Mortality risk of early-onset AD varied depending on the duration of follow-up.</P><P>Copyright © 2013 S. Karger AG, Basel</P>

        Characteristics of Rapid Pyrolysis for Upgrading Heavy Oils in a Circulating Fluidized Bed Reactor

        <P>The hydrodynamic velocity and effect of the reaction temperature on the cracking performance Were investigated for the production of light oil through the rapid pyrolysis of heavy oil in a circulating fluidized bed reactor. From this study, the gas velocity in the loop seal to prevent slugging, the transport gas velocity to form a dense phase of solids on the bottom Of the -' riser, and the minimum bubbling fluidization velocity for stable solid circulation in the reheater were determined. Under the steady supply of feed through the internal mixing with steam inside the feed nozzle, the reactivity for a terriperattire change from 527 to 574 degrees C was investigated. As a result, the residue (>535 degrees C) conversion and liquid yield of up to 71.1 and 78.2%, respectively, were found. The maximum impurity rernoval rates for sulfur, Conradson carbon residue, and metals (nickel and vanadium) were found to be 1.8.7, 50.4, and 83.6%, respectively.</P>

      • Characteristics of slurry-phase hydrocracking for vacuum residue with reaction temperature and concentrations of MoS<sub>2</sub> dispersed catalysts

        <P><B>Abstract</B></P> <P>This study investigated the characteristics of slurry-phase hydrocracking of vacuum residue (VR >524°C) at a high conversion (40–95wt.%) with a change of reaction temperatures and concentrations of the dispersed molybdenum (Mo Conc. 100–2000wt.ppm) catalyst. Experiments were carried out with an initial hydrogen pressure of 80bar (at 80°C) at a reaction temperature from 385 to 440°C for four hours in a batch reactor. As results, it was found that the sediment formation is mainly dependent on the VR conversion by the reaction temperature and started over 70wt.% of VR conversion. Up to 70–80wt.% of VR conversion, the higher catalysts showed the higher asphaltene conversion and prevented the sediment formation at the boundary of its formation, but it didn’t show any effect over 80wt.% of VR conversion anymore. Based on the analysis of the structural change and ξ-potential of asphaltene from the liquid and sediment, and SARA compositional change in the liquid, the reason for sediment formation was explained.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Sediment formation started to form at near 70wt.% VR conversion. </LI> <LI> Higher concentration of catalyst delayed sediment formation. </LI> <LI> Change of resin content in the maltene was found to be significant to form sediment. </LI> <LI> ξ-potential showed the colloidal instability of asphaltene during SHC reaction. </LI> </UL> </P>

        Reaction kinetics of reduction and oxidation of metal oxides for hydrogen production

        <P><B>Abstract</B></P><P>Partial oxidation of methane and water splitting for hydrogen production were carried out. The reaction kinetics of reduction and oxidation (redox) of Mn and Zn iron oxides were determined using a thermogravimetric analyzer. The reduction conversion of the metal oxides increases with temperature above 1073K and all the experimental data are well represented by the diffusion-limited mechanism except ZnFe<SUB>2</SUB>O<SUB>4</SUB> due to the solid state diffusion of iron and the reaction with solid carbon. The oxidation reaction was also controlled by the product-layer diffusion mechanism. Substitution of Mn and Zn cations with iron oxides can lower the reaction temperature and increase the reaction rate. The reaction rate and the activation energy (57–110kJ/mol) for decomposition of water by oxidation of the metal oxides were also determined.</P>

        Effect of Ionic Surfactants on Improving Deasphalting Selectivity in a Nonpolar System

        <P>To reduce the amount of solvent used in the solvent deasphalting process, this study investigated the possibility of improving deasphalting selectivity at a low solvent/oil ratio (SOR) by modifying the stability of the asphaltene colloidal dispersion with ionic surfactants. To do this, an experiment was conducted by changing the solvent types (n-pentane and nheptane), temperature (35-90 degrees C), SOR (3-20, vol/vol), surfactants, and surfactant concentration (0.5-2.0 wt % of the feedstock). As a result, the zeta potential with sodium dodecyl sulfonate (SDS) was shown to decrease, while the zeta potential of pitch with cetylpyridinium chloride (CPCI) rose, as compared to those without surfactant. Also, the difference is bigger at a low SOR than at a high SOR. Finally, it was confirmed that, for SDS, the asphaltene content rose about 6% for the incremental 3% rise of pitch yield, while for CPCI, the asphaltene content rose 9-10% for the incremental 1.8% rise of the pitch yield, at the same time. The different movement of zeta potential and asphaltene content between SDS and CPCI can be explained by the adsorption mechanism.</P>

        Nilotinib is a second-generation tyrosine kinase inhibitor active in patients with chronic myeloid leukemia (CML) resistant to imatinib, and has been recently approved for newly diagnosed patients. We present a case of nilotinib-induced interstitial lung disease (ILD). A 67-year-old female patient was initially treated with imatinib for chronic-phase Philadelphia chromosomepositive (Ph+) CML. Imatinib was replaced by nilotinib because of hematological toxicity. The patient had received nilotinib for about 3 years without significant adverse effects. She visited the clinic due to chronic cough; chest X-ray revealed consolidations in both lung fields. Nilotinib-induced ILD was diagnosed based on intensive workup, including lung biopsy. She responded dramatically to corticosteroid therapy. To our knowledge, this is the first reported case of nilotinib-induced ILD in a patient with Ph+ CML. We emphasize that if unexplained lung abnormalities progress in patients receiving nilotinib, physicians should consider this potentially fatal complication in their differential diagnoses.

      • S-477 : Solitary Abdominal Tuberculous Lymphadenopathy Misdiagnosed as the Recurrence of Ovarian Cancer

        Monitoring cancer antigen 125 (CA-125) serum levels is very useful for determining how epithelial ovarian cancer is responding to treatment and for predicting a patient`s prognosis after treatment. An increase in CA-125 levels within patients in a remission is a strong predictor of the recurrence of ovarian cancer. However, while CA-125 is best known as a tumor marker for ovarian cancer, it may also be elevated in other various malignancies and in some benign conditions such as peritonitis, pancreatitis, chronic liver diseases, pelvic inflammatory diseases, or endometriosis. We describe herein a case of solitary intra-abdominal tuberculous lymphadenopathy mimicking a local relapse of epithelial ovarian cancer. A 51-year-old woman presented with progressive rise in serum CA-125. She had been diagnosed with advanced epithelial ovarian malignancy and undergone debulking surgery about 4 years ago. She had been given platinum-based adjuvant chemotherapy and achieved complete remission (CR). During regular follow-up in CR, a progressive rise in serum CA-125 was detected. The abdominal computed tomography (CT) showed a cystic mass, 3.6 cm in diameter, located adjacent to the right common iliac artery, suggesting a metastatic lymph node. We had planned second-look operation in settings of suspected focal recurrence of ovarian cancer, supported by evidence of rising CA-125 levels. However, the suspected tuberculous lesion was detected incidentally on preoperative chest CT. The patient had no symptoms and signs associated with tuberculosis (TB) and chest X-ray was also normal. We postponed the planned second-look operation and anti-TB medication was started. Mycobacterium TB was identified on culture of sputum specimens later. During anti-TB medication, the suspected metastatic isolated intra-abdominal lymphadenopathy improved and serum CA-125 level was also normalized. We emphasize here to consider not only the recurrence or progression of ovarian cancer itself but also benign conditions including TB in their differential diagnoses, when serum CA-125 level increases during the follow-up of epithelial ovarian cancer patients.
