Endobronchial Neurilemmoma Mimicking a Bronchial Polyp

Neurilemmomas are relatively uncommon, slowly growing tumors which originate from Schwann cells. Intrathoracic neurilemmomas often occur in the chest wall and posterior mediastinum, but endobronchial neurilemmomas are exceedingly rare. These tumors in trachea or bronchus are usually detected by radiologic examinations, mostly computed tomography scan of chest. An 88-year-old man was admitted for management of pneumonia in left lower lobe and parapneumonic effusion. On bronchoscopic examination, there was a small polypoid nodule less than 1 cm in diameter mimicking an endobronchial inflammatory polyp at the bifurcation of the right anterior segmental bronchus and lateral segmental bronchus and under auto-fluorescence imaging, the nodule showed reddish brown area with defined margin. The bronchoscopic biopsy revealed that the bronchial nodule was endobronchial neurilemmoma. This is an interesting case of endobronchial neurilemmoma mimicking a bronchial polyp that is detected incidentally via bronchoscopy.

Induction of NAD(P)H:(quinone-acceptor) oxidoreductase (QR) which promotes obligatory two electron reduction of quinones and prevents their participation in oxidative cycling and thereby the depletion of intracellular glutathione, has been used as a marker for chemopreventive agents. Induction of phase II enzyme is considered to be an important mechanism of cancer prevention. In our previous study, we assessed the quinone reductase QR-inducing activities of 216 kinds of medicinal herb extracts in cultured murine hepatoma cells, BPRc1 and hepa1c1c7 cells. Among the 216 herbal extracts tested in that study, extracts from Chrysanthemum zawadskii showed significant induction of QR. In this study, we examined QR-inducing activity of solvent fractions of the herbal extract. The dichloromethane fraction of the herb showed the highest QR induction among the samples fractionated with four kinds of solvents with different polarity. The fraction also significantly induced the activity of glutathione S-transferase (GST), one of the major detoxifying enzymes, at 4 μg/mL and 2 μg/mL in hepa1c1c7 and BPRc1 cells, respectively. In conclusion, dichloromethane-soluble fraction of Chrysanthemum zawadskii which showed relatively strong induction of detoxifying enzymes merits further study to identify active components and evaluate their potential as cancer preventive agents.

Enhancer of polycomb1 acts on serum response factor to regulate skeletal muscle differentiation.

<P>Skeletal muscle differentiation is well regulated by a series of transcription factors. We reported previously that enhancer of polycomb1 (Epc1), a chromatin protein, can modulate skeletal muscle differentiation, although the mechanisms of this action have yet to be defined. Here we report that Epc1 recruits both serum response factor (SRF) and p300 to induce skeletal muscle differentiation. Epc1 interacted physically with SRF. Transfection of Epc1 to myoblast cells potentiated the SRF-induced expression of skeletal muscle-specific genes as well as multinucleation. Proximal CArG box in the skeletal alpha-actin promoter was responsible for the synergistic activation of the promoter-luciferase. Epc1 knockdown caused a decrease in the acetylation of histones associated with serum response element (SRE) of the skeletal alpha-actin promoter. The Epc1.SRF complex bound to the SRE, and the knockdown of Epc1 resulted in a decrease in SRF binding to the skeletal alpha-actin promoter. Epc1 recruited histone acetyltransferase activity, which was potentiated by cotransfection with p300 but abolished by si-p300. Epc1 directly bound to p300 in myoblast cells. Epc1+/- mice showed distortion of skeletal alpha-actin, and the isolated myoblasts from the mice had impaired muscle differentiation. These results suggest that Epc1 is required for skeletal muscle differentiation by recruiting both SRF and p300 to the SRE of muscle-specific gene promoters.</P>

본 연구는 인공호흡에 영향을 미치는 감염요인을 분석하기 위해 수행했다. OO대학교 응급구조학과 재학생 157명을 대상으로 NAVER Office를 활용한 인터넷 비대면 설문조사를 통해 108건의 응답을 얻었다. 입대입, 페이스쉴드, 포켓마스크 인공호흡 방법별로 환자의 체액유무, 확진, 의심, 비감염 및 응답자의 성별, 심폐소생술 훈련횟수, 심폐소생술 제공유무에 따른 시행 의사를 Chi-Square test로 분석했다. 전체적으로 비감염, 의심, 확진 환자 및 포켓마스크, 페이스쉴드, 입대입 인공호흡 순으로 시행 의지가 컸고 심폐소생술 제공 경험이 없는 경우에도 시행 의지가 컸다. 심폐소생술 제공경험이 없는 경우에 체액이 보이지 않는 비감염 환자에 대한 입대입 인공호흡 시행 의지가 컸고 통계적으로 유의한 차이(85.5 vs. 65.6%; p=.019)가 있었다. 심폐소생술 제공경험이 없는 경우에 체액이 보이는 비감염 환자에 대한 페이스쉴드 인공호흡 시행 의지가 컸고 통계적으로 유의한 차이(71.0 vs. 50.0%; p=.036)가 있었다. 심폐소생술 제공경험이 없는 경우에 체액이 보이지 않는 감염 환자(61.9 vs. 37.5%; p=.020), 체액이 보이는 감염 의심 환자(59.9 vs. 37.5%; p=.039), 체액이 보이지 않는 감염 의심 환자(68.4 vs. 43.7%; p=.016), 체액이 보이는 비감염 환자(81.6 vs. 50.0%; p=.001), 체액이 보이지 않는 비감염 환자(98.7 vs. 87.5%; p=.026)에 대한 포켓마스크 인공호흡 시행 의지가 컸고 통계적으로 유의한 차이가 있었다. This study hypothesized that conditions related to infection negatively affected on performing artificial respirations and analyzed these factors. A total of 157 paramedic students at a university were invited to use NAVER Office to access an online survey and 108 responses were received. The intention to perform cardiopulmonary resuscitation (CPR) was asked to answer yes or no depending on the certainty of the patient’s infection including existence of body fluids, confirmed infection, suspected infection, not infected and artificial respiration methods including mouth to mouth, face shield, pocket mask. The collected data were analyzed by Chi-Square test or Fischer’s exact test. There was a statistically significant difference (p=.019) in the intention to perform mouth to mouth ventilation according to the experience of performing CPR in non-infected patients without bodily fluids. There was a statistically significant difference (p=.036) in the intention to perform face shield ventilation according to the experience of providing CPR in non-infected patients with bodily fluids visible. The intention to perform pocket mask ventilation according to the experience of providing CPR was significantly different in infected patients with no visible bodily fluids (p=.020), patients suspected of having an infection with visible bodily fluids (p=.039), patients suspected of having an infection with no visible bodily fluids (p=.016), non-infected patients with visible bodily fluids (p=.001).

As the result of the study to overcome the disadvantage of administration and therapy in medical fields, the most interesting field has been appeared in the polymers decomposed without having injurious effects on the human body. Poly DL-lactide, poly glycolide and poly (Dl-lactide-co-glycolide)(DL-PLGA) are suited for the medical treatment purpose. Generally the excessive dose of conventional drugs in the medical treatment has caused side effects in an human body as well as the disadvantage of ailments required of the frequent administrations. But the advent of drug delivery of a constant dose long. In this study, the release experiments of drug were operated in the phosphate buffer solutions of pH 7.4 and 37.0±0.5℃ by using micelles prepared in poly(DL-lactide??-co-glycolide??) with clonazepam for delivery drug. The lineary release time according to the kinds of micelles prepared with the copolymer/drug ratio of 20:40, 20:20, and 40:20 was delayed in the order of 20:40>20:20>40:20. Amount of drug release accoding to the kinds of micelles showed similar patterns. In short, the fomulation allows polymeric micelles to suppress the burst effect of the drug release mechanism, which led to the controlled release pattern and micelles.
