We observed clinical features in 43 patients with acute metabolic complications of diabetes mellitus, diabetic ketoacidosis 36 cases and hyperosmolar nonketotic coma 7 cases who were diagnosed and treated in Chunangnam National University Hospital from Jan. 1988 to Dec. 1992. The range of age of the patients with diabetic ketoacidosis was from 2nd decade to 9th decade. The diabetic ketocacidosis was most frequent in the 4th decade(22.2%). But the range of patients with hyperosmolar nonketotic coma was from 6th decade to 9th decade and that compications was most frequent in 6th decade. There were no significant sex difference in the groups of acute metabolic complications. The altered consciousness was the most common complaints both groups of diabetic ketoacidosis and hyperosmolar nonketotic coma. Omission of insulin therapy, infection, and fasting were the most common precipitating factors for the development of diabetic ketoacidosis and hyperosmolar nonketotic coma. Only 3(8.3%) out of 36 patients with diabetic ketoacidosis were presented as the initial manifestations of diabetes mellitus. The mortality rates of the patients with diabetic ketoacidosis and hyperosmolar nonketotic coma 5.6% and 28.6%, respectively.
Fimasartan attenuates renal ischemia-reperfusion injury by modulating inflammation-related apoptosis

Fimasartan, a new angiotensin II receptor antagonist, reduces myocyte damage and stabilizes atherosclerotic plaque through its anti-inflammatory effect in animal studies. We investigated the protective effects of pretreatment with fimasartan on ischemia-reperfusion injury (IRI) in a mouse model of ischemic renal damage. C57BL/6 mice were pretreated with or without 5 (IR-F5) or 10 (IR-F10) mg/kg/day fimasartan for 3 days. Renal ischemia was induced by clamping bilateral renal vascular pedicles for 30 min. Histology, pro-inflammatory cytokines, and apoptosis assays were evaluated 24 h after IRI. Compared to the untreated group, blood urea nitrogen and serum creatinine levels were significantly lower in the IR-F10 group. IR-F10 kidneys showed less tubular necrosis and interstitial fibrosis than untreated kidneys. The expression of F4/80, a macrophage infiltration marker, and tumor necrosis factor $(TNF)-{\alpha}$, decreased in the IR-F10 group. High-dose fimasartan treatment attenuated the upregulation of $TNF-{\alpha}$, interleukin $(IL)-1{\beta}$, and IL-6 in ischemic kidneys. Fewer TUNEL positive cells were observed in IR-F10 compared to control mice. Fimasartan caused a significant decrease in caspase-3 activity and the level of Bax, and increased the Bcl-2 level. Fimasartan preserved renal function and tubular architecture from IRI in a mouse ischemic renal injury model. Fimasartan also attenuated upregulation of inflammatory cytokines and decreased apoptosis of renal tubular cells. Our results suggest that fimasartan inhibited the process of tubular injury by preventing apoptosis induced by the inflammatory pathway.

The risk of psoriasis in patients with allergic diseases: a nationwide population-based cohort study

Background: The spectrum of allergic diseases includes atopic dermatitis (AD), allergic rhinitis (AR), and asthma. To date, the association between allergic diseases and psoriasis has not been evaluated. Objectives: This study was conducted to determine the risk of psoriasis in patients with allergic diseases. Methods: This was a nationwide population-based cohort study in which data of 9,718,722 subjects ≥20 years of age were analyzed. Results: The incidence of psoriasis per 1,000 person-years was 9.57, 3.78, and 4.28 in the AD, AR, and asthma groups, respectively. The AD group exhibited a significantly increased risk of developing psoriasis (hazard ratio [HR], 3.18; 95% confidence interval [95% CI], 3.05-3.31) (P<0.001) after adjustment for confounding factors. The risk of psoriasis was significantly increased in the AR group (HR, 1.32; 95% CI, 1.31-1.34) (P<0.001) and asthma group (HR, 1.30; 95% CI, 1.27-1.33) (P<0.001). The risk of developing psoriasis tended to increase with the number of accompanying allergic diseases. In the presence of all three allergic diseases, the HR of psoriasis was 3.16 (95% CI, 2.70-3.69) (P<0.001). Conclusion: Allergic diseases, particularly AD, may be a risk factor for psoriasis.
Integrated circuit (IC)‐embedded wafer‐level packaging technology for millimeter‐wave power ICs

<P><B>Abstract</B></P><P>A wafer‐level packaging (WLP) technology for millimeter‐wave power devices is presented, which features simple fabrication, low‐signal transition loss, and thermal consideration. Instead of using a rigid interposer, the proposed WLP technology incorporates thick copper sheet and an adhesive tape as the initial carrier. Short electroplated interconnection showed transition loss within 0.2 dB and return loss above 20 dB, at frequency range up to 30 GHz. We demonstrate the WLP with an RF power amplifier at 28 GHz. The power‐added efficiency (PAE) of the power amplifier packaged in the proposed technology is 30% at an output power of 27.5 dBm, which is identical to an unpackaged integrated circuit (IC) wire bonded on a printed circuit board. The size of the IC, and the final packaged sample was 2.2 × 1.5 × 0.1 mm<SUP>3</SUP> and 3.3 × 2.4 × 0.2 mm<SUP>3</SUP>, respectively.</P>
Adaptive Diversity of Echinochloa Species to Osmotic Stress

Three Echinochloa species inhabit various crop fields with different soil moisture conditions. Therefore, a growth pouch test was conducted to investigate adaptive diversity of six Echinochloa species, three from Korea and three from USA, to osmotic stress by assessing shoot and root growths. Echinochloa crus-galli var. praticola showed the greatest tolerance to osmotic stress in both root (GR50=1316.3 g PEG L<SUP>−1</SUP>) and shoot (GR50=212.2 g PEG L<SUP>−1</SUP>) growths, while Korean E. oryzicola was most sensitive to osmotic stress in both root (GR50=116 g PEG L<SUP>−1</SUP>) and shoot (GR50=126.2 g PEG L<SUP>−1</SUP>) growths. Root to shoot (R/S) ratio of Echinochloa crus-galli var. praticola increased with increasing osmotic stress, while that of Korean E. oryzicola decreased, suggesting that R/S ratio is closely related to osmotic stress tolerance in Echinochloa species. Our results clearly demonstrate that E. crus-galli var. praticola maintains high R/S ratio even under high osmotic stress, which enables this species to well adapt to dry upland condition. In contrast, while E. oryzicola fails to maintain sufficiently high R/S ratio, resulting in poor adaptability to dry upland condition.

Background: Ultrasonographic imaging is an efficient tool for diagnosing subcutaneous mass without invasive procedure. However, there are few studies which applied deep neural network to skin ultrasound images. Objectives: The aim of this study was to evaluate the accuracy of deep neural network in diagnosing epidermal cyst, lipoma and other subcutaneous masses. Methods: We created a dataset of 1317 skin ultrasound images from 198 patients who diagnosed as epidermal cyst, lipoma and the others for learning deep neural network. Performance was evaluated using another set of 95 images (26 ultrasound images of epidermal cysts, 27 of lipoma and 44 of the others) from published articles. Results: The overall accuracy of deep neural network was 86.3%. The accuracy in diagnosis of epidermal cyst, lipoma and the others were 95.83%, 66.67% and 95.35%, respectively. As a result of analysis with GradCam, the deep neural network detected posterior enhancement, hypoechoic feature, and well-defined margin in epidermal cyst. Conclusion: We showed that deep neural network combined with ultrasound might be helpful for clinicians diagnosing subcutaneous masses.
