        육군 병사들의 복무기간에 따른 불안, 우울 및 충동성에 대한 연구

        연구목적과 방법 : 저자는 군복무기간에 따른 우울, 불안, 충동성향을 평가하기 위하여 548명의 병사들에게 Beck의 우울척도(BDI), Zung이 개발한 자가평정 불안척도(SAS), Barratt이 개발한 충동성 검사(BIS)를 실시하였다. 일반 병사들은 다음과 같이 3군으로 나뉘어졌다. 제1군 : 군복무기간이 8개월이하인 병사 제2군 : 군복무기간이 9개월에서 16개월인 병사 제3군 : 군복무기간이 17개월이상인 병사 결 과 : 1) 제1군의 우울척도 총점(F value=3.60, p<.05)과 부정적 자기상/정신운동지연의 하부척도 점수(F value=5.82, p<.01)가 제2군의 것보다 의미있게 더 높았다. 2) 제1군의 자가평정 불안척도 점수가 다른 두 군에 비해서 의미있게 높았다(R value=5.80, p<.01). 3) 제1군의 총충동점수(F value=3.42, p<.05)와 운동충동점수(F value=3.29, p<.05)가 제3군에 비해 의미있게 높았다. 결 론 : 비록 병사들의 각 척도별 측정평균점수는 정상범위에 속할지라도, 이번 연구의 결과는 제1군의 경우 다른 두 군에 비해 더 우울하고, 불안하고, 충동적이라는 것을 말한다. 따라서 군복무기간중 사고를 방지하기 위해서, 우리는 군복무기간이 8개월 이하인 사병의 경우 더 많은 주의와 관심을 가지고 관리해야 되겠다. Objectives & Methods : The authors applied Beck's Depression Inventory(BDI), Self-rating Anxiety Scale(SAS) and Barratt Impulsiveness Scale(BIS) to 548 enlisted men to evaluate the differences of anxious, depressive and impulsive trends according to the duration of the military service. The enlisted men were divided into 3 groups as follows : Group 1 : those who had served under 8 months. Group 1 : those who had served from 9 to 16 months. Group 1 : those who had served over 17 months. Results : 1) The scores of BDI(F=3.60, p<.05) and negative self image and psychomotor retardation subscale(F=5.8, p<.01) of group 1 were significantly higher than those of group 2, respectively. 2) The SAS score of group 1 was significantly higher than that of others(F=5.80, p<.05). 3) The scores of total BIS(F=3.42, p<.05) and impulsiveness-motor subscale(F=3.29, p<.05) of group 1 were significantly higher than those of group 3, respectively. Conclusion : In spite of their scores were in normal range, these results suggest that the enlisted men of group 1 are more anxious, depressive and impulsive than those of the others. So we should give more attention to them and offer more concerns for them to protect the military accident.

        전북대학교병원 소아치과에 내원한 교정환자 보호자들의 의삭에 관한 설문조사

        최근 치의학의 눈부신 발전과 소득의 증가에 따른 일반인의 의식 변화로 외모에 대한 관심도가 높아지고 있다. 본 연구에서는 대학병원에 내원한 동기와 교정치료를 시작하게 된 동기,치료방법 및 치료기간을 파악하여,이를 토대로 보호자와 보다 적절한 의사소통을 위한 정보를 파악하고,보호자들의 교정치료에 대한 기대를 파악하고자 본 연구를 시행하였다. 이를 위해 전북대학교 치과병원 소아치과에 내원한 교정환자의 보호자 150명을 대상으로 조사 연구한 결과 다음과 같은 결론을 얻었다. 1. 본 병원의 소아치과를 찾게 된 이유 중 대학병원이 좋을 것 같아서가 52.1%. 친지나 아는 사람의 권유가 25%. 다른 치과병원의 소개가 16.7%로 나타났다. 2. 내원 전 예상하였던 치료기간 중 2년 이상이 37.5%. 12∼18개월이 12.5%로 나타났다. 3. 교정치료를 받으려는 이유 중 부모가 부정교합을 발견하고 걱정스러워서가 58.3%, 주위사람들의 지적이 12.5%로 나타났다. 4. 소아치과 내원 환자의 치료방법은 구강 내 고정성장치가 41.7%, 가철성장치가 29.2% 구외 장치가 2.1%를 차지했다. 5. 예약 후 치료를 위해 기다리는 시간 중 5∼10분이 39.6%, 15∼30분이 4.1%로 나타났으며,기다리는 시간은 어느 정도까지 괜찮다고 생각하는가에 대해 5∼10분이 60.4%, 15∼30분이 2.1%로 나타났다. Recently, in proportion to the remarkable development of dentistry and income increases it is growing more and more a concern about changed awareness in appearance, In this study, it had a grasp of the purpose for university hospital visiting, the motive of the commencement for orthodontic treatment and a method of the treatment as well as term. Based on these, the aim of this study is to keep more of the information between parent and doctors for mutual understanding and to grasp the characteristics for the needs of orthodontic treatment. In order to conduct researches, there has made a survey of 150 persons among orthodontic patients' parents who visit CBNU hospital, the pediatric dentistry. The study has found the results like these. 1. There was a question about the reason to visit CBNU hospital in the department of pediatric dentistry for orthodontic treatment. 52.1% of respondents, the survey found, were more likely to receive a good medical service. 25% of them were counselled from a relative or an acquaintance. 16.7% of them were recommended by another dental clinic. 2. There was a question about the expected orthodontic treatment period, when at first hospital visiting. 37.5% of the respondents answered that it was a 'more than 2 years', 12.5% of them said 'from 12 months to 18 months'. 3. There was a question about the reason to receive orthodontic treatment. 58.3% of the respondents, the survey found, answered the reason was parents' concern about the malocclusion of their children, 12.5% of them said a the orthodontic problem pointed out by entourages. 4. There was a question about the method of orthodontic treatment for patients who visit the department in pediatric dentistry. 41.7% of the respondents said that it was used as 'an intra-oral fixed appliance', 29.2% of them said 'an intra-oral removable appliance', 2.1% of them said 'an extra-oral appliance'. 5. There was a question about the waiting time for treatment after a dental appointment. 60.4% of the respondents said 'from 5 minutes to 10 minutes', 4.1% of them said 'from 15 minutes to 30 minutes'. There was a question about the extent of an acceptable waiting time. It was answered to 'from 5 minutes to 10 minutes' by 60.4% of them, 2.1% of them said 'from 15 minutes to 30 minutes'.

        혼합치열기의 miniscrew를 이용한 교정치료

        Anchorage plays an important role in orthodontic treatment, Skeletal anchorage like the miniscrew is considered a more effective method in anchorage control than conventional anchorage which needs much patient s cooperation. The miniscrew offers many advantages. 1) It is easy to insert and to remove. 2) It can endure the force needed for moving teeth. 3) It can be immediately loaded and 4) Patient cooperation is not needed. 5) It is economic compared to other skeletal anchorage systems, In comparison to adult's bones, children s bones have comparatively poor bone quality and quantity. Therefore, it is hard to obtain primary stability in younger patients. However, if the miniscrew can be retained successfully, it will be effective in many orthodontic treatments. In these cases we used the miniscrew in correcting of diastema, in aligning dental midline, and in rendering a forced eruption of impacted tooth in mixed dentition patient. We obtained satisfactory results. 교정치료 시 고정원은 항상 염두에 두면서 치료를 진행해야 하는 중요한 요소이다. 전통적인 고정원을 이용한 치료는 환자의 적극적인 협조를 필요로 하고 원하는 치아이동에 대한 반작용이 나타날 수 있는 등의 부작용을 가지고 있다. 그러므로 보다 견고한 구강 내 고정원이 요구되어져 왔으며 이를 위해 miniscrew가 임상에서 흔히 이용되고 있다. Miniscrew를 통한 고정원의 확보는 구외력 의존도 감소,치료기간의 단축,식립 후 즉시 교정력 적용,환자의 협조도 불필요,식립의 간편성,저렴한 비용 등의 장점을 가지고 있다. 그러나 성장기 환자는 성인에 비해 골질이 좋지 않아 miniscrew의 성공률이 성인에 비해 낮다. 그러나 일단 고정에 성공하면 많은 교정치료 시 매우 유용하게 이용할 수 있다. 본 증례는 흔합치열기 환자를 miniscrew를 이용하여 정중이개 및 정중선 불일치,매복치에 대한 양호한 교정치료를 하였기에 보고하는 바이다.

      • 만성 화농성 중이염의 세균학적 고찰

        Background and Objectives : Chronic suppurative otitis media is one of the frequent diseases in otolaryngologic field. It is important to choose of antibiotics in the management of infectious disease. But the organisms in infections have been changed and resistance to antibiotics has been grown as the development of antibiotics has been achieved. And so it has been necessary to recognize the changes of organisms and resistance in antibiotics. Our study was performed to identify the pathogens isolated from discharges in chronic otitis media and evaluate the antibiotic agents, to recognize the change of to resistance to·antibiotic agents and to use the appropriate anitiotics. Materials and Methods : Retrospectively, authors reviewed the backeriologic study of 114 cases of chronic suppurative otitis media who visited the Department of Otolaryngology, Soonchunhyang University in Seoul from Nov. 1996 to Oct. 1999 and analyzed 101 strains and its sensitivity test to various antibiotics. Result : In 93 cases in which pathogenic organism was isolated, single infection was 85 cases(91.4%) and mixed infection was 8 cases(8.6%). The most frequent pathogenic organism was Staphylococcus aureus(59.4%), Pseudomonas aeruginosa(13.9%), Providencia(4.8%), Alcaligenes(3.0%) and Streptococcus pneumoniae(2.9%) were the next. Methicilline-Resistance Staphylococcus aureus was 34 cases(57.6%) of Staphylococcus aureus. Staphylococcus aureus was sensitive to Vancomycin(96.7%), Trimethoprim-Sulfamethoxazole (57.6%), Cefuroxime(42.4%) and Ciprofloxacin(40.7%) but resistant to Penicillin(94.9%). Pseudomonas aeruginosa was sentitive Amikacin(100%), Ciprofloxacin(100%) and Gentamicin(67%). Conclusion : As MRSA recently was wide spread in community accquired infection as well as nosocomial infection, we must consider strict control of MRSA.

        Reliable permeability assay system in a microfluidic device mimicking cerebral vasculatures.

        <P>Since most of the bioavailable drugs are impermeable through the blood-brain barrier (BBB), development of a rapid and reliable permeability assay system has been a challenge in drug discovery targeting central nervous system (CNS). Here, we designed a microfluidic device to monitor the drug permeability into the CNS. Human umbilical vein endothelial cells (HUVECs) were shortly (2?~?3 h) incubated with astrocyte-conditioned medium after being trapped on microholes in the microfluidic device and tested for chip-based permeability measurement of drugs. The measured permeability values were highly correlated with those measured by conventional in vitro methods and the brain uptake index representing the quantity of transported substances across the in vivo BBB of rats. Using the microfluidic device, we could easily monitor the effect of hydrogen peroxide on the trans-endothelial permeability, which are consistent with the finding that the same treatment disrupted the formation of tight junctions between endothelial cells. Considering relatively short period of time needed for endothelial cell culture and ability to monitor the BBB physiology continuously, we propose that this novel system can be used as an invaluable first-line tool for CNS-related drug development.</P>

      • Cancer-derived exosomes trigger endothelial to mesenchymal transition followed by the induction of cancer-associated fibroblasts

        <P><B>Abstract</B></P> <P>Cancer-associated fibroblasts (CAFs) play a pivotal role in tumor growth, but very little has been known about its characteristics and origin. Recently, cancer-derived exosome has been suggested to transdifferentiate CAFs, by a new mechanism of endothelial to mesenchymal transition (EndMT), initiating angiogenic processes and triggering metastatic evolution. However, an enabling tool <I>in vitro</I> is yet to be developed to investigate complicated procedures of the EndMT and the transdifferentiation under reconstituted tumor microenvironment. Here we proposed an <I>in vitro</I> microfluidic model which enables to monitor a synergetic effect of complex tumor microenvironment <I>in situ</I>, including extracellular matrix (ECM), interstitial flow and environmental exosomes. The number of CAFs differentiated from human umbilical vein endothelial cells (HUVECs) increased with melanoma-derived exosomes, presenting apparent morphological and molecular changes with pronounced motility. Mesenchymal stem cell (MSC)-derived exosomes were found to suppress EndMT, induce angiogenesis and maintain vascular homeostasis, while cancer-derived exosomes promoted EndMT. Capabilities of the new microfluidic model exist in precise regulation of the complex tumor microenvironment and therefore successful reconstitution of 3D microvasculature niches, enabling <I>in situ</I> investigation of EndMT procedure between various cell types.</P> <P><B>Statement of Significance</B></P> <P>This study presents an <I>in vitro</I> 3D EndMT model to understand the progress of the CAF generation by recapitulating the 3D tumor microenvironment in a microfluidic device. Both cancer-derived exosomes and interstitial fluid flow synergetically played a pivotal role in the EndMT and consequent formation of CAFs through a collagen-based ECM. Our approach also enabled the demonstration of a homeostatic capability of MSC-derived exosomes, ultimately leading to the recovery of CAFs back to endothelial cells. The <I>in vitro</I> 3D EndMT model can serve as a powerful tool to validate exosomal components that could be further developed to anti-cancer drugs.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>
