<P><B>Abstract</B></P> <P><B>Introduction</B></P> <P>Mounting evidence indicates a significant relationship between Sasang constitutional types (SCTs) and metabolic syndrome (MetS), and between metabolic hormones and risk of MetS. However, studies have not yet examined whether the impact of metabolic hormones on MetS differs according to SCT. The aim of this study was to investigate whether the association of adiponectin, ghrelin, leptin with MetS and metabolic components is affected by SCTs.</P> <P><B>Methods</B></P> <P>A total of 2370 subjects (1201 men and 1169 women) who participated in the Korean Genome and Epidemiology Study-Ansan cohort between 2007 and 2008 and completed the health-related questionnaire, anthropometric evaluation, and biochemical measurements were included in the analysis. Each constitutional type was defined using an integrated diagnostic method. Total adiponectin, ghrelin, and leptin levels were determined by enzyme-linked immunosorbent assay or radioimmunoassay.</P> <P><B>Results</B></P> <P>Tae-eum (TE) type had the highest frequency of MetS (29.9%). In multivariate logistic regression analysis adjusted for possible confounders, adiponectin (OR = 0.88, 95% CI: 0.84–0.92; <I>P</I> < 0.0001) and leptin levels (OR = 1.05, 95% CI: 1.01–1.08; <I>P</I> = 0.015) were significantly associated with MetS in TE type, whereas only adiponectin levels were significantly associated with MetS in So-yang type (OR = 0.81, 95% CI: 0.73–0.90; <I>P</I> < 0.0001). Ghrelin levels were not significantly associated with MetS in all SCTs.</P> <P><B>Conclusions</B></P> <P>This population-based study showed different association of adiponectin, ghrelin, and leptin with MetS and its components according to SCT.</P>
Proteomic analysis of plasma proteines from patients with Obstructive sleep apnea

Obstructive Sleep Apnea(OSA) is characterized by episode of complete or partial pharyngeal obstruction during sleep and deeply related to cardiovascular disease and cerebrovascular disease. Many studies show that Hypoxia, Hypertension, and the production of superoxide produced by elevated homocysteine concentration may damage to vascular endothelium. In this study we performed proteomic-based approach to characterize the plasma protein associated with wound healing mechanism. First, overnight polysomnographic study was carried out in sleep laboratory, using standard recording techniques. Five cases of OSA patients and 5 normal controls were analyzed by two dimensional gel electrophoresis (pH 3-10NL, 8-18% gradient). Protein spots were cut from 2-D gels stained by Coomasie blue, and were identified by using matrix-assisted laser desorption ionization mass spectrometry(MALDI-MS). The results were compared with those of paired normal controls. Comparative analysis of the respective spot patterns in 2DE shows that Vitronectin which plays an important role in repairing wounded vessel was increased. Following protein was also increased in OSA patients : lg alpha-1 chain C region

Obstructive sleep apnea(OSA) occcurs when tissues in the upper airway collapse during sleep, and is accompanied by hypoxemia and EDS(excessive daytime sleepiness). Also OSA is related to hypertension and cardio-vascular disease. According to recent study, OSA possibility is estimated through partial gene"s predispoding factor. To study the correlation of OSA and angiotensin converting enzyme and angioten-sinogen genotype, this study examined genetic polymorphism of OSA patient and normal case using PCR and restriction enzyme digestion. We found significant differences between most sevene obstructive sleep apnea and normal control in the ACE genotypes. The freguency of genotypes in MSO was: DD : 15.5%, ID : 82.2%, Ⅱ : 2.2%(p<0.05) The freguency of genotypes in Normal group(NO) was: DD : 8.5%, ID : 71.8%, Ⅱ : 19.7%. But there were no significant differences between obstructive sleep group and normal control in the AGT(M235T) genotypes. The freguency of genotypes in MSO was : MM : 8.9%, MT : 13.3%, TT : 77.8%. In normal group was: MM : 5.6%, MT : 25.4% TT : 69.0%. Our data suggest some effect between ACE polymorphism and OSA but no effect between AGT polymorphism and OSA.

Cytoglobin(Cygb) is a newly recognized globin type, which are almost ubiquitously present in human tissues. Cytoglobin is reported to function as an oxygen scavanging and distribution protein and induced by hypoxia. The physiological role of cytoglobin is not yet clearly understood. We investigated cytoglobin in human blood leukocyte from obstructive sleep apnea patients(n=5) and normal controls(n=5). Immunoreactivity to anticytoglobin in OSA patients were much higher than that in normal control. These results suggest that cytoglobins of leukocyte might be overexpressed in obstructive sleep apnea.

<P>Summary</P><P>Obstructive sleep apnoea (OSA) is a common syndrome, which is associated with a number of medical problems that impact morbidity and mortality. Although the precise mechanisms that underlie these associations are not fully understood, previous studies have suggested that patients with OSA show elevations of several biomarkers that are associated with cardiovascular risk. This study was undertaken to identify serum proteins associated with OSA by using a proteomics technique and to examine changes in identified protein levels after continuous positive airway pressure treatment. The study participants consisted of 40 male patients (aged 40–49 years) with severe OSA and 34 male control subjects matched for age and body mass index. All subjects underwent polysomnography. Using a proteomics approach, we identified nine proteins that were differentially expressed in patients with severe OSA and controls. Three of these nine proteins, haptoglobin, paraoxonase-1 and apolipoprotein M, were quantified by using enzyme-linked immunosorbent assays, kinetic assays and by using Western blotting. Multiple regression analysis showed that haptoglobin and apolipoprotein M levels are independently related to apnoea–hypopnoea index (<I>P</I> < 0.01). A further study is required to determine the nature of associations between these identified proteins and OSA in a large population.</P>
