        Shape Optimization Method of Free-form Cable-braced Grid Shells Based on the Translational Surfaces Technique

        Cable-braced grid shells, being a new type of single-layer reticulated shell, are widely used in glass roofs. However, research on the shape optimization of free-form cable-braced grid shells is relatively lacking. This paper describes a shape optimization method for cable-braced free-form grid shells, with strain energy used as the optimization object, structural height used as the optimization variable, and the conjugate gradient method used as the optimization algorithm. According to the shape forming method for grid shells, their shape optimization can be realized only by adjusting the generatrix and directrix, not by optimizing the whole surface. The B-spline curve method is used to model the generatrix and directrix and maintain an optimized surface fairing. The following conclusions can be drawn from this study. First, the structural mechanical behavior of grid shells can be significantly improved with rapid convergence using the proposed shape optimization method. Second, the plane quadrilateral mesh is maintained and fewer optimization variables are needed during the proposed shape optimization method. Finally, the optimized surface is fairing and the mechanical properties of the optimized surface are somewhat decreased when using the B-spline curve method.

        Prognostic value of hyponatremia in heart failure patients: an analysis of the Clinical Characteristics and Outcomes in the Relation with Serum Sodium Level in Asian Patients Hospitalized for Heart Failure (COAST) study

        Background/Aims: Hyponatremia is a well-known risk factor for poor outcomes in Western studies of heart failure (HF) patients. We evaluated the predictive value of hyponatremia in hospitalized Asian HF patients. Methods: The Clinical Characteristics and Outcomes in the Relation with Serum Sodium Level in Asian Patients Hospitalized for Heart Failure (the COAST) study enrolled hospitalized patients with systolic HF (ejection fraction < 45%) at eight centers in South Korea, Taiwan, and China. The relationship between admissionsodium level and clinical outcomes was analyzed in 1,470 patients. Results: The mean admission sodium level was 138 ± 4.7 mmol/L, and 247 patients (16.8%) had hyponatremia defined as Na+ < 135 mmol/L. The 12-month mortality was higher in hyponatremic patients (27.9% vs. 14.6%, p < 0.001), and hyponatremia was an independent predictor of 12-month mortality (hazard ratio, 1.72; 95% confidence interval, 1.12 to 2.65). During hospital admission, 57% of hyponatremic patients showed improvement without improvement in their clinical outcomes (p = 0.620). The proportion of patients with optimal medical treatment was only 26.5% and 44.2% at admission and discharge, respectively, defined as thecombined use of angiotensin-converting-enzyme inhibitor/angiotensin receptor blocker and β-blocker. Underuse of optimal medical treatment was more pronounced in hyponatremic patients. Conclusions: In hospitalized Asian HF patients, hyponatremia at admission is common and is an independent predictor of poor clinical outcome. Furthermore, hyponatremic patients receive less optimal medical treatment than their counterparts.

        Effects of enzymolysis and fermentation of Chinese herbal medicines on serum component, egg production, and hormone receptor expression in laying hens

        Objective: In the present study, we aimed to investigate the effects of enzymolysis fermentation of Chinese herbal medicines (CHMs) on egg production performance, egg quality, lipid metabolism, serum reproductive hormone levels, and the mRNA expression of the ovarian hormone receptor of laying hens in the late-laying stage. Methods: A total of 360 Hy-Line Brown laying hens (age, 390 days) were randomly categorized into four groups. Hens in the control (C) group were fed a basic diet devoid of CHMs, the crushed CHM (CT), fermented CHM (FC), and enzymatically fermented CHM (EFT) groups received diets containing 2% crushed CHM, 2% fermented CHM, and 2% enzymatically fermented CHM, respectively. Results: Compared with crushed CHM, the acid detergent fiber, total flavonoids, and total saponins contents of fermented CHM showed improvement (p<0.05); furthermore, the neutral and acid detergent fiber, total flavonoids, and total saponins contents of enzymatically fermented CHM improved (p<0.05). At 5 to 8 weeks, hens in the FC and EFT groups showed increased laying rates, haugh unit, albumin height, yolk color, shell thickness, and shell strength compared with those in the C group (p<0.05). Compared with the FC group, the laying rate, albumin height, and Shell thickness in the EFT group was increased (p<0.05). Compared with the C, CT, and FC groups, the EFT group showed reduced serum total cholesterol and increased serum luteinizing hormone levels and mRNA expressions of follicle stimulating hormone receptor and luteinizing hormone receptor (p<0.05). Conclusion: These results indicated that the ETF group improved the laying rate and egg quality and regulated the lipid metabolism in aged hens. The mechanism underlying this effect was likely related to cell wall degradation of CHM and increased serum levels of luteinizing hormone and mRNA expression of the ovarian hormone receptor.

        Multi-objective Morphology Optimization of Free-form Cable-braced Grid Shells

        This paper examines the multi-objective morphology optimization of free-form cable-braced grid shells. First, according to the shape forming method for grid shells, shape optimization can be realized by adjusting the generatrix and directrix rather than optimizing the whole surface. Second, the multi-objective shape optimization of free-form cable-braced grid shells is conducted. According to different practical requirements, the mechanical and geometric indexes, mechanical and economic indexes, or different mechanical indexes are used as multiple optimization objectives. Four main conclusions can be drawn from this study. First, with the above shape optimization method, the optimized surface does not change significantly; therefore, this method is useful in the shape optimization of grid shells with given initial surfaces. Second, among the static mechanical performance indexes, the mechanical behavior of the cable-braced grid shell is better with strain energy as the optimization objective. Third, with the weight of steel tubes and strain energy as the optimization objectives, the result of the multi-objective optimization that combines section optimization with shape optimization is favorable and practical in engineering applications. Fourth, when strain energy and the variance of the tube lengths are used as multiple optimization objectives, the structural mechanical behavior is not sensitive to the weight factor of variance of the tube lengths.

      • Healing Effects of Rebamipide and Omeprazole in <i>Helicobacter pylori-</i>Positive Gastric Ulcer Patients after Eradication Therapy: A Randomized Double-Blind, Multinational, Multi-Institutional Comparative Study

        <P>Abstract</P><P><I>Background and Aims:</I> A recent placebo-controlled trial showed that rebamipide, which is a mucosal-protective antiulcer agent, promoted gastric ulcer healing without affecting the <I>Helicobacter pylori</I> status. We conducted a randomized, double-blind trial to compare the healing effects of rebamipide and the proton-pump inhibitor omeprazole in <I>H. pylori</I>-positive gastric ulcers after <I>H. pylori</I> eradication therapy. <I>Methods:</I> After completion of 1 week of eradication therapy, 132 patients with <I>H. pylori</I>-positive gastric ulcer were enrolled in 5 Chinese and 4 Korean institutions. Patients were randomly assigned to take either 20 mg of omeprazole (n = 63) or 300 mg of rebamipide (n = 65) daily for 7 weeks. Healing was defined as complete recovery and S1 and S2 stage ulcer according to the Sakita-Miwa classification. <I>Results:</I> Healing rates at 12 weeks were 81.5% (53/65) and 82.5% (52/63) in the rebamipide and omeprazole groups, respectively. There was no significant difference in treatment efficacy, as evidenced by gastric ulcer healing rates (absolute difference –1.0%; 95% confidence interval –10.7 to 8.7; p = 0.88). The <I>H. pylori</I> eradication rate and ulcer healing rate did not differ between the groups, the latter regardless of eradication outcome. <I>Conclusions:</I> Rebamipide is as effective as omeprazole in treating of <I>H. pylori</I>-positive gastric ulcer after eradication therapy.</P><P>Copyright © 2011 S.         The Effect of Transformation on the Virulence of Streptococcus pneumoniae

        Although pneumococcus is one of the most frequently encountered opportunistic pathogen in the world, the mechanisms responsible for its infectiveness have not yet been fully understood. In this paper, we have attempted to characterize the effects of pneumococcal transformation on the pathogenesis of the organism. We constructed three transformation-deficient pneumococcal strains, which were designated as Nos. 1d, 2d, and 22d. The construction of these altered strains was achieved via the insertion of the inactivated gene, comE, to strains 1, 2 and 22. We then conducted a comparison between the virulence of the transformation-deficient strains and that of the wild-type strains, via an evaluation of the ability of each strain to adhere to endothelial cells, and also assessed psaA mRNA expression, and the survival of hosts after bacterial challenge. Compared to what was observed with the wild-type strains, our results indicated that the ability of all of the transformation-deficient strains to adhere to the ECV304 cells had been significantly reduced (p < 0.05), the expression of psaA mRNA was reduced significantly (p < 0.05) in strains 2d and 22d, and the median survival time of mice infected with strains Id and 2d was increased significantly after intraperitoneal bacterial challenge (p < 0.05). The results of our study also clearly indicated that transformation exerts significant effects on the virulence characteristics of S. pneumoniae, although the degree to which this effect is noted appears to depend primarily on the genetic background of the bacteria.

        Clinicopathological Features and Immunohistochemical Alterations of Keratinocyte Proliferation, Melanocyte Density, Smooth Muscle Hyperplasia and Nerve Fiber Distribution in Becker`s Nevus

        Background: Although Becker`s nevus (BN) is a relatively common disease, the systematic studies of clinicopathological and immunohistochemical results are poorly reported. Objective: To investigate the clinicopathological features and immunohistochemical alterations of keratinocyte proliferation, melanocyte density, smooth muscle hyperplasia and nerve fiber distribution in BN. Methods: Clinical and pathological data were collected in 60 newly-diagnosed BN cases. Immunohistochemical stain of Ki-67, Melan-A, keratin 15, smooth muscle actin and protein gene product 9.5 was performed in 21 cases. Results: The median diagnostic and onset age was 17 and 12 years, respectively. Skin lesions usually appeared on the upper trunk and upper limbs. The pathological features included the rete ridge elongation and fusion and basal hyperpigmentation. Epidermal Ki-67, Melan-A and keratin 15 expression and dermal nerve fiber length were significantly higher in lesional and perilesional skin than in normal skin (p<0.05∼0.01), while smooth muscle actin expression was upregulated only in skin lesion (p<0.05). Conclusion: Although the clinical diagnosis of BN is often straightforward, histopathology is helpful to differentiate from other pigmentary disorders. The hyperproliferation of keratinocytes, melanocytes, arrector pili muscle and dermal nerve fibers could be involved in the patho-genesis of BN. (Ann Dermatol 28(6) 697∼703, 2016)
