        Identification of a sensitive urinary biomarker, selenium-binding protein 1, for early detection of acute kidney injury

        <P>Acute kidney injury (AKI) is associated with increased mortality rate in patients but clinically available biomarkers for disease detection are currently not available. Recently, a new biomarker, selenium-binding protein 1 (SBP1), was identified for detection of nephrotoxicity using proteomic analysis. The aim of this study was to assess the sensitivity of urinary SBP1 levels as an early detection of AKI using animal models such as cisplatin or ischemia/reperfusion (I/R). Sprague-Dawley rats were injected with cisplatin (6 mg/kg, once i.p.) and sacrificed at 1, 3, or 5 days after treatment. Ischemia was achieved by bilaterally occluding both kidneys with a microvascular clamp for 45 min and verified visually by a change in tissue color. After post-reperfusion, urine samples were collected at 9, 24, and 48 hr intervals. Urinary excretion of protein-based biomarkers was measured by Western blot analysis. In cisplatin-treated rats, mild histopathologic alterations were noted at day 1 which became severe at day 3. Blood urea nitrogen (BUN) and serum creatinine (SCr) levels were significantly increased at day 3. Levels of urinary excretion of SBP1, neutrophil gelatinase-associated lipocalin (NGAL), and a tissue inhibitor of metalloproteinase-1 (TIMP-1) were markedly elevated at day 3 and 5 following drug treatment. In the vehicle-treated I/R group, serum levels of BUN and SCr and AST activity were significantly increased compared to sham. Urinary excretion of SBP1 and NGAL rose markedly following I/R. The urinary levels of SBP1, NGAL, TIMP-1, and KIM-1 proteins excreted by AKI patients and normal subjects were compared. Among these proteins, a marked rise in SBP1 was observed in urine of patients with AKI compared to normal subjects. Based upon receiver-operator curves (ROC), SBP1 displayed a higher area under the curve (AUC) scores than levels of SCr, BUN, total protein, and glucose. In particular, SBP1 protein was readily detected in small amounts of urine without purification. Data thus indicate that urinary excretion of SBP1 may be useful as a reliable biomarker for early diagnosis of AKI in patients.</P>

        과학 문제 대면 상황에서 상태호기심 및 상태불안 유발에 영향을 미치는 학습자 변인에 대한 연구

        본 연구는 초등학교 5∼6학년을 대상으로 과학 문제 대면 상황에서 학습자 변인이 상태호기심 및 상태불안 유발에 미치는 영향을 밝히는 데 목적이 있다. 상태호기심 및 상태불안 유발에 영향을 줄 것으로 예상되는 학습자 변인으로 과학호기심, 흥미, 인지욕구, 과학자아개념, 과학불안, 사전지식, 학생이 인식한 과제난이도를 선정하였다. 분석 결과, 과학 문제 대면 상황에서 유발되는 상태호기심에 유의한 영향을 미치는 변인은 흥미, 인지욕구, 과학호기심, 사전지식 순으로 나타났으며, 이들 변인은 모두 정적인 영향을 미쳤다. 또한 과학 문제 대면 상황에서 유발되는 상태불안에 유의한 영향을 미치는 변인은 과학불안, 학생이 인식한 과제난이도, 인지욕구, 과학자아개념, 사전지식 순으로 나타났다. 이중 사전지식만 상태불안 유발에 부적인 영향을 미쳤고 나머지 변인들은 정적인 영향을 미쳤다. 본 연구의 결과는 과학 학습에서 학생의 정서적 상태에 대한 이해의 폭을 넓히고, 과학 학습에서 유발되는 상태호기심 및 상태불안 연구에 대한 이론적 토대를 마련할 것으로 기대된다. The purpose of this study was to clarify the effect of learner variables on triggering state curiosity and state anxiety in confronting scientific task situation for fifth to sixth grades of elementary school. Science curiosity, interest, need for cognition, science self-concept, science anxiety, prior knowledge, and perceived difficulty were selected as learner variables that are expected to affect state curiosity and state anxiety. As a result of this study, the variables that had a significant influence on evoking state curiosity in confronting scientific task situation were in the order of interest, need for cognition, science curiosity, and prior knowledge, and all of these variables had a positive effect. In addition, the variables that significantly affect on evoking state anxiety in confronting scientific task situation were in the order of science anxiety, perceived difficulty, need for cognition, science self-concept, and prior knowledge. Of these, only prior knowledge had a negative effect on evoking state anxiety, and the other variables had a positive effect. The results of this study are expected to broaden the comprehension of students’ emotional states in science education, and provide a theoretical foundation for the studies of state curiosity and state anxiety in science learning.

        Effects of Long-acting Injectable 3-Monthly Paliperidone Palmitate on the Clinical and Social Performance of Patients with Schizophrenia

        Objective: To investigate the effects of long-acting injectable 3-monthly paliperidone palmitate on the clinical and social functioning of patients with schizophrenia. Methods: This study enrolled patients with schizophrenia receiving long-acting injectable 1-monthly paliperidone palmitate for at least 4 months and who subsequently received 3-monthly paliperidone palmitate. Accordingly, 418 patients were followed up for 24 weeks. Their clinical symptoms and social functioning were measured using the Clinical Global Impression-Severity of Illness and Personal and Social Performance scales. Results: The Personal and Social Performance total score was significantly higher after 3-monthly paliperidone palmitate treatment than at baseline (baseline vs. week 24: 54.3 ± 18.0 vs. 61.0 ± 14.5 [mean ± standard deviation]; p < 0.001; Wilcoxon signed-rank test); the proportion of patients in the mildly ill group (scores 71−100) also increased significantly (baseline vs. week 24: 16.5% vs. 20.6%; p < 0.001; McNemar-Bowker test). The mean Clinical Global Impression-Severity of Illness score decreased significantly (baseline vs. week 24: 3.7 ± 1.0 vs. 3.4 ± 0.9; p < 0.001; Wilcoxon signed-rank test), as did the proportion of patients in the severely ill group (baseline vs. week 24: 4.1% vs. 2.1%; p < 0.001; McNemar-Bowker test). Conclusion: Continuous 3-monthly paliperidone palmitate treatment significantly enhances the personal and social performance of patients with schizophrenia and reduces the proportion of those with severe illness. These findings suggest that long-acting injectable antipsychotic administration at intervals longer than 1 month might improve the social functioning of and promote return to activities of daily living in patients with schizophrenia.

      • 구치부 반대교합을 동반한 성인 구순구개열 환자의 MARPE를 사용한 치험례

        목적: 구순구개열로 인해 상악골의 횡적, 전후방적 성장 결핍과 열 부위 치아 결손을 동반한 환자에서 MARPE를 사용하여 악궁 확장을 시행한 증례를 보고하고자 한다. 임상 증례: 본 증례는 21세 남환으로 이가 반대로 물리며 치열을 가지런하게 하고싶다는 주소로 내원하였다. 환자는 구순구개열에 의한 상악의 성장 부전으로 인해 III급 골격 관계를 갖고 있었으며, 열 부위인 상악 좌측 측절치 결손 상태였다. 환자는 유아기 구순구개열 수술과 골 결손부위 2차 치조골 이식술, 성장기 교정 치료를 진행했으며, 성인이 된 후 치아배열 및 2차교정을 위해 본과 내원했다. 본과에서는 구순구개열에 의한 상악 열성장을 해결하기 위해 수술교정을 권하였으나, 환자분의 요구대로 비수술적 치료를 하게 됐고, 따라서 MARPE를 사용한 상악궁 확장과 전후방적 치아 이동을 통한 절충치료를 진행했다. 결론: 구순구개열을 치료하기 위해서는 출생 시부터 성장이 완료될 때까지 여러 과들의 협진을 통해 진행되며, 각 시기에 필요한 치료를 순서대로 진행하여 성인이 된 후 구순구개열을 성공적으로 치료할 수 있다. 특히 교정적 접근은 결손된 치아 및 부정교합의 회복을 위해 필수적이며, 성장기 교정치료와 더불어 악골 성장이 완료되는 시기에 최종적인 교합의 완성을 위한 치료를 진행한다. 대다수의 구순구개열 환자는 악골의 전후방적 부조화 및 횡적 부조화를 보이며, 이에 따라 치료과정이 더 복잡해지기 때문에 치료계획을 수립함에 있어 환자의 교정적 문제점을 파악하는 데 중점을 두어야 한다. 본 증례는 상악골의 횡적, 전후방적 성장 결핍과 열 부위 치아 결손을 동반한 구순구개열 환자에서 악궁의 회복 및 결손 부위의 공간확보를 통해 양호한 교합을 형성하였고 심미와 기능의 개선을 얻을 수 있었다. Purpose: We report the successful treatment of a patient with cleft lip and palate and anterior-posterior and transverse deficiency of the maxilla using MARPE. Clinical Case: A 21-year-old man presented with the chief complaints of multiple teeth in crossbite and crowding. He had cleft lip and palate in the past, skeletal Class III malocclusion due to growth deficiency of the upper jaw, and a missing upper left lateral incisor. For the treatment of cleft lip and palate, the patient underwent lip and palate surgeries in infancy, secondary alveolar bone graft, and phase I orthodontic treatment. We recommended surgical correction to solve the maxillary deficiency due to cleft lip and palate, but nonsurgical treatment was performed as requested by the patient. Orthopedic treatment through maxillary arch expansion using MARPE along with camouflage treatment through anterior and posterior tooth movement was performed. Conclusion: It is necessary to carry out treatment of cleft lip and palate patients in multiple stages from birth to adulthood through collaboration with various medical and dental departments. Orthodontic approach is essential for the recovery of malocclusion. Treatment for the completion of the final occlusion is performed after growth cessation, while orthodontic treatment also takes place during the growth period.

        2020 Korean guidelines for the management of metastatic prostate cancer

        In 2017, Korean Society of Medical Oncology (KSMO) published the Korean management guideline of metastatic prostate cancer. This paper is the 2nd edition of the Korean management guideline of metastatic prostate cancer. We updated recent many changes of management in metastatic prostate cancer in this 2nd edition guideline. The present guideline consists of the three categories: management of metastatic hormone sensitive prostate cancer; management of metastatic castration resistant prostate cancer; and clinical consideration for treating patients with metastatic prostate cancer. In category 1 and 2, levels of evidence (LEs) have been mentioned according to the general principles of evidence-based medicine. And grades of recommendation (GR) was taken into account the quality of evidence, the balance between desirable and undesirable effects, the values and preferences, and the use of resources and GR were divided into strong recommendations (SR) and weak recommendations (WR). A total of 16 key questions are selected. And we proposed recommendations and described key evidence for each recommendation. The treatment landscape of metastatic prostate cancer is changing very rapid and many trials are ongoing. To verify the results of the future trials is necessary and should be applied to the treatment for metastatic prostate cancer patients in the clinical practice. Especially, many prostate cancer patients are old age, have multiple underlying medical comorbidities, clinicians should be aware of the significance of medical management as well as clinical efficacy of systemic treatment.

      • Multidisciplinary methodology to predict the performance of modular actuator-based manipulator

        <P><B>Abstract</B></P> <P>In this paper, we propose a multidisciplinary methodology to predict the performance of a modular actuator-based robotic manipulator considering the flexibilities of the linkage and the joint. Generally, fast dynamics of manipulator caused by structural flexibility often leads to the residual vibrations and deflections. However, it is quite difficult to predict these types of phenomena analytically since the dynamic characteristics of the manipulator considerably vary with its posture. To address this issue effectively, we propose to utilize the finite element method as well as the Lagrange formulation for more accurate modeling of the mechanical flexibility in manipulator structure. To verify the viability of proposed method, we perform the simulation using a 3 degree-of-freedom (DOF) planar manipulator constructed with three modular actuators. Through the simulation, the sensitivity analysis and principal component analysis (PCA) are carried out for the settling time and deflection with respect to the motion profile and end effector position. It is observed that the structural flexibility of the link and flexibility of modular actuator originated from the harmonic drive as well as the end effector position have a significant effect on the settling time and the deflection. The parameters in the S-curve motion profile are other key factors to the motion time and settling time, which should be selected to minimize the cycle time for task completion.</P> <P><B>Highlights</B></P> <P> <UL> <LI> The practical methodology is proposed to estimate the performance of manipulator. </LI> <LI> The flexibilities of manipulator are modeled via FEM and Lagrange formulation. </LI> <LI> The nonlinear compliance of harmonic drive is identified to consider the joint flexibility. </LI> <LI> The proposed methodology can reduce the cost and the time required for robot design. </LI> </UL> </P>

        Performance enhancement of mobile antenna by ground decomposition

        <P><B>Abstract</B></P><P>We propose a ground decomposition method (GDM) to improve the performance of mobile antennas. In the GDM, overall ground impedance is analyzed as the combination of independent decomposed ground parts. In a proposed design, the overall ground mode resonance is tuned by adjusting the impedance of a flexible printed circuit board, which is decomposed according to GDM. This greatly improves the radiation performance, as verified through simulation and measurement. This proposed method is effective for mobile antenna design, and especially for actual devices that have very complicated ground configurations.</P>

        <P>In recent years, numerous research activities have been devoted to the controlled release of nitric oxide (NO) due to its potential as a restenosis inhibitor which inhibits the proliferation of vascular smooth muscle cells, the apoptosis of vascular endothelial cells, and aggregation of platelets. This work has demonstrated the development of a novel NO-conjugated gel system comprising of thermosensitive Pluronic F127, branched polyethylenimine (BPEI), and diazeniumdiolates (NONOates). Synthesis of conjugated Pluronic-BPEI-NONOates involved coupling of activated F127 to BPEI followed by the installation of NONOates at the secondary amine sites of branched PEI backbone under high pressure. NO-conjugated gel system, F127-BPEI-NONOates, reduced the initial burst of NO release and prolonged NO release. Furthermore, F127-BPEI-NONOates polymer coated on cell culture dish displayed much higher increase of endothelial cell proliferation and reduction of smooth muscle cell proliferation than that exhibited by non-NO releasing control.         Ctbp2 Modulates NuRD-Mediated Deacetylation of H3K27 and Facilitates PRC2-Mediated H3K27me3 in Active Embryonic Stem Cell Genes During Exit from Pluripotency : Ctbp2 Modulates H3K27-State in Active ESC Genes

        <P>For cells to exit from pluripotency and commit to a lineage, the circuitry of a core transcription factor (CTF) network must be extinguished in an orderly manner through epigenetic modifications. However, how this choreographed epigenetic remodeling at active embryonic stem cell (ESC) genes occurs during differentiation is poorly understood. In this study, we demonstrate that C-terminal binding protein 2 (Ctbp2) regulates nucleosome remodeling and deacetylation (NuRD)-mediated deacetylation of H3K27 and facilitates recruitment of polycomb repressive complex 2 (PRC2)-mediated H3K27me3 in active ESC genes for exit from pluripotency during differentiation. By genomewide analysis, we found that Ctbp2 resides in active ESC genes and co-occupies regions with ESC CTFs in undifferentiated ESCs. Furthermore, ablation of Ctbp2 effects inappropriate gene silencing in ESCs by sustaining high levels of H3K27ac and impeding H3K27me3 in active ESC genes, thereby sustaining ESC maintenance during differentiation. Thus, Ctbp2 preoccupies regions in active genes with the NuRD complex in undifferentiated ESCs that are directed toward H3K27me3 by PRC2 to induce stable silencing, which is pivotal for natural lineage commitment.</P>

        Effect of Home-based Self-administered Transcranial Direct Stimulation in Patients with Mild to Moderate Major Depressive Disorder: A Single-arm, Multicentral Trial

        Objective: Although the effects and safety of transcranial direct current stimulation (tDCS) treatment in depressive patients are largely investigated, whether the self-administration of tDCS treatment at patient’s home is comparable to clinic-based treatment is still unknown. Methods: In this single-arm, multi-center clinical trial, 61 patients with mild to moderate major depressive disorder were enrolled. tDCS treatment was delivered at the patient’s home once a day, 5 to 7 times a week for 6 weeks, and each session lasted for 30 minutes. The primary outcome was a total Beck-Depression Inventory-II score, and no concurrent antidepressants were used. Results: The remission rates in both Full-Analysis (FA) (n = 61) and Per-Protocol (PP) (n = 43) groups were statistically significant (FA: 57.4% [0.44−0.70], PP: 62.8% [0.47−0.77]; percent [95% confidence interval]). The degree of depression- related symptoms was also significantly improved in 2, 4, and 6 weeks after the treatment when compared with baseline. There was no significant association between treatment compliance and remission rate in both FA and PP groups. Conclusion: These results suggest that acute treatment of patient-administered tDCS might be effective in improving the subjective feeling of depressive symptoms in mild to moderate major depressive disorder patients.
