      • Eosinophils play a role in the pathogenesis of psoriasis by stimulating inflammatory cytokine and chemokine production and neutrophil chemotaxis

        Background: Eosinophilic cytokine profile including IL-23/Th17 axis and their immunomodulatory interaction with other immune cells imply their possible involvement in psoriasis pathogenesis. Objectives: This study was to examine the potential role of eosinophils in imiquimod (IMQ)-induced psoriasiform dermatitis using ΔdblGATA mice, an eosinophil lineage-ablated mouse line. Methods: After topical IMQ application, inflammatory cytokine gene expression and Inflammatory cell populations were analyzed by qRT-PCR and flow cytometry, respectively. Results: Psoriatic features like epidermal thickening, erythema, and scale were less prominent in ΔdblGATA mice after IMQ treatment. Inflammatory cytokines important in psoriasis, like IL-1β, IL-6, IL-22, IL-23, and IL-17, and Th1-/neutrophil-related chemokines CCL2 and CXCL12 were also downregulated, and neutrophils were constantly decreased in the skin, LN, and spleen of ΔdblGATA mice compared to WT mice after IMQ treatment. Although IMQ application increased eosinophils in the skin of WT mice, the absolute cell count was much lower than neutrophils, a major player in psoriasis. Conclusion: The small size of eosinophil population and constant reduction of neutrophil in ΔdblGATA mice than WT mice after IMQ treatment suggest that eosinophils would influence psoriasis indirectly via their inflammatory profile and affecting other mediators in psoriasis, such as neutrophils.

      • [P300] Two cases of primary cutaneous nocardiosis caused by intralesional injection

        Nocardiosis is a rare localized or systemic suppurative disease caused by several species of bacteria of the genus Nocardia. Nocardia is an aerobic, filamentous gram-positive, partially acid-fast bacteria belonging to the Actinomycetales order and found in soil, decayed organic matter, water, and air worldwide. Cutaneous nocardiosis is an uncommon infectious disease that presents a primary cutaneous infection or as a part of disseminated pulmonary nocardiosis. First case is a 50-year-old woman who had erythematous patches with swelling on her right arm, who had been treated with triamcinolone intralesional injection at the private clinic. Histopathology of the skin showed diffuse lymphohistiocytic infiltration in the dermis and subcutaneous layer. On Gram staining, gram-positive bacilli were seen, and on modified AFB staining, the branching hyphae stained red purple. White colonies were formed on blood agar medium in pus culture. By bacterial culture and rDNA gene sequencing, the causative organism was identified as N. farcinica. Second case is a 34-year-old woman who presented with erythematous patches and tenderness on Lt.arm. She had been also treated with triamcinolone intralesional injection for keloid at the same clinic as the first patient. She was diagnosed with primary cutaneous nocardiosis by pus culture. Herein, we report rare two cases of primary cutaneous norcardiosis after intralesional injection.

      • A case of drug-induced hemolytic anemia associated with antibiotic prophylaxis

        A 68-year-old male patient presented with a dark brownish nodule on nose. In a local clinic, he underwent skin biopsy and was diagnosed as basal cell carcinoma. We performed wide excision to remove remaining lesion and clear margin obtained. Immediately before and after the operation he received intravenous cefotetan for antibiotic prophylaxis. On post-operate day 8, he felt dizziness and fell down. Hemoglobin was 3.5g/dL and hematocrit was 12.8%, whereas the initial CBC was normal. There was no evidence of bleeding on physical exam. Total bilirubin (6.86mg/dL), reticulocyte (6.2%) were elevated and direct antiglobulin test was positive. He was diagnosed as drug-induced immune hemolytic anemia (DIIHA) due to cefotetan. After systemic steroid treatment and transfusion a CBC revealed a hemoglobin of 9.4g/dL and hematocrit of 29.3%. He had complete resolution of symptoms and was discharged 5 days later. DIIHA is a rare acquired form of hemolytic anemia caused by interaction of drug with the immune system. Drug adsorption, drug-dependent antibody, and autoimmune induction are the three mechanism of hemolysis. Cefotetan-induced hemolytic anemia (CIHA) has been described to occur through all three mechanisms. Prompt diagnosis and massive systemic steroid administration are essential in minimizing mortality from hemolysis. Herein, we report a rare case of CIHA and hope to be aware of occurrence of CIHA associated with antibiotic prophylaxis.

      • Postoperative radiation therapy following keloidectomy

        Keloid is a problem that dermatologists encounter. Patients typically present with cosmetic concerns, although keloids can cause pruritus, pain. Keloids have been treated with multimodal therapy. Radiotherapy treatment after keloidectomy is known to be an effective method for reducing the rate of recurrence. A 40-year-old male presented with erythematous hard plaques on chin, shoulder, right foot. He was treated with steroid intradermal injection for several years but there was no improvement. A 26-year-old female presented an erythematous hard plaque on Lt. upper arm appeared 20 years earlier because of vaccination. She was performed surgical excision but keloid recurred. They had thought in common about new treatment. They were treated with steroid intralesional injection three times before surgery. Keloids were treated with surgical excision, followed by a total radiation dose of 1,800 cGy in three fractions over three days. We will observe their lesion whether keloids recur for 18 months. Postoperative electron-beam irradiation may control collagen synthesis by inhibiting the promotion of abnormal activated and normal fibroblasts. Furthermore, many researchers are trying to set the proper protocols of radiation, keloids should be treated with dose protocols customized by site. Postoperative electron-beam irradiation expects effectively to prevent recurrence of keloid and a safe treatment modality due to absence of complications.

        Aplasia cutis congenita (ACC) is a heterogeneous group of disorders in which localized or widespread areas of skin are absent at birth. ACC manifests as a solitary defect on the scalp in 70% of cases. Although most commonly seenon the scalp, ACC can affect any part of the body. Although pathogenesis of ACC is still unclear, proposed hypotheses are divided into two categories : endogenous causes (developmental failure, congenital syndromes) and exogenous ones (intrauterine trauma, infarction, skin fissure caused by rapid growth). Hair collar sign is a ring of darker and coarser terminal hairs on the scalp, typically surrounding ACC, dermoid cyst, encephalocele, or meningocele. It has been described as a relatively specific marker of neuroectodermal defect. A 5-month-old boy presented with round bald patches with underlying a large reddish patch on posterior occiput. It was surrounded by a rim of terminal hairs. Hair collar sign can be a specific clue for diagnosing ACC. Pregnancy and the delivery had been unremarkable and the baby wasotherwise healthy. Histopathology of the skin showed flattened epidermis, vascular dilatation in the superficial dermis, thickening of vessel walls in the reticular dermis, and absence of adnexal structures. It is assumed that association of ACC with nevus flammeus is that fetal skin development including capillary network formation might have been affected since cranial tube was not closed properly.

      • High fluence 1064-nm Q-Switched Nd:YAG laser : safe and effective treatment of cafe-au-lait macules in Asian patients

        Background: Cafe-au-lait macules (CALMs) are benign cutaneous hyperpigmentary disorders. Usually, laser therapies for cosmetic concerns result in more severe side effects in the people of Asian descent than that of Caucasians. Unfortunately, there is no gold standard for the laser treatment of CALMs in skin of people of Asian descent. Objectives: To investigate the efficacy and safety of a high fluence 1064-nm Q-switched neodymium-doped yttrium aluminum garnet (Nd:YAG) laser treatment of CALMs in Asian patients. Methods: The medical records of thirty-five Korean patients (age range: 1 to 40 years old, mean age : 18.5 years) diagnosed with isolated CALMs were reviewed retrospectively. The patients were treated with a 1064-nm Q-switched Nd:YAG laser. The parameters were a spot size of 7 mm, a fluence of 2.2-2.4 J/cm2 with a slow single sliding-stacking pass, and a pulse rate of 10Hz with a one-week interval for 20-50 sessions. Results: At the week of the final treatment, all treated CALMs showed considerable pigmentation removal without any permanent side effects, such as scaring, mottled hypopigmentation, and post-inflammatory hyperpigmentation (PIH). All treated CALMs showed more than 50% clinical improvement. No recurrence was observed in any of the patients after 12 months of follow up. Conclusion: A high fluence 1064-nm Q-switched Nd:YAG laser treatment of CALMs in Asian patients is a safe and effective method without side effects and recurrence.

        거주의 한계와 초월

        바빌로니아와 페르시아 시대에 일어난 유다인들의 유배와 귀환은 고대세계의 유일한 사례는 아니다. 특이한 것이라면 민족의 역사와 문화 회복이 모국 본토가 아니라 유배지역에서 일어났다는 사실이다. 이 회복 운동은 본토에서 성공을 거두고 다시금 모든 디아스포라에 영향을 주었다. 이스라엘 회복은 귀환공동체의 엘리트 에스라와 느헤미야가 터를 놓았고, 그 위에 바로 고대 유대교가 세워졌다. 그들은 율법의 백성이라는 정체성과 성전공동체라는 종교적 자치형태를 수립했고, 이러한 형태는 이집트의 엘레판티네 유대인 공동체의 정체성과 성전재건에도 영향을 미쳤을 것이다. 그 증거로 P. Cowley 21번과 30번을 검토할 수 있다. 하지만 에스라와 느헤미야가 추진한 이상주의적 율법 준수는 본토는 물론 디아스포라 유대인 성전공동체가 사회문화적으로 자신의 정체성을 위해 투쟁하여 주변으로부터 고립되는 결과를 초래했다. 기원전 200년경부터 시리아가 팔레스타인을 지배하고 그 후 마카베오 혁명으로 유대성전국가가 수립되면서 디아스포라 성전공동체의 운명은 급격히 본토의 정치적 상황에 따라 요동쳤다. 그에 따라 이집트의 유대인들은 이집트인들과 문화적 갈등을 경험해야 했다. 이런 문화적 동화와 저항의 파고를 넘나들며 그들이 성전공동체와 율법의 백성이라는 문화적 정체성을 유지했던 원천에 고대 유대교의 이스라엘 회복의 신앙이 있었다. The exile and return of the Jews in the Babylonian and Persian eras is not the only example of the ancient world. If there was something unusual in them: The restoration of Israelite tradition took place in the exile, not the motherland, and eventually succeeded in influencing the other diaspora again. The foundations of Israel’s restoration erected by the elite Ezra and Nehemiah of the returning community were ancient Judaism. The identity of the people of the law and the religious autonomy of the temple community would have affected the identity of the Elephantine Jewish community in Egypt and the rebuilding of the temple. We can examine P. Cowley 21 and 30 as their evidences. On the other hand, Ezra and Nehemiah’s adherence to the idealistic law led to the isolation of the Diaspora Jewish temple community as well as the struggle for their own identity socially and culturally. Since about 200 BC, the fate of the diaspora temple community shook sharply in response to the political situation in the mainland. As Syria dominated Palestine and then the Maccabean revolution established a Jewish temple nation, the Jews of Egypt had to experience cultural conflicts with the Egyptians. The source of these cultural assimilation and resistance, which maintained their cultural identity as a temple community and a people of the law, was the faith of the restoration of ancient Judaism in Israel.

        교회공동체의 갈등 관리

        마태복음의 공동체 규율(18:15-17)은 죄짓는 형제를 경책하여 회개하도록 용서하라는 어록전승(눅 17:3f.)에서 시작되었지만, 유대교의 3단계 증인규정과 결합되어 출교라는 마지막 공동체적 치리까지 실행되었던 초기 유대 기독공동체의 현실을 반영한다. 마태는 공동체 강화에서 우선 권고와 용서를 핵심으로 하는 어록전승의 관점에서 15-20절과 21-35절을 결합한다. 공동체 강화는 ‘작은 자’를 돌보아야 할 ‘더 큰 자’와 권위를 맡은 ‘높은 자들’은 그리스도의 낮아짐과 하나님의 무한한 용서를 공동체의 근본정신으로 삼아, 실족한 자와 잃은 자, 죄지은 형제를 죄에서 돌이켜 공동체의 품에서 살아가도록 돌보라고 권고한다. 이른바 “은총 모델”은 이런 측면에서 본문해석의 근간이 된다. 하지만 신앙공동체의 회개를 향한 무한한 용서와 보살핌은 도덕적인 헤이와 구성원 사이의 갈등을 증폭시키고 방치할 경우 공동체의 성결을 보존할 수 없어 공동체는 마침내 스스로 와해될 수밖에 없다. 마태공동체는 베드로라는 ‘인물’에게 수여되었던 ‘매고 푸는 권위’를 공동체 전체의 권위로 향유하였던 마태공동체에서, 마태는 이 ‘매고 푸는’ 권위를 통해 사적인 관계에서 발생되는 죄로 인한 공동체의 갈등을 관리한다. 요컨대, 교회구성원의 과실을 판단하는 ‘매고 푸는’ 권위는 오직 그 공동체가 일치될 때, 그리고 예수와 함께 있을 때 유효하다(18:18-20). 여기서 “언약신학”의 해석이 가능하다. 다른 한편으로, 이른바 “한계 모델”은 특수한 죄의 경우 추방까지 갈 수 있다는 극단의 경우에만 해당되지만 공동체 강화는 일상의 죄와 갈등의 문제를 다룬다. 그런 의미에서 “형제가 [너에게] 죄를 범하고” 마침내 “너에게는 이방인과 세리같이 될 것이다”(18:17)라는 공동체적 치리선언은 죄를 범한 자를 개인과 공동체로부터 관계를 단절시키는 조치로 해석할 수 있다. 그럼에도 공동체의 치리는 죄짓는 형제를 회개하여 얻으려는 근본동기를 떠날 수 없다. 이는 교회 공동체의 이상과 현실의 딜레마를 단적으로나마 보여준다. The first part of Matthew’s discourse on community deals with community conflicts. The authoritarian “greater” (18:1-5), whoever causes “this little one” to sin (18:6-9), and whoever looks down on (18:10-14) him or her are strongly warned. Various issues of conflicts are generalized in 18:15-20 as problems to commit sins against brothers and sisters of the community. The overall thesis of the discourse on community is that community members must give every effort to bring those who are hurt back into the community. The leaders having great authority should take care of the community’s “little one” in imitation of Christ’s lowering and God’s infinite forgiveness. It is necessary to warn brothers and sisters who had sinned in private so that they could turn away from sin and live within the community. Matthew passes down and applies a three-stage admonishing process to manage conflicts (18:15-17). However, while implementing such rules, the community would have been forced to excommunicate the guilty, which might also result in risks of questioning authority and even leading to division. In order to overcome such risks, On the one hand, Matthew develops the theology of ‘the authority of forgiving’ in the discourse on community. The authority of ‘binding and loosing’, which was conferred on Peter(18:18), is now practiced under the authority of the whole community (18:17-18). The authority to judge the faults of church members is valid only when the community is united and with Jesus (18:19-20). On the other hand, the infinite forgiveness and generous care given to sinners might create further internal conflicts and moral hazard. Moreover, the neglect of sin cannot preserve the integrity of the community, which could eventually lead to its own collapse. Here is the dilemma of the ideal and reality of the church community. Throughout history, almost all communities had to resolve their internal conflicts by implementing internal regulations and disciplines.
