Background: The clinical characteristics of cutaneous lupus erythematosus (CLE) are well delineated in adults, but pediatric data, particularly in Asian populations, are limited. Objectives: Therefore, we evaluated the characteristics of pediatric cases by retrospectively reviewing the medical records of children with CLE during a 15-year period in a tertiary care dermatology clinic in South Korea. Methods: The study included 21 children (8 males and 13 females), 4 of whom had neonatal lupus erythematosus (NLE). Results: Among 17 patients with CLE, discoid lupus erythematosus (DLE) was most common (47.1%), followed by acute CLE (ACLE, 35.3%). All ACLE cases had systemic lupus erythematosus (SLE). Female predominance was conspicuous in ACLE/SLE (6/11 females versus 0/6 males), as was older age, whereas DLE and NLE showed near equal sex distributions. The median age at the diagnosis of CLE was significantly higher in females than in males (15 years versus 4.5 years, p=0.02). All patients with ACLE/ SLE simultaneously showed skin and systemic symptoms from onset. The kidney was the most commonly involved organ. Conclusion: This study revealed unique characteristics of pediatric CLE, further warranting a comprehensive review among various ethnicities to understand the wide spectrum of CLE in the pediatric population.

Background: Breast cancer is the second most common cancer and the most common cause of cancer deaths in Korean women. Although tumor-induced mediators and cancer therapy can suppress cell-mediated immunity, the concurrence of herpes zoster in breast cancer patients has not been well-recognized. Objective: This study aimed to delineate the characteristics of herpes zoster in patients with breast cancer, particularly its association with patient age and breast cancer severity, treatment, and clinical course. Methods: We retrospectively reviewed the medical records of breast cancer patients at a tertiary referral center in Korea from January 2003 to June 2018, identified patients with a subsequent diagnosis of herpes zoster, and analyzed their clinical characteristics. Results: Among 8,124 patients with breast cancer, 2.04% further developed zoster during a median 31-month follow-up period. Age at the diagnosis of breast cancer was higher in the zoster group than in the no zoster group. Cytotoxic chemotherapy and radiotherapy significantly increased the risk of zoster. Time from the diagnosis of breast cancer to zoster development was significantly shorter for invasive cancers than for in-situ cancers, with higher risk in the initial 2 years from the cancer diagnosis. Conclusion: This study showed that breast cancer patients are at an increased risk of zoster, particularly in the time following cancer diagnosis. Therefore, a recent diagnosis of breast cancer should warrant clinical suspicion of zoster for patients with suggestive symptoms, and active management should be started. Background: Topical applications of a silicone gel have been shown to be effective in promoting accelerated epithelialization, reducing the inflammatory response. Objectives: This study was performed to compare the effectiveness of a topical film-forming silicone gel and a moisturizing cream in reducing radiation dermatitis in patients receiving radiotherapy for breast cancer. Methods: This was a single-centre, unblinded, randomized controlled parallel study that included patients with breast cancer treated with adjuvant breast radiotherapy protocol of 50Gy. A total of 56 patients with breast cancer were randomized to use topical silicone gel or moisturizing cream (1:1). The severity of radiation dermatitis was graded using physiological skin parameters, including the erythema index, melanin index, and transepidermal water loss (TEWL), clinician-assessed visual rating scales and patient-reported symptoms. Results: Two-way repeated-measures ANOVA revealed different patterns of changes in the erythema index (F=3.609, p=0.008) and melanin index (F=3.475, p=0.015). The post hoc analysis demonstrated a significantly lower erythema index and melanin index in the patients allocated to the silicone gel group. There were no significant differences between two groups in the TEWL measurements. Conclusion: This study demonstrated the superiority of a topical film-forming silicone gel compared to moisturizer in reducing severity of radiation dermatitis during breast radiotherapy.

An 81-year-old male had multiple erythematous to dusky patches on whole body for several years and two asymptomatic violaceous nodules on the back with unknown onset. He had numbness of both hands and feet, and edema of both hands and face during the previous 6 weeks, and therapies for diabetic neuropathy was unsuccessful. Three years ago, the patient had skin lesions and neural symptoms, similar to current clinical manifestations. Leprosy was clinically suspected, but the evaluations was negative according to Hansen welfare association. The biopsies were performed on the patch and nodule of the back. The nodule showed dense infiltrations of large foamy histiocytes in the dermis, many of which contained microbial components, whereas the patch showed superficial dermal perivascular and periappendageal lympho-histiocytic infiltration. AFB stain was positive with many acid-fast bacilli with globi. The patient was diagnosed with lepromatous leprosy. This case emphasizes that specimens biopsied from dissimilar lesions from the same patient can reveal different histopathological results. In this case, histopathology of the specimen from nodular lesion looked more consistent with the diagnosis than that from the erythematous patch. Considering the possibility of clinico-pathologic discordance, re-biopsy should be considered if a certain diagnosis is strongly suspicious clinically, even when the initial histopathologic findings are ambiguous.
A case of granuloma faciale in an 8-year-old girl treated by topical pimecrolimus

Granuloma faciale (GF) is a rare benign chronic skin disease of unknown etiology. GF commonly presents as asymptomatic or tender, red to brown papules or plaques on the face of middle aged to older people. An 8-year-old girl presented with asymptomatic erythematous confluent scaly papules on right malar area for 3 months. Although the lesions improved with topical steroid, they got worsened with additional appearance of a few pustules, after stopping the application. Histopathologic examination revealed superficial dermal perivascular and perifollicular lymphocytic and neutrophilic infiltration with copious nuclear dusts and extravasation of RBCs. The patient was diagnosed with GF and treated with 1% pimecrolimus cream and retapamulin ointment. The lesions were much improved in two weeks. GF is usually unresponsive to therapy. Topical or intralesional corticosteroids are primarily used, and topical calcineurin inhibitors including tacrolimus or pimecrolimus can also be effective. If GF is resistant to topical treatments, systemic treatments, like dapsone, are available. Herein we report an uncommon case of childhood GF which was treated with topical pimecrolimus. Considering the side effect of skin atrophy and telangiectasia associated with corticosteroids, pimecrolimus could be used as a first line therapy in childhood GF.

Identifying Mycobacterium using matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) is challenging. We evaluated the performance of MALDI-TOF MS in identifying nontuberculous mycobacteria (NTM) using the ASTA MicroIDSys system (ASTA Inc., Suwon, Korea) with the MycoDB v1.95s and upgraded MycoDB v2.0-beta databases. We tested 124 NTM isolates collected from Ogawa medium at Severance Hospital, Seoul, Korea, between January and April 2019. MicroIDSys scores were categorized into three groups: ≥140, reliable identification; 130–139, ambiguous identification; and <130, invalid identification. To validate the results, we used the reverse blot hybridization assay (Molecutech REBA MycoID, YD Diagnostics Corp., Korea). Initial analysis using MycoDB v1.95s resulted in 26.6% (33/124) reliable, 43.5% (54/124) ambiguous, and 29.8% (37/124) invalid identifications. Re-analysis using the upgraded MycoDB v2.0-beta database resulted in 94.4% (117/124) reliable, 4.0% (5/124) ambiguous, and 1.6% invalid (2/124) identifications. The percentage of reliable identifications that matched with the reference increased from 26.6% (33/124) with MycoDB v1.95s to 93.5% (116/124) with MycoDB v2.0-beta. The upgraded databases enable substantially improved NTM identification through deep learning in the inference algorithm and by considering more axes in the correlation analysis. MALDI-TOF MS using the upgraded database unambiguously identified most NTM species. Our study lays a foundation for applying MALDI-TOF MS for the simple and rapid identification of NTM isolated from solid media.

<P>In this paper, we first propose and demonstrate an interposer-level wireless power transfer (WPT) scheme to reduce the number of wired power supply interconnections for high-density silicon interposer-based 2.5-D and 3-D integrated circuits (2.5-D and 3-D ICs). We first suggest the concept of an active silicon interposer, fabricate it with active circuits for WPT, and finally proved its WPT operation. The active circuits are the rectifier and dc–dc converter designed with 0.18- <TEX>$\mu \text{m}$</TEX> SK-Hynix CMOS process to convert wirelessly delivered ac power from a printed circuit board (PCB) package into dc power. We improved the power transfer efficiency by applying magnetic field resonance coupling with proper coil structure selection. The equivalent circuit model of the WPT interconnection scheme is suggested with analytic equations. For experimental demonstration, the fabricated active silicon interposer is attached on PCB package. The average dc output voltage of the designed CMOS rectifier with an MIM smoothing capacitor was measured 2.21 V, and that of the designed dc–dc converter was measured 1 V, which is a suitable voltage level for 1 V devices in 2.5-D and 3-D ICs.</P>
