Background/Aims: The ballooning time in endoscopic papillary large balloon dilation (EPLBD) remains controversial. The aim of this study was to evaluate the significance of the ballooning time comparing an immediate balloon deflation method with a conventional ballooning time of > 45 seconds. Methods: Between January 2010 and December 2010, 126 patients with bile duct stones treated with EPLBD and endoscopic sphincterotomy were divided according to the ballooning time: the immediate deflation group (n=56) and the conventional inflation group (ballooning time 45s to < 60s) (n=70). Results: The overall success rate and the success rate of the first attempt of ERCP (endoscopic retrograde cholangio-pancreatography) were 96.4% (54/56) and 80.4% (45/56) in the immediate group and 97.1% (68/70) and 77.1% (54/70) in the conventional inflation group. There were no statistically significant differences in the overall success and the first attempt of ERCP success rate (p=0.99, p=0.66). The frequency of mechanical lithotripsy was 0% in the immediate deflation group and 7.1% in the conventional inflation group (p=0.065). Complications occurred in 3.6% (2/56) patients in the immediate deflation group and 8.6% (4/70) patients in the conventional inflation group (p=0.298). Conclusions: The ballooning time in EPLBD does not affect the outcomes of the treatment for bile duct stones. And the feasibility of the immediate deflation method in EPLBD is acceptable. Ovarian cancer is generally primary cancer and less frequently originates from metastasis from non-gynecological cancer. Ovarian metastasis from lung cancer represents only 2~4% of all ovarian metastatic cancers. We report a case of ovarian metastasis of non-small cell lung cancer with epidermal growth factor receptor mutation. The patient underwent cytoreductive surgery for the ovarian mass and erlotinib therapy for the metastatic lung cancer. Erlotinib therapy markedly decreased the size of lung mass.

In this study temperature distribution and gas flow inside a planetary type reactor in which a number of satellites on a spinning susceptor were rotating were analyzed using numerical simulation. Effects of flow rates on gas flow and temperature distribution were investigated in order to obtain design parameters. The commercial computational fluid dynamics software CFD-ACE+ was used in this study. The multiple-frame-of-reference was used to solve continuity, momentum and energy conservation equations which governed the transport phenomena inside the reactor. Kinetic theory was used to describe the physical properties of gas mixture. Effects of the rotation speed of the satellites was clearly seen when the inlet flow rate was small. Thickness of the boundary layer affected by the satellites rotation became very thin as the flow rate increased. The temperature field was little affected by the incoming flow rate of precursors.

Seroepidemiology of spotted fever group and typhus group rickettsioses in humans, South Korea.

<P>The prevalence of spotted fever group (SFG) and typhus group (TG) rickettsioses was investigated in 3,362 sera by immunofluorescence assay. The serum samples were obtained from patients with acute febrile episodes in South Korea from December 1992 to November 1993. The number of polyvalent positive sera against SFG rickettsial agents at the level of 1: 40 dilution was 269 (8%) in Rickettsia sibirica, 482 (14.34%) in R. conorii, and 546 (16.24%) in R. akari. Many of the positive sera contained immunoglobulin (Ig) M antibodies rather than IgG antibodies. These results strongly suggest that SFG rickettsioses are prevalent in Korea. For TG rickettsial agents, the number of positive sera was 1,096 (32.60%) in R. typhi and 951 (28.29%) in R. prowazekii. Only a few epidemic typhus positive sera contained IgM antibodies. The result suggests that recent and/or primary infections of epidemic typhus were very rare in Korea during the said period. Among seven patients who had high titers (1:5,120) of IgG antibody to R. prowazekii, six were over 50 years old. The result suggests that Brill-Zinsser disease was prevalent in Korea.</P>

Interior permanent-magnet synchronous motors (IPMSMs) produce both magnetic and reluctance torques. The reluctance torque is due to the difference between the d- and q-axis inductances based on the geometric rotor structure. The steady-state performance analysis and precise control of the IPMSMs greatly depend on the accurate determination of the parameters. The three essential parameters of the IPMSMs are the armature flux linkage of the permanent magnet, the d-axis inductance, and the q-axis inductance. In the basic design step of an IPMSM, the inductance parameters are very important for determining the motor characteristics, such as the input voltage, torque, and efficiency. Thus, it is very important to accurately estimate the values of the motor inductances. The inductance parameters of IPMSMs have nonlinear characteristics along the magnet size because the iron core is saturated by the magnet and armature reaction fluxes. In this study, the inductance parameters were calculated using both the magnetic-equivalent-circuit method and the finite-element method (FEM). Then the calculated parameters were compensated by the saturation coefficient function, which was also calculated via the magnetic-equivalent-circuit method and FEM.

<P>In this paper, a method for predicting vibrations due to an electromagnetic force from among the other types of vibrations that occur in an electric motor is proposed. Among electromagnetic forces, the forces applied in the radial direction were separated into their spatial and temporal harmonic components for developing the vibration-prediction method. Each separated radial force densities acts as a vibration source and generates vibrations combined with the mechanical characteristics of the motor. The separation of vibration sources makes it possible to predict the magnitude of the vibration velocity occurring in the motor. This enables optimal motor design considering vibration. To this end, a description explaining the characteristics of electromagnetic vibrations that occur in an eight-pole nine-slot permanent magnet synchronous motor has been provided in this paper as an example. In addition, the proposed method was verified by comparing the obtained vibration velocity with that obtained through electromagnetic-vibration coupled analysis.</P>

<B>ABSTRACT</B><P>A nested PCR assay was developed for the detection of spotted fever group (SFG) rickettsiae in serum samples. The assay was based on specific primers derived from the rickettsial outer membrane protein B gene (<I>rompB</I>) of <I>Rickettsia conorii</I>. An SFG rickettsia-specific signal is obtained from <I>R. akari</I>, <I>R. japonica</I>, <I>R. sibirica</I>, and <I>R. conorii</I>. Other bacterial species tested did not generate any signal, attesting to the specificity of the assay. As few as seven copies of the <I>rompB</I> gene of <I>R. conorii</I> could be detected in 200 μl of serum sample. The assay was evaluated with a panel of sera obtained from patients with acute-phase febrile disease tested by immunofluorescent antibody assay (IFA). The SFG rickettsia-specific DNA fragment was detected in 71 out of 100 sera, which were proven to have immunoglobulin M antibodies against SFG rickettsial antigen by IFA. The results were further confirmed by restriction fragment length polymorphism and sequencing analysis of the DNA fragments. The results indicated that this PCR assay is suitable for the diagnosis of spotted fever group rickettsiosis in Korea.</P>
S-97 The Predictors of Follow up Loss in Patients with Inflammatory Bowel Disease

There were several studies about the predictors of non-adherence to inflammatory bowel disease (IBD) therapy. However, there were few studies about the predictors of stopping visiting clinics in patients with IBD. Our aims were to evaluate the predictors of follow up loss of visiting clinics in IBD patients. Methods: The medical record of 224 patients with IBD who were followed up between January 2010 and December 2013 were reviewed retrospectively. Follow up loss was defined that patients didn't visit clinics at least 6 months. Results: Among 224 patients with IBD, there were 94 patients with Crohn’s disease and 130 patients with ulcerative colitis. Mean follow up duration was 58.51 months. The follow up loss was noted in 32.1% of patients. The follow up loss rate in patients with CD and UC were 28.7% and 34.6 % respectively. Age, gender and disease severity index at the last visit before being absent the clinics could not show any difference according to the status of follow up. Mean follow up loss duration was 17.72 months. All of the patients who followed up loss revisited clinics and the most common cause of revisit was flare up of the diseas . Lower Education status and C reactive protein also increased the risk for follow up loss by 2.13 and 1.81 times, respectively. By multivariate analysis, risk factors of follow up loss were C reactive protein level below 1.2mg/dL and lower educational status. Conclusions: The follow up loss rate in patient of IBD was 32.1%. Lower educational status and low C reactive protein level increased the risk of follow up loss in IBD patients. Clinician takes an effort to help the patients with lower educational status or low C reactive protein level understand the disease and disease course to prevent follow up loss.
