        결합조직형성 법랑모세포종의 방사선학적 소견 : 증례보고 3례

        Desmoplastic ameloblastoma is a rare histologic variant of ameloblastoma. It shows important differences in anatomic distribution, histologic appearance, and radiographic findings compared with the general type of ameloblastoma. It is histologically characterized by an abundance of densely collagenous stroma and radiographically a mixed radiolucent-radiopaque lesion. We present three cases of desmoplastic ameloblastoma. All the patients complained on buccal swelling with or without pain and the sites of occurrence were the anterior and the premolar region. Plain radiographs showed mixed radiopaque-radiolucent lesion with ill-defined or diffuse sclerotic margin and no external root resorption. Additionally, CT scans revealed buccal expansion and relatively well-defined margin of the lesions. The clinical and radiographic features of the presented cases were compared with those of the desmoplastic ameloblastoma in the previous literatures.       • 온도·습도가 돈분뇨에서 발생되는 암모니아에 미치는 영향

        대량의 유기물을 함유하는 가축 분뇨는 최근 대규모화된 가축 사육에 의하여 그 발생량이 늘어나고 있다. 가축 분뇨를 자원화하거나 에너지화하는 연구가 현재 활발히 진행되고 있지만, 악취문제는 쉽게 해결이 되지 않아 주변 민가와의 분쟁이 심각한 상황이다. 특히 가축분뇨의 발생량 중 40%를 차지하는 돼지의 분뇨는 수분함 유량이 90%나 되어 미부숙된 분뇨로 퇴비화할 경우 대기 중으로 휘산되기 쉬어 악취 발생에 취약하다. 돼지분뇨 악취의 대표 물질 중의 하나인 암모니아는 배출 규제 농도로 저감되어도 실제 주변 민가는 악취로 인한 고통을 호소한다. 돼지분뇨의 악취는 온도와 습도에 상당한 영향을 받기 때문에 본 연구에서는 이러한 돼지 분뇨의 악취를 효율적으로 억제하기 위하여 온도와 습도를 제어하여 미치는 영향을 알아보았다. Lab-Scale 실험을 위해 항온항습기(HB-105SG, 500×500×600(W×D×H)mm)를 제작하여 돈분뇨 슬러리 시료 25ml를 이용하여 1시간 동안 악취를 발생시켰다. 샘플링은 다양한 온도와 다양한 습도에서 발생되는 가스시료를 진공감압박스와 흡인펌프를 이용하여 1분에 3L의 유량으로 1L Tedlar bag에 포집하였고, 포집된 가스는 현장에서 암모니아 검지관(Kitagawa/Japan)을 이용하여 즉석에서 악취를 측정하였다. 검지관의 측정농도 범위는 0.2-20ppm과 1-200ppm으로 100ml의 가스시료를 채취하여 암모니아의 농도를 측정하였다. 사육 시설 내 겨울철의 평균 기온으로 가정하여 온도는 10℃-20℃, 습도는 50%-80%의 온·습도로 설정하였다. 실험 결과, 10℃의 온도에서 습도가 50%일 때 암모니아 농도가 4.8ppm이였는데 습도 80%로 증가 시 7.5ppm으로 56.3% 증가하였고, 20℃의 온도에서는 습도가 50%일 때 11.15ppm에서 80%일 때 13.7ppm으로 22.9% 증가하였다. 습도를 기준으로 보았을 때에, 습도 50%에서 온도가 10℃에서 20℃로 상승할 때 4.8ppm에서 11.15ppm으로 132% 증가하였다. 10℃에서 20℃영역에서는 습도의 증가가 암모니아 발생에 미치는 영향이 동일한 습도조건에서 온도의 상승에 의한 발생보다 더 큰 발생요인으로 보여 진다.

      • Clinical evaluation and study on the health-related behaviors of Korean patients with rosacea

        Background: Traditionally, rosacea is classified into erythematotelangiectatic rosacea (ETR), papulopustular rosacea (PPR), and phymatous and ocular rosacea. Recently, rosacea is regarded as a combination of various phenotypes that are classified as one or more subtypes. It is important to control the daily health-related behaviors because rosacea is easy to recur. Objectives: To analyze the clinical characteristics of Korean rosacea patients and which behavior factors affect disease symptoms Methods: We classified phenotypes of rosacea according to the classification provided by the National Rosacea Society in 2017 and recorded health-related behaviors. Statistical analysis was performed to confirm the association between the presence and the severity of each subtype of rosacea and clinical and health-related behavior factors. Results: A Total of 82 patients included. ETR was 62 patients and PPR was 59 patients, and 39 cases were both. Flushing was the most frequent phenotype. The presence of fixed erythema and papules and pustules was associated with increased severity of ETR and PPR, respectively. The severity of ETR was correlated with the frequency of sun exposure and the severity of PPR showed inverse correlation with the frequency of exposure of hot foods. The increase in the grade of ETR did not lead to more PPR. Conclusion: This study can be used to understand the characteristics of Korean rosacea and to educate the patients about behavior factors to control the disease.

        어깨에 발생한 관상 에크린암 1예

        Syringoid eccrine carcinoma (SEC) is a rare cutaneous malignant tumor thought to be derived from eccrine sweat glands. It is usually present in the scalp and face and often occurs in the fourth to seventh decades of life. A 76-year-old female patient visited our department with a 3-year history of a lesion showing a 5×4 cm-sized erythematous firm plaque with ulceration on her right shoulder. Histological findings revealed a tumor consisting of numerous proliferating tubular structures with two layers of basaloid cells with cellular atypia. Some ductal structures showed a tadpole appearance. Based on these findings, the final diagnosis was SEC. The patient was treated with slow Mohs micrographic surgery and a full-thickness skin graft and did not show any recurrence during the follow-up period of 6 months. Herein, we report a very rare case of a 76-year-old woman diagnosed with SEC that developed on the right shoulder. (Korean J Dermatol 2021;59(2):136∼139)

      • CD47 deficient mice exhibit enhanced human peripheral blood mononuclear cell engraftment in NOD-Rag2null IL2rgnull condition

        Human immune system (HIS) mice have become valuable animal models for the study of human diseases, and peripheral blood mononuclear cell engrafted humanized mice (PBMCHu mice) mice provide a useful platform for cancer immunotherapy research. However, there have been limitations such as insufficient engraftment of human immune cells and development of graft versus host disease (GVHD). To overcome these limitation, we generated CD47; Rag2; IL2rg triple KO mice (TKO) using NOD-Rag2null IL2rgnull mice (DKO) and transplantation efficacy of human leukocytes and GVHD incidence were compared in PBMC-Hu mice. Examining flow cytometry results, more than 25% Human CD45+ cells were observed in both TKO groups from 3 weeks after PBMC administration, and it was observed more than 1 week later in DKO groups. Most of the engrafted cells were CD3+ T cells, and some CD19 B cells and CD66b+ granulocytes were also observed. The ratio of human leukocytes engraftment was greater in TKO mice, however, clinical symptoms of GVHD such as weight loss, jaundice, anemia and death of animals were more severe than in DKO mice. In histopathology, inflammatory cell aggregation, necrotic or apoptotic cells were observed in liver, lung and kidney, and semi-quantitative lesion scores were greater in TKO mice groups. Human CD45, CD3 and CD19 positive cells were also observed in immunohistochemistry. CD47 protein present on myeloid cells and inhibit phagocytosis through ligation of signal-regulatory protein alpha (SIRPα). In this study, we generate TKO mice to improve PBMC-Hu mice. Engraftment of human leukocytes was increased, but symptoms of GVHD were also more severe in TKO mice than DKO mice. Further study for overcoming development of GVHD and poor engraftment of leukocytes other than T cells is needed.

        Characterization of a Squalene Synthase from the Thraustochytrid Microalga Aurantiochytrium sp KRS101

        The gene encoding squalene synthase (SQS) of the lipidproducing heterotrophic microalga Aurantiochytrium sp. KRS101 was cloned and characterized. The krsSQS gene is 1,551 bp in length and has two exons and one intron. The open reading frame of the gene is 1,164 bp in length, yielding a polypeptide of 387 predicted amino acid residues with a molecular mass of 42.7 kDa. The deduced krsSQS sequence shares at least four conserved regions known to be required for SQS enzymatic activity in other species. The protein, tagged with His6, was expressed into soluble form in Escherichia coli. The purified protein catalyzed the conversion of farnesyl diphosphate to squalene in the presence of NADPH and Mg2+. This is the first report on the characterization of an SQS from a Thraustochytrid microalga.

        Vaccination Policy in Korean Armed Forces: Current Status and Future Challenge

        <P>Infectious diseases have historically resulted in suspended or cancelled military operations. Vaccination for disease prevention is a critical component of the military's force readiness doctrine. Until recently, Korea had not recognized the importance of vaccinating military personnel. However, a 2011 meningococcal disease outbreak at an army recruit training center led to dramatic changes in the paradigm of traditional medical practice in the Korean armed forces. A new vaccination policy was formed by a 2012 Military Healthcare Service Act. Since then, <I>Neisseria meningitidis</I>, hepatitis A, and measles-mumps-rubella vaccines have been routinely administered to all new recruits early in basic training to ensure protection against these diseases. All active-duty soldiers also receive seasonal influenza vaccination annually. Despite quantitative improvements in vaccination policies, several instances of major infectious diseases and adverse vaccine reactions have threatened soldier health. In the future, vaccination policies in the Korean armed forces should be based on epidemiologic data and military medical research for vaccine use and safety management.</P>

      • Sequence Control of Small-scaled ITER Power Supply for Reactive Power Compensation

        A technology based on thyristors will be used to manufacture the super-conducting coil AC/DC converters because of the low ratio of cost over installed power compared to a design based on GTO or similar technology. But phase-controlled converter suffers from fundamental disadvantage. They inject current harmonics into the input ac mains due to their nonlinear characteristics, thereby distort the supply voltage waveform, and demand reactive power from the associated ac power system at retarded angles. To overcome this disadvantage, in the case of two series converters at the DC side, connected to the same step-down transformer, apply for the sequence control. It is the most simple and efficient way to reduce the reactive power consumption at low cost. Analytical sequence control algorithm is suggested, the validity of the proposed scheme has been verified by experimental results with the small-scaled International Thermonuclear Experimental Reactor (ITER) Power Supply to minimize reactive power consumption.

      • Genetic polymorphism of DOCK8, IL17RA and KLK12 in Koreans with atopic dermatitis and allergic march

        Background: Atopic dermatitis (AD) is one of the most common childhood diseases and has a complex etiology involving genetic and environmental factors. Thus, a broad understanding of genetic background is needed for early diagnosis of AD. Objectives: The purpose of this study was to identify of novel candidate functional genetic variants in Koreans with atopic dermatitis and allergic march. Methods: Whole-exome sequencing (WES) was performed in 20 AD and 20 AM Korean patients, and 40 Korean personal genome project (KPGP) controls. Sanger sequencing was carried out to validate found variants in 86 AD, 72 AM patients and 81 controls. Results: Three candidate variants of DOCK8 (rs529208), IL17RA (rs1248468), and KLK12 (rs3745540) were identified. rs529208 had no correlation with the development of AD but the AA genotype of DOCK8 had significantly increased total IgE level. rs1248468 had significantly increased risk of AD and the CA genotype of IL17RA had also increased total IgE level. rs3745540 was significantly associated with AD. Conclusion: DOCK8 (rs529208), IL17RA (rs1248468) and KLK12 (rs3745540) were deemed functionally interesting based on WES. Our case-control study suggests that the three variants detected in WES may enhance the risk of AD.
