        선천성 피부 형성부전증 1례 보고

        Aplasia cutis congenita is a disorder of heterogeneous group characterized by isolate or multiple, localized or widespread areas of skin defect at birth. Since the first report by Cordon in 1767, it has been relatively rarely reported disorder abroad as well as domestically . It has several clinical subtypes classified by the location and pattern of skin absence, the presence of associated malformations and the mode of inheritance with unknown cause. We had a newborn male infant with this disorder clinically diagnosed, who showed wide full thickness skin defects on right knee and medial surface of ankle from birth, that covered with thin yellowish membrane and showed focal ulcerative area. On his familial history, there were four aplasia cutis congenita patients in the patient's mother in his maternal family side through four generations. Patient mother showed wide spread atrophic scarred looking appearance on her right lower leg. Chromosomal analysis revealed as normal male karyotype. The biopsy of lesions showed ulcerated surface and underlying chronic inflammation with total absence of epidermis. He had no associated congenital anomaly. The lesions healed rapidly for four weeks by conservative treatment as moist wound dressing and systemic antibiotic administrations. As a relatively uncommon skin anomaly, one case of aplasia cutis congenita is reported with review of references.

        하악 대구치 Ⅱ형 근심 근관에서 치근단 부위의 만곡도 조사

        Objectives: The purpose of this study was to evaluate the buccolingual curvature at the apical one third in type Ⅱ mesial canals of mandibular molars using the radius and angle of curvature. Materials and Methods: Total 100 mandibular molars were selected. Following an endodonticaccess in the teeth, their distal roots were removed. #15 H- or K-files (Dentsply Maillefer) were inserted into the mesiobuccal and mesiolingual canals of the teeth. Radiographs of the teeth were taken for the proximal view. Among them, type Ⅱ canals were selected and divided into two subgroups, Ⅱa and Ⅱb. In type Ⅱa, two separate canals merged into one canal before reaching the apex and in type Ⅱb, two separate canals merged into one canal within the apical foramen. The radius and angle of curvature of specimens were examined. Results: In type Ⅱ, mean radius of curvature in mesiolingual and mesiobuccal canals were 2.82 mm and 3.58 mm, respectively. The radius of the curvature of mesiolingual canals were significantly smaller than that of mesiobuccal canals in type Ⅱ, and especially in type Ⅱa. However, there were no statistically significant differences in radius of curvature between mesiobuccal and mesiolingual canals in type Ⅱb and there were no significant differences in angle of curvature between type Ⅱa and Ⅱb. Conclusion: In this study, type Ⅱ mesial canals of mandibular molars showed severd curvature in the proximal view. Especially, mesiolingual canals of type Ⅱa had more abrupt curvature than mesiobuccal canals at the apical one third.

        Effects of Sleep Patterns on the Subjective Health Status in Older Men from the 7th Korea National Health and Nutrition Examination Survey, 2016

        Background: Sleeping is a major component of health. The prevalence of sleep disorders is expected to be high in older adults, and sleep habits generally differ on weekdays and weekends. This study aimed to clarify the associations between sleep habits and the subjective health status of older adults.         Increase in the genetic polymorphism of varicella-zoster virus after passaging in in vitro cell culture

        Primary infections with the varicella-zoster virus (VZV) result in varicella, while latent reactivation leads to herpes zoster. Both varicella and zoster can be prevented by live attenuated vaccines. There have been reports suggesting that both clinical VZV strains and those in vaccine preparations are genetically polymorphic, containing mixtures of both wild-type and vaccine-type sequences at certain vaccine-specific sites. In this study, the genetic polymorphism of the VZV genome was examined by analyzing the frequencies of minor alleles at each nucleotide position. Next-generation sequencing of the clinical VZV strain YC02 passaged in an in vitro cell culture was used to identify genetically polymorphic sites (GPS), where the minor allele frequency (MAF) exceeded 5%. The number of GPS increased by 7.3-fold at high passages (p100) when compared to low passages (p17), although the average MAF remained similar. GPS were found in 6 open reading frames (ORFs) in p17, 35, and 54 ORFs in p60 and p100, respectively. GPS were found more frequently in the dispensable gene group than the essential gene group, but the average MAF was greater in the essential gene group. The most common two major/minor base pairs were A/g and T/c. GPS were found in all three passages at 16 positions, all located in the reiterated (R) region. The population diversity as measured by Shannon entropy increased in p60 and p100. However, the entropy remained unchanged in the R regions.

        Genetic changes in plaque-purified varicella vaccine strain Suduvax during in vitro propagation in cell culture

        Infection by varicella-zoster virus (VZV) can be prevented byusing live attenuated vaccines. VZV vaccine strains are knownto evolve rapidly in vivo, however, their genetic and biologicaleffects are not known. In this study, the plaque-purified vaccinestrain Suduvax (PPS) was used to understand the geneticchanges that occur during the process of propagation in invitro cell culture. Full genome sequences of three different passages(p4, p30, and p60) of PPS were determined and comparedfor genetic changes. Mutations were found at 59 positions. The number of genetically polymorphic sites (GPS) andthe average of minor allele frequency (MAF) at GPSs were notsignificantly altered after passaging in cell culture up to p60. The number of variant nucleotide positions (VNPs), whereinGPS was found in at least one passage of PPS, was 149. Overall,MAF changed by less than 5% at 52 VNPs, increased by morethan 5% at 42 VNPs, and decreased by more than 5% at 55VNPs in p60, compared with that seen in p4. More complicatedpatterns of changes in MAF were observed when geneticpolymorphism at 149 VNPs was analyzed among the threepassages. However, MAF decreased and mixed genotypes becameunequivocally fixed to vaccine type in 23 vaccine-specificpositions in higher passages of PPS. Plaque-purified Suduvaxappeared to adapt to better replication during in vitro cellculture. Further studies with other vaccine strains and in vivostudies will help to understand the evolution of the VZV vaccine.

        The Effect of Oral Health Belief Influence on the Preventive Oral Health Behaviors in College Students

        Objective: The purpose of this study was to investigate the effects of oral health belief on the personal oral health practice and preventive dental care in college students. Methods: The total 319 individuals completed a self-reported questionnaire on oral health belief such as importance, seriousness, benefits and barriers, modifying factors and oral health behaviors. Results: 1. The Female students showed a higher level of seriousness, importance, benefits (p<0.05), and the respondents who were older than 21 years showed a higher level of seriousness (p<0.05). 2. The group who didn't experience oral health educations showed a higher level of barriers scores (p<0.05) and the group who perceived showed a lower level of perceived oral health status (p<0.05). 3. 'Perceived oral health status' effects on seriousness according to gender, age, oral health education and perceived oral health status (B=0.174, p<0.05). The factor effecting importance were age (B=.0.044) and perceived oral health status (B=0.074). The perceived oral health status effected on perceived barriers (B=0.191). 4. Using multiple regression analysis and logistic regression analysis, the study showed that importance (B=0.076, p<0.05) and barriers (B=0.098, p<0.05) affected on tooth brushing practice and importance (B=0.434, OR=1.543, p<0.05) and oral health education (B=0.795, OR=2.214, p<0.05) affected on regular scaling. Conclusion: Factors affecting oral health behaviors were 'importance', 'barriers' in oral health belief and the oral health education experienece in cue to action.

        The purpose of the study was to propose a direction for residential planning to relieve depression by the change of residential function in the New Normal Era. Thus this study seeks to identify the degree of depression for young adults in their 20s, comparing and analyzing their specific and practical behavior, and derive stress factors and common needs to propose the direction for residential planning. The data for the analysis was collected through the interview and the questionnaire for depression(K-BDI-Ⅱ), from July 1 to August 15 of 2021, and the sample consisted of 20 respondents. Among them, only 16 respondents’ interview results with a score of 10 or more were analyzed by using AEIOU framework. The results are summarized as follows : 1) Considering the factors of skip-floor or folding door to allow space separation and to embrace various activities. 2) Residential elements such as balconies, which have been overlooked due to space expansion should be reconsidered so that the residents can feel open even inside the residential space. 3) It should be possible to meet the needs of realization of various concepts through utilizing LDK formation with workspace, or using elements such as lighting.

        Quantitative Light-Induced Fluorescence-Digital을 이용한 치면세균막 검사법의 임상적 활용 가능성 평가

        The aim of this study was to suggest a convenient method of monitoring the gingival state through plaque detection. Quantitative Light-Induced Fluorescence-Digital (QLF-D), which can assess mature plaque, can be used to assess the oral hygiene status of individuals and to establish an adequate intervention plans for them. The subjects of the study participated in the clinical training at Department of Dental Hygiene, N University. The subjects completed questionnaires on general characteristics and oral hygiene methods. Then, photographs of maxillary and mandibular anterior teeth of the subjects were taken using the QLF-D. After the oral examinations, gingival state was recorded according to the Loe & Silness`s Gingival Index (GI). In addition, a plaque control record was calculated in percentage using disclosing agent. The analysis of the relation between the plaque control record and the QLF-D scores showed positive correlation (r=0.638, p<0.001), and the analysis of the relation between the QLF-D scores and the gingival bleeding index scores also showed positive correlation (r=0.562, p<0.001). Besides, the study classified the participants into healthy gingiva group and the gingivitis group according to the classification criteria of GI, and when the difference of QLF-D scores between the groups was analyzed, the QLF-D scores were statistically significantly higher (t=-2.785, p=0.007) in the gingivitis group (1.71±1.545) than the healthy gingiva group (0.74±1.290). When the differences in mean values of the QLF-D scores were analyzed against and the gingival bleeding index scores, the QLF-D scores of 0 and 1 belonged to the category of normal gingival state while the QLF-D scores of 2, 3 and 4 belonged to the category of gingivitis state. Therefore, it is deemed that the red fluorescence detected by the QLF-D from the mature supragingival plaque can be used for monitoring the state of gingivitis.

        일부 치과환자 및 대학생에서 개량치과불안척도(MDAS)에 따른 고도 치과불안의 관련 요인

        Objectives. The aim of this research was to determine factors associated with high dental anxiety and explain demographic characteristics and dental utilization behaviors in patients with high dental anxiety and students. Methods. In relation to dental anxiety levels classified based on sociodemographical characteristics, significance was determined by a t-test and one-way ANOVA. In addition, the influence of the dental anxiety group was analyzed by logistic regression. Results. Among subjects in the high dental anxiety group, the college student group 19.2% (cut-off score 19) showed a higher frequency number than the patient group 9.7% (cut-off score 19). Among the variables of the high dental anxiety group in the college student group, females 23.7% (cut-off score 19) outnumbered males. Also, subjects aged below 20, which were 24.3% (cut-off score 19) showed the highest frequency number while the highest frequency number in case subjects that postponed their dental appointment was 32.8% (cut-off score 19) or 29.6% for case subjects that cancelled their dental appointment (p<0.05). Among the variables belonging to the high dental anxiety group in the patient group, 21.5% of subjects postponed their dental appointment whereas 20.0% cancelled, and this result was statistically significant (p<0.05). Conclusions. It was found that dental anxiety had a negative effect on a patient`s oral cavity health condition because it was an obstacle to dental utilization behavior, thereby ultimately lowering the quality of life associated with oral cavity health. Therefore, it is important that effective plans to reduce dental anxiety should be considered by offering an appropriate clinical & behavioral science-based mediation service after sorting the high dental anxiety group in advance for dental treatment.

        치과내원 환자의 불안취약사고 경향 및 치과경험과 치과공포의 관련성

        Introduction The aim of this study, personal trait and their dental care based on experience level of dental fear is to analyze the differences. Catastrophizing is an irrational belief that something is far worse than it actually is. Because it makes it possible to analyze patients` anxious thoughts and tendencies. Methods The Dental Fear Survey(DFS) and Anxious Thoughts and Tendencies(AT&T) were used as measuring tools, and the independent sample ttest and ANOVA were performed on the basis of the mean value of the summated scale scores and the standard deviation. On the factors to dental anxiety, the multiple regression analysis was performed. Results Demographically, the total DFS score was higher in women(25.73±8.27) than in men. Also, AT&T was higher in women(31.01±7.05) (p<0.05). Dental fear was intense in the group of patients who visited dental clinics only problems(25.29±8.57) than in those regular visited(22.29±7.78). In relation to dental experiences, the DFS score was significantly higher in children. In many cases, patients experienced therapeutic pains in children(26.40±9.54) (p=0.004). Also in many patients, dental anxiety began to occur in adolescence(44.3%). The multiple regression analysis result, it was found that dental treatment pain and their AT&T wielded great influence upon dental fear.
