        Case Reports : Basal Cell Carcinoma on the Pubic Area: Report of a Case and Review of 19 Korean Cases of BCC from Non-sun-exposed Areas

        Basal cell carcinoma (BCC) is one of the most commonly diagnosed malignant skin tumors and develops characteristically on sun-exposed areas, such as the head and neck. Ultraviolet light exposure is an important etiologic factor in BCCs, and BCCs arising from non-sun- exposed areas are, therefore, very rare. In particular, the axilla, nipple, the genital and perianal areas are not likely to be exposed to ultraviolet light; thus, if BCC develops in these areas, other predisposing factors should be considered. Herein, we report a case of BCC arising on the pubic area in a 70-year-old man. We also performed a survey of the literature and discussed the 19 cases of BCC from non-sun-exposed areas reported to date in Korea.

        The Efficacy and Safety of 4-n-butylresorcinol 0.1% Cream for the Treatment of Melasma: A Randomized Controlled Split-face Trial

        Background: Melasma is a common acquired symmetrical hypermelanosis that occurs on sun-exposed areas, and it is frequently observed among women. Various treatment modalities have been tried, but none are completely satisfactory. 4-n-butylresorcinol, which is a resorcinol derivative that has an inhibitory effect on both tyrosinase and tyrosinase-related protein-1, was introduced in 1995 and it has received increasing attention as a new hypopigmenting agent. However, the hypopigmenting effect of 4-n-butylresorcinol in human subjects has only been shown in a few studies. Objective: The aim of this study was to investigate the hypopigmenting efficacy and safety of 4-n-butylresorcinol 0.1% cream for the treatment of melasma. Methods: Twenty patients with melasma were enrolled to this randomized, double-blind, vehicle-controlled, split-face comparative study. The patients were instructed to apply 4-n-butylresorcinol 0.1% cream or vehicle to each side of the face twice daily for 8 weeks. Mexameter measurements were performed along with photography at baseline, 4 weeks and 8 weeks. Adverse events were observed and recorded throughout the study. Results: All the patients completed the study. Mexameter measurements demonstrated that the melanin index of the treated side showed a significant decrease when compared with that of the vehicle-treated side after 4 weeks (p=0.006) and after 8 weeks (p<0.0005). All the adverse reactions were mild and transient. Conclusion: 4-n-butylresorcinol 0.1% cream showed rapid efficacy and it was well tolerated when used for the treatment of melasma.         Geosynchronous Magnetic Field Response to Solar Wind Dynamic Pressure

        The present study examines the morning-afternoon asymmetry of the geosynchronous magnetic field strength on the dayside (magnetic local time [MLT] = 06:00~18:00) using observations by the Geostationary Operational Environmental Satellites (GOES) over a period of 9 years from February 1998 to January 2007. During geomagnetically quiet time (Kp < 3), we observed that a peak of the magnetic field strength is skewed toward the earlier local times (11:07~11:37 MLT) with respect to local noon and that the geosynchronous field strength is larger in the morning sector than in the afternoon sector. That is, there is the morning-afternoon asymmetry of the geosynchronous magnetic field strength. Using solar wind data, it is confirmed that the morning-afternoon asymmetry is not associated with the aberration effect due to the orbital motion of the Earth about the Sun. We found that the peak location of the magnetic field strength is shifted toward the earlier local times as the ratio of the magnetic field strength at MLT = 18 (B-dusk) to the magnetic field strength at MLT = 06 (B-dawn) is decreasing. It is also found that the dawn-dusk magnetic field median ratio, B-dusk/B-dawn, is decreasing as the solar wind dynamic pressure is increasing. The morning-afternoon asymmetry of the magnetic field strength appears in Tsyganenko geomagnetic field model (TS-04 model) when the partial ring current is included in TS- 04 model. Unlike our observations, however, TS-04 model shows that the peak location of the magnetic field strength is shifted toward local noon as the solar wind dynamic pressure grows in magnitude. This may be due to that the symmetric magnetic field associated with the magnetopause current, strongly affected by the solar wind dynamic pressure, increases. However, the partial ring current is not affected as much as the magnetopause current by the solar wind dynamic pressure in TS-04 model. Thus, our observations suggest that the contribution of the partial ring current at geosynchronous orbit is much larger than that expected from TS-04 model as the solar wind dynamic pressure increases.

        A rare case of abdominal adenoid basal cell carcinoma in a patient with a history of radiation therapy

        Basal cell carcinoma (BCC) is the most common skin cancer and its incidence is steadily increasing. Prior radiation therapy is one of the most important risk factors for BCC. Although the mechanism remains undefined, long-term studies have shown that people exposed to radiation have an increased risk of BCC. Despite the fact that BCC occurs most frequently in sun-exposed areas of the body, patients with a history of radiation therapy have an increased risk of BCC in areas previously exposed to radiation. Here, we report a case of adenoid BCC on the abdomen in a 67-year-old woman after radiation therapy post-hysterectomy.

        백서에서 Depulpin®과 Formocresol에 대한 치수와 치근단 조직의 반응

        One fifth dilution of formocresol is usually for pulpotomy of the primary teeth and emergency pulpotomy of the permanent teeth. However, the use of formaldehyde has been subjected to criticism because it may be absorbed into the blood stream and become distributed systemically, it may also alter the pulp tissue rendering it immumologically active, and have carcinogenic potential. Recently Depulpin®(VoCo., Germany) gains popularity as a devitalizing agent during root canal therapy in spite of high concentration of 49% paraformaldehyde because it facilitate devitalization of pulp and make root canal therapy easier. But there have been not enough publications about the reaction of pulp and periapical tissue caused by Depulpin. This study was performed to evaluate the histological changes in pulp and periapical tissue of rats after pulpotomy using formocresol and Depulpin and to elucidate the toxic effects of these agents. Thirty six Sprague-Dawley rats were anesthetized by intraperitoneal injection of ketamine. Maxillary first molar teeth were used for pulpotomy with formocresol and Depulpin. Rats were sacrificed after 2 days, 4 days, 1 week, 2 weeks, 3 weeks and 4 weeks respectively. Specimens were histologically observed by light microscope changes in pulp and periapical tissue. The obtained results were as follows. 1. Formocresol group A zone of fixed tissue, in which odontoblasts could clearly be defined, was present directly underneath the pulpotomy dressing in almost all teeth of this group. This was followed by an area of necrotic tissue which resembled dried out fibrous tissue with no cellular detail except some pyknotic nuclei. In the specimens of after 2 days, 4 days, 1 week, 2 weeks in which vital tissue was present. it was separated from the fibrous area by a zone of inflammation. In the specimens of after 3 weeks and after 4 weeks, inflammatory infiltrate was in the periodontal ligament adjacent to the apical foramina of the teeth. 2. Depulpin® group The area of necrotic tissue which had no cells and fibers , was present adjacent to the dressing. This was followed by dried out fibrous tissue with no cellular details except some pyknotic nucleli. A short stump of vital pulp with odontoblasts was present at the end of the canal after 2 days. Inflammatory infiltrate was in the periodontal ligament after 4 days and after 1week. Severe root resorption and necrosis of periapical tissue opposite the root resorption site were defined after 2 weeks and after 3 weeks. Periapical lesion which consist of necrotic tissue surrounded by a fibrous connective wall. was found after 4 weeks. The results indicated that Depulpin can cause more adverse reaction to the dental pulp and periapical tissue than formocresol, and further studies are needed for its clinical use with safety.

      • Violin 奏法에 있어서의 左手

        In violin performance the fundamental works of the left hand include the proper use of the whole left arm and the each part of that arm as a unit. So the concepts on the fundamental techniques of the left hand are based on the formation of fingering due to the use of left hand and its members. The essential finctons of the left arm in violin performance are well known to us, but a good player can create his own techniques. Though there exists the limits of the finger-ing and the work of the left arm, we can obtain much technical mode and formation of fingering, And, generally, the synthetic techniques of the left hand are based on the fol-lowing five actions, 1. The falling actions of the four fingers. 2. The actions for the chromatic scale and for the stretching of the fingers. 3. The actions of the fingers for chords. 4. The actions of the thumb returning from the 3rd position to the lst positon. 5. The combined actions of the hand and articulations in the lst position, which are found when the fingers go up to produce the highest sound, on the contrary, go down to make the lowest one. To give a full detail of them, they are as follows; ① The position of the left arm. ②Fingering ③Vibrato The theory of the essential techniques of the left hand is formed by above mentional actions and compositions in which the actions are available. In other words. we must observe those actions more carefully in the mechanical point of view. Each finger has its own flexibility and force balanced to its own weight, and the player make it fall on the string. Remembering all these points, we had better to practising the violin, and must devel-ope the playing techniques.

      • 소아 그레이브스병에서 부신호르몬 및 IgE의 변화

        Graves' disease is a autoimmune thyroid disease. In pathogenesis of Graves' disease, T cell is known as that helps production of thyroid autoantibodies. Endocrine and immune systems are connected and interdependent. Adrenal gland plays an important role in this network and control the balance between serum levels of dehydroepiandrosterone sulfate(DHEAS) and cortisol. The authors evaluated the change of adrenal androgen, cortisol and immunoglobulin E as immune modulators in Graves' disease. From March, 1999 to August, 2002, Serum DHEAS, 24 hour urine free cortisol(F) and IgE and thyroid autoantibodies(TsAb, TBII, AMA, ATA) examined in 22 Graves' patients Serum IgE increased in 41.2% of all patients(97.41±99.07IU/mL). DHEAS was increased in only 5.5% of all patients, and it was decreased or included in normal range in 94.5% of patients(99.3±91.49μg/dL). DHEAS was correlated with ATA(r=0.517).Twenty-four hour urine free cortisol was increased in 72.2% of patients(64.45±47.59μg/dL), but it had no interrelationship with thyroid autoantibodies. Ratio of DHEAS/F had 2.08±2.06, and it had negative correlation with TsAb and TBII, respectively(r=-0.487, r=-0.565). These data demonstrate that there are change of adrenal hormones(DHEAS and cortisol) and IgE, and dis-equilibrium between two adrenal hormones in children with Graves' disease, and Thse might be some relevant to the T cell immune response induction of Graves' disease.
