Objective: The addition of maintenance olaparib to bevacizumab demonstrated a significant progression-free survival (PFS) benefit in patients with newly diagnosed, advanced ovarian cancer in the PAOLA-1/ENGOT-ov25 trial (NCT02477644). We evaluated maintenance olaparib plus bevacizumab in the Japan subset of PAOLA-1. Methods: PAOLA-1 was a randomized, double-blind, phase III trial. Patients received maintenance olaparib tablets 300 mg twice daily or placebo twice daily for up to 24 months, plus bevacizumab 15 mg/kg every 3 weeks for up to 15 months in total. This prespecified subgroup analysis evaluated investigator-assessed PFS (primary endpoint). Results: Of 24 randomized Japanese patients, 15 were assigned to olaparib and 9 to placebo. After a median follow-up for PFS of 27.7 months for olaparib plus bevacizumab and 24.0 months for placebo plus bevacizumab, median PFS was 27.4 versus 19.4 months, respectively (hazard ratio [HR]=0.34; 95% confidence interval [CI]=0.11–1.00). In patients with tumors positive for homologous recombination deficiency, the HR for PFS was 0.57 (95% CI=0.16–2.09). Adverse events in the Japan subset were generally consistent with those of the PAOLA-1 overall population and with the established safety and tolerability profiles of olaparib and bevacizumab. Conclusion: Results in the Japan subset of PAOLA-1 support the overall conclusion of the PAOLA-1 trial demonstrating that the addition of maintenance olaparib to bevacizumab provides a PFS benefit in patients with newly diagnosed, advanced ovarian cancer. Study Design: Prospective study based on magnetic resonance (MR) imaging of the lumbar spinal root of the intervertebral foramen. Purpose: This study was to compare MR three-dimensional (3D) sequences for the evaluation of the lumbar spinal root of the intervertebral foramen. Overview of Literature: The diagnosis of spinal disorders by MR imaging is commonly performed using two-dimensional T1- and T2-weighted images, whereas 3D MR images can be used for acquiring further detailed data using thin slices with multi-planar reconstruction. Methods: On twenty healthy volunteers, we investigated the contrast-to-noise ratio (CNR) of the lumbar spinal root of the intervertebral foramen with a 3D balanced sequence. The sequences used were the fast imaging employing steady state acquisition and the coherent oscillatory state acquisition for the manipulation of image contrast (COSMIC). COSMIC can be used with or without fat suppression (FS). We compared these sequence to determine the optimized visualization sequence for the lumbar spinal root of the intervertebral foramen. Results: For the CNR between the nerve root and the peripheral tissue, these were no significant differences between the sequences at the entry of foramen. There was a significant difference and the highest CNR was seen with COSMIC-FS for the intra- and extraforamen. Conclusions: In this study, the findings suggest that the COSMIC-FS sequences should be used for the internal or external foramen for spinal root disorders.

Purpose: The fixed flexion view (FFV) of the knee is considered useful for evaluating the joint space when assessing the severity of osteoarthritis (OA) of the knee. To clarify the usefulness of FFV for evaluation of the joint space and severity of knee OA, this study evaluated changes in the joint space on the FFV and standing extended view (SEV) in patients with knee OA. Materials and Methods: The SEV and FFV images were acquired in 567 patients (1,102 knees) who visited the hospital with a chief complaint of knee joint pain. Medial joint space width (MJSW) and Kellgren-Lawrence (K-L) classification assessed using the SEV and FFV images were compared. Results: Mean MJSW was significantly smaller when assessed on the FFV than on the SEV (3.02±1.55 mm vs. 4.31±1.30 mm; p<0.001). The K-L grade was the same or higher on the FFV than on the SEV. Conclusions: The FFV is more useful than the SEV for evaluating the joint space in OA knees. Treatment strategies in patients with knee OA should be determined based on routinely acquired FFV images.

Objective: This study was conducted to ascertain the correlation between preserved pelvic nerve networks and bladder function after laparoscopic nerve-sparing radical hysterectomy. Methods: Between 2009 and 2011, 53 patients underwent total laparoscopic radical hysterectomies. They were categorized into groups A, B, and C based on the status of preserved pelvic nerve networks: complete preservation of the pelvic nerve plexus (group A, 27 cases); partial preservation (group B, 13 cases); and complete sacrifice (group C, 13 cases). To evaluate bladder function, urodynamic studies were conducted preoperatively and postoperatively at 1, 3, 6, and 12 months after surgery. Results: No significant difference in sensory function was found between groups A and B. However, the sensory function of group C was significantly lower than that of the other groups. Group A had significantly better motor function than groups B and C. No significant difference in motor function was found between groups B and C. Results showed that the sensory nerve is distributed predominantly at the dorsal half of the pelvic nerve networks, but the motor nerve is predominantly distributed at the ventral half. Conclusion: Various types of total laparoscopic nerve-sparing radical hysterectomies can be tailored to patients with cervical carcinomas.
Medial and Lateral Discoid Menisci of Both Knees

Discoid menisci on both the medial and lateral sides are rare, and there are very few reports on cases involving both sides. We report a case of a 52-year-old female with medial and lateral discoid menisci in both knees. Arthroscopy revealed the lateral menisci of both knees were complete discoid menisci, and partial meniscectomy was performed. The medial menisci were incomplete discoid menisci, but there were no findings of abnormal mobility or injury; therefore, the medial menisci were observed without treatment. At six months postoperatively, her pain and range of motion restrictions disappeared.

As a major component of a power plant, a turbine generator must have sufficient reliability. Longer blades have lower natural frequency, thereby requiring that the design of the shaft and blade takes into account the coupling of the blade vibration mode, nodal diameter k=0 and k=1 with vibration of the shaft. The present work analyzes the coupling of the translation motion of the shaft with in-plane vibration of the blades with k=1 modes. At a rotational speed ${\Omega}_1=|{\omega}_s-{\omega}_b|$, the resonance of the blades has a relatively large amplitude. A violent coupled resonance was observed at a rotational speed ${\Omega}_2=|{\omega}_s+{\omega}_b|$. Resonance in blade vibration at ${\Omega}_1=|{\omega}_s-{\omega}_b|$ was experimentally confirmed.

Objective: To investigate the relationship between the precursors of high grade serousovarian cancer (HGSOC) and the characteristics of patients with a low HGSOC risk in termsof the effects of pregnancy. Methods: We prospectively examined consecutive cases in which the bilateral fallopian tubeswere removed during benign gynecological or obstetric surgery and assessed the relationshipbetween the patient characteristics, including parity and pregnancy, and the incidence ofHGSOC precursors. All the fallopian tubes were examined by applying the Sectioning andExtensively Examining the Fimbriated End (SEE-FIM) Protocol. Results: Of the 113 patients enrolled, 67 were gynecological and 46 were obstetric. The p53signature was identified in 21 patients. No other precursors were identified. In a comparisonof the p53 signature-positive and negative groups, parous women and pregnant womenwere significantly fewer in the p53 signature-positive group (53% vs. 86%, p=0.002, 10%vs. 47%, p=0.001, respectively). Current pregnancy was also associated with a significantlylower incidence of the p53 signature after multivariate adjustment (odds ratio [OR]=0.112;95% confidence interval [95% CI]=0.017–0.731; p=0.022). Among gynecological patients,parous women were fewer in the p53 signature-positive group on univariate (47% vs. 73%,p=0.047) and multivariate analysis (OR=0.252; 95% CI=0.069–0.911; p=0.036). No othercharacteristics were associated with p53 signature positivity. Conclusions: The incidence of the p53 signature was significantly lower in parous womenand pregnant women. This decreased incidence of early phase serous carcinogenesis may beone of the possible mechanisms underlying HGSOC risk reduction among parous women.

The bluegill sunfish (Lepomis macrochirus) is a notorious exotic fish, which causes serious damages to freshwater ecosystems in the world. Therefore, it is a matter of urgency to control this species for the conservation of ecosystem. New microsatellite loci were developed for L. macrochirus using high throughput 454 GS-FLX+ pyrosequencing. We selected 24 primer pairs that were tested on 47 individuals from Lake Biwa, which encompasses the oldest and largest population of L. macrochirus in Japan. Allele per locus varied 2–17. Expected and observed heterozygosities ranged from 0.185 to 0.888 and 0.120 to 0.872, respectively. Applied to L. macrochirus in Lake Biwa, these markers could detect genetic differentiation among populations, which showed a pattern of isolation by distance. These markers are expected to be useful in the estimation of population structure and ecology of L. macrochirus.
