Background: The U.S. Food and Drug Administration has prohibited epidural steroid injection (ESI) with particulate steroids. Thus, this study aimed to compare the efficacy and safety of ESI with two nonparticulate steroids, dexamethasone and betamethasone. Methods: The eligible patients (n = 600) who received ESI (0 week) with dexamethasone (ESI-dexa) or betamethasone (ESI-beta) had follow-up visits at 2, 4, and 8 weeks with a phone interview at 12 weeks. The primary endpoint was the proportion of effective responders without pain or who were much improved at 2 weeks. The secondary endpoints were the proportion of crossover injections at 2 weeks; changes in the visual analog scale (VAS) and disability index scores at 2, 4, and 8 weeks; the number of additional ESIs in 12 weeks; the number of participants having spinal surgery, as well as the incidence of adverse events over the 12 weeks. Results: The proportion of effective responders at 2 weeks was not different between ESI-beta (72/216, 33.3%) and ESI-dexa (63/200, 31.5%; P = 0.670). Adverse events were more common with ESI-dexa (40/200, 20.0%) than with ESI-beta (24/216, 11.1%; P = 0.012). VAS scores decreased more with ESI-beta than with ESI-dexa at 2 weeks (difference, 0.35; P = 0.023) and 4 weeks (difference, 0.42; P = 0.011). The disability score improved significantly more with ESI-beta compared with ESI-dexa at 2 weeks (difference, 3.37; P = 0.009), 4 weeks (difference, 4.01; P = 0.002), and 8 weeks (difference, 3.54; P = 0.007). Conclusions: Betamethasone would be more appropriate for ESI.

The introduction of a digiral subscriber loop transmission system necessitates an optimized line interface solution. To meet this objective an adaptive line equalizer has been developed. The equalizer can be compensated up to 42 dB line loss at 200KHz, and operated up to 3.2 Km transmission length (0.4 mm\ulcornercable)at a rate of 400Kb/s. This has been builted using a variable \ulcorner equalizer to compensate a frequency-attenuation characteristics of metallic cable, an AGC (automatic gain control) circuits with simple control algorithm, and various filters to minimize a transmission constraints over subscriber loop. The purpose of this paper is to present a short description of a design of the adaptive line equalizer with a summary of implementation results. Some design concepts and considerations which results in an implementation of the equalizer are also given.

A High Frequency Isolated Single-Phase Impedance-Source AC-AC Converter

This paper deals with a new family of single-phase impedance source ac-ac converters which employs a high frequency transformer between the impedance source network and the load. The proposed structure have unique features: providing a wide range of output ac voltage with buck-boost, maintaining or reversing the phase angle, and reduced inrush and harmonic current component. In addition to the aforementioned benefits the proposed converter incorporates a high frequency transformer which provides isolation between input and output, and removes the need for bulky 60 Hz low frequency transformer which improves the efficiency of the converter in terms of size, weight and cost of the transformer. Moreover, high frequency transformer can also be utilized in adjusting the gain of the converter. An experimental prototype of proposed converter is built and tested to prove its performance and effectiveness.

Red ginseng, a steamed and sun-dried ginseng, is a popular health-promoting food in Korea and other Asian countries. We introduced nanofertilizer technology using gold nanoparticles in an effort to develop red ginseng with an elevated level of ginsenosides, the main active compounds of ginseng. Shoots of 6-year-old ginseng plants were fertilized three times with colloidal gold nanoparticle sprays. Red ginseng extract was prepared from the main roots. The concentrations of gold and ginsenosides were measured following gold nanoparticle treatment. To evaluate the anti-inflammatory effects, mouse peritoneal macrophages of male BALB/c mouse were stimulated with lipopolysaccharide plus interferon-γ in the presence of extracts from red ginseng with or without gold nanoparticle treatment. The content of ginsenosides, such as Rg1, Re, Rf, and Rb1, increased in ginseng treated with gold nanofertilizer whereas the steaming process increased only the levels of Rd and Rg3. The levels of nitric oxide, inducible nitric oxide synthase, and interleukin-6, but not tumor necrosis factor-α, were more suppressed in macrophages treated with extract from gold nanoparticle-treated red ginseng. Our results show that the use of a colloidal gold nanoparticle fertilizer improved the synthesis of ginsenosides in ginseng and enhanced the anti-inflammatory effects of red ginseng. Further research is required to elucidate the causal factors for the gold-induced change in ginsenoside synthesis and to determine the in vivo effect of gold nanoparticletreated ginseng.
Single-Phase Isolated Impedance-Source Inverters

This paper presents a new class of impedance source inverters which offers high frequency isolation between the inverter bridge and the load. Conventional Zsource inverter which employs a impedance network consisting of inductors and capacitors have unique features which can be realized as both buck and boost functions without the need of dead time and overlap time, that cannot be achieved by traditional voltage-fed and current-fed inverters. This paper extends this novel concept by using electrical isolation in impedance source inverters. High frequency isolation itself have limitless advantages in terms of safety, reliability etc. when applied with impedance source inverter makes it a preferable choice for industrial applications. In PV systems a dc circulating current emerges and flows into grid which damages the whole system, electrical isolation prevents the injection of this dc current into the grid. Moreover the gain of the proposed inverter can also be adjusted by varying the turns ratio of the high frequency transformer giving more freedom in utilizing a higher modulation index, results in increased efficiency in terms of waveform quality, improved power factor, lower component voltage stress etc. An experimental prototype is constructed to validate the working operation of the proposed inverter.

<P>The purpose of this study was first, to determine the sensitivity, specificity, and accuracy of MDCT arthrography (CTA) for the diagnosis of acetabular labral tear and sulcus; second, to correlate tear types using the Lage classification system on CTA compared with the arthroscopic classification; and third, to correlate CTA localization with arthroscopic localization.</P>
