        Association of CD40 gene polymorphisms and immune thrombocytopenic purpura in the adult Egyptian population

        Background The pathophysiology underlying primary adult immune thrombocytopenic purpura (ITP) has not yet been identified. However, many mechanisms affect the immune system, causing defective tolerance to self-platelets and megakaryocytes. Cluster of differentiation 40 (CD40) contributes to both humoral and cell-mediated immune responses. Methods This case‒control study was conducted to detect rs4810485G>T and rs1883832C>T polymorphisms of CD40 in Egyptian patients with persistent/chronic ITP to clarify their possible association with chronic disease evolution. This study included 50 patients with persistent/chronic ITP and 50 healthy controls. Genotyping was performed using the polymerase chain reaction‒restriction fragment length polymorphism technique. Results Genotyping of rs1883832 and rs4810485 revealed no statistically significant differences between the two groups. However, combined gene polymorphism genotyping showed a statistically significant difference between the two groups (P <0.01). Conclusion Our results indicate a strong association between the combined polymorphism of both genes and susceptibility to developing ITP among adult Egyptian patients. Targeting this pathway using novel therapeutic approaches is promising.

        Clinical Utility of Chest Sonography in Chronic Obstructive Pulmonary Disease Patients Focusing on Diaphragmatic Measurements

        Background: There are many methods of evaluating diaphragmatic function, includingtrans-diaphragmatic pressure measurements, which are considered the key rule of diagnosis. We studied the clinical usefulness of chest ultrasonography in evaluating stablechronic obstructive pulmonary disease (COPD) patients and those in exacerbation,focusing on diaphragmatic measurements and their correlation with spirometry andother clinical parameters. Methods: In a prospective case-control study, we enrolled 100 COPD patients dividedinto 40 stable COPD patients and 60 patients with exacerbation. The analysis included20 age-matched controls. In addition to the clinical assessment of the study population,radiological evaluation included chest radiographs and chest computed tomography. Transthoracic ultrasonography (TUS) was performed for all included subjects. Results: Multiple A lines (more than 3) were more frequent in COPD exacerbation thanin stable patients, as was the case for B-lines. TUS significantly showed high specificity,negative predictive value, positive predictive value, and accuracy in detecting pleuraleffusion, consolidation, pneumothorax, and lung mass. Diaphragmatic measurementswere significantly lower among stable COPD subjects than healthy controls. Diaphragmaticthickness and excursion displayed a significant negative correlation with bodymass index and the dyspnea scale, and a positive correlation with spirometry measures. Patients in Global Initiative for Chronic Obstructive Lung Disease (GOLD) groupD showed lower diaphragmatic measurements (thickness and excursion). Conclusion: The TUS of COPD patients both in stable and exacerbated conditions andthe assessment of diaphragm excursion and thickness by TUS in COPD patients andtheir correlations to disease-related factors proved informative and paved the way forthe better management of COPD patients.

      • A Representation Model for Reusable Assets To Support User Context

        In the field of software reuse, several methods for storage and retrieval of assets abound. However, these methods often fined their limits; they fail to turn up the suitable reusable assets that satisfy the needs of a particular software system under development. Two problems are the root cause of this situation. One is the lack of accurate semantics for describing software assets. The other is the ignorance of the user query context. In such a context, this paper presents an XML-based asset representation model for describing all kinds of software asset that can be reused within software development process. The proposed model provides semantic metadata for describing assets oriented user context in order to build the foundation for semantic reasoning in the retrieval process.

        Pedagogically-Driven Courseware Content Generation for Intelligent Tutoring Systems

        This paper describes a novel approach to adaptive courseware generation. This approach adopts its structure from existing intelligent tutoring systems and introduces a new component called pedagogical scenario model to support pedagogical flexibility in the adaptation process of courseware generation system. The adaptation is carried out using Dynamic Constraint Satisfaction Problem framework, which is a variant of classical Constraint Satisfaction Problem, to deliver courseware tailored to individual learner. Such a framework provides a high level of expressiveness to deal with the particular characteristics of courseware generation problem. Further, it automatically designs a sound courseware satisfying the design constraints imposed by the domain, the pedagogical scenario and learner models.

      • Synergistic Activity of Paclitaxel and Sorafenib Against Liver Cancer Stem Cells

        Aims: Cancer stem cells (CSCs), also known as tumor-initiating cells (TICs), are suggested to be responsible for drug resistance and cancer recurrence. Current treatments with conventional chemotherapy are not highly efficient against the cancer stem cells (CSCs). The combination of anticancer drugs, of which functions are different from the other, enhances efficiency compared to the mono-therapy because it targets cancer cells in a synergistic or an additive manner. In this study, the effect of paclitaxel and sorafenib on cancer stem cells (CSCs) developed from mouse iPSCs in very low concentration was evaluated. Methods: To investigate the effect of combination therapy, CSCs were exposed to paclitaxel and/or sorafenib at different concentrations of 1, 2 and 4 nM, respectively. Cell viability was assessed with 3-(4,5-dimethylthiazolyl)-2,5-diphenyltetrazolium bromide (MTT). The same concentrations of the agents were assessed for the effect on the self-renewal potential of CSCs subpopulation by sphere formation ability. Results: As a result, a combination of sorafenib and paclitaxel significantly reduced the resistance while the CSCs exhibited drug resistance against paclitaxel alone. Also, combination of these agents reduces the self-renewal potential of CSCs when compared to single treatment. Simultaneously, combination significantly suppressed not only the colony formation but also the tube formation of the Cancer stem cells. Conclusions: These results suggest the combination therapy of paclitaxel and sorafenib in low doses be an attractive approach to target cancer stem cells in the future.

        Fe/Fe<sub>2</sub>O<sub>3</sub> nanoparticles as anode catalyst for exclusive power generation and degradation of organic compounds using microbial fuel cell

        <P><B>Abstract</B></P> <P>Iron/iron oxide (Fe/Fe<SUB>2</SUB>O<SUB>3</SUB>) nanoparticles were deposited on the surface of different carbonaceous anode materials: carbon felt (CF), carbon cloth (CC), and graphite (G) as an effective catalyst to improve the anode performance of microbial fuel cell (MFC) based on the real industrial wastewater. Interestingly, the results of the characterization indicated the novel structure of the iron nanoparticles enveloped with a thin layer of iron oxide formed on the anode surfaces. This novel structure enhances the surface wettability of the electrode, the degradation reactions rate of organic compounds, and the microorganism adhesion on the electrode surface, and decreases the electron transfer resistance. Therefore, the generated power and current were considerable improved, where, the generated power was increased by 385%, 170%, and 130%, for the CF, CC, and G electrodes, respectively.       • Cobalt oxides-sheathed cobalt nano flakes to improve surface properties of carbonaceous electrodes utilized in microbial fuel cells

        <P><B>Abstract</B></P> <P>A novel nanoflakes of cobalt sheathed with cobalt oxide is electrodeposited on four different carbonaceous anodes; carbon cloth (CC), carbon paper (CP) graphite (G) and activated carbon (AC), to introduce as high-performance anodes of microbial fuel cell (MFC). Interestingly, characterizations results indicated that novel metallic nanoflakes that sheathed by a thin layer of cobalt oxide were formed on the surface of the different anode materials. Moreover, using a simple and effective electrodeposition technique for fabricating of cobalt/cobalt oxide nanoflakes is introduced to overcome the hydrophobicity and the interfacial electron transfer of the anodes. The thin layer of cobalt/cobalt oxide nanoflakes significantly enhanced the microbial adhesion, the wettability of the anode surface and decrease the electron transfer resistance. Alternatively, the toxicity risk of the pure cobalt is overcome by the cobalt oxide layer. The application of the modified anodes in an air-cathode MFCs fed by industrial wastewater resulted in a significant improving in cell performance for the different anode materials. Where, the observed increasing in the power was 103, 137, 173 and 71% for the CC, CP, G and AC electrodes, respectively.
