        BRCA1/2 mutation status in patients with metachronous breast and ovarian malignancies: clues towards the implementation of genetic counseling

        Objective: The characteristics of patients with metachronous breast and ovarian malignancies and the pathogenic role of BRCA1/2 mutations remain poorly understood. We investigated these issues through a review of hospital records and nationwide Taiwanese registry data, followed by BRCA1/2 mutation analysis in hospital-based cases. Methods: We retrospectively retrieved consecutive clinical records of Taiwanese patients who presented with these malignancies to our hospital between 2001 and 2017. We also collected information from the Data Science Center of the Taiwan Cancer Registry (TCR) between 2007 and 2015. Next-generation sequencing and multiplex ligation-dependent probe amplification were used to identify BRCA1/2 mutations and large genomic rearrangements, respectively. When BRCA1/2 mutations were identified in index cases, pedigrees were reconstructed and genetic testing was offered to family members. Results: A total of 12,769 patients with breast cancer and 1,537 with ovarian cancer were retrieved from our hospital records. Of them, 28 had metachronous breast and ovarian malignancies. We also identified 113 cases from the TCR dataset. Eighteen hospital-based cases underwent BRCA1/2 sequencing and germline pathogenic mutations were detected in 7 patients (38.9%, 5 in BRCA1 and 2 in BRCA2). All BRCA1/2 mutation carriers had ovarian high-grade serous carcinomas. Of the 12 patients who were alive at the time of analysis, 5 were BRCA1/2 mutation carriers. All of them had family members with BRCA1/2-associated malignancies. Conclusions: Our results provide pilot evidence that BRCA1/2 mutations are common in Taiwanese patients with metachronous breast and ovarian malignancies, supporting the clinical utility of genetic counseling.

        Effect of sodium sulfite, tartaric acid, tannin, and glucose on rheological properties, release of aroma compounds, and color characteristics of red wine

        In this study, we evaluated the effect of additionof non-volatile compounds (sodium sulfite, tartaric acid,tannin, and glucose) on the rheological properties, releaseof aroma compounds, and color of the red wine. Whiledetermining the rheological properties of the supplementedsamples, non-Newtonian fluidic and shear-thinningbehavior of samples was noticed. The viscosity of thesesamples was found in negative correlation with the dose ofaddition of various non-volatile substances. The aromaprofile of red wine after additions showed the change in therelease of the nine key aroma compounds. Among themethyl hexanoate, phenylethyl alcohol, octanoic acid, diethylsuccinate, and ethyl octanoate were profoundly increased. Further, the color of red wines was improved in the presenceof tartaric acid and tannin. Overall, supplementationof various substances during storage period of three differentwines could enormously affect the sensory characteristicsin a dose dependent manner.

        Metal Dusting Behavior of 321 Stainless Steel: Effects of Edge and Corner

        The metal dusting behavior of 321 stainless steel (SS) in a flowing mixed CO/H_2/H_2O gas stream at 600 °C for 500 h and 1000 h was investigated. The microstructures and chemical compositions of the reaction products at the surface and those in the substrate under the pits were examined by using a scanning electron microscope (SEM) and a transmission electron microscope (TEM), each combined with an energy dispersive spectrometer (EDS). The phenomenon of a pitting attack that occurred preferentially at the edges and corners of the specimens was the focus of this study. The carburization behavior in the steel substrate under the pits was also characterized. Matrix carbide in the form of Cr_7C_3 and grain oundary carbide in the form of Cr_23C_6 were identified by TEM.

        <P><B>Background</B></P><P>Although invariant natural killer T (iNKT) cells have been shown to play a critical role in the pathogenesis of asthma, the role of iNKT cells in atopic dermatitis (AD) has not been well evaluated.</P><P><B>Objective</B></P><P>We investigated whether iNKT cells in patients with AD increased and whether iNKT cells were activated by thymic stromal lymphopoietin (TSLP), which is highly expressed in keratinocytes of AD.</P><P><B>Methods</B></P><P>We assessed the population of iNKT cells in PBMCs of patients with AD and healthy controls (HCs) using flow cytometry. Immunohistochemistry was used to evaluate iNKT cells and TSLP expression in AD and HC skin. We also evaluated whether iNKT cells expressed the TSLP receptor, the effects of TSLP on iNKT cells, and iNKT cell–dendritic cell interactions in a TSLP-rich environment.</P><P><B>Results</B></P><P>There were more iNKT cells among PBMCs of patients with moderate to severe AD than mild AD (<I>P</I> < .05) and HC (<I>P</I> < .001). The number of iNKT cells was significantly larger in severe AD skin lesions than in mild (<I>P</I> < .001) or moderate AD skin lesions (<I>P</I> < .05). TSLP expression increased in lesional skin (<I>P</I> < .001) but not in the sera of patients with AD (<I>P</I> = .729) compared with HC. iNKT cells expressed TSLP receptor protein and mRNA. TSLP directly activated iNKT cells to secrete IL-4 and IL-13, and the concurrent addition of dendritic cells further activated IFN-γ expression.</P><P><B>Conclusion</B></P><P>Increased iNKT cells activated by TSLP, especially in patients with severe AD, might play an essential role in the innate allergic immune response in AD.</P>

      • Dental arch wires with tooth-like color

        Unique tooth-like (milky white) color ${\beta}$-Ti dental arch wires are prepared by anodization in a 1M $H_2SO_4$ electrolyte at $30^{\circ}C$ and 30 V for 88 min and 40 s. Aggregates are formed on these surfaces of the anodized wires with tooth-like colors, and the results are different from those of the anodized wires with monochromatic colors which have smooth oxide surfaces. Similar to the monochromatic wires, the composition of the oxide layer is $TiO_{2-x}$ and the x approaches zero at the outer layer. But different from the amorphous structure observed in monochromatic wires, the oxide layers are partially crystallized with an anatase structure. The milky white colors result from the rough and crystalized oxide layers, not by the interference effect as observed in monochromatic wires.

        Compound glycyrrhizin injection for improving liver function in children with acute icteric hepatitis: A systematic review and meta-analysis

        Background: Compound glycyrrhizin injection (CGI) is a preparation with glycyrrhizin as the main active ingredient extracted from licorice. As clinical trials suggest that CGI is effective in improving liver function for acute icteric hepatitis in children (AIHC), this systematic review aimed to evaluate and verify its therapeutic effects and safety. Methods: Six electronic databases were searched from their inception to 15 May 2021. Randomized controlled trials (RCTs) assessing therapeutic effects and safety of CGI for AIHC were included. The risk of bias for each trial was assessed using the Cochrane Risk of Bias Tool 2.0. Primary outcomes were indexes related to liver function, including total bilirubin (TBiL), alanine aminotransferase (ALT) and aspartate transaminase (AST). RevMan 5.4 software was used for data analyses. The certainty of the evidence was assessed using the online GRADEpro tool. Results: Six RCTs involving 608 children were included. The overall bias was assessed as having “high risk of bias” in all trials. All trials compared the combination of CGI and conventional western medicine (CWM) with CWM alone. Regarding the effects of CGI for AIHC, results showed that CGI plus CWM was superior to CWM alone in reducing the levels of TBiL (mean difference (MD) = -8.19 mmol/L, 95% CI -9.86 to -6.53), ALT (MD = -24.09 U/L, 95% CI -30.83 to -17.34) and AST (MD = -18.67 U/L, 95% CI -21.88 to -15.45). No trial reported adverse events. The certainty of the evidence for outcomes were all evaluated as low or very low. Conclusion: CGI may have adjuvant therapeutic effects on improving the liver function of children with AIHC. There is no evidence to determine the safety of CGI for AIHC. As current evidence is weak, further well-designed RCTs are required for verification of the therapeutic effects of CGI.         Effect of gelatin nano-coating containing Gardenia pigment on the preservation of pork slices

        The nano-coating composed of gelatin and Gardenia pigment (GP) was successfully prepared and showed strong antioxidant activity. The average particle sizes of the nano-coating containing 0.1% and 0.3% GP were 269.58 and 394.13 nm, respectively. The pork slices uncoated and coated with the nano-coating were preserved at 4 C for 15 days. The pork slices’ pH, total volatile basic nitrogen (TVB-N), total viable counts (TVC), water-bind- ing capacity (WHC), and thiobarbituric acid reactive sub- stances (TBARS) were measured to assess the preservation effect of the nano-coating. The results showed that the pork coated with the nano-coating had lower pH, TVC, TVB-N, TBARS, and higher WHC, significantly different (p \ 0.05) than the uncoated pork. It is suggested that the proposed nano-coating can be used to effectively improve the pork’s quality and shelf life during refrigeration storage.

        Initial Factors Influencing Duration of Hospital Stay in Adult Patients With Peritonsillar Abscess

        Objectives. To review cases of peritonsillar abscess and investigate the initial clinical factors that may influence the duration of hospitalization. To determine the predictive factors of prolonged hospital stay in adult patients with peritonsillar abscess. Methods. Subjects were adults hospitalized with peritonsillar abscess. We retrospectively reviewed 377 medical records from 1990 to 2013 in a tertiary medical center in southern Taiwan. The association between clinical characteristics and the length of hospital stay was analyzed with independent t-test, univariate linear regression and multiple linear regression analysis. Results. The mean duration of hospitalization was 6.2±6.0 days. With univariate linear regression, a prolonged hospital stay was associated with several variables, including female gender, older ages, nonsmoking status, diabetes mellitus, hypertension, band forms in white blood cell (WBC) counts, and lower hemoglobin levels. With multiple linear regression analysis, four independent predictors of hospital stay were noted: years of age (P<0.001), history of diabetes mellitus (P<0.001), ratio of band form WBC (P<0.001), and hemoglobin levels (P<0.001). Conclusion. In adult patients with peritonsillar abscess, older ages, history of diabetes mellitus, band forms in WBC counts and lower hemoglobin levels were independent predictors of longer hospitalization.
