        The Relationships Among Occupational Safety Climate, Patient Safety Climate, and Safety Performance Based on Structural Equation Modeling

        Objectives: The aim of the present study was to investigate the relationships among hospital safety climate, patient safety climate, and safety outcomes among nurses. Methods: In the current cross-sectional study, the occupational safety climate, patient safety climate, and safety performance of nurses were measured using several questionnaires. Structural equation modeling was applied to test the relationships among occupational safety climate, patient safety climate, and safety performance. Results: A total of 211 nurses participated in this study. Over half of them were female (57.0%). The age of the participants tended to be between 20 years and 30 years old (55.5%), and slightly more than half had less than 5 years of work experience (51.5%). The maximum and minimum scores of occupational safety climate dimensions were found for reporting of errors and cumulative fatigue, respectively. Among the dimensions of patient safety climate, non-punitive response to errors had the highest mean score, and manager expectations and actions promoting patient safety had the lowest mean score. The correlation coefficient for the relationship between occupational safety climate and patient safety climate was 0.63 (p<0.05). Occupational safety climate and patient safety climate also showed significant correlations with safety performance. Conclusions: Close correlations were found among occupational safety climate, patient safety climate, and nurses’ safety performance. Therefore, improving both the occupational and patient safety climate can improve nurses’ safety performance, consequently decreasing occupational and patient-related adverse outcomes in healthcare units.

        Y Stent Rendezvous to Treat Symptomatic Innominate Artery Stenosis

        A male in his 60s presented with transient ischemic attacks 5 years after aortic arch branch graft repair for type A aortic dissection. Computed tomographic angiography demonstrated 80% stenosis of the brachiocephalic artery close to the origins of the right common carotid and subclavian arteries. The case was reviewed at our multidisciplinary aortic meeting and a plan for endovascular management was made. Percutaneous endovascular Y stenting from the innominate artery into the left common carotid and subclavian arteries was achieved using self-expanding nitinol stents with a rendezvous technique that included retrograde right radial artery, retrograde right external carotid artery, and retrograde right femoral arterial approaches. At 6 months review, the stents remained widely patent and the patient was symptom-free.

        Hybrid fuzzy model to predict strength and optimum compositions of natural Alumina-Silica-based geopolymers

        This study introduces the supervised committee fuzzy model as a hybrid fuzzy model to predict compressive strength (CS) of geopolymers prepared from alumina-silica products. For this purpose, more than 50 experimental data that evaluated the effect of Al2O3/SiO2, Na2O/Al2O3, Na2O/H2O and Na/[Na+K] on (CS) of geopolymers were collected from the literature. Then, three different Fuzzy Logic (FL) models (Sugeno fuzzy logic (SFL), Mamdani fuzzy logic (MFL), and Larsen fuzzy logic (LFL)) were adopted to overcome the inherent uncertainty of geochemical parameters and to predict CS. After validating the model, it was found that the SFL model is superior to MFL and LFL models, but each of the FL models has advantages to predict CS. Therefore, to achieve the optimal performance, the supervised committee fuzzy logic (SCFL) model was developed as a hybrid method to combine the benefits of individual FL models. The SCFL employs an artificial neural network (ANN) model to re-predict the CS of three FL model predictions. The results also show significant fitting improvement in comparison with individual FL models.

        Evaluation of the antinociceptive effects of a selection of triazine derivatives in mice

        Background: The authors showed in a previous study that some novel triazine derivatives had an anti-inflammatory effect. The present study was designed to evaluate the antinociceptive effect of five out of nine compounds including two vanillintriazine (5c and 5d) and three phenylpyrazole-triazine (10a, 10b, 10e) derivatives which showed the best anti-inflammatory effect. Methods: Male Swiss mice (25–30 g) were used. To assess the antinociceptive effect, acetic acid-writhing, formalin, and hot plate tests were used after intraperitoneal injection of each compound. Results: All compounds significantly (P < 0.001) reduced acetic acid-induced writhing at tested doses (50, 100, and 200 mg/kg). Also, the percent inhibition of writhing in the acetic acid test showed that at the maximum tested dose of these compounds (200 mg/kg), the order of potencies is as follows: 10b > 10a > 10e > 5d > 5c. In the formalin test, compounds 5d, 10a, and 10e showed an antinociceptive effect in the acute phase and all compounds were effective in the chronic phase. In the hot plate test, compounds 5c, 5d, and 10a demonstrated an antinociceptive effect. Conclusions: The results clearly showed that both vanillin-triazine and phenylpyrazole- triazine derivatives had an antinociceptive effect. Also, some compounds which showed activity in the early phase of formalin test as well as in the hot plate test could control acute pain in addition to chronic or inflammatory pain.

        Detection of surface anomalies through fractal analysis and their relation to morphotectonics (High Zagros belt, Iran)

        Fractal geometry is considered as a new method for quantitative analysis and explanation of surface complexities and roughness in self-similar or self-affine landforms. In the present study, the surface fractal dimensions were investigated by a cellular model by covering divider method and remote sensing data, in a complex morphotectonic region in terms of tectonic, geological, and geomorphological structures along the margin of the High Zagros Belt. Results of this study indicated that surface anomalies can be detected by cellular fractal model due to variations at the boundary of lithological units and structural zones, and along faults that can change the characters of the fractal dimension of landforms. Investigation of wavelet analyses on two profiles of study area shows that the amplitude and frequency of the fractal dimension is related to lithological and structural zones boundaries, and to the presence of faults. In this study, the lowest fractal dimension is associated with the integrated units of Mesozoic orbitolina limestone on the border of the two structural zones of Sanandaj-Sirjan and High Zagros belt. However, the presence of friable and erodible Quaternary formations increases the fractal dimension. There is an inverse relationship between the fractal dimension and elevation and Terrain Ruggedness Index, indicating that mountains have lower fractal dimensions than lowlands. The results of the present study show that fractal dimensional changes in topographically complex zones depend on the interaction of a set of lithological, tectonic, and geomorphological factors, and allow for a systematic quantitative analysis of landforms.

        Application of mesoporous magnetic carbon composite for reactive dyes removal: Process optimization using response surface methodology

        Discharging the effluents of textile wastewaters into potable water resources can endanger the ecosystem, due to their reactivity, toxicity, and chemical stability. In this research, the application of powder activated carbon modified with magnetite nanoparticles (PAC-MNPs) as an adsorbent for removal of reactive dyes (Reactive black 5 (RB5) and reactive red 120 (RR120)) was studied in a batch system. The adsorption performance was evaluated as a function of temperature, contact time and different adsorbent and adsorbate concentrations. The levels of factors were statistically optimized using Box-Behnken Design (BBD) from the response surface methodology (RSM) to maximize the efficiency of the system. The adsorption process of both dyes was fit with the pseudo-second order kinetic and Langmuir isotherm models. The identified optimum conditions of adsorption were 38.7 oC, 46.3 min, 0.8 g/L and 102mg/L for temperature, contact time, adsorbent dose, and initial dyes concentration, respectively. According to the Langmuir isotherm, the maximum sorption capacities of 175.4 and 172.4mg/g were obtained for RB5 and RR120, respectively. Thermodynamics studies indicated that the adsorption process of the reactive dyes was spontaneous, feasible, and endothermic. After five cycles, the adsorption efficiency was around 84 and 83% for RB5 and RR120, respectively. A high value of desorption was achieved, suggesting that the PAC-MNPs have a good potential in regeneration and reusability, and also can be effectively utilized in industrial applications. PAC-MNPs also show a good anti-interference potential for removal of reactive dyes in dye-industry wastewaters.

        CaRotid Artery Filtering Technique (CRAFT): A Technique for Carotid Artery Stenting with Intrinsic Embolic Protection

        Purpose: Carotid artery stenting (CAS) is an established treatment for symptomatic carotid artery stenosis as an alternative to carotid endarterectomy. A variety of techniques and devices have been devised to minimise periprocedural stroke risk using either proximal or distal embolic protection. This study presents a method of embolic protection during CAS–the CaRotid Artery Filtering Technique (CRAFT). Materials and Methods: The CRAFT technique employs aspects of both proximal and distal embolic protection. The CASPER RX stent (MicroVention, Tustin, CA, USA), which is a double-layered, closed-cell, micromesh nitinol stent, is deployed across the carotid artery stenosis with the assistance of a FlowGate balloon guide catheter (Stryker Neurovascular, Fremont, CA, USA). The partially deployed stent acts as a distal filter while the balloon guide is deflated midway during stent deployment to prevent distal plaque embolisation, followed by completion of stent deployment and angioplasty. Results: A total of 94 patients underwent CAS using the CRAFT technique between June 2016 and March 2021. Successful stent deployment was achieved in all patients. Preliminary results demonstrated acute stent occlusion in 6 patients (6.4%) and distal embolic stroke in 5 patients (5.3%). The median procedural fluoroscopy time was 34 minutes with an interquartile range of 22 to 55 minutes. Conclusion: The CRAFT technique of CAS presented by this study can be applied in the treatment of symptomatic carotid artery stenosis in both emergency and elective procedure settings with a high technical success and low distal embolic stroke risk.

        Endovascular Treatment of Intracranial Aneurysms Using the Novel Low Profile Visualized Intraluminal Support EVO Stent: Multicenter Early Feasibility Experience

        Purpose Low-profile, self-expandable stents have broadened therapeutic options available for definitive treatment of intracranial aneurysms. The novel Low-Profile Visualized Intraluminal Support (LVIS) EVO stent extends upon the success of its predecessor, the LVIS Jr stent, aiming to enable higher visibility and greater opening ability within a self-expandable and fully retrievable microstent system. In this study, we aim to report the early safety and feasibility experience with the LVIS EVO stent. Materials and Methods A multicenter, retrospective, observational study was conducted on patients who had intracranial aneurysms treated with the LVIS EVO stent across 3 Australian neurovascular centers between February 2020 and September 2020. Short-term technical and clinical outcomes were evaluated. Results A total of 22 LVIS EVO stents were successfully implanted to treat 15 aneurysms (3 ruptured, 12 unruptured) in 15 patients. Aneurysms ranged from 2 mm to 35 mm in dome height. The LVIS EVO stent was used for stent-assisted coiling in 11 patients and flow diversion in 4 patients. There were no device-related procedural complications. There were 2 cases of peri-procedural symptomatic thromboembolic complications and no procedure-related mortality. At early radiological follow up, 10 patients had complete occlusion, 4 patients had small neck remnants, and 1 patient who was managed with flow diversion had a residual aneurysm. Conclusion Early experience with the LVIS EVO stent demonstrated safety and feasibility for stent-assisted coiling as well as flow diversion for intracranial aneurysms. In this heterogeneous cohort, including ruptured, complex, and large aneurysms, all cases were technically successful.
