A Heuristic Algorithm for Designing Near-Optimal Mobile Agent Itineraries

Several distributed architectures, incorporating mobile agent technology, have been recently proposed to answer the scalability limitations of their centralized counterparts. However, these architectures fail to address scalability problems, when distributed tasks requiring the employment of itinerant agents is considered. This is because they lack mechanisms that guarantee optimization of agents' itineraries so as to minimize the total migration cost in terms of the round-trip latency and the incurred traffic. This is of particular importance when MAs itineraries span multiple subnets. The work presented herein aspires to address these issues. To that end, we have designed and implemented an algorithm that adapts methods usually applied for addressing network design problems in the specific area of mobile agent itinerary planning. The algorithm not only suggests the optimal number of mobile agents that minimize the overall cost but also constructs optimal itineraries for each of them. The algorithm implementation has been integrated into our mobile agent framework research prototype and tested in real network environments, demonstrating significant cost savings.

Nonsteroidal anti-inflammatory drugs (NSAIDs) are the most commonly prescribed and self-prescribed drugs to treat inflammation and pain associated with several conditions. Although their efficacy and overall safety have been recognized when used according to medical prescriptions and for a short period time, their acute impact on enteric physiology has rarely been studied. NSAIDs are known to cause gastrointestinal side effects due to their intrinsic mechanism of action, which involves prostaglandins synthesis, leading to impaired mucopolysaccharide layer production. Despite this well-known and investigated side effect, the short- and long-term influences of acute administration of these drugs on the biochemical environment of enteric cells are not well understood. This study investigates the rate of adenosine triphosphate (ATP) loss and permeability alterations occurring in a model of human enteric cells, as a consequence of acute administration of NSAIDs as major perpetrators of enteric toxicity. For the first time, we investigate the ability of a novel ATP-containing formulation to prevent ATP hydrolysis in the stomach and ensure its delivery at the proximal duodenal site.

<P>Crouch gait, a pathological pattern of walking characterized by excessive knee flexion, is one of the most common gait disorders observed in children with cerebral palsy (CP). Effective treatment of crouch during childhood is critical to maintain mobility into adulthood, yet current interventions do not adequately alleviate crouch in most individuals. Powered exoskeletons provide an untapped opportunity for intervention. The multiple contributors to crouch, including spasticity, contracture, muscle weakness, and poor motor control make design and control of such devices challenging in this population. To our knowledge, no evidence exists regarding the feasibility or efficacy of utilizing motorized assistance to alleviate knee flexion in crouch gait. Here, we present the design of and first results from a powered exoskeleton for extension assistance as a treatment for crouch gait in children with CP. Our exoskeleton, based on the architecture of a knee-ankle-foot orthosis, is lightweight (3.2 kg) and modular. On board sensors enable knee extension assistance to be provided during distinct phases of the gait cycle. We tested our device on one six-year-old male participant with spastic diplegia from CP. Our results show that the powered exoskeleton improved knee extension during stance by 18.1° while total knee range of motion improved 21.0°. Importantly, we observed no significant decrease in knee extensor muscle activity, indicating the user did not rely solely on the exoskeleton to extend the limb. These results establish the initial feasibility of robotic exoskeletons for treatment of crouch and provide impetus for continued investigation of these devices with the aim of deployment for long term gait training in this population.</P>
iPOJO-based Middleware Solutions for Self-Reconfiguration and Self-Optimization

In recent years, ubiquitous and pervasive scenarios have emerged as a complex ecosystem where differentiated software/hardware components interoperate wirelessly and seamlessly. The goal is to enable users to continuously access services and contents, and to always get the best out of their current environment and available resources. In such dynamic and flexible scenarios, the need emerges for flexible and general solutions for continuous runtime self-reconfiguration and self-optimization of ubiquitous support software systems. This paper proposes a fully reconfigurable middleware approach that aims at reconfiguring complex software systems made up of heterogeneous off-the-shelf components from both functional and non-functional perspectives. Our middleware can also extend already existing and non-reconfigurable middleware/applications in an easy and flexible way, with no need to re-design them. The proposed design principles have been practically applied to the implementation of a runtime self-reconfigurable middleware called Off-The-Shelf Ready To Go (OTS-RTG), implemented on top of iPOJO. The reported experimental results both exhibit a limited overhead and show the wide applicability of the proposed solution to many application scenarios, including complex, industrial, Enterprise Service Bus-based ones.

The area under a receiver operating characteristic (ROC) curve (AUC) is a popular measure of pure diagnostic accuracy that is independent from the proportion of diseased subjects in the analysed sample. However, its actual usefulness in the clinical context has been questioned, because it does not seem to be directly related to the actual performance of a diagnostic marker in identifying diseased and non-diseased subjects in real clinical settings. This study evaluates the relationship between the AUC and the proportion of correct classifications (global diagnostic accuracy, GDA) in relation to the shape of the corresponding ROC curves. We demonstrate that AUC represents an upward-biased measure of GDA at an optimal accuracy cut-off for balanced groups. The magnitude of bias depends on the shape of the ROC plot and on the proportion of diseased and non-diseased subjects. In proper curves, the bias is independent from the diseased/non-diseased ratio and can be easily estimated and removed. Moreover, a comparison between 2 partial AUCs can be replaced by a more powerful test for the corresponding whole AUCs. Applications to 3 real datasets are provided: a marker for a hormone deficit in children, 2 tumour markers for malignant mesothelioma, and 2 gene expression profiles in ovarian cancer patients. In conclusion, the AUC is a measure of accuracy with potential clinical relevance for the evaluation of disease markers. The clinical meaning of ROC parameters should always be evaluated with an analysis of the shape of the corresponding ROC curve.
Surgical Reconstruction of Traumatic Pseudoaneurysm of Palmar Arch Caused by Blunt Trauma

Although rare, pseudoaneurysms (PAs) of the palmar arch are mostly considered benign. However, they can cause severe complications if left untreated or misdiagnosed. There are a few data on traumatic PAs of the palmar arch, particularly those most commonly caused by penetrating hand injuries. However, PAs caused by blunt trauma are more insidious in onset, presenting as a painful pulsatile mass in the palmar area of the hand, and require prompt diagnosis and management to avoid catastrophic sequelae. Our case is the first study to describe a patient with traumatic PA of the palmar arch caused by blunt trauma that was treated with surgical reconstruction and venous bypass interposition.
