        Evaluation of the drug-induced lymphocyte stimulation test for diagnosing mesalazine allergy

        Background/Aims: Mesalazine is an effective drug for treating ulcerative colitis (UC), but causes allergic symptoms in a few cases. Therefore, the objective of this study was to evaluate the usefulness of the drug-induced lymphocyte stimulation test (DLST) for the diagnosis of mesalazine allergy. Methods: Patients with UC treated with mesalazine with or without a history of associated adverse events (AEs) were enrolled at Kyorin University Hospital from July 2016 to April 2017. Results: The DLST was performed in 104 patients with UC, of which 24 had a history of AEs due to mesalazine treatment. The control value of DLST was 337.4±296.3 counts per minute (cpm) in the AE<sup>+</sup> group and 408.0±371.9 cpm in the AE<sup>-</sup> group. The measured value of DLST was 578.8±424.7 cpm in the AE<sup>+</sup> group and 476.5±471.8 cpm in the AE<sup>-</sup> group. The stimulation index (SI) was 243.9%±291.1% in the AE<sup>+</sup> group and 119.8%±53.0% in the AE<sup>-</sup> group. The SI value and DLST positivity were significantly higher in the AE<sup>+</sup> group than in the AE<sup>-</sup> group (P=0.030 and P=0.029, respectively). The test sensitivity and specificity were 0.240 and 0.805, respectively, and the false-positive and false-negative rate was 0.195 and 0.760, respectively. Conclusions: The DLST for mesalazine showed low sensitivity and high specificity, suggesting that it may be useful for the definitive diagnosis of allergy to mesalazine. (Intest Res 2018;16:273-281)

      • Significantly high polarization degree of the very low-albedo asteroid (152679) 1998 KU<sub>2</sub>

        <P>We present a unique and significant polarimetric result regarding the near-Earth asteroid (152679) 1998 KU2, which has a very low geometric albedo. From our observations, we find that the linear polarization degrees of 1998 KU2 are 44.6 ± 0.5% in the <I>R</I>C band and 44.0 ± 0.6% in the <I>V</I> band at a solar phase angle of 81.0°. These values are the highest of any known airless body in the solar system (i.e., high-polarization comets, asteroids, and planetary satellites) at similar phase angles. This polarimetric observation is not only the first for primitive asteroids at large phase angles, but also for low-albedo (<0.1) airless bodies. Based on spectroscopic similarities and polarimetric measurements of materials that have been sorted by size in previous studies, we conjecture that 1998 KU2 has a highly microporous regolith structure comprising nano-sized carbon grains on the surface.</P>

        Black Soybean Extract Improves Lipid Profiles in Fenofibrate-Treated Type 2 Diabetics with Postprandial Hyperlipidemia

        Black soybeans (Glycine max (L.) Merr.) are known to be rich in polyphenols, including anthocyanins, and they have been consumed since ancient times for their beneficial effects on health. In addition, it has been reported that black soybean (BS) seed coat may ameliorate obesity and insulin resistance. In the present study, we administered BS extract to type 2 diabetics for 2 months to investigate the effects of BS on glycemic control and lipid metabolism parameters. In addition, we administered BS and antihyperlipidemic agent, fenofibrate, to patients with type 2 diabetes complicated by postprandial hyperlipidemia for 2 months and assessed the combined effects of fenofibrate and BS on serum lipid profile. The results showed that administration of the BS alone had no effect on the blood glucose or lipid levels, but that administration of fenofibrate alone and fenofibrate in combination with the BS significantly lowered their serum triglyceride (TG) level at fasting state, and the percent decrease in the serum TG level after combined administration was significantly higher than in the subjects who received fenofibrate alone. Furthermore, the serum LDL cholesterol concentration, which did not decrease when fenofibrate was administered alone, decreased significantly when the BS and fenofibrate were administered in combination. These results suggest that combined administration of the BS with fenofibrate enhanced the antihyperlipidemic action of fenofibrate, and the results of this study demonstrated the usefulness of the BS in clinical practice.

        Effects of Compost Tea Making from Differently Treated Compost on Plant Disease Control

        Antifungal activity of compost teas was evaluated in vitro and in vivo experiments. In vitro test, fourteen compost teas were produced using five different composts and through six different methods for compost tea production. Eleven pathogenic fungi were used as indicators of antifungal activity of compost teas. In vivo test, one of the compost teas used in vitro test was sprayed to cucumber leaves which were infected with powdery mildew in order to evaluate antifungal activity of compost tea. From the results of the tests, it was found that all compost teas used this study showed antifungal activity and the proper application of compost teas against pathogenic fungi for obtaining optimal effects was important to know.

        Efficacy of Anti-NaV1.7 Antibody on the Sensory Nervous System in a Rat Model of Lumbar Intervertebral Disc Injury

        Purpose: The pathophysiology of discogenic low back pain is not fully understood. Tetrodotoxin-sensitive voltage-gated sodium (NaV) channels are associated with primary sensory nerve transmission, and the NaV1.7 channel has emerged as an analgesic target. Previously, we found increased NaV1.7 expression in dorsal root ganglion (DRG) neurons innervating injured discs. This study aimed to examine the effect of blocking NaV1.7 on sensory nerves after disc injury. Materials and Methods: Rat DRG neurons innervating the L5/6 disc were labeled with Fluoro-Gold (FG) neurotracer. Twenty-four rats underwent intervertebral disc puncture (puncture group) and 12 rats underwent sham surgery (non-puncture group). The injury group was divided into a saline infusion group (puncture+saline group) and a NaV1.7 inhibition group, injected with anti-NaV1.7 antibody (puncture+anti-NaV1.7 group); n=12 per group. Seven and 14 days post-surgery, L1 to L6 DRGs were harvested and immunostained for calcitonin gene-related peptide (CGRP) (an inflammatory pain marker), and the proportion of CGRP-immunoreactive (IR) DRG neurons of all FG-positive neurons was evaluated. Results: The ratio of CGRP-IR DRG neurons to total FG-labeled neurons in the puncture+saline group significantly increased at 7 and 14 days, compared with the non-puncture group, respectively (p<0.05). Application of anti-NaV1.7 into the disc significantly decreased the ratio of CGRP-IR DRG neurons to total FG-labeled neurons after disc puncture at 7 and 14 days (40% and 37%, respectively;p<0.05). Conclusion: NaV1.7 antibody suppressed CGRP expression in disc DRG neurons. Anti-NaV1.7 antibody is a potential therapeutic target for pain control in patients with lumbar disc degeneration.

      • Numerical Prediction of Effect of Seawater-Quality Enhancer Installed in Isahaya Bay

        Since the construction of Isahaya Dyke, it is said that the tidal current in Ariake Bay has changed. The change may bring on the environmental deterioration in Ariake Bay. Especially in Isahaya Bay and the offshore of Oura, the emergence of oxygen-deficient water has been prominent. In this study, firstly, we simulated the emergence of oxygen-deficient water by incorporating an ecosystem model into an ocean current model to elucidate the mechanism of the oxygen-deficient water both from physical and biological viewpoints. Next, we proposed a local engineering solution, a seawater-quality enhancer, and simulated numerically the diffusion of nutrient-rich water emitted by the apparatus and its effects on the ecosystems, particularly, on the oxygen-deficient water. The apparatus spreads horizontally the intermediate-density mixture of surface and deep waters. The numerical model considered 8 principal rivers because the nutrient-rich density currents of fresh water largely affect the currents and the ecosystems in Ariake Bay. It was shown that 16 % of the oxygen-deficient water in Isahaya Inlet was reduced by the apparatus, which emits 100-million-ton water per day, at 8 weeks after the start of operation. This may imply the possibility of improvement of water quality by this engineering countermeasure.

        Accuracy of Percutaneous Lumbosacral Pedicle Screw Placement Using the Oblique Fluoroscopic View Based on Computed Tomography Evaluations

        Study Design: Retrospective. Purpose: This study aims to investigate the accuracy of the oblique fluoroscopic view, based on preoperative computed tomography (CT) images for accurate placement of lumbosacral percutaneous pedicle screws (PPS). Overview of Literature: Although PPS misplacement has been reported as one of the main complications in minimally invasive spine surgery, there is no comparative data on the misplacement rate among different fluoroscopic techniques, or comparing such techniques with open procedures. Methods: We retrospectively selected 230 consecutive patients who underwent posterior spinal fusion with a pedicle screw construct for degenerative lumbar disease, and divided them into 3 groups, those who had undergone: minimally invasive percutaneous procedure using biplane (lateral and anterior-posterior views using a single C-arm) fluoroscope views (group M-1), minimally invasive percutaneous procedure using the oblique fluoroscopic view based on preoperative CT (group M-2), and conventional open procedure using a lateral fluoroscopic view (group O: controls). The relative position of the screw to the pedicle was graded for the pedicle breach as no breach, <2 mm, 2–4 mm, or >4 mm. Inaccuracy was calculated and assessed according to the spinal level, direction and neurological deficit. Inter-group radiation exposure was estimated using fluoroscopy time. Results: Inaccuracy involved an incline toward L5, causing medial or lateral perforation of pedicles in group M-1, but it was distributed relatively equally throughout multiple levels in groups M-2 and controls. The mean fluoroscopy time/case ranged from 1.6 to 3.9 minutes. Conclusions: Minimally invasive lumbosacral PPS placement using the conventional fluoroscopic technique carries an increased risk of inaccurate screw placement and resultant neurological deficits, compared with that of the open procedure. Inaccuracy tended to be distributed between medial and lateral perforations of the L5 pedicle, as a result of pedicle morphology and the PPS pathway. Oblique fluoroscopic views, based on CT measurement, may allow accurate PPS insertion with a shorter fluoroscopy time.
