        치매노인의 서비스 희망과 이용의 일치 여부에 영향을 미치는 요인

        Korean government is preparing the long-term care financing and delivery system in order to cope with rapid population aging. The system should be designed to provide demented patients with an appropriate services that the patients want to take, and considered to be necessary for them. In this regard, this study aims to analyse empirically a relationship between the types of long-term care services that demented patients wanted to take and they actually received during 2004. The caregivers of 609 dementia patients, who were randomly selected in a manner of proportional allocation from a nationwide claim database of the Korean National Health Insurance Corporation, were interviewed in September, 2005. Independent variables include soeio-demographic characteristics, Activities of Daily Living(ADL) and Instrumental Activities of Daily Living(IADL). To explore the correspondence of the types of long-term care services that demented patients wanted to take and that they actually received, and its affecting factors, we conducted chi-square test and logistic regression analysis. Main findings are as follows. First, while only 20% of study subjects wanted home services as a long-term care services, those who wanted to use the long-term care facilities and general hospital were 37%, 43% respectively. Second, the correspondence rate was just 38% on average, and extremely low in the demented patients who wanted to use long-term care facilities. Third, the demented patients who resided in urban areas and received relatively high level of education showed high correspondence rate. Fourth, the high ADL score was closely related to low correspondence rate.

        한국 의료체계의 이원화와 변혁 과제

        The health care practitioner in Korea is dualized into the traditional and westernized parts. The dualization was originated in the characteristic and process of introducing western medicine and health care institution. The purpose of this thesis is to review the historical background of Korean health care system and the transformation. This will present some implications useful for recent reform process in the health care system. Korean traditional health care system had been operated through some organizations such as Choneugam(典醫監), Hyeminsuh(惠民暑), Hwarinsuh(活人暑). In the late 19th century, while the traditional health care organizations were almost abolished and replaced by westernized forms, the traditional physicians barely kept themselves in existence. After the emancipation from the Japanese colonial rule, the status of Korean traditional physicians was restored, and thus Korean health care system was dualized consisting of two heterogeneous forms. As this dualization has brought about many problems, some reform in health care became necessary. Conflicts in the health care system are manifested as various forms including the discrepancy between traditional and westernized systems. The resolution of these conflicts will be the critical point of the transforming process in health care. Concrete and active programs should be suggested and carried out in direction of a unitary health care system. The case of unitarian health care system in China can be an example, which provide some opportunities of curriculum interchange between traditional and westernized medical educations. It would be desirable to start with a fundamental review of the necessity of unification between two health care systems, and for this purpose some principles should be provided, that is, a) to fulfill pluralistic health care needs resulted from recent social changes, b) to conform the basic purpose of medicine, like health maintenance and promotion, and people's health behavior, c) to pursue health care reform based on the communication and collaboration of the two professional associations, d) and lastly to complete the reform by the consolidation of laws related with health care.

        환경문제에 대한 일반대중과 전문가의 위해도 인식 관련 요인

        This study was to investigate factors of risk perception on major environmental issues among general public and environmental experts in Korea. Structured questionnaire surveys were conducted to samples from general public and environmental experts during March and April, 2000. Total numbers of respondents were 773 general public and 353 experts. Analysis of the data showed that risk perception was correlated with health concern, environmental satisfaction, perception of environmental pollution, interest on the environmental issues, knowledge, experiences, and social-cultural consciousness, and so on. Overall environmental risk perception was affected by knowledge on environmental issues, experience of damage from environmental problems, sex and perception of environmental pollution. According to the results, environmental risk perception is not independent from social and psychological factors, and it may be desirable to consider the related factors in the process of risk communication for a more effective risk management or environmental policy.

        Role of Healthcare in Korean Long-Term Care Insurance

        With the rapid aging of the population, Korea introduced public long-term care insurance for older people in 2008. The long-term care insurance was designed as a separate scheme from the national health insurance, with eligibility qualifications and the certification process based on functional disability, benefits and coverage of community-based and institutional care, and a financing structure through multi-party contributions. Delivering appropriate health services to long-term care beneficiaries who manifest a high prevalence of comorbid chronic conditions with rising healthcare costs, however, presents a particular challenge. The lack of coordination between the health and long-term care sectors, limited consideration of physicians’ assessments in the certification process, inadequate provision of health services in long-term care facilities, and overlapping and inefficient use of care resources act as barriers to providing comprehensive healthcare for older beneficiaries. Through active participation in the long-term care system, health professionals can help older patients navigate through the complex long-term care terrain to obtain quality healthcare.

        건강보험공단 건강검진과 자비 건강검진 수검자간의 만족도 비교

        Backgrouds : This study was performed to compare the satisfaction levels for health examination programs provided by between the Korea National Health Insurance Corporation (KNHIC) and private healthcare organizations in Korea. We investigated factors associated with the satisfaction level for the KNHIC health examination. Also, factors explaining for the additional receipt of private healthcare organization's examination were identified. Methods : A self-administered questionnaire survey was conducted in June 2004 with 250 beneficiaries of the KNHIC industrial workers' program and 100 beneficiaries of the KNHIC selfemployed program. A total of 297 completed the questionnaire, including 213 (85.2%) and 84 (84.0%) for each insurance program. Ten questions measuring satisfaction levels for each examination program were rated on a scale ranging from 1 (strongly unsatisfied) to 5 (strongly satisfied). Results : Among the subjects receiving both examinations, the mean satisfaction level for the KNHIC examination (3.07) was significantly lower than that for the private healthcare organization's examination (3.50) (p<0.05). The KNHIC examination showed lower satisfaction for all of the 10 items than the counterpart. In particular, the KNHIC examination had the scores of lower than 3.0 for the items reflecting post-examination management services. According to the multiple regression analysis results, the satisfaction for the KNHIC examination was positively associated with the positive attitude toward health examination(•‚=0.38, p<0.00). The logistic regression results showed that the likelihood of receiving the private healthcare organization's examination in addition to the KNHIC examination increases as the respondents were less healthier(OR=0.29, 95% CI = 0.10¢¶0.84), the satisfaction level for the KNHIC examination decreased(OR=0.46, 0.28¢¶0.75) or the attitude toward health examination was more positive (OR=2.56, 1.31¢¶5.12). Conclusion : The relatively low satisfaction level for both examination programs suggests that there's ample room for improvement of health examination services in Korea. The negative association between additional receipt of private healthcare organization's examination and satisfaction level for the KNHIC examination implies that the improvement of the satisfaction for the KNHIC examination would help to reduce the national expenditure spent on the additional examination.
