Background/Aims: The proper treatment of subclinical hypothyroidism and the normal range of serum thyroid stimulating hormone (TSH) concentration are intensely debated. However, few reports have investigated TSH concentrations in Asian ethnic groups. Therefore, the present study was designed to define the TSH reference range in a Korean population and to investigate the metabolic significance of TSH concentration. Methods: We enrolled patients who underwent medical examination at the CHA Bundang Medical Center. Anthropometric data were evaluated, and serum TSH, free T4, and lipid profiles were assayed. Results: A total of 7,270 subjects were included. Mean TSH concentration of the study population was 1.82 ± 0.95 mU/L, and we observed a sex-related difference in TSH concentration (male, 1.67 ± 0.87 mU/L; female, 2.02 ± 1.01 mU/L; p < 0.01). When the 2.5 and 97.5 percentiles were calculated, 95% TSH reference limits were 0.52-4.29 mU/L. TSH concentration was higher in elderly subjects, during winter, in postmenopausal women, and in obese males. Moreover, TSH showed significantly positive correlations with serum total cholesterol, triglyceride, and low density lipoprotein cholesterol regardless of sex, age, season, obesity, or menopausal status (all p < 0.01). Finally, TSH concentration was positively related to the prevalence of metabolic syndrome. Conclusions: We demonstrated the association between TSH concentration within the normal reference range and serum lipid levels. TSH concentration varies according to sex, age, season, and body mass index (only in males). Moreover, high normal TSH levels were significantly associated with an increased prevalence of metabolic syndrome, which may be of importance when evaluating subjects with high normal TSH concentration.

가축사체 액상부산물을 이용하여 액비의 최적조건(미생물, pH, 미생물 주입량)을 조사하였고, 이들 최적 조건하에서 112일간 부숙하여 액비의 품질을 평가하였다. 가축사체 액상 부산물 액비 부숙시 최적 LP 미생물의 주입량은 0.5 mL/100mL이었으며, pH는 7 조건에서 각각 50점으로 완숙판정을 받았다. 최적조건하에서 112일 동안 부숙시킨 액비의 부숙도 는 부숙 후 28일에 50점을 받아 완숙판정을 받았으며, 부숙기 간이 길어짐에 따라 부숙 56일에는 온도가 60℃를 넘어 최고 점인 55점을 받았고, 이후 온도가 조금씩 낮아져 부숙 후 112 일에는 실온조건에 이르렀다. 완숙된 가축사체 액상부산물 액 비의 품질을 평가해본 결과, 최적조건하에서 부숙시킨 액비의 경우에는 T-N, P2O5 및 K2O의 함량이 28일에 가장 높았으며, 시간이 경과함에 따라 약간 감소하는 경향이었다. 또한, 유해성분(As, Cd, Cu, Cr, Hg, Ni, Pb 및 Zn)의 함량은 28일, 56일 및 112일 부숙시킨 액비에서 모두 비료공정규격 기 준치에 적합하였다. 이상의 결과를 미루어 볼 때 가축사체 액상부산물을 농업적 재활용을 위한 액비화 조건은 pH 7조건 에서 LP 미생물을 0.5 mL/100 mL 주입한 경우이다. 하지 만 본 연구에서 비교된 가축분뇨 발효액은 공시재료(가축사체)가 상이하여 향후 가축사체를 이용한 액비의 부숙도 기준이 개선되어야 할 것으로 판단된다. BACKGROUND: Globally, concern about emerging infectious diseases of livestock is growing. For the disposal of the animal carcass, it is necessary to recycle the carcass into an agriculturally usable product. The objective of this study was to investigate the composting conditions of liquid by-product obtained from degradation of animal carcass. METHODS AND RESULTS: Optimum conditions of liquid fertilizer were investigated using different microorganisms, pHs, and volumes of microorganisms (Lactobacillus rhamnosus + Pichia deserticola). Based on the results from the optimum conditions, compost maturity and quality of liquid fertilizer were evaluated for 112 days. The compost maturity of liquid fertilizer were higher in the order of LP(Lactobacillus rhamnosus + Pichia deserticola) > BC(Bacillus cereus) > BS (Bacillus subtilis). The optimum condition under different volumes of LP was injection of 0.5 mL/100 mL. The compost maturity under different pHs were higher in the order of pH 7 > 5 ≥ 9 ÷ 11. The liquid by-product at 56 days after composting was completely decomposed. The concentrations of T-N, T-P and K2O in liquid fertilizer at 56 days were 0.94, 0.17 and 3.78%, respectively, and the sum of those concentrations was 4.89%. CONCLUSION(S): Liquid fertilizer of by-product using pig carcass was decomposed with optimum conditions(LP, pH 7, injection of 0.5 mL/100 mL) in 56 days after composting, and was suitable for official standard of commercial fertilizer.

Background: The aim of this study was to examine the influence of body mass index (BMI) on the development of acute kidney injury (AKI) in critically ill patients in intensive care unit (ICU). Methods: Data of patients admitted to medical ICU from December 2011 to May 2014 were retrospectively analyzed. Patients were classified into three groups according to their BMI: underweight (< $18.5kg/m^2$), normal ($18.5-24.9kg/m^2$), and overweight (${\geq}25kg/m^2$). The incidence of AKI was compared among these groups and factors associated with the development of AKI were analyzed. AKI was defined according to the Risk, Injury, Failure, Loss of kidney function, and End-stage (RIFLE) kidney disease criteria. Results: A total of 468 patients were analyzed. Their mean BMI was $21.5{\pm}3.9kg/m^2$, including 102 (21.8%) underweight, 286 (61.1%) normal-weight, and 80 (17.1%) overweight patients. Overall, AKI occurred in 82 (17.5%) patients. The overweight group had significantly (p<0.001) higher incidence of AKI (36.3%) than the underweight (9.8%) or normal group (15.0%). In addition, BMI was significantly higher in patients with AKI than that in those without AKI ($23.4{\pm}4.2$ vs. $21.1{\pm}3.7$, p<0.001). Multivariate analysis showed that BMI was significantly associated with the development of AKI (odds ratio, 1.893; 95% confidence interval, 1.224-2.927). Conclusion: BMI may be associated with the development of AKI in critically ill patients.

Biochar, widely recognized for its capacity to counteract climate change impacts, has demonstrated substantial benefits in agricultural ecosystems. Nevertheless, empirical studies exploring its efficacy during climatic aberrations such as heavy rainfall are limited. This study investigated the effects of compost and biochar addition on corn growth attributes, yield, and soil CO2 and N2O fluxes under heavy rain (exceeding 5-yr average) and waterlogging conditions. Here, treatments included compost (CP, 7.6 t ha−1); rice husk biochar (RB, 7.6 t ha−1); wood biochar (WB, 7.6 t ha−1); and control (Cn). Under high rainfall and waterlogging, the CP treatment manifested a pronounced enhancement in corn biomass and productivity, exceeding biomass and productivity of Cn treatment by 12.6 and 32.2%, RB treatment by 120 and 195%, and WB treatment by 86.1 and 111%, respectively. Corn yield increased in the order: CP > Cn > WB > RB. Intriguingly, negligible disparity occurred between the RB and WB treatments in straw yield, grain yield, grain index, and corn productivity but both treatments recorded distinctively lower values than CP treatment. Also, the CO2 and N2O fluxes remained largely similar for two biochar treatments but lower than CP treatment. Overall, CP increased corn yield, straw, and grain yield whereas biochars reduced N2O flux during waterlogging. Although derived from a short-term experimental window, these pivotal findings furnish invaluable insights for devising soil amendments for yield and environmental benefits in contexts of extreme climatic perturbations. Our findings offer a robust foundation for refining nutrient management strategies confronted with waterlogging challenges, but long-term studies are necessary for definitive conclusions.
The diagnostic value of F-18 FDG PET/CT findings in nonepithelial ovarian cancer

To evaluate the diagnostic value of F-18 2-fluoro-2-deoxyglucose (FDG) positron emission tomography combined with computed tomography (PET/CT) in nonepithelial ovarian cancer. A retrospective chart review of 33 patients who were diagnosed nonepithelial ovarian cancer including granulose cell tumor (GCT, n=10), dysgerminoma (DG, n=4), mixed germ cell tumor (MGCT, n=3), Sertoli-Leydig cell tumor (SLCT, n=2), immature teratoma (IT, n=9) and yolk sac tumor (YST, n=5) and had preoperative PET/CT for diagnosis from January 2005 to December 2011 at our institution was conducted. We compared ratio (SUV(standard uptake value) max/liverSUV) and SUVmax according to diagnosis. And correlation between SUVmax, ratio(SUVmax/liverSUV) and tumor size, CA 125 level and stage was evaluated respectively. F-18 FDG PET/CT findings of nonepithelial ovarian cancer had no significant findings because of various cystic portion and solid portion in the image by and large. But the ratio (SUVmax/liverSUV) (9.13±6.10) and the SUVmax (14.19±8.59) in mixed germ cell tumor had significant differences(P value=0.003, 0.002) compared with other nonepithelial ovarian cancer (GCT (1.61±0.50, 2.90±1.13), DG (3.98±7.67, 7.86±5.16), SLCT (2.36±1.05, 4.65±1.77), IT(3.81±2.07, 7.77±4.20), YST(3.78±1.35, 7.22±1.50)). There was positive linear correlation between CA 125 (205.33±242.38) and SUVmax (P value=0.003), ratio (P value=0.002). And there was no significant correlation between SUVmax, ratio and tumor size (247.36±235.79(cm2)) and stage (3.40±3.00) respectively. F-18 FDG PET/CT might have a role in discriminating nonepithelial ovarian cancer especially mixed germ cell tumor. Small number of patients is limitation of this study. Therefore further study is needed to assess the diagnostic value of F-18 FDG PET/CT in nonepithelial ovarian cancer.

Background: Recent genomic studies identified four discrete molecular subgroups of medulloblastoma (MB), and the risk stratification of childhood MB in the context of subgroups was refined in 2015. In this study, we investigated the effect of molecular subgroups on the risk stratification of childhood MB. Methods: The nCounter® system and a customized cancer panel were used for molecular subgrouping and risk stratification in archived tissues. Results: A total of 44 patients were included in this study. In clinical risk stratification, based on the presence of residual tumor/metastasis and histological findings, 24 and 20 patients were classified into the average-risk and high-risk groups, respectively. Molecular subgroups were successfully defined in 37 patients using limited gene expression analysis, and DNA panel sequencing additionally classified the molecular subgroups in three patients. Collectively, 40 patients were classified into molecular subgroups as follows: WNT (n = 7), SHH (n = 4), Group 3 (n = 8), and Group 4 (n = 21). Excluding the four patients whose molecular subgroups could not be determined, among the 17 average-risk group patients in clinical risk stratification, one patient in the SHH group with the TP53 variant was reclassified as very-high-risk using the new risk classification system. In addition, 5 of 23 patients who were initially classified as high-risk group in clinical risk stratification were reclassified into the low- or standard-risk groups in the new risk classification system. Conclusion: The new risk stratification incorporating integrated diagnosis showed some discrepancies with clinical risk stratification. Risk stratification based on precise molecular subgrouping is needed for the tailored treatment of MB patients.
