        Effects of Sodium/Glucose Cotransporter 2 (SGLT2) Inhibitors on Cardiac Imaging Parameters: A Systematic Review and Meta-analysis of Randomized Controlled Trials

        Recent studies have shown that sodium/glucose cotransporter 2 (SGLT2) inhibitors might exert favourable changes on cardiac parameters as observed on cardiovascular imaging. We conducted a systematic review and meta-analysis to determine the effects of SGLT2 inhibitors on cardiac imaging parameters. Four electronic databases (PubMed, Embase, Cochrane, Scopus) were searched for studies in which the effects of SGLT2 inhibitors on cardiac imaging parameters were examined. Studies in which a population was administered SGLT2 inhibitors and analysed by echocardiography and/or cardiac magnetic resonance (CMR) imaging were included. Random-effects pair-wise meta-analysis models were utilized to summarize the studies. A total of 11 randomized controlled trials was included with a combined cohort of 910 patients. Comparing patients receiving SGLT2 inhibitors with subjects receiving placebo, the mean change in CMR-measured left ventricular mass (LVM) was −3.87 g (95% confidence interval [CI], −7.77 to 0.04), that in left ventricular end-systolic volume (LVESV) was −5.96 mL (95% CI, −10.52 to −1.41) for combined LVESV outcomes, that in left atrial volume index (LAVi) was −1.78 mL/m2 (95% CI, −3.01 to −0.55) for combined LAVi outcomes, and that in echocardiography-measured E/e′ was −0.73 (95% CI, −1.43 to −0.03). Between-group differences were not observed in LVM and LVESV after indexation. The only between-group difference that persisted was for LAVi. Treatment with SGLT2 inhibitors resulted in reduction in LAVi and E/e′ on imaging, indicating they might have an effect on outcomes associated with LV diastolic function.

        Lactobacillus plantarum DR7 Reduces Cholesterol via Phosphorylation of AMPK That Down-regulatedthe mRNA Expression of HMG-CoA Reductase

        Hypercholesterolemia is one of the primary risk factors for cardiovascular diseases. The use of lactobacilli probiotics to reduce blood cholesterol levels have been extensively reported. However, more information is needed to evaluate the possible mechanisms involved and to identify possible targets for further therapeutic development. In this study, strains of lactobacilli were screened based on the ability to assimilate cholesterol, and prevention of cholesterol accumulation in hepatic (HepG2) and intestinal (HT-29) cells. Cell free supernatant (CFS) from Lactobacillus plantarum DR7 showed a higher ability to assimilate cholesterol, reduction in cholesterol accumulation in both HepG2 and HT-29 cells, accompanied by reduced mRNA expression of HMG-CoA reductase (HMGCR) in HepG2 (p<0.05), compared to other lactobacilli. The reduction of HMGCR expression was also diminished in the presence of an AMPK inhibitor (Compound C), suggesting that L. plantarum DR7 exerted its effect via the AMPK pathway, typically via the phosphorylation of AMPK instead of the AMPK mRNA expression in HepG2 (p<0.05). Altogether, our present study illustrated that lactobacilli could exert cholesterol lowering properties along the AMPK pathway, specifically via phosphorylation of AMPK that led to reduced expression of HMGCR.

        Probiotics: The Next Dietary Strategy against Brain Aging

        Owing to their long history of safe use, probiotic microorganisms, typically from the genus Lactobacillus, have long been recognized, especially in traditional and fermented food industries. Although conventionally used for dairy, meat, and vegetable fermentation, the use of probiotics in health foods, supplements, and nutraceuticals has gradually increased. Over the past two decades, the importance of probiotics in improving gut health and immunity as well as alleviating metabolic diseases has been recognized. The new concept of a gut-heart-brain axis has led to the development of various innovations and strategies related to the introduction of probiotics in food and diet. Probiotics influence gut microbiota profiles, inflammation, and disorders and directly impact brain neurotransmitter pathways. As brain health often declines with age, the concept of probiotics being beneficial for the aging brain has also gained much momentum and emphasis in both research and product development. In this review, the concept of the aging brain, different in vivo aging models, and various aging-related benefits of probiotics are discussed.

        Correlation of Aortic Valve Calcification Volume Using Cardiac-Gated Contrast-Enhanced CT with Aortic Stenosis Severity Using Echocardiographic Indices

        Objective: The aim of the study is to assess feasibility of using aortic valve calcification volume (AVCV) as a surrogate marker for aortic stenosis (AS) by studying the correlation between AVCV on contrast enhanced CT (CECT) and severity of AS using echocardiographic indices. Materials and Methods: Ninety-four patients with AS, who underwent CECT and echocardiography as part of pre-transcatheter aortic valve implantation assessment were analysed. AVC was quantified on CECT using methodology validated by Bischoff et al. Severity of AS was categorized by echocardiography indices [transaortic jet velocity, mean pressure gradient, aortic valve area (AVA) by continuity and planimetry equations, indexed AVA to body surface area and dimensionless index (DI)]. Results: Moderate to strong correlation was found between AVCV with transaortic jet velocity (rs=0.397, p<0.001), peak pressure gradient (rs=0.410, p<0.001) and mean pressure gradient (rs=0.381, p<0.001). There was statistically significant correlation between AVCV with indexed AVA (rs=-0.225, p<0.001) and DI (rs=-0.644 p<0.001) but not with AVA by continuity equation (rs=-0.024, p<0.826) and by planimetry (rs=-0.185, p<0.158). Although not statistically significant, patients with higher degree of AS were noted to have higher AVCV burden on CECT. Conclusion: Quantification of AVCV using CECT is a potential complementary marker for grading of AS severity. AVCV has additive value when there is discrepancy between clinical and echocardiography examinations or discordant echocardiographic indices due to variation in techniques of imaging and small body size in Asian population.
