        Siriraj Stroke Score 따른 뇌졸중 환자 감별의 적합성

        Background: The differentiation between hemorrhagic(HS) and norhemorrhagic(NHS) stroke is the most important first step in the management of acute stroke because clinical management of the two disorders differs substantially. Neuroimaging studies are useful in diagnosing and distinctioning between HS and NHS. The use of clinical variables, such as Siriraj stroke scores(SSS), has led to good sensitivity, specificity and predictive values. The aim of our study was to evaluate the use of the SSS in the Korean population and assess whether it could aid to expedite treatment decisions. Methods: We reviewed 111 cases of stroke patients admitted to our hospital via the emergency department over a 6 months period from July to December 1998. Levels of consciousness, vomiting, headache, and atheroma markers used in the SSS were applied to these patients who met the criteria for a stroke. Results: Of the 111 patients, the SSS classified 83 with sensitivities of 81.1%(NHS) and 73.3%(HS) and positive predictive values of 84.3% and 68.8%, respectively. The overall accuracy rate was 78.3%. Conclusion: Our results suggest that the SSS is not reliable in distinguishing stroke types in the Korean population. Definite neuroimaging studies are needed prior to thrombolytic therapy.

      • CybexⅡ를 이용한 여자 태권도 선수와 유도 선수들의 등속성 근력에 관한 연구

        The purpose of this study was to assess isokinetic factors between female taekwondo and judo players. Fourteen female subjects employed for this study were all national team levels. They were all university students and average age were 21 years old. The main findings as follows: 1) Taekwondo players were significantly higher than Judo players in Peak Torque. 2) Taekwondo players were significantly wider than Judo players in angle of Peak Torque. 3) Taekwondo players were significantly faster than Judo players in Accel Time. 4) Taekwondo players were significantly higher than Judo players in Total Work. 5) Taekwondo players were significantly higher than Judo players in Average Power. 6) Taekwondo players were significantly higher than Judo players in TAE. 7) Taekwondo players were significantly higher than Judo players in Set Total Work. 8) There was no significant differences exist in endurance rate between female Taekwondo and Judo players.

        전기화상 환자에 대한 임상적 고찰

        Background: Good documentation of electrical injuries at the time of presentation is very important to emergency management, so this study was designed to investigate the clinical characteristics and the outcomes of patients with electrical injury. Methods: A review of 75 cases of electrical injuries admitted to our hospital via the emergency department over a 4 year period from 1996 to 1999 was conducted. Results: There were 49 patients with high-voltage injuries and 19 patients with low-voltage injuries. All but 4 patients were males, with a mean age of 29.5 years. The most common type of injury was electrothermal burn. The most common point of entry was hand. The average size of burn wounds was 14.5% TBSA in the high-voltage group and 2.5% in the low-voltage group. Forty-nine(72.1%) of the injuries were work related. The number of patients with compartment syndrome was 19, and fasciotomies were performed in all but one patient. Myoglobinuria was noted in 22 patients, but no patient developed acute renal failure due to myoglobinuria. In the high-voltage group, 10 limb amputations were performed. Complication were observed in 12 patients. The most common complication was neurological injury. The average length of hospital stay was 50.7 days in the high-voltage group and 13.8 days in the low-voltage group. The overall mortality rate was 4.3%. Couclusion: Prevention of electrical injuries is very important. Education and compliance with safety measures, as well as basic knowledge and precaution in dealing with electricity, are essential to avoid these injuries.

        무연탄을 열원으로 한 연도측위 내부 송풍식 목재 건조실의 제작에 관하여

        I. In the past, the smoke-pipe type dry kiln being characterized by the low cost of installation and easy operation was only used by the small scale of wood processing enterprisers for drying softwood and finishing air-dried stocks that were belonged to easy drying properties. The smoke-pipe type dry kiln usually was not suitable for drying hardwoods and green woods because of the difficulties of controlling drying conditions. 2. However, the smoke-pipe type dry kiln heated with smokeless coal which was designed and constructed by the anthers was demonstrated to maintain higher accuracy of controlling temperature and humidity than the other smillar types of kiln. Also, since it was constructed with home made equipments, it is supposed to offer the opportunity to install the kiln by the small scale of wood processing enterprisers who could not install the dry kiln because of economical and technical difficulties. Thus. it is expected that promoting the spread of installing the dry kiln will realize the rational use of wood.

      • Extremity CT검사 시 Cradle Angle이 피사체의 선량과 화질에 미치는 영향에 관한 연구

        Purpose : Correct positioning patients is the important factor for diagnosis and direction of treatment. Despite of technicians effort, it is difficult because of patients build and extremity, splint, and gips. In this study, we try to compare and analyze the effect of cradle angle on the dose and image quality in the case that the subject is correct with the center of cradle and the opposite case that subject get tilted by angle of cradle. Material and method : The phantoms used in the study were acryl phantom sized 32 cm, called body phantom and the anthropomorpic phantom (RS-114T, RS-116T Radiology Support Devices, Alderson, USA). We compared the dose and the image quality using the value measured in the three cases that the center of cradle was correct with phantom, 10 cm far to the left, and 10 cm far to the right. All evaluation of the dose was done using Kruskal-Wallis test to evaluate the significance. In case of Acryl phantom, the index of coincidence was evaluated by Intra calss correlation, ICC Result : As a result, each surface dose in the direction of 3 o clock and 9 o clock was 2.9±0.5 and 2.7±0.1 compared to the depth dose, 1.8±0.1 at the center. Using Kruskal-Wallis test, we found out there was significant result at the center and in 3 o clock and 9 o clock (0.001<p). According to the result of ICC test using Acryl phantom, there was statistically significant result by 0.915 (95 % CI=0.786~0.966) between the center and the direction of 9 c clock, and 0.991 (95 % CI=0.977~0.99) between the center and the direction of 9 o clock.In the evaluation of the dose using the anthropomorpic phantom, there was significant result from hand & wrist phantom and ankle & foot phantom by using Kruskal-Wallis test, because the measure about CTDI volume value at hand and wrist was 0.02<p, and at foot and ankle was 0.01<p. In case to evaluate image quality, SNR when the subject was correct with the center of cradle was 46.6 ±2.1, 44.6±3.4. SNR in the case that subject was 10 cm far to the left from the center of the cradle was 52.7±3.1, 50.2±1.4, and 10 cm far to the right was 42.2±2.8, 46.0±3.0. There was the significant result statistically (0.03<0.05). Also there was difference more than twice in the case that the subfect was far to the left or right compared to the alignment with center. Conclusion : In this study, it didn t matter in the dose and image quality even though the subject moved to the left or right in case of Extremity CT, but the distortion of image happened and the reproducibility of the shape got wores by the CT cradle tilted. That is the reason why the algorithm is applied using software to compensate for that. Also the appropriate instrument need to be used to raise the reproducibility of image. 목적 : Extremity CT 검사 시 올바른 환자의 위치 잡이는 진단 및 치료방향에 있어 중요한 요소이며, 좁은 cradle과 cradle angle, 환자의 체구와 양측 사지 그리고 각종 부목 (Splint), 고정붕대 (Gips) 등은 검사자의 노력에도 불구하고 적절한 환자의 자세를 잡는 데 한계가 준다. 이에 피사체가 cradle 중심에 일치한 경우와 cradle angle의 영향으로 피사체가 기울어졌을 경우의 cradle angle이 선량과 화질에 미치는 영향을 비교 분석하고자 한다. 대상 및 방법 : 실험에 사용된 팬텀은 Acryl phantom (지름 32 cm body phantom), 인체모형팬텀인 (RS-114T, RS-116T Radiology Support Devices, Alderson, USA)사용하였고, phantom의 중심부의 일치하였을 때 측정된 값과 좌측으로 10 cm, 우측으로 10 cm 이동하여 측정된 값을 이용하여 선량과 화질을 비교 하였다. 모든 선량 평가는 Kruskal-Wallis test 통해 유의성을 평가하였고, acryl phantom은 급내상관계수 (Intra calss correlation, ICC)를 이용하여 측정값의 일치도를 평가하였다. 결과 : acryl phantom을 이용한 선량평가에서는 중심부인 심부선량보다 표면선량 (3시 방향, 9시 방향)에서의 선량은 9시방향의 경우 2.7±0.1, 3시방향의 경우 2.9±0.5, 중심부인 심부선량에서는 1.8±0.1로 측정되었고 중심부와 9시 3시 방향에서 Kruskal-Wallis 검정을 통해 유의한 결과를 나타내었다 (0.001<p). acryl phantom을 이용한 선량의 ICC test결과 중심부와 9시 방향에서는 0.915 (95 % CI=0.786~0.966), 중심부와 3시에서는 0.991 (95 % CI=0.977~0.99)으로 중심부와 3시, 9시 방향에서의 측정값의 일치도는 통계적으로 유의한 결과를 나타내었다. 인체팬텀 선량 평가에서는 Hand & wrist phantom, foot & ankle 측정한 결과 중심에 일치하는 경우와 좌, 우측으로 이동한 경우 Kruskal-Wallis 검정을 통해 CTDIvol값에 대해 hand & wrist는 0.02<p, foot & ankle은 0.01<p로 유의성이 있었고, DLP 값에 대해 hand & wrist는 0.9>p로 유의성이 없는 반면 foot & ankle 0.01<p 으로 유의성을 보였다. 화질평가의 경우 cradle의 중심으로부터 일치한 경우의 SNR 46.6±2.1, 44.6±3.4이며, 좌측으로 10 cm 벗어난 경우 SNR은 52.7±3.1, 50.2±1.4, 우측으로 10 cm 벗어난 경우 SNR은 42.2±2.8, 46.0±3.0 으로 측정되었고, 통계적으로 유의한 결과를 나타내었다 (0.03<0.05). 화질의 정성적 평가에서는 4개의 항목 중에 B, D항목 (영상의 회전, 3D reformation)에서 좌, 우측으로 이동한 경우 중심정렬보다 2배 이상의 차이를 보였다. 결론 : 본 연구를 통해 Extremity CT 검사 시 피사체가 좌, 우측으로 이동하여 검사하면 선량과 화질에는 큰 영향이 없다는 결과를 나타내었지만, CT cradle의 기울기에 따라 영상의 왜곡과 형태의 재현성이 떨어지므로 이에 대한 보상으로 소프트웨어를 이용한 영상 기법을 적용 시키거나 cradle 기울기의 적절한 보상 및 보조기구의 활용을 통해 영상의 재현성을 높여야 할 것으로 생각된다.

      • Highly efficient and bending durable perovskite solar cells: toward a wearable power source

        <P>Perovskite solar cells are promising candidates for realizing an efficient, flexible, and lightweight energy supply system for wearable electronic devices. For flexible perovskite solar cells, achieving high power conversion efficiency (PCE) while using a low-temperature technology for the fabrication of a compact charge collection layer is a critical issue. Herein, we report on a flexible perovskite solar cell exhibiting 12.2% PCE as a result of the employment of an annealing-free, 20 nm thick, amorphous, compact TiO<SUB><I>x</I></SUB> layer deposited by atomic layer deposition. The excellent performance of the cell was attributed to fast electron transport, verified by time-resolved photoluminescence and impedance studies. The PCE remained the same down to 0.4 sun illumination, as well as to a 45° tilt to incident light. Mechanical bending of the devices worsened device performance by only 7% when a bending radius of 1 mm was used. The devices maintained 95% of the initial PCE after 1000 bending cycles for a bending radius of 10 mm. Degradation of the device performance by the bending was the result of crack formation from the transparent conducting oxide layer, demonstrating the potential of the low-temperature-processed TiO<SUB><I>x</I></SUB> layer to achieve more efficient and bendable perovskite solar cells, which becomes closer to a practical wearable power source.</P> <P>Graphic Abstract</P><P>We report annealing-free compact TiO<SUB><I>x</I></SUB> layer by atomic layer deposition for high efficiency flexible perovskite solar cells, and maintained 95% of the initial PCE after 1000 bending cycles with 10 mm bending radius. <IMG SRC='http://pubs.rsc.org/services/images/RSCpubs.ePlatform.Service.FreeContent.ImageService.svc/ImageService/image/GA?id=c4ee02441a'> </P>

        Application of Cartilage Extracellular Matrix to Enhance Therapeutic Efficacy of Methotrexate

        BACKGROUND: Rheumatoid arthritis (RA) is characterized by chronic inflammation and joint damage. Methotrexate (MTX), a commonly used disease-modifying anti-rheumatic drug (DMARD) used in RA treatment. However, the continued use of DMARDs can cause adverse effects and result in limited therapeutic efficacy. Cartilage extracellular matrix (CECM) has anti-inflammatory and anti-vascular effects and promotes stem cell migration, adhesion, and differentiation into cartilage cells. METHODS: CECM was assessed the dsDNA, glycosaminoglycan, collagen contents and FT-IR spectrum of CECM. Furthermore, we determined the effects of CECM and MTX on cytocompatibility in the SW 982 cells and RAW 264.7 cells. The anti-inflammatory effects of CECM and MTX were assessed using macrophage cells. Finally, we examined the in vivo effects of CECM in combination with MTX on anti-inflammation control and cartilage degradation in collagen-induced arthritis model. Anti-inflammation control and cartilage degradation were assessed by measuring the serum levels of RA-related cytokines and histology. RESULTS: CECM in combination with MTX had no effect on SW 982, effectively suppressing only RAW 264.7 activity. Moreover, anti-inflammatory effects were enhanced when low-dose MTX was combined with CECM. In a collagen-induced arthritis model, low-dose MTX combined with CECM remarkably reduced RA-related and pro-inflammatory cytokine levels in the blood. Additionally, low-dose MTX combined with CECM exerted the best cartilage-preservation effects compared to those observed in the other therapy groups. CONCLUSION: Using CECM as an adjuvant in RA treatment can augment the therapeutic effects of MTX, reduce existing drug adverse effects, and promote joint tissue regeneration.         Cloning of the short-tailed Gyeongju Donggyeong dog <i>via</i> SCNT: conserving phenotypic inheritance

        <P>Somatic cell nuclear transfer is a useful tool to maintain genetic information of animals. The Gyeongju Donggyeong dog is a breed registered as natural monument in Korea. The unique feature of the Donggyeong dog is its tail, as the Donggyeong dog can be classified as either short tailed or tailless. The aim of this study was to preserve the Donggyeong dog’s unique feature by cloning. Fibroblasts were obtained from a short-tailed Donggyeong dog. <I>In vivo</I> matured oocytes were enucleated, microinjected with a donor cell and fused electrically. Reconstructed embryos were transferred to six recipient dogs. One surrogate became pregnant, and one short-tailed Donggyeong dog was delivered. This study demonstrated that the phenotype of the Donggyeong dog could be conserved by somatic cell nuclear transfer.</P>
