3차 의료기관 응급실로 내원한 안면부 열상 환자에 대한 임상적 고찰

Background: Facial laceration is a common clinical problem in the emergency room that is frequently associated with other craniofacial injuries. It elicits an emotional response from the patient because of the underlying concern of permanent scar and secondary facial disfigurement, so they demand prompt and adequate treatment. The aim of this study is to analyze the incidence, etiology, depth, and site of facial lacerations to provide basic data for further understanding. Method: This study reviewed 1,043 facial-laceration patients treated in the emergency room at Pusan Paik Hospital between March 1999 and February 2000. This retrospective study was done by reviewing and analyzing the sex and age distributions, the monthly and daily distributions, the causes of injury, the types of injury, the sites of injury, the lengths of the lacerations, and associated injuries. Results: The sex ratio of men to wemen was 2.4:1, and the 0~10 age group was at the top of the age distribution. The number of patients was the highest on sundays and during March. About 9.2% of the patients visited the emergency room between 10:00 P.M. and 11:00 P.M. Falls(38.1%) were the most common cause of facial laceration, and deep laceration(51.8%) was the most common type of facial laceration. The most common laceration site was the forehead, followed by the chin and the nose. The most common associated craniofacial injury was facial bone fracture(39%), Facial-lacerations were sutured by plastic surgeons(80%), emergency physicians(10%), and oromaxillofacial surgeons(10%). We found statistical significance in the types and lengths of lacerations between men and women, among age groups(p<0.05). Conclusion: Facial laceration is one of the most common craniofacial injuries in the emergency room. By now, most lacerations have been repaired by plastic surgeons, But, the causes of lacerations are diverse and many patients are admitted due to the associated injuries. Therefore, emergency physicians should participate more aggressively in the care of facial-laceration patients to improve the outcome of the patients.

<P>Carbapenems (imipenem, meropenem, biapenem, ertapenem, and doripenem) are β-lactam antimicrobial agents. Because carbapenems have the broadest spectra among all β-lactams and are primarily used to treat infections by multi-resistant Gram-negative bacteria, the emergence and spread of carbapenemases became a major public health concern. Carbapenemases are the most versatile family of β-lactamases that are able to hydrolyze carbapenems and many other β-lactams. According to the dependency of divalent cations for enzyme activation, carbapenemases can be divided into metallo-carbapenemases (zinc-dependent class B) and non-metallo-carbapenemases (zinc-independent classes A, C, and D). Many studies have provided various carbapenemase structures. Here we present a comprehensive and systematic review of three-dimensional structures of carbapenemase-carbapenem complexes as well as those of carbapenemases. We update recent studies in understanding the enzymatic mechanism of each class of carbapenemase, and summarize structural insights about regions and residues that are important in acquiring the carbapenemase activity.</P>

Consistent thermal analysis procedure of LNG storage tank

As the LNG (Liquefied Natural Gas) tank contains cryogenic liquid, realistic thermal analyses are of a primary importance for a successful design. The structural details of the LNG tank are so complicated that some strategies are necessary to reasonably predict its temperature distribution. The proposed heat transfer model can consider the beneficial effects of insulation layers and a suspended deck on temperature distribution of the outer concrete tank against cryogenic conditions simply by the boundary conditions of the outer tank model. To this aim, the equilibrium condition or heat balance in a steady state is utilized in a various way, and some aspects of heat transfer via conduction, convection and radiation are implemented as necessary. Overall thermal analysis procedures for the LNG tank are revisited to examine some unjustifiable assumptions of conventional analyses. Concrete and insulation properties under cryogenic condition and a reasonable conversion procedure of the temperature-induced nonlinear stress into the section forces are discussed. Numerical examples are presented to verify the proposed schemes in predicting the actual temperature and stress distributions of the tank as affected by the cryogenic LNG for the cases of normal operation and leakage from the inner steel tank. It is expected that the proposed schemes enable a designer to readily detect the effects of insulation layers and a suspended deck and, therefore, can be employed as a useful and consistent tool to evaluate the thermal effect in a design stage of an LNG tank as well as in a detailed analysis.

Purpose: The purpose of the current study was to determine the intra- and inter-rater reliability of muscle thickness measurement of the TA using ultrasonography (US) conducted at different inward pressures of approximately 0.5 kg, 1.0 kg, and no pressure control. Methods: Twenty healthy subjects were recruited for this study. Two different examiners measured the thicknesses of the dominant TA of each subject randomly to assess the intra- and inter-rater reliability. The measurement values were analyzed using the intra-class correlation coefficient (ICC) with a 95% confidence interval, standard error of measurement, minimal detectable change, and coefficient of variance. Results: All intra-rater reliability ICC values showed high reliability above 0.9. Inter-rater reliability ICC values showed high reliability above 0.9 with 0.5 and 1.0 kg of inward pressure. In contrast, Inter-rater reliability ICC values showed poor reliability (0.23) with no pressure control of inward pressure. Conclusion: The findings showed that maintaining consistent inward pressure is essential for reliable results when the muscle thickness of the TA is measured by different examiners in a clinical setting.

Purpose: Yield and quality problems can be caused by clustering defects in display manufacturing processes. This article proposes a detection method for clustering defects in thin film transistor(TFT) processes of a display. Methods: A detection method for display defects was used to solve an imbalanced data problem. We introduce α-index derived from a negative binomial yield model, as one of the cluster indices. Results: We applied the proposed method to detect data collected from the TFT processes of the display. We compared the detection accuracy of the α-index. The application results show that the detection accuracy of clustering defects was 92.11%. Conclusion: In this article, the α-index as a cluster index is proposed to detect the clustering defects in TFT processes. The proposed method provides the best detection accuracy of the defect data in TFT processes.
