        The accuracy of a 3D printing surgical guide determined by CBCT and model analysis

        PURPOSE. The aim of this clinical study was to assess the accuracy of the implants placed using a universal digital surgical guide. MATERIALS AND METHODS. Among 17 patients, 28 posterior implants were included in this study. The digital image of the soft tissue acquired from cast scan and hard tissue from CBCT have been superimposed and planned the location, length, diameter of the implant fixture. Then digital surgical guides were created using 3D printer. Each of angle deviations, coronal, apical, depth deviations of planned and actually placed implants were calculated using CBCT scans and casts. To compare implant positioning errors by CBCT scans and plaster casts, data were analyzed with independent samples t-test. RESULTS. The results of the implant positioning errors calculated by CBCT and casts were as follows. The means for CBCT analyses were: angle deviation: 4.74 ± 2.06°, coronal deviation: 1.37 ± 0.80 mm, and apical deviation: 1.77 ± 0.86 mm. The means for cast analyses were: angle deviation: 2.43 ± 1.13°, coronal deviation: 0.82 ± 0.44 mm, apical deviation: 1.19 ± 0.46 mm, and depth deviation: 0.03 ± 0.65 mm. There were statistically significant differences between the deviations of CBCT scans and cast. CONCLUSION. The model analysis showed lower deviation value comparing the CBCT analysis. The angle and length deviation value of the universal digital guide stent were accepted clinically.

      • Ecological study for refrigerator use, salt, vegetable, and fruit intakes, and gastric cancer.

        <P>We used an ecological approach to determine the correlation between vegetable, fruit and salt intakes, refrigerator use, and gastric cancer mortality in Korean population. Information on fruit and vegetable intakes per capita from the National Health and Nutrition Survey, death certificate data from the National Statistical office, refrigerator per household data from Korean Statistical Information Service, and salt/sodium intake data from a cross-sectional survey were utilized. Correlation coefficients were calculated between vegetable and fruit intakes, refrigerator per household, and gastric cancer mortality and between salt and sodium intakes, and gastric cancer mortality and incidence in the four areas. With 5, 10, and 15?years lag time, refrigerator usage and fruit intake were negatively associated with gastric cancer mortality (p?<?0.01), but vegetable intake was not associated with gastric cancer mortality. When estimates of salt/sodium intake evaluated by 24-h urine collection in four areas of Korea were compared to the gastric cancer mortality and incidence in these regions, positive correlation was shown between salt/sodium intake, and gastric cancer incidence and mortality. Negative associations between refrigerator use, fruit intake, and gastric cancer mortality and positive associations between salt/sodium intake and gastric cancer mortality and incidence were suggested.</P>

      • Dietary intake and breast cancer among carriers and noncarriers of <i>BRCA</i> mutations in the Korean Hereditary Breast Cancer Study

        <P><B>Background:</B> Soy intake is associated with a lower risk of breast cancer. However, it is unclear whether the same reduction in risk associated with high soy intake is also applicable to familial or genetic breast cancer.</P><P><B>Objective:</B> The aim of this study was to assess the dietary factors among carriers and noncarriers of <I>BRCA</I> mutations in the Korean Hereditary Breast Cancer Study (KOHBRA).</P><P><B>Design:</B> The KOHBRA Study is an ongoing project composed of affected breast cancer patients and familial members of breast cancer cases with <I>BRCA</I> mutations. To assess the association between dietary diversity and breast cancer risk, an HR was estimated by comparing affected subjects with their familial nonaffected members. To assess the interaction between the combination of <I>BRCA</I> mutation and diet diversity, the case-only OR (COR) was estimated by comparing <I>BRCA</I> mutation carriers and noncarriers only in affected subjects.</P><P><B>Results:</B> Soy product intake was associated with a lower risk of breast cancer in carriers (HR: 0.39; 95% CI: 0.19, 0.79 for the highest quartile). The highest quartile of meat intake was associated with a higher risk of breast cancer regardless of <I>BRCA</I> mutation in carriers (HR: 1.97; 95% CI: 1.13, 3.44) and noncarriers (95% CI: 1.41; 1.12, 1.78). The associations of meat intake and soybean intake for breast cancer were more prominent in <I>BRCA2</I> mutation carriers. In the analysis with only cases, the highest quartile of soy intake, but not meat intake, was associated with <I>BRCA</I>-related breast cancer (COR: 0.57; 95% CI: 0.36, 0.91).</P><P><B>Conclusion:</B> Our study suggests that soy product consumption is associated with lower breast cancer risk and it had an interaction with <I>BRCA</I> mutation. This trial was registered at clinicaltrials.gov as NCT00595348.</P>

        Reliability of Quadruplicated Serological Parameters in the Korean Genome and Epidemiology Study

        OBJECTIVES: The aim of this study was to evaluate whether clinical test values from different laboratories in the Korean Genome and Epidemiology Study (KoGES) can be integrated through a statistical adjustment algorithm with appropriate intra- and inter-laboratory reliability. METHODS: External quality control data were obtained from the Korean Society for Laboratory Medicine and quadruplicated standardized serological samples (N=3,200) were manufactured in order to check the intra-and inter-laboratory reliability for aspartic acid transaminase (AST), alanine transaminase (ALT), gamma-glutamyl transpeptidase (γ-GTP), blood urea nitrogen (BUN), creatinine, uric acid (UA), fasting blood sugar (FBS), cholesterol, and triglyceride (TG). As an index of inter- and intra-rater reliability, Pearson’s correlation coefficient, intraclass correlation coefficients and kappa statistics were estimated. In addition, to detect the potential for data integration, we constructed statistical compensation models using linear regression analysis with residual analysis, and presented the R-square values. RESULTS: All correlation coefficient values indicated good intra- and inter-laboratory reliability, which ranged from 0.842 to 1.000. Kappa coefficients were greater than 0.75 (0.75-1.00). All of the regression models based on the trial results had strong R-square values and zero sums of residuals. These results were consistent in the regression models using external quality control data. CONCLUSION: The two laboratories in the KoGES have good intra- and inter-laboratory reliability for ten chemical test values, and data can be integrated through algorithmic statistical adjustment using regression equations.

        과도한 치조제 흡수를 보이는 무치악 환자에서 Centric tray와 폐구법 인상을 이용한 총의치 수복 증례

        기존의 의치 인상채득 방법은 치조제 흡수가 심한 환자에게 사용하기에 한계점이 있으며, 의치상을 과도하게 연장하여 문제를 발생시킬 수 있다. 특히나 가동성조직이 존재하는 경우에는 정확한 인상 채득을 하기가 어렵고, 따라서 총의치의 적절한 안착을 방해할 수 있다. 치조제 흡수가 심한 환자에서 폐구법 인상을 이용하여 총의치를 제작하면 더 나은 안정성과 유지를 얻을 수 있다. 본 증례는 86세의 여자환자로 상악 전치부에 치은열성비대가 존재하며 하악의 심한 치조제 흡수를 보이는 무치악 환자이다. 따라서 Centric tray를 이용하여 잠정적인 교합고경을 결정 후 고딕아치 장치를 부착한 맞춤형 트레이를 제작하였고, 폐구법 인상채득을 시행하여 상하악 총의치를 제작하였으며, 의치의 충분한 안정성과 유지력을 얻어 환자의 만족도를 높였다. Conventional denture impression techniques have limitations for edentulous patients with severe alveolar bone resorption and can cause problems from excessive border extension. Especially when a patient has movable tissue it is difficult to make accurate impression, thus might interrupt stable seating of complete denture. Fabrication of complete denture using closed mouth technique for edentulous patient with severe ridge resorption is thought to provide better stability and retention. In this case, an 86-year-old patient had both edentulous jaws with epulis fissuratum on maxillary anterior ridge and severe mandibular ridge resorption. Thus, tentative vertical dimension was determined by using Centric trayand individual tray attached with gothic arch tracer was fabricated. Complete denture was fabricated using closed mouth technique and the patient was satisfied with better stability and retention of the complete denture.

        한국판 Early Years Toolbox (EYT)의 타당화 연구

        Objectives: The aim of this study is to validate the Early Years Toolbox (EYT), which can measure executive function of children. Methods: Participants in this study were 197 3 to 5-years-old children in one kindergarten and two day care centers. An analysis of EYTs the construct validity, the concurrent validity, and reliability were performed. Results: As a result, each tool has a reciprocal static correlation (r = .22~.41, p < .05) appeared to be present, indicating that there was no difficulty in assembling them into a single factor. Also, Mr. Ant and The Eight Boxes Task (r = .19, p < .05), Go/No-Go and Lift Flag Task (r = .28, p < .05), Not This and Dig it span - Backward (r = .27, p < .05), Card Sorting and Dimensional Change Card Sort (DCCS)(r = .57, p < .05), Expressive Vocabulary and Receptive and Expressive Vocabulary Test–Expressive (REVT-E)(r = .66, p < .05), Numbers and Mathematical Ability Picture Test for Young Children (r = .73, p < .05) all had significant correlation. However, it was impossible to confirm the correlation between Childrens Social Behavior Questionnaire (CSBQ) and Preschool and Kindergarten Behavior Scales (PKBS). Finally, the EYT demonstrated high internal consistency, which enabled the verification of the reliability of the tool. Conclusions: The Early Years Toolbox is a reliable and valid executive function instrument for children and is recommended for evaluating executive function.
