Purpose This systematic review aimed to evaluate the effects of concurrent cartilage procedures on cartilage regeneration when performed alongside high tibial osteotomy (HTO). Materials and methods The systematic review followed the guidelines outlined in the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA). A comprehensive search was conducted on databases including PubMed, Embase, Cochrane Library, and Google Scholar, covering articles published until August 31, 2023. Results Sixteen studies (1277 patients) revealed that HTO, with or without concurrent cartilage procedures, leads to cartilage regeneration based on the International Cartilage Repair Society (ICRS) grade during second-look arthroscopy. No concurrent procedure showed improvement in ICRS grade (mean difference: - 0.80 to - 0.49). Microfracture (mean difference: - 0.75 to - 0.22), bone marrow aspirate concentrate (BMAC) (mean difference: - 1.37 to - 0.67), and human umbilical cord blood-derived mesenchymal stem cell (hUCB-MSC) (mean difference: - 2.46 to - 1.81) procedures also demonstrated positive outcomes. Clinical outcome assessments for each cartilage procedure were also improved during postoperative follow-up, and no specific complications were reported. Conclusions HTO with or without concurrent cartilage procedures promotes cartilage regeneration observed during second-look arthroscopy, with improved clinical outcomes. Future randomized controlled trials on the same topic, along with subsequent meta-analyses, are necessary for conclusive findings.

<B>Abstract</B><P>Angiopoietin-1 (Ang1) plays a crucial role in vascular and hematopoietic development, mainly through its cognate receptor Tie2. However, little is known about the precise role of Ang1 in embryonic stem cell (ESC) differentiation. In the present study, we used COMP-Ang1 (a soluble and potent variant of Ang1) to explore the effect of Ang1 on endothelial and hematopoietic differentiation of mouse ESCs in an OP9 coculture system and found that Ang1 promoted endothelial cell (EC) differentiation from Flk-1+ mesodermal precursors. This effect mainly occurred through Tie2 signaling and was altered in the presence of soluble Tie2-Fc. We accounted for this Ang1-induced expansion of ECs as enhanced proliferation and survival. Ang1 also had an effect on CD41+ cells, transient precursors that can differentiate into both endothelial and hematopoietic lineages. Intriguingly, Ang1 induced the preferential differentiation of CD41+ cells toward ECs instead of hematopoietic cells. This EC expansion promoted by Ang1 was also recapitulated in induced pluripotent stem cells (iPSCs) and human ESCs. We successfully achieved in vivo neovascularization in mice by transplantation of ECs obtained from Ang1-stimulated ESCs. We conclude that Ang1/Tie2 signaling has a pivotal role in ESC-EC differentiation and that this effect can be exploited to expand EC populations.</P>
The Effect of Methanol on the Structural Parameters of Neuronal Membrane Lipid Bilayers

The structures of the intact synaptosomal plasma membrane vesicles (SPMVs) isolated from bovine cerebral cortexs, and the outer and the inner monolayer separately, were evaluated with 1,6-diphenyl-1,3,5-hexatriene (DPH) and 1,3-di(1-pyrenyl)propane (Py-3-Py) as fluorescent reporters and trinitrophenyl groups as quenching agents. The methanol increased bulk rotational and lateral mobilities of SPMVs lipid bilayers. The methanol increased the rotational and lateral mobilities of the outer monolayers more than of the inner monolayers. n-(9-Anthroyloxy)stearic acid (n-AS) were used to evaluate the effect of the methanol on the rotational mobility at the 16, 12, 9, 6, and 2 position of aliphatic chains present in phospholipids of the SPMVs outer monolayers. The methanol decreased the anisotropy of the 16-(9-anthroyloxy)palmitic acid (16-AP), 12-(9-anthroyloxy)stearic acid (12-AS), 9-(9-anthroyloxy)stearic acid (9-AS), and 6-(9-anthroyloxy)stearic acid (6-AS) in the SPMVs outer monolayer but it increased the anisotropy of 2-(9-anthroyloxy)stearic acid (2-AS) in the monolayers. The magnitude of the increased rotational mobility by the methanol was in the order at the position of 16, 12, 9, and 6 of aliphatic chains in phospholipids of the outer monolayers. Furthermore, the methanol increased annular lipid fluidity and also caused membrane proteins to cluster. The important finding is that was far greater increase by methanol in annular lipid fluidity than increase in lateral and rotational mobilities by the methanol. Methanol alters the stereo or dynamics of the proteins in the lipid bilayers by combining with lipids, especially with the annular lipids. In conclusion, the present data suggest that methanol, in additions to its direct interaction with proteins, concurrently interacts with membrane lipids, fluidizing the membrane, and thus inducing conformational changes of proteins known to be intimately associated with membranes lipids.

Absence of exhalation before inhaler use is associated with acute exacerbation in patients with COPD

Background: and Aims: Inhalers are essential for pharmacotherapy in airway diseases. Poorly use of the inhalers is associated with poor clinical outcomes. However, few studies have examined which kind of inhaler techniques have the most clinical significance. This study sought to investigate what stage of the inhaler technique is associated with severe acute exacerbations (SAE) of airway diseases. Methods: Data from a prospective observational cohort which had enrolled patients with airway diseases including asthma and COPD, from two university hospitals, was used. SAE was defined as an exacerbation with an emergency room visit or hospitalization. These inhaler techniques of the subjects were checked by research assistants at the time of enrollment regardless of the class of prescribed inhalers : “open and hold device properly”, “load and prime device”, “exhale to residual volume slowly”, “inhale slowly and deeply”, “hold breath for 5-10 seconds” and “rinse mouth”. The association of the inhaler techniques with previous SAE in a year was analyzed. Results: Of 350 participants, 75 (asthma: n=33/133, 44%; COPD: n=30/111, 40%; ACO: n=12/51, 16%) had experienced SAE in the previous year. Overall, 62.8% of the subjects had proper inhaler techniques for the six items. Among these, only inadequate “exhale to residual volume slowly” was associated with the history of SAE (odds ratio, OR=2.39; confidence interval, CI=1.23-4.66) on multivariate logistic regression model adjusting covariates including age, gender, smoking history, dyspnea, percent of the predicted value in forced expiratory volume in one second. Interestingly, this association was significant only in the subjects with COPD (OR=6.20; CI=1.91-20.12), not asthma or ACO. Conclusions: In this cohort of airway diseases, absence of “exhale to residual volume slowly” had a significant association with SAE, suggesting that exhalation before inhaler use should be emphasized in the education of patients, especially with COPD.

공간분석모델링에 의한 산지의 토사붕괴방재기능 적합도 평가

The objective of this study is to develop a spatial analysis modeling technique to evaluate the functional suitability of forest lands for land slide prevention. The functional suitability is classified into 3 categories of high, medium and low according to the potential of land slide on forest lands. The potential of land slide hazards is estimated using the measurements of 7 major site factors : slope, bed rock, soil depth, shape of slope, forest type and D.B.H. class of trees. The analytic hierarchical process is applied to determining the relative weight of site factors in estimating the potential of land slides. The spatial analysis modeling starts building base layers for the 7 major site factors by 25m × 25m grid analysis or TIN analysis, reclassifies them and produces new layers containing standardized attribute values, needed in estimating land slide potential. To these attributes, applied is the weight for the corresponding site factor to build the suitability classification map by map algebra analysis. Then, finally, cell-grouping operations convert the suitability classification map to the land unit function map. The whole procedures of the spatial analysis modeling are presented in this paper.
