        30% Minocycline 국소 약물송달제제의 생체내, 외방출역학, 세포독성 및 세포활성도 측정

        Present study was performed to examine in vitro and in vivo release kinetics, cell cytotoxicity and cellular growth survival of local 30% Minocycline delivery. The purpose of this study was to determine the possibility of clinical use of 30% Minocycline load polycaprolactone film(Mino-Strip). The size of Mino-strip is 6.5㎜×2.5㎜×0.3㎜, Mwt: 5.7㎎). In vitro releasing test, each film showed a large initial burst effect within first one and two hours and more than 84% of Minocycline was releasing test, each film showed a large burst effect within first two and three hours. A steady state kinetics was observed for 8 days both in vivo and in vitro releasing test. In cytotoxicity test, there was no significant cytotoxic effect in Mino-strip to human gingival fibroblast. In cellular growth survival test, there was no significant cellular growth survival effect in Mino-strip to human gingival fibroblast.

        Comparison of Indomethacin, Diclofenac and Aspirin-Induced Gastric Damage according to Age in Rats

        Background/Aims: Aging gastric mucosa is known to have decreased mucosal defenses and increased susceptibility to injury by nonsteroidal anti-infl ammatory drugs. Depending on the type of nonsteroidal anti-infl ammatory drug (NSAID), the underlying mechanisms and the extent of damage to the stomach or intestine may differ. This study was performed to evaluate the acute gastric damage caused by different doses of indomethacin, diclofenac and aspirin in rats of various ages. Methods: For the acute models, indomethacin (10, 20 or 40 mg/kg), diclofenac (40 or 80 mg/kg) or aspirin (100 mg/kg) was given to 7- and 25-week-old and 1-year-old Sprague-Dawley rats by intragastric gavage. The gross ulcer index, damage area as assessed by imaging, histological index, myeloperoxidase (MPO) activity, and cytosolic phospholipase A2 (cPLA2) levels were measured after 24 hours. Results: The gross ulcer index and damage area increased with age in the presence of three NSAIDs (p<0.05). The increases in MPO levels induced by diclofenac and aspirin were signifi cantly higher in 1-year-old than 7-week-old rats (p<0.05). cPLA2 expression induced by indomethacin (10 and 40 mg/ kg) was greater in the 1-year-old rats, compared with 7-weekold rats (p<0.05).       • Origin of Size Dependency in Coherent-Twin-Propagation-Mediated Tensile Deformation of Noble Metal Nanowires

        <P>Researchers have recently discovered ultrastrong and ductile behavior of Au nanowires (NWs) through long-ranged coherent-twin-propagation. An elusive but fundamentally important question arises whether the size and surface effects impact the twin propagation behavior with a decreasing diameter. In this work, we demonstrate size-dependent strength behavior of ultrastrong and ductile metallic NWs. For Au, Pd, and AuPd NWs, high ductility of about 50% is observed through coherent twin propagation, which occurs by a concurrent reorientation of the bounding surfaces from {111} to {100}. Importantly, the ductility is not reduced with an increase in strength, while the twin propagation stress dramatically increases with decreasing NW diameter from 250 to 40 nm. Furthermore, we find that the power-law exponent describing the twin propagation stress is fundamentally different from the exponent describing the size-dependence of the yield strength. Specifically, the inverse diameter-dependence of the twin propagation stress is directly attributed to surface reorientation, which can be captured by a surface energy differential model.         시화호 표층퇴적물의 특성과 오염도 평가

        시화호 표층퇴적물 중 지구화학적 특성과 미량금속의 오염도 평가를 위해 입도, 강열감량(IL), 화학적산소요구량(COD) 및 미량금속(As, Pb, Cd, Cu, Cr, Zn, Ni, Mn, Hg, Fe, Li)을 분석 하였다. 시화호 표층퇴적물의 평균입도는 2.94~6.35Ø 의 범위로 분포하였으며, 퇴적상은 전반적으로 sandy silt(sZ)를 나타내었고, 이러한 표층퇴적물의 성상은 유기물 및 미량금속의 농도와 상관관계를 나타내었다 (p<0.05). 특히, 표층퇴적물 중 As, Pb, Cd, Cu, Cr, Zn, Fe, Hg은 평균입도와 상관성을 보일 뿐만 아니라, IL(Ignition Loss)과도 높은 상관성을 보여 이들 원소의 농도가 입도와 유기물의 영향을 받고 있는 것으로 나타났다. 이에 시화호의 표층퇴적물 관리에 있어서 미량금속의 유입 뿐 만 아니라 유기물의 유입을 병행하여 조사되어야 할 것으로 판단되었다. 오염도 평가에 있어서 표층퇴적물 중 미량금속 함량의 농축계수와 농집지수는 대부분 약간 오염된 상태를 나타낸 가운데, As의 경우 농축계수와 농집지수 가 오염된 상태를 보였다. 이는 특정 중금속에 의한 농축이 여전히 해결되지 않은 것으로 나타나, 이를 해결할 수 있는 방안마련이 시급할 것으로 사료되었다. 아울러 비교적 공단지역 및 배수갑문에 근접한 지역은 오염도가 높은 반면, 시화호 중앙부분은 오염도가 낮은 경향을 보여, 지역적 편차를 고려한 지속적인 모니터링시 이 필요할 것으로 판단되었다. The purpose of this study was to understand the pollution level of the surface sediment of Sihwa Lake by assessing its geochemical characteristics and investigating the spatial distribution of trace metals and organic matter. In the surface sediment of Sihwa lake, the mean grain size was between 2.94 and 6.35 Ø and the main type of sediment was sandy silt. The concentrations of As, Co, Cr, Ni, V and Li among the metal elements in the surface sediment were correlated with the mean crust concentration (p<0.05). Based on the strong correlation between the metals (Cd, Cu, Pb and Zn) and organic matter (Ignition Loss), the concentrations of these metal elements seem to be controlled by the organic matter dilution effect. The trace metal pollution level, determined by applying the Republic of Korea Marine environmental standard and the US National Oceanic and Atmospheric Administration"s sediment quality guidelines, showed the pollution level of As to be either close to or in excess of the above-mentioned standards at almost all levels. The enrichment factor and geoaccumulation index of As showed that there was an incremental increase of pollution by elements other than V, Cr, Co, Fe, Al and Mn. Moreover, the nearby industrial area and dike were more polluted than the other areas, so the surface sediments in Sihwa lake should be monitored by taking into consideration the geological variations.

        JNM : Original Article ; Association of Sleep Dysfunction and Emotional Status With Gastroesophageal Reflux Disease in Korea

        Background/Aims Sleep disturbances and emotional dysfunction are commonly associated with gastroeophageal reflux disease (GERD). The aims of this study were to evaluate GERD symptoms and disturbance in erosive reflux disease (ERD) and nonerosive reflux disease (NERD) patients, and to compare sleep dysfunction, depressive mood, anxiety and quality of life (QOL) among the control, ERD and NERD patients in Korea. Methods The Korean subjects were enrolled between 2010 and 2012, classified into 3 groups; the control group with no symptom and normal endoscopic findings, the ERD group with erosive esophagitis and the NERD group with more than one episode of heartburn or acid regurgitation per week, positive response to proton pump inhibitor, and normal endoscopic findings. Questionnaire included GERD symptoms, GERD impact scale (GIS) and daytime pathological sleepiness (Epworth sleepiness scale), sleep dysfunction (Pittsburgh sleep quality index, PSQI), depression and anxiety (Hospital anxiety and depression scale, HADS) and QOL (WHO quality of life scale abbreviated version, WHOQOL-BREF). Results A total of 217 subjects were enrolled as follows; control (n = 70), ERD (n = 70) and NERD (n = 77). Impact of symptom of GIS score was higher in the NERD (9.2 ± 0.4) than in the ERD (6.5 ± 0.3) group (P< 0.001). Sleep dysfunctions were more frequent in GERD than the control group (PSQI score [P = 0.021]). Anxiety subscale of HAD score was higher in NERD (7.0 ± 0.5, P = 0.002) and ERD (6.2 ± 0.7, P = 0.004) groups than control (4.3 ± 0.7) group. WHOQOL-BREF scores in NERD (54.9 ± 2.3) and ERD (57.8 ± 2.4) groups were significantly lower than those in the control group (63.8 ± 2.4) (P = 0.002; P = 0.014, respectively).Conclusions The patients with NERD than ERD suffered more from the symptoms and disturbance in Korea. Sleep dysfunction and anxiety mood were higher and QOL was decreased in GERD, especially in NERD, suggesting that those factors might affect the severity of NERD.       • Two-dimensional MoS2/Fe-phthalocyanine hybrid nanostructures as excellent electrocatalysts for hydrogen evolution and oxygen reduction reactions

        <P>Two-dimensional (2D) MoS2 nanostructures have been extensively investigated in recent years because of their fascinating electrocatalytic properties. Herein, we report 2D hybrid nanostructures consisting of 1T′ phase MoS2 and Fe-phthalocyanine (FePc) molecules that exhibit excellent catalytic activity toward both the hydrogen evolution reaction (HER) and oxygen reduction reaction (ORR). X-ray absorption spectra revealed an increased Fe-N distance (2.04 Å) in the hybrid complex relative to the isolated FePc. Spin-polarized density functional theory calculations predicted that the Fe center moves toward the MoS2 layer and induces a non-planar structure with an increased Fe-N distance of 2.05 Å, which supports the experimental results. The experiments and calculations consistently show a significant charge transfer from FePc to stabilize the hybrid complex. The excellent HER catalytic performance of FePc-MoS2 is characterized by a low Tafel slope of 32 mV dec<SUP>−1</SUP> at a current density of 10 mA cm<SUP>−2</SUP> and an overpotential of 0.123 V. The ORR catalytic activity is superior to that of the commercial Pt/C catalyst in pH 13 electrolyte, with a more positive half-wave potential (0.89 <I>vs</I>. 0.84 V), a smaller Tafel slope (35 <I>vs</I>. 87 mV·dec<SUP>−1</SUP>), and a much better durability (9.3% <I>vs</I>. 40% degradation after 20 h). Such remarkable catalytic activity is ascribed to the HER-active 1T′ phase MoS2 and the ORR-active nonplanar Fe-N4 site of FePc.</P>

        대장 게실출혈과 연관된 위험인자

        목적: 대장게실출혈은 하부위장관 출혈의 30-40%를 차지하며 이 중 3-5%에서는 대량출혈로 나타난다. 이번 연구에서는 대장내시경검사를 통해 게실이 확인된 환자들을 대상으로 대장게실출혈의 위험인자에 대하여 후향적으로 알아보고자 하였다. 대상 및 방법: 대장게실증, 대장게실염, 대장게실출혈의 상병명을 통해 확인된 1,003명 중 대장내시경에서 대장게실이 확인된 216명을 대상으로 출혈이 확인되었거나 추정된 환자군과 출혈이 없는 환자군으로 나누어 나이, 성별, 게실 위치에 따른 차이를 비교하였고 동맥경화증 관련 기저질환, 흡연 및 음주 그리고 복용약제 등의 위험인자에 대하여 두 군 간의 차이를 평가하였다. 결과: 216명 중 게실출혈은 35명에서 관찰되었다(16.2%). 평균 연령은 출혈군에서 비출혈군에 비해 유의하게 높았으나 남녀비는 두 군 간에 유의한 차이는 없었다. 두 군에서 모두 우측 대장게실의 비율이 높았으며 게실이 양측성으로 존재하는 경우 출혈 환자의 비율이 더 높았다. 고령, 양측성 게실, 고혈압, 당뇨병, 허혈성 심장질환, 비만이 있을 때 그리고 아스피린, NSAIDs, 칼슘통로차단제의 복용이 출혈과 유의한 관계를 보였다. 다변량 분석에서 아스피린과 양측성 대장게실증이 출혈의 독립적인 위험인자로 나타났다. 결론: 대장게실이 양측에 존재하거나 또는 아스피린을 복용중인 환자에서 대장게실 출혈의 위험성이 증가하므로 이에 대한 주의와 함께 환자에 대한 교육이 필요할 것으로 생각된다. Background/Aims: Colonic diverticular bleeding cases account for 30-40% of the lower gastrointestinal bleeding, among which, 3-5% appear to be massive bleeding. The purpose of this study was to evaluate the risk factors for colonic diverticular bleeding diagnosed by colonoscopic examination. Methods: Among the 1,003 patients, who were identified to have colonic diverticulosis including sleeding by diverticulitis and diverticular bleeding coding search, 216 patients had diverculosis, and they were divided into two groups: one with diverticular bleeding, and the other without bleeding. We evaluated the potential risk factors for diverticular bleeding, based on age, gender, location of diverticulum, comorbidities related to atherosclerosis, smoking, alcohol and medications, and compared them between both groups. Results: Among the 216 patients, we observed colonic diverticular bleeding in 35 patients (16.2%). The mean age of the bleeding group was significantly older than that of non-bleeding group. No difference was observed regarding gender ratio. Right colonic diverticula were common in both groups, but there were higher proportion of patients with bleeding in bilateral diverticuosis. Old age, bilateral diverticulosis, presence of atherosclerosis related diseases (hypertension, diabetes mellitus, ischemic heart disease, obesity), use of aspirin, NSAIDs and calcium channel blocker, increased the risk of bleeding. In a multivariate analysis, use of aspirin and bilateral diverticulosis were identified as independent risk factors for colonic diverticular bleeding. Conclusions: Since the patients who took aspirin and/or had bilateral colonic diverticulosis increased the risk of bleeding from divertuculi. As such, caution and education of patients are required.

        만성 B형간염 환자에서 심한 피부 발진을 보인 라미부딘 부작용 1예

        Lamivudine is widely used for the treatment of chronic hepatitis B infection because of it`s remarkable antiviral efficacy and safety. We report a case of severe skin eruption caused by lamivudine. A 47-year-old female was admitted because of jaundice and itching sensation. She was diagnosed as chronic hepatitis B infection a few years ago but did not receive any specific treatment. Laboratory data showed acute deterioration of chronic hepatitis B infection. We prescribed lamivudine as a rescue therapy. Her general condition improved and lab data showed improvement in liver function test thereafter. However, she complained of severe skin eruption and itching sensation a few days after the discharge. We stopped lamivudine because the symptoms did not improve despite the use of anti-histamine. Skin biopsy showed interface dermatitis. After stopping lamivudine, her symptoms improved. However, the skin eruption developed again after lamivudine was restarted. Adefovir was used instead, and the patient did not experience any further skin problems since then. (Korean J Gastroenterol 2006;48:281-285)
