N/A Background : Hepatopulmonary syndrome (HPS) refers to the association of hypoxemia, intrapulmonary shunting and chronic liver disease. But there is no clear data about the prevalence of HPS in postnecrotic liver cirrhosis by hepatitis B virus(HBV), the most common cause of liver disease in Korea. The aim of this study was to investigate the prevalence of HPS in poorly compensated postnecrotic liver cirrhosis by HBV, and the correlation of the hepatopulmonary syndrome with clinical aspects of postnecrotic liver cirrhosis by HBV. Methods : Thirty-five patients underwent pulmonary function test, arterial blood gas analysis and contrast-enhanced echocadiography. All patients were diagnosed as HBV-induced Child class C liver cirrhosis and had no evidence of intrinsic cardiopulmonary disease. Results : Intrapulmonary shunt was detected in 6/35 (17.1%) by contrast- enhanced echocariography. Two of six patients with intrahepatic shunts had significant hypoxemia (PaO2 < 70 mmHg) and four showed increased alveolar- arterial oxygen gradient over 20 mmHg. Only cyanosis could reliably distinguish between shunt positive and negative patients. Conclusions : The prevalence of intrapulmonary shunt in poorly compensated postnecrotic liver cirrhosis by HBV was 17.1% and the frequency of hepatopulmonary syndrome was relatively low (5.7%). ‘Subclinical’ hepatopulmonary syndrome (echocardiographically postive intrapulmonary shunt but without profound hypoxemia) exists in 11.4% of cases with poorly compensated postnecrotic liver cirrhosis by HBV. Cyanosis is the only reliable clinical indicator of HPS of HBV- induced poorly compensated liver cirrhosis. Further studies are required to determine if the prevalence and clinical manifestations of HPS varies with etiology or with geographical and racial differences.

This study is intended to examine the tDCS and Montoya stair task(MST) on sensorimotor recovery and glial scar expression in MCAo induced stroke model of rat. To achieve this goal, this study selected 80 SD rats of 8 weeks. The experiment groups were divided them into four groups, and assigned 20 rats to each group. GroupⅠ was a experimental control group; GroupⅡ was a tDCS application group after MCAo; GroupⅢ was a MST application group after MCAo; GroupⅣ was a tDCS and MST application group after MCAo. In each group, neurological function test measurement, motor behavior test, montoya stair task test, immunohistochemistric finding of GFAP expression finding were analyzed. In motor behavior test, the outcome of groupⅠ was significantly difference than the other group, especially from 14days. In montoya stair task test, the outcome of groupⅠ was significantly lower than the other group especially, groupⅡ were significantly different on 14days and group Ⅳ was most significantly difference than the other group. In immunohistochemistric finding, groupⅡ, Ⅲ, Ⅳ were decrease GFAP expression on depend on time stream. These results throughout the MCAo due to focal ischemic brain injury rat model four weeks tDCS and MST was applied, when the neurobehavioural, upper extremity function and ability, histopathologic data suggest that sensorimotor function recovery and a positive influence on glial scar decrease and confirmed that.

A three-year-old male Thoroughbred horse with corneal opacity in his left eye was donated to Chonbuk National University for anatomical study. Upon gross observation, two whitish parasites were moving swiftly within the vitreous chamber of the eyeball. The worms obtained from the eye after anatomical dissection were identified as Setaria digitata by morphological observation with light and scanning electron microscopes; one male (43 mm in length) and one female (55 mm) were found. This aberrant ocular infection by S. digitata is the first case reported in horses in Korea.

<P><B>Abstract</B></P> <P><B>Background</B></P> <P>Dual antiplatelet therapy (DAPT) is recommended in patients receiving drug-eluting stents (DES). However, bleeding risk should be weighed against ischemic risk. Utility of GRACE risk score and ACUITY-HORIZONS bleeding risk score was assessed in patients with acute myocardial infarction (MI) according to use of P2Y12 blocker.</P> <P><B>Methods</B></P> <P>From the Korea Acute Myocardial Infarction Registry-National Institute of Health database, 7791 patients with acute MI receiving DES were divided into ticagrelor (<I>n</I> =1554) and clopidogrel (<I>n</I> =6237) groups. Propensity-matched 12-month mortality and bleeding event rates were compared according to GRACE and ACUITY-HORIZONS scores. Patients who received thrombolysis, prasugrel or anticoagulants, or who discontinued or switched DAPT were excluded.</P> <P><B>Results</B></P> <P>In all patients, high-risk patients more often received clopidogrel. After propensity score matching (<I>n</I> =1553 in each group), 12-month mortality was not different, but TIMI major bleeding rate was higher with ticagrelor (2.8% vs. 1.4%, <I>p</I> =0.007). On subgroup analysis, 12-month mortality was lower with ticagrelor in patients with high (>140) compared to low-to-moderate risk GRACE score (5.1% vs. 7.9%, <I>p</I> =0.04). When combined with ACUITY-HORIZONS bleeding score, 12-month mortality was lower with ticagrelor in patients with high GRACE score but without very high (≥20) ACUITY-HORIZONS score (2.4% vs. 5.3%, <I>p</I> =0.03).</P> <P><B>Conclusions</B></P> <P>In patients with acute MI receiving DES, GRACE and ACUITY-HORIZONS scores may help guide DAPT. In patients with high GRACE score, a more potent P2Y12 blocker may be considered, particularly in the subset not at very high risk of bleeding.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Both ischemic and bleeding risks are correlated with mortality post-percutaneous coronary intervention. </LI> <LI> GRACE and ACUITY-HORIZONS scores may help guide dual antiplatelet therapy (DAPT) in patients with acute myocardial infarction (MI). </LI> <LI> The optimal DAPT regimen in Asian patients with acute MI needs to be determined. </LI> </UL> </P>

Casting defects, which are closely related to entrapped air bubbles and metallic oxides, occur very frequently in the casting process. Many researchers have shown that these defects can be reduced by adopting an appropriate gating system design. But, it is difficult for field engineers to identify a specific gating system that is more appropriate for their products. In this study, we tried to draw a comparison of gating system designs with and without ceramic foam filters. A ceramic foam filter was added to the horizontal runner just after the sprue to prevent air bubble generation and to reduce turbulence without change of the gating system design. To verify the effects of initial pouring velocity, the experiment was conducted with four different amounts of water volume in the reservoir. Results of the water modeling experiment applying the filter showed remarkably changed flow characteristics. Although the study confirmed that use of the filter may change the flow characteristics, it needs to be noted that only filter use alone cannot solve all the problems caused by a poorly designed gating system.
