1. Objective: Gastric Stuffiness (心下?證) in the Soeumin Taeeum symptomatology (少陰人 太陰病) in Sasang Constitutional Medicine shows an interesting evolutionary process. Originally found in the texts of Sanghanlun (傷寒論), the perspectives regarding the symptoms of Gastric Stuffiness were expanded by many medical scholars and Lee Jema. To comprehend the Jema's concept of Taeeum symptomatology and utilize his prescription clinically, it deserves to study on the Gastric Stuffiness in Donguisuseowon (東醫壽世保元). 2. Methods: Many texts including Sanghanlun (傷寒論) and Dongeuibogam (東醫寶鑑) were reviewed in order to track the evolutionary process concerning the symptoms of Gastric Stuffiness. Also, Lee Jema's works including the different versions of Donguisuseowon (東醫壽世保元) were reviewed to examine the development of Lee's concepts regarding the Gastric Stuffiness in the Soeumin Taeeum symptomatology. 3. Results and Conclusions: 1) Most of the previous concepts on Gastric Stuffiness were generally similar to those found in Sanghanlun, especially in that they addressed Yang deficiency of the middle triple energizeer as the central pathogenic process. 2) Lee Jema's views on the epigastric discomfort in the Soeumin Taeeum symptomatology evoloved over time and culminated in the finalized pathology and therapeutics appearing in his last work, the Shinchuk version of Donguisuseowon (東醫壽世保元). 3) 4 kinds of prescription which invented by Lee Jema to treat the Gastric Stuffiness in the Soeumin Taeeum symptomatology can be matched to the prescription of Sanghanlun respectively. However through evolutionary process of Jema's prescriptions, the indication of them expanded: not only to treat Gastric Stuffiness but to diverse symptoms of Soeumin.

저자들은 하지 자색반과 고환 통증으로 내원한 알코올성 간경화증 환자에서 LCV의 원인 감별하는 과정에서 HCC와 하대정맥 막성폐쇄를 진단하고 이에 대한 치료 후 LCV의 호전된 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Vasculitis is a heterogeneous group of diseases that destroy blood vessel walls by inflammation. Approximately half of vasculitis cases are idiopathic, but sometimes associated with genetic factors, medicines, chronic infection, autoimmune diseases, and malignancies. Although the mechanism remains unclear, vasculitis secondary to malignancy, also known as paraneoplastic vasculitis, has been reported. It is generally associated with hematologic malignancies rather than solid malignancies and commonly presents as leukocytoclastic vasculitis or polyarteritis nodosa. We experienced a case of leukocytoclastic vasculitis in a patient with hepatocellular carcinoma and membranous obstruction of the inferior vena cava. 주정중독 환자에서 Marchiafava-Bignami Disease로 판명된 환자 2례

Among the many neuropsychiatric complications in chronic alcoholics, the Marchiafava-Bignami disease is a rare clinical condition, which is pathologically characterized by selective degeneration of the corpus callosum as well as the anterior and posterior commissural fibers. It can share many of the common symptoms of chronic alcoholism, but frontal lobe signs(motor and mental slowness, apathy, and frontal releasing signs) are the most consistent findings. We report two cases of Marchiafava-Bignami disease that showed abnormal signal density in the corpus callosum on MRI or CT, and that presented with initial symptoms of psychiatric problems such as personality and behavioral changes.
Thermocycling이 상아질과 복합레진의 결합강도에 미치는 영향

The purpose of this study was to evaluate the effect of thermocycling on the tensile bond strength and marginal leakage of composite bond to dentin surface. Four kinds of dentin conditioner and adhesives including Superbond D-Liner, All-Bond 2, Prisma Universal Bond 3, and Clearfil Photobond were used for this study. Thermocycling consisted of 500 cycles, 15 seconds dwell time each in 5℃ and 55℃ bath. Tensile bond strength was measured using an Instron testing machine with a crosshead speed of 1mm/min, marginal leakage was evaluated using an image analyzer, and applicated dentin surface was observed by SEM. The results obtained were summerized as follows; 1. Tensile bond strength was 6.64 ±2.32 MPa at Superbond D-Liner group, 6.35 ±1.56 MPa at All-Bond 2 group, 4.75 ±0.95 MPa at Prisma Universal Bond 3 group and 4.66 ±1.89 MPa at Clearfil Photobond group. Superbond D-Liner group and All-Bond 2 group were larger than Clearfil Photobond group(P<0.05). 2. After thermocycling, tensile bond strength showed significant difference between All-Bond 2 group and the others, Superbond D-Liner group and clearfil Photobond group(P<0.05). 3. Marginal leakage after thermocycling of Superbond D-Liner group and All-Bond 2 group were less than Prisma Universal Bond 3 group and Clearfil Photobond group(P<0.05).
마취된 흰쥐 해마신경세포에서 Neurobiotion 이온주입으로 인한 신경세포의 생리적 특성의 변화

마취된 흰쥐를 사용하여 해마의 CA영역에 위치한 피라밋 세포들의 세포막 특성을 in vivo의 세포내 기록법에 의해서 관찰한 후 2.5% neurobiotin을 세포내 기록용 미세전극에 채워 세포내로 충진시킨후 충진전과 동일한 실험순서로 반응을 다시 관찰하고 ABC kit를 이용하여 면역조직염색을 행하여 형태학적인 관찰을 하였다. 피라밋세포의 세포내 반응 특성은 높은 휴지막 세포막전위, 낮은 input resistance 그리고 큰 활동전위를 가졌다. neurobiotin 충진 전·후에 따른 세포막 특성의 변화는 sustained AHP의 duration과 amplitude, input resistance. 그리고 세포외 및 세포내 자극에 따른 AP 수에서 유의한 차이를 보였고(P<0.05). 세포외 자극에 의한 억제는 주로 전반부에 나타났으며 CA3 영역에 위치한 이 세포의 형태학적 관찰 결과 세포체는 피라밋층에서 분명한 피라미드 형태를 띄고 있었고, 기저 및 선단 가지가 각각 백색판층 및 섬유방-분자층까지 뻗어 있어으며 축삭은 겉질을 향해 기저가지 돌기면에서 수직으로 뻗어 있었다. 해마의 주세포인 피라밋세포의 세포막 특성과 세포내 염색지시체(marker)로 주로 쓰이는 neurobiotin에 의해 세포막 특성중 일부가 변화됨을 알 수 있었고, 뇌내의 신경세포연결망이 완전히 보존되어 세포들 사이에 시냅스관계를 추측할 수 있는 in vivo 실험 모델이 응용될 수 있음을 제시하였다. Pyramidal cells in the hippocampal CA area were recorded from and filled with neurobiotin in anesthetized rats. The extent of their dendrites and the electropharmacological properties of membrane as well as the effect before and after neurobiotion injection were examined. Pyramidal cells had a high resting membrane potential, a low input resistance, and a large amplitude action potential. A afterhyperpolarization was followed a single action potential. Most pyramidal cells did not display a spontaneous firing. Pyramidal cell displayed weak inward rectification and anodal break excitation in response to negative current injection into the cell. Membrane properties of recorded neurons before and after neurobiotin injection with consecutive current injection were compared. Some properties were significantly increased after labelling(P>0.05) ; the duration and amplitude of sustained AHP, input resistance, and the number of action potentials for simultaneous intra-and extracellular stimulations. Neurobiotin-filled neurons showed pyramidal morphology. Cells were generally bipolar dendrite processes ramifying in stratum lacunosum-moleculare, radiatum, and oriens.
