Companding algorithms have been used to enhance speech recognition in noise for cochlea implant users. The efficiency of using companding for digital hearing aid users is not yet validated. The purpose of this study is to evaluate the performance of the companding for digital hearing aid users in the various hearing loss cases. Using HeLPS, a hearing loss simulator, two different sensorinerual hearing loss conditions were simulated; mild gently sloping hearing loss(HL1) and moderate to steeply sloping hearing loss(HL2). In addition, a non-linear compression was simulated to compensate for hearing loss using national acoustic laboratories-non-linear version 1(NAL-NL1) in HeLPS. In companding, the following four different companding strategies were used changing Q values(q1, q2) of pre-filter(F filter) and post filter(G filter). Firstly, five IEEE sentences which were presented with speech-shaped noise at different SNRs(0, 5, 10, 15 dB) were processed by the companding. Secondly, the processed signals were applied to HeLPS. For comparison, signals which were not processed by companding were also applied to HeLPS. For the processed signals, log-likelihood ratio(LLR) and cepstral distance(CEP) were measured for evaluation of speech quality. Also, fourteen normal hearing listeners performed speech reception threshold(SRT) test for evaluation of speech intelligibility. As a result of this study, the processed signals with the companding and NAL-NL1 have performed better than that with only NAL-NL1 in the sensorineural hearing loss conditions. Moreover, the higher ratio of Q values showed better scores in LLR and CEP. In the SRT test, the processed signals with companding(SRT = -13.33 dB SPL) showed significantly better speech perception in noise than those processed using only NAL-NL1(SRT = -11.56 dB SPL).

Background:Delayed encephalopathy after carbon monoxide (CO) poisoning is a severe neurologic impairment that occurs after apparent recovery from the acute event. These patients rarely remain neurologically independent, and no effective treatment has yet been validated. Case Report:Two patients had survived acute CO intoxication without neurologic sequelae. Three weeks after each of the two patients recovered, the patients gradually developed cognitive impairment, urinary incontinence, akinetic mutism and parkinsonism. After the administration of high dose prednisolone, the treatment response was prompt within two weeks, and the patients have recovered with mild cognitive impairment within one year. Conclusion:High dose prednisolone can be used for treatment of delayed encephalopathy after CO poisoning.

Depression in drug-naïve, early-stage PD patients is associated with an earlier occurrence of postural instability. This suggests shared nondopaminergic pathogenic mechanisms and potentially enables the prediction of early development of postural instability.
Syringomyelia Manifesting With Pseudodystonia: A Case Report

Diagnosing dystonia is occasionally difficult because a condition that mimics dystonia, pseudodystonia, is also characterized by a wide spectrum of abnormal muscle contractions. Pseudodystonia may be caused by various neurological diseases that involve the spinal cord or peripheral nervous system but not the brain; alternatively, pseudodystonia may be caused by non-neurological diseases, including arthritis and other musculoskeletal diseases
Guillain-Barre syndrome after generalized tetanus infection

Guillain-Barre syndrome (GBS) is an auto-immune disease of peripheral nerve system. It occurs mainly after preceding infection such as upper respiratory or gastrointestinal infection and other antecedent events as tetanus vaccinations. However, any case of GBS after tetanus infection has not been reported. Recently, when analyzed the clinical aspects of 13 tetanus patients including ours, 2 GBS occurred after tetanus infection. We report the neurological and electrophysiologic findings of two cases of Guillain-Barre Syndrome after generalized tetanus.
Idiopathic Hypertrophic Cranial Pachymeningitis Masquerading as Tolosa-Hunt Syndrome

Idiopathic hypertrophic cranial pachymeningitis (ICHP) is diffuse inflammatory process of the dura mater. ICHP can produce similar presentation with Tolosa-Hunt syndrome (THS) if it involves cavernous sinus. A-29-year old male with persistent headache and no definite neurologic dysfunction was noted. Two weeks later, he complained of ophthalmoplegia, and his symptoms were thought to be manifestations of THS. Brain magnetic resonance images revealed diffuse thickened, enhanced pachymeninges in left tentorium. The patient was diagnosed with IHCP. We report a IHCP patient who showed very similar presentation as THS.

Associations between various metabolic conditions and Parkinson’s disease (PD) have been previously identified in epidemiological studies. We aimed to investigate the causal effect of lipid levels, type 2 diabetes mellitus (T2DM), and body mass index (BMI) on PD in a Korean population via Mendelian randomization (MR).
