        간질성 방광염의 사암침법(舍巖鍼法) 자침 1례

        Objectives : The purpose of this study was to evaluate the effects of $Sa-am$ acupuncture treatment on an interstitial cystitis patient. Methods : A 51 year-old woman who diagnosed as interstitial cystitis, was enrolled in this study. The clinical data was analyzed on a patient with interstitial cystitis due to $Shinheo$(腎虛). The patient had visited Korean medicine department of Yangsan public health care center, and was treated with a $Sa-am$ acupuncture($Sinjeongkyeok$) for 4 weeks. Results : After 4 weeks of $Sa-am$ acupunture treatment($Sinjeongkyeok$), improvement was seen in the subsection of the international prostate symptom score and the part of qualiveen questionnaire score. Conclusions : This clinical case indicates that acupuncture treatment is significantly effective in treatment of a interstitial cystitis.

        폐색성 요로 장애로 발생한 신후성 급성신부전의 치험 1례

        A 64 year-old male patient with acute postrenal failure from obstructive uropathy was treated. His symptoms were difficulty voiding, general weakness, fever, chills and dyspepsia. We diagnosed his symptoms as acute postrenal failure from obstructive uropathy and administered Hwajung-tang(和中湯), Jasin-hwan(滋腎丸) and nelaton catheterization. After 30 days, these clinical symptoms had improved and results of BUN:creatinine also showed improvement, so I report this case, but more clinical case reports are needed.

        인체 신경세포에서 청뇌명신환(淸腦明神丸)의 산화적 스트레스에 대한 세포보호 효과

        Objectives : Oxidative stress due to excessive accumulation of reactive oxygen species (ROS) is one of the risk factors for the development of several chronic diseases, including neurodegenerative diseases. Methods : In the present study, we investigated the protective effects of cheongnoemyeongsin-hwan (CNMSH) against oxidative stress?induced cellular damage and elucidated the underlying mechanisms in neuronal-derived SH-SY5Y cells. Results : Our results revealed that treatment with CNMSH prior to hydrogen peroxide (H2O2) exposure significantly increased the SH-SY5Y cell viability, indicating that the exposure of the SH-SY5Y cells to CNMSH conferred a protective effect against oxidative stress. CNMSH also effectively attenuated H2O2?induced comet tail formation, and decreased the phosphorylation levels of the histone ${\gamma}H2AX$, as well as the number of apoptotic bodies and Annexin V?positive cells. In addition, CNMSH exhibited scavenging activity against intracellular ROS generation and restored the mitochondria membrane potential (MMP) loss that were induced by H2O2, suggesting that CNMSH prevents H2O2?induced DNA damage and cell apoptosis. Moreover, H2O2 enhanced the cleavage of caspase-3 and degradation of poly (ADP-ribose)-polymerase, a typical substrate protein of activated caspase-3, as well as DNA fragmentation; however, these events were almost totally reversed by pretreatment with CNMSH. Furthermore, CNMSH increased the levels of heme oxygenase-1 (HO-1), which is a potent antioxidant enzyme, associated with the induction of nuclear factor-erythroid 2-related factor 2 (Nrf2). According to our data, CNMSH is able to protect SH-SY5Y cells from H2O2-induced apoptosis throughout blocking cellular damage related to oxidative stress through a mechanism that would affect ROS elimination and activating Nrf2/HO-1 signaling pathway. Conclusions : Therefore, we believed that CNMSH may potentially serve as an agent for the treatment and prevention of neurodegenerative diseases caused by oxidative stress.

        청뇌명신환(淸腦明神丸)이 뇌혈류저하 흰쥐의 학습 및 기억 장애 개선에 미치는 영향

        Objectives : Cheongnoemyeongsin-hwan (CNMSH) is a herb medicine to treat cognitive impairment. This study was investigated the effects of CNMSH on learning and memory impairment induced by cerebral hypoperfusion. Cerebral hypoperfusion was produced chronically by permanent bilateral common carotid artery occlusion (BCCAO) in rats. Methods : CNMSH was administered orally once a day (250 mg/kg) for 28 days starting at 4th week after the BCCAO. The acquisition of learning and the retention of memory were tested on 9th week after the BCCAO using the Morris water maze. In addition, effect of CNMSH on neuronal apoptosis and ${\beta}-amyloid$ accumulation in the hippocmapus was evaluated with immunohistochemistry and Western blotting. Results : 1. CNMSH and ChAL significantly shortened the escape latencies on the 2nd day of acquisition training trials. 2. ChAL significantly prolonged the swimming time spent in the target and peri-target zones and CNMSH also significantly prolonged the swimming time spent in the peri-target zone. 3. CNMSH and ChAL significantly increased the number of target heading in the retention test. 4. ChAL significantly shortened the time of the 1st target heading in the retention test, but CNMSH insignificantly shortened the time of that. 5. CNMSH and ChAL significantly increased the memory score in the retention test. 6. CNMSH and ChAL significantly attenuated the reduction of CA1 neurons, but insignificantly attenuated the reduction of CA1 thickness. 7. CNMSH and ChAL significantly attenuated the up-regulation of Bax expression in the CA1 of hippocampus. 8. CNMSH and ChAL significantly attenuated the up-regulation of cascapse-3 expression in the CA1 of hippocampus. 9. CNMSH and ChAL significantly attenuated the ${\beta}-amyloid$ accumulation in the CA1 of hippocampus. 10. CNMSH and ChAL significantly attenuated the up-regulation of APP expression in the CA1 of hippocampus. 11. CNMSH and ChAL significantly attenuated the up-regulation of BACE-1 expression in the CA1 of hippocampus. Conclusions : The results show that CNMSH attenuates neuronal apoptosis and ${\beta}-amyloid$ accumulation in the hippocampus and alleviates the impairment of learning and memory produced by chronic cerebral hypoperfusion. These results suggest that CNMSH may be a beneficial medicinal herb to treat cognitive impairment associated with neurodegenerative diseases.

        간질성 방광염의 사암침법(舍巖鍼法) 자침 1례

        Objectives : The purpose of this study was to evaluate the effects of $Sa-am$ acupuncture treatment on an interstitial cystitis patient.         갈근추출액이 알레르기 염증반응에 미치는 영향

        Objectives To evaluate that Okwada affected which factors for treatment of lung fibrosis. Methods Bleomycin induced lung fibrosis model made in mice. After Okwada lyophilized, power sample obtained and melt in distilled water. Okwada solution administered mice through oral route on 21 days after bleomycin instillation and this procedure performed once a day for 7 days. We divided by three groups; normal (control), bleomycin induced lung fibrosis without treatment (experimental), bleomycin induced lung fibrosis with treatment (treatment). On six weeks after bleomycin instillation, mice sacrificed and removed lung. We performed Western blot analysis for TGF-beta, phosphodiesterase 5A, interleukin (4,5,13) and compared therapeutic effects of Okwada. Results On western blot analysis, all normal and experimental mice detected TGF-beta, phosphodiesterase 5A, interleukin 4,5,13. The amount of band of TGF-beta, phosphodiesterase 5A, interleukin 5 in experimental and treatment group was similar. However, interleukin 4,13 of treatment group decreased compared with experimental group. Conclusions Okwada would be effected the lung fibrosis through suppression of interleukin 4,13.

        부자의 메탄올 추출물에 의한 신세포암의 증식억제 및 세포사멸 유도

        Aconiti Tuber is a traditional medicinal plant generally used in Oriental medicine therapy. In this study, we investigated the biochemical mechanisms of anti-proliferative effects by the methanol extract of Aconiti tuber (MEBJ) in Caki-1 human renal cell carcinoma cells. It was found that MEBJ could inhibit, in a dose-dependent manner, cell growth which was associated with apoptotic cell death such as formation of apoptotic bodies, DNA fragmentation and increased populations of apoptotic-sub G1 phase. Apoptosis of Caki-1 cells by MEBJ was associated with an up-regulation of pro-apoptotic Bax expression, and a down-regulation of anti-apoptotic Bcl-2 in a dose-dependent manner; however, the levels of IAP family were not affected. MEBJ treatment also induced the proteolytic activation of caspase-3 and -8, and a inhibition of poly(ADP-ribose) polymerase (PARP) and $PLC{\gamma}1$ protein. Furthermore, MEBJ treatment caused a dose-dependent inhibition of iNOS and cyclooxygenase-2 (Cox-2). Though further studies will be needed to identify the active compounds that confer the anti-cancer activity of MEBJ, the present findings provide important new insights into the possible molecular mechanisms of the apoptotic activity of MEBJ in cancer cells.

        포제부자(?製附子) 및 포제부자(?製附子) 함유 삼부탕(蔘附湯)의 aconitine 함량 분석과 단회투여 독성시험

        Objectives : The contents of aconitine in aconiti radix lateralis preparata, purified hot water extract of Aconiti Radix lateralis preparata, and purified hot water extract of Sambutang-P that contains Aconiti Radix lateralis preparata was analyzed to compare toxicity. Toxicity of Sambutang-P that contains Aconiti Radix lateralis preparata was assessed with a single oral toxicity test on 6-week-old male and female Sprague-Dawley rats. Methods : 1. The contents of aconitine in Aconiti Radix lateralis preparata, purified hot water extract of Aconiti Radix lateralis preparata, and purified hot water extract of Sambutang-P that contains Aconiti Radix lateralis preparata was analyzed using the purity test according to the "Korean Herbal Pharmacopoeia". 2. 2,000mg/kg was injected for the single oral toxicity test of purified hot water extract of Sambutang-P that contains Aconiti Radix lateralis preparata, and the test was done for a test group (injection) and a control group, each with 5 male and 5 female rats. For 14 days after injection, rats were observed for general symptoms and changes in weight. Afterwards, blood biochemical test, autopsy, and histophathological exam of the liver was conducted. Results : 1. The contents of aconitine was 0.0785% for Aconiti Radix lateralis preparata, 0.1510% for purified hot water extract of Aconiti Radix lateralis preparata, and 0.1248% for purified hot water extract of Sambutang-P that contains Aconiti Radix lateralis preparata. 2. There was no death of either male or female rats in both the control group and the test group (injection of 2,000mg/kg). 3. No unusual symptom was observed in both the control group and the test group (injection of 2,000mg/kg). 4. No significant change in weight was observed for both male and female rats in the test group (2,000mg/kg). 5. The histopathological exam of ALT, AST, ALP, GGT and LDH showed no significant changes for both male and female rats in the test group (2,000mg/kg). 6. According to the autopsy results, no visible abnormality of organs or tissues was found in both the control group and the test group (2,000mg/kg). 7. According to the histopathological exam of the liver, the effect of the injected material was not observed for either male or female rats in the test group (2,000mg/kg). Conclusions : The contents of aconitine in Aconiti Radix lateralis preparata was lower for decoction of Sambutang-P with ginseng radix alba than for decoction of only Aconiti Radix lateralis preparata. This suggests that ginseng radix alba can dilute toxicity of Aconiti Radix lateralis preparata. As for a single oral toxicity test of Sambutang-P that contains Aconiti Radix lateralis preparata, no abnormal reaction was observed even when the injection amount far exceeded a toxic dose or a lethal dose. Thus, it is deemed that using Sambutang-P at a clinically prescribed dose would not lead to hepatoxicity.

        Apoptosis 유도에 의한 톳 ethyl alcohol 추출물의 인체 유방암세포 증식 억제

        H. fusiformis는 갈조식물, 모자반과의 해조류로서 우리나라의 서해안, 남해안, 및 제주도를 비롯해 중국, 일본 등 아시아 전역에 서식하는 천연자원식물이다. 최근 톳의 항산화 활성에 대한 연구는 활발히 이루어지고 있으나 항암에 대한 연구 및 분자생물학적 기전연구가 많이 미흡한 편이다. 따라서 본 연구에서는 인체 유방암세포주인 MDA-MB-231 및 MCF-7 세포를 대상으로 톳 에탄올 추출물(EAHF)에 의한 증식억제에 대하여 조사한 결과, 두 세포주 모두에서 EAHF을 처리하지 않은 군과 비교하여 EAHF의 처리 농도가 증가할수록 암세포의 증식이 현저하게 억제되었고 심한 형태적 변화를 관찰할 수 있었다. 이러한 유방암 세포의 증식억제 및 형태변화는 MDAMB-231에서는 apoptosis 유발과 밀접한 관련이 있음을 알 수 있었고, EAHF에 의해 유발된 apoptosis 정도를 정량적으로 분석하기 위하여 flow cytometry를 이용하여 apoptosis 유발 세포군에 해당하는 sub-G1기에 속하는 세포들의 빈도를 측정하였다. MDA-MB-231세포의 경우는 EAHF의 처리 농도의 증가에 따라 sub-G1기에 해당하는 세포의 빈도가 증가하여 400㎍/㎖의 농도에서는 약 23% 정도가 나타났지만 MCF-7 세포에서는 apoptosis 유발보다는 G1 arrest 효과가 높게 나타났다. 또한 염색질 응축과 연관된 apoptotic body 형성과 함께 apoptosis 유발의 직접적인 증거에 해당하는 DNA fragmentation 여부를 agarose gel 전기영동으로 조사하였다. MDAMB-231 세포에서는 apoptosis가 일어난 세포들에서 볼 수 있는 전형적인 DNA laddering을 관찰 할 수 있었지만 MCF-7세포에서는 DNA laddering이 거의 관찰되지 않았다. 또한 apoptosis 유발에 관여하는 여러 유전자들의 발현에 EAHF이 어떠한 영향을 미치는지를 조사한 결과, MDA-MB-231 및 MCF-7 세포 모두에서 death receptor에 속하는 여러 유전자의 발현에는 아무런 변화가 나타나지 않았으나, MDA-MB-231 세포에서 pro-apoptotic Bax의 발현 증가와 caspase-9 및 -3의 활성화, caspase-3의 기질 단백질들의 분해 등이 관찰되었다. Hizikia fusiforme is a kind of brown edible seaweed that mainly grows in the temperate seaside areas of the northwest pacific, including Korea, Japan and China, and has been widely used as a health food for hundreds of years. Recently, H. fusiforme has been known to exert pharmacological activities including antioxidant, antimutagenic and anticoagulant activities. However, the molecular mechanisms of H. fusiforme in malignant cells have not been clearly elucidated yet. In this study, the effects of ethyl alcohol extract of H. fusiforme (EAHF) on the anti-proliferative effects of MDA-MB-231 and MCF-7 human breast cancer cells were investigated. EAHF treatment resulted in a concentration-dependent growth inhibition by including apoptosis in MDA-MB-231 cells and G1 phase arrest in MCF-7 cells, which could be proved by MTT assay, DAPI staining, agarose gel electrophoresis and flow cytometry analysis. In MDA-MB-231 cells, the increase in apoptosis induced by EAHF treatment correlated with up-regulation of pro-apoptotic Bax expression. EAHF treatment induced the proteolytic activation of caspase-3 and caspase-9, and a concomitant inhibition of poly (ADP-ribose) polymerase, β-catenin, phospholipase-γ1 protein and DNA fragmentation factor 45/inhibitor of caspase-activated DNase. Taken together, these findings provide important new insights into the possible molecular mechanisms of the anti-cancer activity of H. fusiforme.
