        턱목뿔근신경의 재조명

        The mylohyoid nerve is also called nerve to mylohyoid (muscle). This nerve is a branch from the inferior alveolar nerve, which is very familiar to our dentists due to the frequently used block anesthesia for mandibular tooth treatment. The motor fiber is known to be the main component of the nerve that is distributed in the mylohyoid muscle and anterior belly of digastric muscle. In this review article, the following contents will be discussed: the naming story based on the etymology, the general route and main function of the nerve, the research related to the failure of inferior alveolar nerve block anesthesia, and finally other miscellaneous anatomical research results and their clinical significance.

        전자 서베잉을 이용한 서베이드 금관과 국소의치 프레임워크 디자인

        There are still many limitations on fabricating dentures using digital method while computerized production of fixed prostheses utilizing intraoral scanner and CAD/CAM technology has propagated rapidly. Recently the digital solution of fabricating removable partial denture by applying haptic input device, electronic surveying, and rapid prototyping was introduced. In this case presentation, five patients were treated with surveyed crown and removable partial dentures by this digital solution. Fit of dentures was excellent except for one case which showed morphological difference between the actual teeth and that of master cast by the erroneous impression process. There also was not any problem of stability and retention after adaptation in the clinical setting. 구강 스캐너와 CAD/CAM 기술로 인해 컴퓨터로 제작하는 방식이 활발히 보급된 고정식 보철 증례와는 달리, 의치를 디지털 방식으로 제작하는 데에는 아직 한계가 많다. 촉각을 느낄 수 있는 입력 장비와 전자 서베잉의 개념, 그리고 쾌속 조형법의 적용으로 인해 국소의치를 디지털로 제작할 수 있는 솔루션이 최근 소개되고 있다. 본 증례보고에서는 이러한 국소의치의 디지털 솔루션을 이용하여 총 다섯 명의 환자에서 서베이드 금관 및 국소의치를 제작하였다. 인상 과정상의 문제로 모형과 실제 치아 사이의 형태적 차이가 있었던 경우를 제외하고는 국소의치의 적합도는 우수했으며, 장착 후 안정성과 착탈 시의 유지력 또한 임상에서 적용하는 데에 문제가 없었다.

        구강 내 디지털 인상채득을 통한 맞춤형 지대주와 시멘트 합착 후 나사형 임플란트 보철 수복 증례

        최근 디지털 인상채득 시스템이 개발되면서 이를 기존의 CAD/CAM 시스템과 접목한 인레이, 온레이, 크라운이나 간단한 계속가공의치 등의 고정성 보철물의 제작이 활발히 소개되고 있다. 그러나 임플란트 보철에서는 주로 기존의 아날로그식 인상채득 후 이를 스캔하여 제작하는 방법이 사용되어 왔으나, 구강 내 디지털 스캔한 데이터를 이용하여 나사형 보철물을 제작하는 술식은 아직 활발히 이루어지지 않았다. 이에 본 증례보고에서는 6명의 환자를 대상으로 구강 내 스캐너인 iTero를 사용하여 디지털 스캔을 시행한 후 그중 다섯명은 맞춤형 지대주와 시멘트 합착 후 나사형 임플란트 보철물(screw-retained implant prosthesis after cementation, or SCRP)을 제작하였고, 임상적으로 기능과 심미적인 측면으로 만족할 만한 결과를 얻었기에 이를 보고하고자 한다. Fixed dental prostheses such as inlay, onlay, crown, and bridge fabricated by CAD/CAM technique combined with digital impressions is getting popular due to the recent rapid progress of digital impression taking system. For the scope of implant prosthesis, however, digital intra-oral scan hasn’t been actively utilized for the fabrication of superstructures. In this case report, 6 cases of titanium-milled custom abutment based on the iTero intra-oral scan data were introduced, five of them were restored with screw-type prosthesis after cementation (SCRP) and the clinical results were satisfactory on restoring the function and esthetics.

      • 헬기 조종사의 임무수행에 지장을 주지 않는 형태의 맞춤형 구강보호장치 제작에 관한 연구

        Objective : The purpose of this study is to fabricate custom-made mouthguard for helicopter pilots who need to protect oral and maxillofacial system(teeth, periodontal tissue, muscle of head and neck, and temporomandibular joint) from external damage such as bruxism and clenching not to disturb to perform their task. Methods : seventeen helicopter pilots who had bruxism or clenching habit, or severe teeth attrition, teeth fracture, and temporomandibular disorder participated in this study. At first, four type custom-made mouthguard(1mm, 2mm, 3mm, 4mm thickness) were constructed by soft material by using vaccum machine. And then, each mouthguard was adapted for one week respectively. Finally, participant were asked to fill out a questionnaire with a five point rating scale regarding the wearability. Results : All items showed significantly greater satisfaction level in 1mm mouthguard. Except for the 'occlusion' item, there is significantly lowest satisfaction level in 4mm mouthguard. In comparison to 2mm and 3mm mouthguard, no significant difference is seen in all items. Among all items, 'occlusion' item showed highest, and 'speaking' showed lowest satisfaction level. Conclusions : Despite of limitation of this study, 3mm thickness mouthguard which is minimum thickness to do it's own function is considered not to disturb to perform helicopter pilots's task.

      • 군 치과 고정성 보철물의 재제작 및 실패 요인 분석 : 지르코니아 기반 완전도재전장관에 한해서

        Objectives : The aim of this study is to analyze about cause of remake and failure of zirconia-based all ceramic crown and to discuss to reduce possible error while prosthetic process both in clinic and in laboratory. Methods : This study is performed from Oct 2015 to Aug 2016 (about ten months). All remake cases are classified according to cause of failure ; marginal problem(open or overhanging margin), improper proximal contact(loosening or tightening), tooth shade problem, posterior occlusal unanatomical morphology, anterior labio-lingual shape, posterior occlusal fracture, endodontic treatment caused by post-cementation hypersensitivity or pulpal necrosis, anterior porcelain fracture of margin or incisal area over zirconia coping. Results : Among all prosthetic treatment by zirconia crown by using CAD/CAM system, total twelve cases needed remake because of failure of final setting. marginal problem is reported by four cases, esthetic problem of shape of prostheses in anterior area is reported by three cases, posterior occlusal surface fracture and contact problem in posterior area is reported by two cases inspectively, endodontic treatment caused by pulpal damage after cementation of zirconia crown is reported by one case. Conclusions : In this study, most cause of remaking and failure of prostheses was marginal problem of zirconia-based all ceramic crown prostheses. In order to reduce possible problem of fabrication of zirconia prostheses, close communication and cooperation between dentist and dental technician is need

      • 군 치과 CAD/CAM 시스템으로 제작한 보철물의 변연적합도 평가 : In vivo study

        Objectives : The purpose of this study is to evaluate effectiveness of ROK dental CAD/CAM(computer-aid design/computer-aid manufacture) system, which is newly introduced since 2015 by measuring marginal fitness of all ceramic final prostheses(zirconia crown) in labial(buccal), palatal(lingual), mesial and distal marginal area on molar and premolar teeth. Methods : twenty patients who need caries removal and prosthetic treatment participated in this study to fabricate crown prostheses by using ROK dental CAD/CAM system. total twenty-four teeth(seven maxillary premolar, six maxillary and mandibular molar, five mandibular premolar) are treated to CAD/CAM prostheses. final prostheses is adapted to teeth, and marginal fitness was evaluated by using replica technique. Results : labial/buccal marginal gap of bucco-lingual section is 57.1μm(±11.8μm), palatap/lingual is 64.9μm(±17.3μm), and mesial marginal gap of mesio-distal section is 61.9μm(±9.6μm), distal area is 62.5μm(±13.8μm). In comparison of tooth area, the marginal gap of maxillary premolar, mandibular premolar, maxillary molar, mandibular molar is respectively 54.9μm, 63.9μm, 64.0μm, 64.8μm. In periodic check after 1 month, non specific finding is seen such as periodontal or prosthetic problem. Conclusions : In this study, mean marginal distance of zirconia crown prostheses by using ROK dental CAD/CAM system is 61.6μm, so it seems that it had clinically acceptable marginal fitness.

      • 일부 지역 육군 장병들의 구강건강관리 인식 및 구강건강관리 실태 조사 결과 분석

        Objectives: The purpose of this study was to investigate Korean soldier’s oral healthcare recognition and oral health status by performing oral examination and questionnaires, and then to provide the basic data for the contents development of the oral health policies to the military. Methods: The subjects in this study were 653 soldiers serving in Gyeonggi, Gangwon and Chungchong provinces. The study involved an oral examination to record decayed, missing, filled teeth and periodontal status to evaluate oral health status, combined with a questionnaire to evaluate oral health recognition. Results: 52% of soldiers have received dental treatment in the army dental clinic or local clinic after enlistment, and most received treatment was caries treatment(53%). Second, In the result of oral health recognition, the most of the soldiers(46%) answered that they thought to be in moderate oral health, but 27% of respondents answered that they thought to be in poor oral health. and there is no significant difference between the rank of soldiers. (p>0.05) Next, According to this study, 45% of subjects need caries treatment, and 37% of subjects need scaling and periodontal treatment. Finally, Most soldiers brush their teeth three times a day(40%) or twice a day(39%). Only 34% of respondents used oral care products, and most used oral care products was gargle solution(30%), floss(27%), proximal brush(26%) in sequence. Conclusions: This study suggests that it is necessary to organize the oral health care program for soldiers to provide self-oral health care themselves, and to develop the contents of an oral health education customized to them to improve concern and recognition of oral health for members.
