Tuberculin 반응과 혈청 IgE와의 관련성에 대한 연구

목 적 : 아토피는 IgE에 의해 매개되는 여러가지 항원(allergen)에 대한 알레르기 반응상태이다. 최근 수 십년간 선진국에서 소아 알레르기의 유병률은 증가추세에 있으며, 소아시기에 일부감염에 대한 노출 기회의 감소가 아토피 반응을 증진시킬 가능성이 있을 것으로 추정되고 있다. 저자들은 우리나라에서 비교적 유병률이 높은 Mycobacterium tuberculosis에 의한 면역 반응이 아토피 질환을 억제할 수 있는지를 보기 위해 tuberculin 반응과 혈청 총 IgE, 항원 특이 IgE 등과의 관련성을 비교, 분석하였다.방 법 : 1995년 1월부터 1997년 12월까지 부산메리놀병원 소아과에 급성 상기도 감염, 급성 기관지염, 폐렴, 기관지 천식 등으로 입원한 환아중 내원 당시 흉부청진상 wheezing이 있었던 환아 93례를 대상으로, tuberculin 반응 검사(5TU PPD)를 시행하여 경결 직경이 10mm 이상인 양성군(A군)과 10mm 미만인 음성군(B군)으로 나누어, 두 군에서의 연령, 성별, 혈청 총 IgE, allergen-specific IgE 양성률을 비교하였다.결 과 : 전체 대상 환아 93명중 A군이 21명, B군이 72명이었으며, 혈청 총 IgE 농도는 A군에서 152.2±360.5IU/ml, B군에서 309.4±437.5IU/ml로 B군에서 의미있게 높았고, 연령별 정상치와 비교해서 높은 경우도 B군에서 의미있게 많았다(P<0.05, Table 2). 항원 특이 IgE 양성을 보인 경우는 A군 14.3%(3/21명), B군 73.6%(53/72명)로 유의한 차이를 나타내었다(P< 0.05, Table 2). Purpose : Atopy is a state of allergic response, mediated by IgE, to common environmental allergens. In recent decades, there has been an increase in prevalence of atopic disorders in children in developed countries. The factor associated with the rise of atopy incidence may be declining exposure to some infection opportunities. The aim of this study is to determine if the immune response to Mycobacterium tuberculosis can suppress atopy by investigating the association between tuberculin responses and serum IgE. Methods : We divided the patients into two groups according to tuberculin skin test. Group A showed positive response(induration diameter■10mm), while Group B showed negative response (induration diameter<10mm). A comparison was made between the two groups in terms of age, sex, serum total IgE and allergen-specific IgE. Results : Twenty-one patients comprised Group A and 72 comprised Group B. The mean serum total IgE concentration and the number of patients showing positive MAST in Group B was significantly higher than that of Group A(P<0.05). Conclusion : In our study, there was an inverse association between delayed hypersensitivity to Mycobacterium tuberculosis and atopy.

Agenesis of corpus callosum occurs sporadically and may be transmitted as sex-linked, or autosomal-dominant or recessive traits. It has been associated with different syndromes. Clinical pictures vary from severe intellectual and neurologic abnormalities to asymptomatic and normaly intelligent cases. Agenesis of corpus callosum may occur alone, but it is more frequently associated with a high incidence of other anomalies. We report a male infant with agenesis of corpus callosum who was diagnosed to have ileal atresia and duplication. 저자들은 출생시부터 약한 울음소리와 기면을 보였고, 생후 3일경부터 심한 구토와 복부팽만을 보였던 뇌량무발육증과 동반된 소장폐쇄와 장중복 환아를 경험하였다. 뇌량무발육증의 경우 환아에서 동반된 선천성 기형에 대한 주의깊은 조사가 필요하며, 심한 신경발달적 지연과 경련의 발생을 고려해야 한다.

저자들은 2주간의 간헐적 상복부 동통과 식욕결핍을 호소하는 15세 남아에서 위 내시경과 조직 생검으로 진단된 원발성 위 림프종 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Primary gastric lymphoma is relatively uncommon, accounting for 3% to 8% of all malignancies arising in the stomach. The most common symptom is abdominal pain, closely followed by weight loss, anorexia, weakness due to anemia, nausea, and vomiting. The diagnosis of gastric lymphoma usually requires a biopsy at the time of gastroscopy or laparotomy. Microscopically, the vast majority of gastric lymphoid tumors are non-Hodgkin's lymphomas of B cell origin. Survival rates for all types of gastric lymphoma generally exceed those for adenocarcinoma and other malignancies of the stomach. We experienced one case of primary gastric lymphoma in puberty with a brief review of the literature.

Primary hypomagnesemia is a rare inherited disorder and it is considered to be due to either a defect in the intestinal transport of magnesium or a defect in renal tubular transport. It is important to measure the urinary excretion of magnesium to differentiate the causes of magnesium deficiency. We report here an one-month-old female infant of primary hypomagnesemia who presented generalized tonic-clonic seizures. She had hypomagnesemia(<1.5mg/dL) and several seizure attacks but normal magnesium creatinine ratio in random urine and normal magnesium excretion in 24-hour urine. Continuous oral magnesium supplementation was necessary to avoid the recurrence of symptoms and maintain serum magnesium levels. 저자들은 증상소실을 위해 마그네슘의 지속적 경구투여가 필요하였던 원발성 저마그네슘혈증의 1례를 1개월 여아에서 경험하였기에 문헌고찰과 함께 보고하는 바이다.
