목적 초음파 자궁조영술로 확인된 자궁내막폴립의 변화 양상을 알아보고자 하였다. 연구방법 2005년 10월 1일부터 2008년 7월 31까지 초음파 자궁조영술로 자궁내막폴립이 추정되는 57명의 폐경 전 여성을 대상으로 후향적으로 분석하였다. 이들에서 2회 이상의 정상 생리 후 다시 초음파 자궁조영술을 시행하여 변화양상을 확인하였다. 결과 대상 환자의 평균 연령은 37.7세였으며 모두 폐경 전 여성이었다. 42명의 환자에서 출혈증상이 있었으며 가장 흔한 증상은 생리사이 소량 출혈이었다. 57명의 환자 중 15명(26.3%)에서 자궁내막폴립이 저절로 소실되었다. 소실된 군과 소실되지 않은 군 간의 환자의 증상, 나이, 체중, 신장, 체질량지수, 산과력, 두 번째 초음파 자궁조영술까지의 기간 및 폴립의 크기를 비교하였을 때 통계학적으로 유의한 차이는 없었다. 자궁내막폴립이 소실되지 않은 42명의 환자 중 29명에서 수술적 치료를 시행하였으며 모두에서 악성 소견은 보이지 않았다. 결론 악성 가능성이 적은 폐경 전 여성에서 자궁내막폴립이 진단된 경우 폴립의 자연소실 가능성을 고려하여 2회 정도의 정상 생리 후 폴립의 지속 여부에 따라 치료 방법을 결정해야 할 것으로 생각된다. Objective To assess changes in endometrial polyps by sonohysterography. Methods We conducted a retrospective analysis of 57 premenopausal women who were suspected to have endometrial polyps and were examined by sonohysterography from October 2005 to July 2008. We evaluated changes in endometrial polyps by sonohysterography after observation for two or more menstrual cycles. Results The mean age of the patients was 37.7, and all patients were premenopausal. Forty-two patients had bleeding symptoms, the most common being intermenstrual spotting. Endometrial polyps regressed spontaneously in 15 out of 57 patients (26.3%). The endometrial polyp regression rate was not associated with symptoms, age, body mass index, gravida, parity, initial largest polyp diameter or interval between sonohysterography. Of the 42 patients with persistent polyps, 29 received surgical treatment. None of the patients had malignant lesions. Conclusion Endometrial polyps in premenopausal patients with low malignant potential may be observed for about two menstrual cycles in the hope of spontaneous regression.
