        We have clinico-statically analysed the 235 cases of esophageal foreign body during the period the past 12 years from March InS to December 1996 at the Department of Otolaryngology-Head and Neck Surgery, Korea University College of Medicine, Guro Hospital. The results were as follow : 1) Coin was the most frequent foreign body in the esophagus(172 cases : 73.2%). Bone was the next frequent foreign body (21 cases 8.8%). 2) In sex distribution, 124 cases(52.8%) of patients were the female and 111 cases(47.2%) were the male, so the sex ratio was 1:1.12. 3) In the age distribution, 134 cases(56.9%) were under 5 years of age, coin was the most frequent materials in this age group(111 cases 47.2%). 4) The most common symptom was vomiting(136 cases : 57.9%). The next common symptom was dysphagia and odynophagia was 41 cases(17.4%) 5) The most prevalent site of lodgement was the first narrowing(181 cases : 77.0%), the third narrowing(31 cases : 13.2%) and the second narrowing of esophagus(23 cases 9.8%) in order. 6) In duration of lodgement, 207 cases(88.1%) were lodged for one day. 7) The foreign bodies of esophagus were removed successfully by esophagoscopic procedure under local or general anesthesia. There were 3 cases of complication, one case of esophageal bleeding and two cases of esophageal rupture.

        내용 생성을 위한 학생 필자의 자료 탐색 과정에 관한 연구 -6학년 필자 사례를 중심으로-

        본 연구는 글의 내용을 생성하기 위해 학생 필자들이 어떻게 다양한 자료들을 탐색하는지 살피는 것을 목표로 하였다. 이에 따라 6학년 필자 5명의 쓰기 과제 수행 전 과정 녹화 자료, 면담, 글 자료를 분석하였다. 연구의 결과, 자료 기반 쓰기에서는 자료 탐색의 과정이 글의 주제 선정에 큰 영향을 주는 것으로 풀이되었다. 또한 글을 쓰기 전 자료 탐색 및 선택을 모두 완료하는 것이 아니라, 자료 탐색과 쓰기 과정이 번갈아서 반복적으로 나타났다. 끝으로, 참여자들은 자료의 질을 평가하기도 하였다. 그러나 자료의 질을 제대로 평가하지 못하는 문제가 있었고, 선택한 자료의 수가 매우 적어 정보의 종합에 이르지 못하기도 하였다. 자료 탐색의 의의를 이해하고 목적에 맞는 자료를 제대로 선택하여 작문할 수 있도록 하는 구체적인 교육 방안들이 모색되어야 할 것이다. The purpose of this study was to investigate how student writers search various sources to generate text`s contents. For this purpose, 6th student writers` visual recording data, interview data, texts data were col-lected and analysed. Study results showed that sources searching process has an effect on topic decision. And participants didn`t finish sources searching and selecting before writing but search sources and write repeatedly. Third, participants evaluate the quality of sources. However they could not eval-uate the quality properly and could not synthesize information properly because of a few source. Based on the results, we need to understand the importance of writ-ers` sources searching process and to seek teaching strategies to help writers to select proper sources to write.

        예비 초등 교사의 아동 도서 선정 기준에 관한 연구

        본 연구에서는 교육대학교 재학생 50명이 교육 활동에 쓸 아동 도서를 어떤 기준으로 선정하는지 분석하였다. 분석 결과, 참여자들은 크게 ‘도서에 권위를 부여하는 사회적 지표’, ‘초등학교 교육과정’, ‘예비 교사의 개인적인 독서 경험’, ‘예비 교사의 개인적인 교육 경험’, ‘아동 독자의 특성 및 교육 환경을 둘러싼 사회적 요구’, ‘아동 독자의 이해’, ‘아동 독자의 흥미’, ‘교육 제재로서 도서의 특성’, ‘태도 측면 교육’, ‘심동 측면 교육’ 등의 기준을 고려한 것으로 드러났다. 또한 문학 도서를 선택할 때와 정보 도서를 선택할 때 주목하는 요인이 서로 다른 것으로 드러났다. 참여자들은 다양한 기준을 사용해 도서를 꼽았지만 문학 장르에 편향하여 도서를 선정했다는 점, 도서 선정 목적에 어울리는 구체적 교육 방법을 제시하지 못했다는 점, 개인적 취향에 따라 도서를 선정하기도 한다는 점 등의 제한점도 드러내었다. 이에 따라 이들이 아동 도서 및 아동 독자의 특성을 더욱 잘 이해할 수 있도록 돕는 적극적인 교육 기회가 제공되어야 할 것으로 보인다 In this study, two researchers analysed how 50 pre-service elementary teachers select children’s books. Study results showed that the participants selected children’s books taking into account the ‘social indicators’, ‘pre-service teacher’s unofficial teaching experience’, ‘children readers’ nature and social needs’, ‘children readers’ comprehension’, ‘children readers’ interest’, ‘book’s trait as teaching material’, ‘attitude education’ and ‘behavior education’. And participants considered different elements due to the genre of the texts. Pre-service teachers considered various elements selecting books, there were, however, inadequacies that they couldn’t present proper teaching methods, overvalued literature books, focused on personal taste. Based on the results, this study proposes that pre-service teachers need active support to understand the traits of children’s books and children readers.

        복합양식으로서 만화 텍스트 표현 활동 사례에 대한 연구

        만화는 글과 그림이 종합된 복합양식 텍스트이다. 또한 학습자들이 친숙하게 접할 수 있어 교육의 도구로 자주 사용되는 텍스트이기도 하다. 그러나 만화를 교육 텍스트로 다룬 선행 연구는 학습자를 만화의 수용자에 제한하거나, 의사소통의 도구로서 글과 그림의 결합을 깊이 있게 다루지 못한 경향이 있다. 이에 본 연구는 11주간 진행된 만화 텍스트 표현수업의 사례를 살펴봄으로써, 복합양식 텍스트 표현 교육의 의의 및 방향을 탐색해 보았다. 수업 사례에 대한 관찰 및 분석 결과, 첫째, 학습자들은 만화에 있어서 내용의 전달보다 그림의 표현에 집중하는 모습을 보였다. 둘째, 학습자는 교과 학습의 수단으로 만화 텍스트 표현 교육의 의의를 인지하고 있었다. 셋째, 복합양식의 분석 기준에 대한 연구가 필요하다는 점이 도출되었다. 연구의 결과는 학습자들이 다양한 기호의 의미와 기능을 이해하고 능동적으로 복합양식 텍스트를 표현할 수 있도록 도와야 하다는 점을 시사한다. Cartoon is multimodal text concluding written texts and pictures. Moreover, cartoon is used for instructional materials because of learners’ being intimacy with cartoon. Prior researches, however, restricted learners’ position as cartoon readers and could not investigate the relationship between written texts and pictures sufficiently. As a result, this research was set for study the meanings and intention points of multimodal text expressing by analyze a case of cartoon text expressing class lasting 11 weeks. The result of this research showed that participants focused on painting rather than contents delivering. Second, learners recognized the importance of the composition education of cartoon texts. Third, the standards of analysis for multimodal texts are needed to develop the teaching and learning method in new literacy. This research’s results suggest that teachers and researchers need to help students to understand and use the meaning and functions of various codes and to express multimodal texts actively.

        Aspiration of foreign bodies into the airway is a common problem in spite of efforts to educate the public, and it can be sometimes fatal accidents. We clinically analyzed 101 cases of airway foreign bodies treated in the Department of Otolaryngology-Head and Neck Surgery, Korea University College of Medicine for the past 10 years comparing data with the previous articles. There was male predominance in the occurrence rate, being 72% in the male patients. The peak age was 1 to 3 years of age, and 84.2% was below age of 10. The most common symptom after foreign body aspiration was intractable cough, and obstructive emphysema was most commonly found on the initial chest X-rays. Foreign bodies were mainly located at the right main bronchus and left main bronchus was the next. Peanuts were the most common airway foreign bodies. Ventilating bronchoscopy was performed in the 99 cases and foreign bodies were successfully removed in most cases. There was 2 deaths(2%) resulted from hypoxia after bronchoscopy. The fact that complication rates increase with the duration of the foreign bodies in situ is clear. Therefore, prompt bronchoscopy in patients with suspected airway foreign bodies is essential for lower complication rates.

        좋은 텍스트 요건에 대한 학습자의 인식과 쓰기 실행에 대한 연구

        본 연구는 학습자들이 좋은 글의 요건으로 인식한 내용을 본인의 작문에 어떻게 적용하는지 탐색하기 위한 목적으로 수행되었다. 연구에서는 먼저 내용 영역의 적절성을 고려하여 의학 칼럼으로 텍스트의 범위를 한정하고 의학 전공생들을 참여자로 선정하였다. 두 명의 연구자는 51명의 참여자들이 좋은 의학 칼럼의 요건으로 무엇을 들었는지, 그리고 자신의 작문에는 이를 어떻게 반영하였는지 분석하여 다음의 결론을 도출하였다. 첫째, 참여자들은 좋은 의학 칼럼의 요건으로 ‘독자 고려’, ‘가치’ 등을 포함하는 10가지 항목을 제시하였다. 둘째, 참여자들이 좋은 의학 칼럼의 요건을 적절히 인식하고도 이를 스스로의 작문에 적용하는 데 어려움을 겪는 것으로 확인되었다. 셋째, 참여자들은 적절한 요건을 인식하고 스스로의 작문에 이를 적용하기도 했으나 전체적으로 양질의 글을 생산하는 데 실패하기도 하였다. 본 연구에서는 쓰기 지식에 대한 인식과 실제 쓰기 수행 간에 존재하는 뚜렷한 차이를 확인하였다. 추후에는 학습자의 읽기와 쓰기에 영향을 주는 요건들이 더욱 세밀하게 연구되어야 할 것으로 보인다. The purpose of this study was to investigate how learners evaluate requisites for good texts and how they apply these requisites to their own composition. This study limited the scope of texts to medical column and collected medical students as participants considering the suitability of content area. Two researchers analyzed participants’ awareness of good medical texts and their own writing outcomes and found several traits. First, participants suggested ten categories of conditions for good medical column including ‘regarding readers’ and ‘value’. Second, some participants struggled to make use of their awareness of good texts in their writing. Third, some participants who even could aware and apply requites for good texts failed to make good texts. Researchers identified the gap between writing knowledge and authentic writing implementation. We need to inquire the traits and reflected factors on learners’ reading and writing more minutely.

      • 초기 후두암의 치료

        Background: Laryngeal cancer, the most common malignancy of head and neck, is being detected and treated in earlier and more effectively due to introduction of endoscope, continuous development of radiographic technology, and advancement in new surgical techniques such as laser surgery and conservation laryngeal surgery Objectives To compare the different treatment results for early laryngeal cancer to determine which methos provides the good prognosis. Materials and Methods: Retrospective study was done for 135 patients with early laryngeal squamous cell carcinoma(Stage I or II) who were treated at our institute between 1989 to 1996. There were 105 patients with glottic cancer(Stage I: 68 patients, Stage II:37 Patients) and 30 patients with supraglottic cancer(Stage I: 12 patients, Stage II:18 patients) Initial treatment included radiation therapy for 54 patients, endoscopic laser surgery for 8 patients, laryngofissure and cordectomy for 8 patients, vertical partial laryngectomy for 37 patients, supraglottic laryngecomy for 14 patients and supracricoid laryngectomy for 5 patiens, and total laryngectomy for 9 patients. Results : Salvage treatments, such as total laryngectomy, conservation laryngeal surgery, radiation therapy and neck dissection were performed for initial treatment failure with 9 patients after radiation therapy, 8 patients after conservation laryngeal surgery, 2 patients after endoscopic laser and 3 patients after total laryngectomy. Overall 3-year survival rate for glottic 71 was 92.4%, glottic f 84.3%, supraglottic 7172.7%, and supraglottic f was 63.%%. However, survival rate of the same stage was variable according to the choice of initial treatment. Conclusion New classification of the early laryngeal cancer was necessary and helpful for the choice of the initial treatment.

        Carotid body tumors are uncommon tumors of the head and neck Surgery is the primary treatment for the tumor. Large carotid body tumors frequently encircle the common, internal, and exernal carotid arteries, and extensive bleeding often complicates the resection, increasing the risk of carotid artery rupture and damange to major cranial nerves. Grafting should be used in high-risk patients. We have experienced a case of carotid body tumor which encircle the common, internal and external carotid arteries, treated with ligation of external carotid artery and grafting using Gortex between common carotid artery and internal carotid artery.

        일반논문 : 쓰기 과정에서 드러나는 필자와 협조자의 상호작용 연구

        This article attempted to analyze how college student writers and their supporters interact in actual writing process. Focusing on how writers and supporters interact, usefulness of collaborative writing could be investigated. Participants were 37 junior students majoring Korean language education in the college of education and they learned theories about collaborative writing process. Researchers collected and analyzed participants’ texts, reflective journals, written brain storming materials, conversation maerials and so on. Study results showed that writers selected their supporters depending on their writing purpose and personal situation. And participants who had oral conversation made better results than who had merely written comments. Moreover, having interaction in planning and revising stage both was more desirable than in revising stage only. This study proposes that educators need to teach student writers how to interact supporters and how to accept others’ help in writing more than ever.
