        침구의학에서 플라시보 대조군과 플라시보 효과

        Objectives: Clinical improvements can be observed following placebo administrations in clinical trials. Randomized controlled trials have been conducted to disentangle the specific effects of therapeutic interventions over the past decade. In acupuncture trials, non-penetrating placebo needles offer the opportunity to determine the specific effects of needling. Methods: The present review provides an overview of the concept of the placebo effect and the characteristics of the placebo needles. Results: Placebo control can contribute to minimize for bias and the contextual and psychological components of therapeutic interventions. Placebo control should meet two criteria: blinding efficacy (indistinguishable from active treatment) and physiological inertness. In the case of acupuncture, however, it is difficult to meet both criteria simultaneously. The dilemma of placebo needles suggests that placebo needles do not constitute proper control in acupuncture research. Considering the characteristics of placebo needles, patients are more likely to perceive placebo needles as active treatment in acupuncture trials compared to placebo pills in pharmaceutical trials. Placebo response might be observed more frequently to placebo needles than to placebo pills. When acupuncture treatments are utilized in clinical use, placebo effects can be enhanced by exploiting patients' expectations in the contextual or environmental cues that surround medical intervention. Conclusions: We have to consider these unique characteristics of placebo needles in order to avoid drawing premature conclusions that acupuncture itself is just a placebo.

        데이터마이닝을 이용한 동의보감에서 경락의 주치특성 분석

        Objectives : DongUiBoGam is one of the representative medical literatures in Korea. We used text mining methods and analyzed the characteristics of the indications of each meridian in the second chapter of DongUiBoGam, WaeHyeong, which addresses external body elements. We also visualized the relationships between the meridians and the disease sites. Methods : Using the term frequency-inverse document frequency (TF-IDF) method, we quantified values regarding the indications of each meridian according to the frequency of the occurrences of 14 meridians and 14 disease sites. The spatial patterns of the indications of each meridian were visualized on a human body template according to the TF-IDF values. Using hierarchical clustering methods, twelve meridians were clustered into four groups based on the TF-IDF distributions of each meridian. Results : TF-IDF values of each meridian showed different constellation patterns at different disease sites. The spatial patterns of the indications of each meridian were similar to the route of the corresponding meridian. Conclusions : The present study identified spatial patterns between meridians and disease sites. These findings suggest that the constellations of the indications of meridians are primarily associated with the lines of the meridian system. We strongly believe that these findings will further the current understanding of indications of acupoints and meridians.

        오수혈 자침 깊이와 맥기의 천심

        Objectives : The purpose of this study was to investigate the needling depth of five-phase acupoints and discuss the association with the depth of meridian qi. Methods : DongUiBoGam was used to determine the depth of five-phase acupoints. The depth of needling at 60 five-phase acupoints was compared between well, spring, stream, river, and sea acupoints. Results : The proximal part of the extremities had deeper needling depth than the distal part of the extremities. The targeted deqi sensation can be related to the needling depth. Conclusions : The depth of the meridian qi is related to the distinct patterns of needling depth of five-phase acupoints.

        아시혈의 의미와 특성의 이해

        Objectives : Acupoints are divided into three categories: classical acupoints, extra-acupoints, and Ashi points. The aim of this study was to understand the meaning and features of Ashi points. Methods : We examined the original meaning of Ashi points from the classical medical texts, including the Beijiqianjinyaofang, the Essential Prescriptions Worth a Thousand Gold for Emergencies, and the Huangdi Neijing, the Yellow Emperor's Canon of Internal Medicine. Results : First, the Ashi method is to locate the points for the acupuncture and moxibustion based on the patients' reactions mainly manifested by sensations of comfort and pain, which can help identify not only Ashi points but also classical and extra-acupoints. Thus, Ashi points may or may not match to classical or extra-acupoints, and we propose that Ashi points should not be classified mutually exclusive to classical or extra-acupoints. Second, there are several similarities between Ashi points and myofascial trigger points. They are located by palpation and have no fixed anatomical positions. Patients experience painful but pleasant feeling when Ashi and myofascial trigger points are pressed, and stimulation of these points have treatment effects. Conclusions : We suggest that Ashi method be used to identify the acupoints based on how patients react when these points are pressed. Ashi points may or may not correspond to classical or extra-acupoints, and share traits with myofascial trigger points.

        침 치료 임상연구 동향에 대한 계량서지학적 분석

        Objectives : As acupuncture treatment has been widely practiced in many countries around the world, clinical trials of acupuncture treatments also have become popular. The objective of the study was to explore the trends of research investigating the effect of acupuncture treatment in clinical trials using a bibliometric approach, a quantitative analytical methods. Methods : Publications related to clinical trials using acupuncture from 2000 to 2019 were retrieved from the Web of Science database. Extracted articles were analyzed in terms of publication year, country, journal, research area, organizations and authors. Trends in research on acupuncture in clinical trials were visualized using the VOSviewer program. Results : A total of 3,166 articles of acupuncture clinical trials published from 2000 to 2019 were identified and analyzed. The country producing the most articles in this field was USA followed by China, England, South Korea, and Germany. A network analysis based on the co-occurrence of keywords showed following three clusters: clinical studies, pain management studies, and methodology studies. Conclusions : This study provided a macroscopic overview of research in acupuncture clinical trials. These findings provide an expansive strategy for researchers in this field to cooperate with other researchers or organizations.

        『동의보감』 내 미용 처방 선별 연구 : 기능과 용법을 중심으로

        As Korea's representative medical text, "Donguibogam" reveals Korean Medicine's unique perspective on the human body. The text also contains abundant resources related to skin, leading to diverse researches on the subject today. In this paper, authors tried to elicit values of "Donguibogam" in the general cosmetics field, through literature review. First, prescriptions related to main functions of modern cosmetics (anti-aging, whitening, and moisturizing) were classified. In relation to anti-aging, concepts such as 返老 (reversing aging), 還童 (to become young again), 皺 (wrinkle), and 皮急 (tense skin) were extracted and 69 examples of treatments were introduced. For whitening care, 41 treatments were mentioned on hyper-pigmentation and general complexion. Concepts related to moisturizing included expressions such as 潤肌膚 (hydration of skin) and 光澤 (glow), induced by 48 treatment prescriptions. Second, cosmetics related prescriptions were classified according to the application methods of modern cosmetics. 78 prescriptions of cleansing category included washing-off local area with herbal liquid, bathing, washing-off after steaming, and soaking the body in the herbal liquid. Total of 308 topical application prescriptions covered the conditions of various skin inflammations, scalp illnesses, and etc. In conclusion, this research examines the value of "Donguibogam" as a useful of skincare. Further studies are expected to extend our understanding of the field.

        침자 보사에서 자침 자극 강도의 특성

        Objectives : The purpose of this study was to characterize the stimulation intensity in acupuncture manipulation techniques for tonification and sedation therapy. Methods : To describe the level of stimulation used in acupuncture manipulation techniques for tonification and sedation therapy, we reviewed the acupuncture manipulation techniques described in classical medical textbooks. Results : Based on the patients' conditions and pattern identification, acupuncture manipulation strategies for tonification and sedation therapy were chosen. For example, the excess condition was treated with sedative therapy, and the deficiency condition was treated with tonification therapy. For tonification therapy, weak to modest stimulation was applied in acupuncture manipulation techniques, whereas intense stimulation was applied for sedative therapy. Even though the intensity of acupuncture stimulation was chosen based on the clinical examination, deqi sensation is a crucial component of acupuncture treatment, and during acupuncture practice, the practitioner should choose the right intensity of acupuncture stimulation based on deqi response in each patient. Conclusions : We concluded that the tonifying and sedative effects of acupuncture treatment are related to the stimulation intensity of acupuncture manipulation techniques. For individualized medicine, the right amount of acupuncture stimulation should be administered based on the patients' conditions and responses, such as deqi responses.

        훈침의 미주신경 실신 측면으로 이해와 적절한 예방과 조치

        Objectives : Needle sickness is one of the adverse events of acupuncture, although substantial adverse effects during a routine acupuncture treatment seem to be highly unusual. In this work, we propose that an acupuncture-related vasovagal response resembles needle sickness during acupuncture therapy. Methods : In this article, we discussed the general characteristics of vasovagal syncope and went into more detail on vasovagal syncope in people who have a fear of blood injection and injury. We also offer a recommendation for the prevention and management of vasovagal syncope brought on by acupuncture. Results : The vasovagal reaction related to acupuncture is closely associated with needle sickness. The prevention can be done using PEACHES (position, experience, anxiety, constitution, hydration, environment, symptom recognition) principles. The management should be conducted using the RIPCORD (recognize, initiate, position, communicate, order treatments, reassess, document) techniques. Conclusions : It is important to comprehend the characteristics of needle sickness as a vasovagal reaction related to acupuncture. According to the recommendation, practitioners should effectively prevent and manage needle sickness.

        수지부 정혈과 십선혈의 관계에 대한 고찰

        Objectives : Our goals were to examine how the well points in the fingers came to be and how their placements have changed, as well as to determine how they relate to the EX-UE11 points. Methods : We reviewed the classic textbooks to understand the origin and the changes of locations of the well points in the fingers. We also compared the location and indications between well points in the fingers and EX-UE11 points. Results : At first, the tips of the fingers, which are now thought to be the locations of EX-UE11 points, were once described as containing well points. Currently, well points are positioned 0.1 F cun distal-medial (or lateral) to the medial (or lateral) corner of the nail. In addition to the locational commonality, we found similarities between the well points in the fingers and the EX-UE11 points in terms of their indications; for example, bloodletting at these places is frequently utilized to treat emergencies, including acute stroke and fever. Conclusions : We suggest that it is highly likely that well points in the fingers and EX-UE11 points were initially the same acupuncture point and later classified into two different acupuncture points, given their identical locations and indications. If the clinical relevance between the change process of the well points’ locations in the fingers and the EX-UE11 is studied in the historical literature, it is anticipated that the significance and clinical application of well points can be expanded.
